Radiologic assessment of bone healing after orthognathic surgery using fractal analysis

  • Purpose : To evaluate the radiographic change of operation sites after orthognathic surgery using the digital image processing and fractal analysis. Materials and Methods : A series of panoramic radiographs of thirty-five randomly selected patients who had undergone mandibular orthognathic surgery (bilateral sagittal split ramus osteotomy) without clinical complication for osseous healing, were taken. The panoramic radiographs of each selected patient were taken at pre-operation (stage 0), 1 or 2 days after operation (stage 1), 1 month after operation (stage 2), 6 months after operation (stage 3), and 12 months after operation (stage 4). The radiographs were digitized at 600 dpi, 8 bit, and 256 gray levels. The region of interest, centered on the bony gap area of the operation site, was selected and the fractal dimension was calculated by using the tile-counting method. The mean values and standard deviations of fractal dimension for each stage were calculated and the differences among stage 0, 1, 2, 3, and 4 were evaluated through repeated measures of the ANOVA and paired t-test. Results : The mean values and standard deviations of the fractal dimensions obtained from stage 0, 1, 2, 3, and 4 were 1.658±0.048, 1.580±0.050, 1.607±0.046, 1.624±0.049, and 1.641 ±0.061, respectively. The fractal dimensions from stage 1 to stage 4 were shown to have a tendency to increase (p < 0.05). Conclusion: The tendency of the fractal dimesion to increase relative to healing time may be a useful means of evaluating post-operative bony healing of the osteotomy site.

외측 하악각 골절제술을 동반한 시상분할골절단술을 통한 골격성 3급 하악골 비대칭 환자의 치료 (THE CORRECTION OF CLASS III MANDIBULAR ASYMMETRY USING BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY AND LATERAL ANGLE REDUCTION)

  • Purpose: The aim of this study is to identify the usefulness of unilateral mandibular angle ostectomy, so-called "Lateral Angle Reduction", in asymmetric prognathism patients by the assessment of postoperative stability and esthetic results Patients and methods: For the retrospective study, 10 skeletal class III mandibular asymmetry patients who were performed SSRO and unilateral mandibular angle ostectomy, Lateral Angle Reduction, was selected. Lateral and posterioanterior cephalogram was taken before surgery (T0), 1day after surgery (T1) and 6month after surgery (T2). To know the esthetic results the facial width and lateral facial contour were examined on posterioanterior cephalogram and to know the postoperative stability B point and Incisor inferius was examined on lateral cephalogram. Statistical analysis was performed. Results: From T0 to T1, Intergonial width was significantly decreased, dominantly at shortened side but no significant changes at lengthened side. Those were well-maintained during 6 months. Lateral facial angle and Ramus angle was significantly decreased on only shortened side from T0 to T1. As a result, after surgery, there were no significant differences in all measurements between shortened side and lengthened side. Ramus deviation angle in shortened side and ramus angle in lengthened side which reflect the angulation of ramus on frontal plane didn't show significant changes after surgery and during postsurgical periods. Lower dental midline showed no statistical changes during postsurgical period. The relapse rate on B-point was 11.92%. Conclusion: Unilateral "Lateral angle reduction" in the asymmetric mandible is valuable to obtain the narrow lower face and symmetric facial contour with a good stability.

Retrospective study of changes in pharyngeal airway space and position of hyoid bone after mandibular setback surgery by cephalometric analysis

  • Background: The posterior movement of mandible was known as the main cause of the changes in the pharyngeal airway space (PAS) and the postoperative obstructive sleep apnea (OSA). The purpose of this study was to know the changes of PAS and position of hyoid bone. Methods: Lateral cephalographies of 13 patients who had undergone sagittal split ramus osteotomy (SSRO) setback surgery were taken preoperatively (T1), postoperatively within 2 months (T2), and follow-up after 6 months or more (T3). On the basis of F-H plane, diameters of nasopharynx, oropharynx, and hypopharynx were measured. The movements of the soft palate, tongue, and hyoid bone were also measured. Results: The amount of mandible setback was $7.5{\pm}3.8mm$. In the measurements of PAS, there was a statistically significant decrease of $2.8{\pm}2.5mm$ in nasopharynx (P < 0.01), and $1.7{\pm}2.4mm$ in oropharynx (P < 0.01) were observed after surgery. The hypopharynx decreased $1.0{\pm}2.1mm$ after surgery and continuously decreased $1.0{\pm}2.8mm$ at follow-up. The changes in hyoid bone position showed the posterior movement only after surgery and posteroinferior movement at follow-up. Conclusions: The PAS such as nasopharynx, oropharynx, and hypopharynx showed relatively high correlation with the amount of mandibular setback. The change of resistance in upper airway may be important for the prevention of OSA after mandibular setback surgery.

Evaluation of the course of the inferior alveolar canal in the mandibular ramus using cone beam computed tomography

  • Objectives: This study sought to provide guidelines in order to decrease the incidence of nerve injury during mandibular ramus bone harvesting, and to improve understanding of the anatomical structure of the inferior alveolar canal (IAC) to include its distance from the exterior buccal cortex. Materials and Methods: In January and February 2009, 20 patients who visited the Wonkwang University Department of Oral and Maxillofacial Surgery reporting various conditions underwent cone beam computed tomography and were included in this study. Patients with missing left or right mandibular first molars or incisors, or who had jaw fracture or bone pathologies, were excluded. The reference point (R point) was defined as the point where the occlusal plane reached the anterior ramus of the mandible. The position of the IAC in relation to the R point, the buccal bone width (BW), the alveolar crest distance (ACD), the distance from the alveolar crest to the occlusal plane (COD), and the distance from the IAC to the sagittal plane (CS) were determined using proprietary image analysis software which produced cross-sectional coronal and axial images. Results: The distance medially from the R point to the IAC along the axial plane was $6.19{\pm}1.21mm$. The HD from the R point, posteriorly to IAC, in the lateral view was $13.07{\pm}2.45mm$, the VD from the R point was $14.24{\pm}2.41mm$, and the ND from the R point was $10.12{\pm}1.76mm$. The pathway of the IAC was positioned almost in a straight line along a sagittal plane within $0.56{\pm}0.70mm$. The distance from the buccal bone surface to the IAC increased anteriorly from the R point. Conclusion: Marking osteotomy lines in the retromolar area in procedures involving bone harvesting should be discouraged due to the risk of damage to IAC structures. Our measurements indicated that the area from the R point in the ramus of the mandible to 10 mm anterior can be safely harvested for bone grafting purposes.

하악지 시상분할 절단술 후 감각 변화에 관한 연구 (CLINICAL STUDY OF SENSORY ALTERATIONS AFTER SAGITTAL SPLIT RAMUS OSTEOTOMY)

  • The bilateral sagittal split ramus osteotomy (BSSRO) is preferred method of surgical correction for mandibular prognathism, retrognathism and asymmetry. This technique performed from primarily an intraoral incision to avoid a scar. After forward movement of the distal segment of the mandible, healing of bone by primary or secondary intention is easily accomplished through large areas of cancellous bony overlap. When rigid fixation is used for the BSSRO, it is possible to open the mouth during the immediate post-operative period because it promotes the healing process. Although this surgical procedure has been well-documented, the incidence of postoperative trigeminal neurosensory disorder in the region of the inferior alveolar nerve and the mental nerve remains one of the major complication. However, evaluation of objective methods for sensory recovery patterns is insufficient although most patients find their sensory return. Neurometer electrodiagnostic device performs automated neuroselective sensory nerve conduction threshold evaluation by determining current perception threshold (CPT) measures. The purpose of this study was to evaluate the sensory recovery patterns of inferior alveolar and mental nerve over time. Nerve examination with a neurometer was performed in 30 patients undergoing the BSSRO at pre-operative, post-operative 1-, 2-, 4- week, and 2-, 3-, 4-, 5-, 6- month follow-up visits after the osteotomy to compare the differences of nerve injury and recovery patterns after the BSSRO with or without genioplasty and sensory recovery patterns associated with the kind of nerve fiber.

안면 비대칭환자의 악교정 수술을 동반한 완전구강회복 (Full mouth Rehabilitation with Orthognathic Surgery in Facial Asymmetry Patient : Case Report)

  • 안면 비대칭 중 가장 흔히 나타나는 하악 비대칭은 성장 중 골격성장의 비대칭을 유발하는 다양한 원인에 의해 발생한다. 안면 비대칭이 있을 경우 외모 뿐 아니라 저작과 기능에 지장을 줄 수 있다. 심한 골격성 악간관계 부조화가 있는 경우 보철치료만으로는 이상적인 교합관계를 형성하기 어려우며, 힘의 분산이 적절히 이루어지지 않아 자연치와 수복물의 수명이 단축될 수 있다. 악간관계 부조화로 인한 부정교합이 존재하여 치아의 수복 시 예후가 불량할 것으로 예상되는 경우 먼저 교정치료를 통한 악간관계의 개선을 위한 교합안정이 우선되어야 한다. 부조화가 심한 경우 기능적 및 심미적인 치료 결과를 위해 악교정 수술이 필요하며, 진단결과에 따라 편악 또는 양악수술이 시행된다. 상 하악 또는 안면 비대칭증의 악교정 수술시 이부 성형술이 동반될 수 있으며, 이는 하악골의 이부의 수직적, 횡적, 전후방적 위치를 외과적으로 변화시킴으로써 더욱 심미적인 안모개선을 얻기 위해 시행된다. 본 증례의 환자는 안모의 비대칭과 돌출된 하악 및 불량 보철물을 개선하고자 술전 교정을 시행한 뒤 상악 Le-Fort I 골절단술, 하악 시상분할골절단술, 이부 성형술(전방 4 mm), 우측 하악각 증대술을 시행하였다. 술후 교정치료를 진행하고 치관연장술 및 완전구강회복을 진행하였다. 보철 수복 완료 후, 환자의 평가와 객관적 결과로 만족할만한 결과를 얻었기에 이를 보고하고자 한다.

Surgical approach and orthodontic treatment of mandibular condylar osteochondroma

  • Osteochondroma is a common benign tumor of bones, but it is rare in the mandibular condyle. With its outgrowth it manifests clinically as deviation of the mandible limitation of mouth opening, and facial asymmetry. After the tumor is diagnosed on the basis of clinical symptoms and radiographic examination including cone-beam computed tomography (CBCT) analysis, an appropriate surgery and treatment plan should be formulated. Herein, we present the case of a 44-year-old female patient who visited our dental hospital because her chin point had been deviating to the left side slowly but progressively over the last 3 years and she had difficulty masticating. Based on CBCT, she was diagnosed with skeletal Class III malocclusion accompanied by osteochondroma of the right mandibular condyle. Maxillary occlusal cant with the right side down was observed, but it was confirmed to be an extrusion of the molars associated with dental compensation. Therefore, after intrusion of the right molars with the use of temporary anchorage devices, sagittal split ramus osteotomy was used to remove the tumor and perform orthognathic surgery simultaneously. During 6 months after the surgery, continuous bone resorption and remodeling were observed in the condyle of the affected side, which led to a change in occlusion. During the postoperative orthodontic treatment, intrusive force and buccal torque were applied to the molars on the affected side, and a proper buccal overjet was created. After 18 months, CBCT revealed that the rate of bone absorption was continuously reduced, bone corticalization appeared, and good occlusion and a satisfying facial profile were achieved.

하악전돌증환자의 악교정수술후 하악각변화에 관한 임상적 분석 (CLINICAL ANALYSIS OF GONIAL ANGLE CHANGE AFTER ORTHOGNATHIC SUGERY IN PATIENTS WITH THE MANDIBULAR PROGNATHISM)

  • 저자는 1996년 1월에서 1998년 10월까지 본원에서 하악지시상분할골절단술을 시행한 하악전돌증환자중 35명(남자15명, 여자 20명)을 무작위로 선정하여 수술전, 수술직후 및 술후 1년의 하악각변화를 분석한 결과 다음과 같은 결론을 얻었다. 1. 35명 환자의 하악각평균은 술직후에는 술전에 비해 $9.3^{\circ}$ 감소하였다가 술후 1년까지 $4.0^{\circ}$증가하여 술전에 비해 술후 1년의 하악각은 $5.3^{\circ}$감소하였다(p<0.01). 2. 35명 환자의 악교정수술전 하악각평균은 $129.4^{\circ}({\pm}6.27)$로 정상에 비해 유의있게 큰 값을 보였으며 하악지시상분할골절단술 후 1년의 하악각평균은 $124.1^{\circ}({\pm}6.33)$로 비교적 개선되었다. 3. 술전과 술후 1년 사이의 남녀의 하악각변화는 여자는 $5.4^{\circ}$감소, 남자는 $5.3^{\circ}$감소로서 남녀간에 유의한 차이는 없었다(p>0.05). 4. 35명의 하악지시상분할골절단술을 받은 환자의 술전과 술후 1년 사이의 하악각변화에서 하악각감소에 관여하는 가장 주요한 변수는 하악후퇴량이었다. 5. 35명의 하악지시상분할골절단술을 받은 환자의 술직후부터 술후 1년까지의 하악각변화에서 하악각증가에 관여하는 가장 주요한 요인은 수평회귀율이었으며 원심골편후방부의 흡수현상도 하악각증가에 관여하는 것으로 생각되었다. 결론적으로 하악전돌증 환자에 있어 하악각이 정상보다 큰 경우 하악지시상분할골절단술은 하악각개선에 효과적이었으며 하악각이 정상에 가깝고 과도한 하악후퇴량이 요구되는 경우는 Short lingual cut 방법, 부가적인 원심골편의 후방변연절제술, Obwegeser II 방법과 같은 하악우각부 형태를 유지하는 수술법을 선택함이 유리할 것으로 예상되고 술후 회귀방지책이 안정된 하악각유지에도 도움이 될 것으로 사료된다.

전산화단층촬영상에서 하악후퇴수술 후 인두기도 공간의 변화 (Changes of Pharyngeal Airway Space after Mandibular Setback Surgery in Computed Tomography Images)

  • Purpose: This study evaluated the pharyngeal airway space changes in CT images in patients receiving bilateral sagittal split osteotomy (BSSRO) for the surgical correction of mandibular prognathism. Methods: A total of 22 patients with mandibular prognathism were treated using BSSRO. Computed tomography was performed 1 month (T0) before surgery and, 1 month after surgery (T1). The anteroposterior length (AP), lateral width (LAT) and cross-sectional area (AREA) at the level of soft palate (C2) and base of the tongue (C3) were measured using CT images. Results: The mean amount of mandibular setback was 7.41 mm (${\pm}$3.46 mm). All the AP, LAT and AREA at the C2 and C3 level were decreased significantly 1 month after surgery (P<0.001). As the amount of mandibular setback was increased, the AP, LAT and AREA levels at the level of C2 and C3 had decreased. In addition, the reduction of the AREA at the C3 level was associated with the amount of mandibular setback (P<0.05). Conclusion: A significant decrease in pharyngeal airway space was observed 1 month after the operation. The cross-sectional area at the level of base of tongue was decreased with increasing amount of mandibular setback.

하악전돌증의 악교정수술후 연조직 변화에 관한 연구 (CHANGE OF LATERAL SOFT TISSUE PROFILE AFTER SURGICAL CORRECTION OF MANDIBULAR PROGNATHISM)

  • The purpose of this paper is to investigate changes in soft tissue in 22 patients treated by vertical ramus osteotomy and sagittal split ramus osteotomy for the correction of mandibular prognathism. 22 individuals, 12 males and 10 females, were selected from the patients with mandibular prognathism at the Department of Oral and Maxillofacial Surgery, Colledge of Dentistry, Kyung Hee University. Patient were analyzed with cephalogram taken 1 week before and at least 6 weeks after surgery under the same condition. Measurements were made constructed hard tissue and sop tissue points located on each before-and-after film tracing. Comparision were made of these figures to estimate the amount that the soft tissue followed the hard tissue structures in each surgical procedure : ratio of sop and hard tissue changes were formulated. The results were as follows. 1. The horizontal changes of Pogs and Bs as a ratio of the horizontal changes of Pog and B point were 1.02 and 1.16 respectively. 2. One millimeter of posterior changes at Pog resulted in 0.86mm of posterior change at Li and 0.09mm of posterior change at Ls. The greatest amount of sop tissue change occurred at Pogs, with substantially less posterior displacement at Bs, even less at Li and at least at Ls. 3. The ratio of LI to Li was 1:0.81 and the ratio of LI to Ls was not significant.(1 : 0.17) 4. The ULA(Cm-Sn-Ls) and the relative lower lip projection (LLP) was incnease4 but the relative upper lip projection (ULP) was slightly decreased 5. The angular change of the upper lip inclined angle (Ls-Sn/ANS-PNS) and lower lip inclined angle(Li-Pogs/Me-Go) expressed as a ratio of the posterior change of Pog were 0.57 and 0.20 respectively. 6. The ratio of the lower anterior facial height change of the soft tissue(Sn-Mes) to the hard tissue(ANS-Gn) were 0.78 and and the ratio of vertical height changes of the hard tissue and sop tissue to the posterior change of the Pog were 0.18 and 0.19 respectively. 7. The sop tissue angular change of facial convexity(G-Sn-Pogs) expressed as a ratio of the angular change of the hard tissue angle of facial convexity(N-A-Pog) was 1.24.
