하악골 과두에 생긴 골연골종의 치험례 (OSTEOCHONRDOMA OF THE MANDIBULAR CONDYLE : A CASE REPORT)

  • The osteochondroma, also known as osteocartilaginous exostosis, is one of the most common benign tumors of the axial skeleton. These tumours rarely affect the facial skeleton and a true osteochondroma of the mandibular condyle is a very uncommon entity. The tumors are usually slow growing, and symptoms develop over a long time. The most common presentation of the condylar osteochondroma consists of a changing occlusion, the development of facial asymmetry, and a posterior open bite on the affected side. In this case, we treated an osteochondroma of Lt. mandibular condyle through excision of the lesion via the temporal approach to the temporomandibular joint.

최근 10년간 소아 청소년에서 발생한 하악 과두 골절에 대한 후향적 연구 (Retrospective Study of the Mandibular Condyle Fracture in Children and Young Adolescents)

  • 본 연구의 목적은 소아 청소년에서 발생한 하악 과두 골절의 빈도, 원인, 골절의 임상적 양상 및 치료 방법에 대한 후향적 분석을 통해 연령에 따른 하악 과두 골절의 임상적 특징 및 치료방법을 조사하는 것이었다. 최근 10년간 아주대학교 치과병원에서 하악 과두 골절로 진단받고 치료받은 만 15세 이하의 환자 44명을 대상으로 후향적 분석을 시행하였다. 하악골 골절 중 하악 과두 골절의 빈도는 환아의 연령이 어릴수록 높았다. 하악 과두 골절의 원인으로 넘어짐의 비율이 가장 높았다. 만 4 - 7세 군에선 과두부(condyle head) 골절이 91.7%를 차지하였으며, 연령이 증가할수록 과두경부(condyle neck) 골절 비율이 증가하였다. 하악 과두 골절 환아의 54.5%에서 하악 정중부 골절이 동반되었으며, 정중부 골절이 동반된 경우 구치부 치아 손상 빈도는 하악 과두 골절 단독인 경우에 비해 높았다. 44명 환아 중 43명에서 보존적 치료가 시행되었으며, 양측 과두의 심각한 골편 변위를 가진 1명 환아에서 관혈적 수술이 시행되었다. 본 연구 결과는 소아 청소년에서 발생한 하악 과두 골절의 진단 및 치료 계획 수립에 의미있는 자료로 적용될 수 있을 것이다.

  • The author has evaluated the panoramic image of the mandibular condyle according to its horizontal condylar angle (0˚, 10˚, 20°, 30°, 40°) and mandibular position (standard, 25㎜ forward and reverse position). The results were as follows: 1. The larger the horizontal condylar angle was, the larger the horizontal magnification was in all positions. 2. In case of small horizontal condylar angle, profile view could be obtained in 25㎜ forward and reverse position. 3. In case of large horizontal condylar angle, profile view could not be obtained in any positions.

Threaded Kirschner Wire와 외부 고무줄 견인을 통한 소아 하악골 관절돌기 골절의 치료 (The Treatment for Mandibular Condyle Fracture of Children by a Threaded Kirshcner Wire and External Rubber Traction)

  • Purpose: The treatment of children mandibular condyle fracture that is severely displaced is controversial. The conservative treatment of it may lead to complications - mandibular deficiency, asymmetry, malocclusion and temporomandibular joint dysfunction. Moreover, open reduction carries risks for growth retardation, facial nerve injury, scarring and joint stiffness. The aim of this article is to present an alternative technique of the treatment by using a threaded Kirschner wire and external rubber traction. Materials: From November 2005 to May 2008, three patients underwent the management by using a threaded Kirschner wire and external rubber traction. A threaded Kirschner wire was inserted in the condylar segment by using a C-arm. We applied the external rubber traction, and we reducted the segment progressively until complete reduction. The mandibular - maxillary fixations were removed after 3 weeks, and patients went into training for mouth opening. Results: The technique didn't result in complications - joint dysfunction, facial nerve injury, sore, infection and nonunion during follow - up period. Radiologic follow - up examinations revealed correct reduction in all patients. In all cases, we found restoration of preinjury occlusion and temporomandibular joint function. Conclusions: Closed reduction of children mandibular condyle fracture by using a threaded Kirschner wire and external rubber traction did achieve anatomic reduction and restore mandibular height. This alternative technique is simple, effective, inexpensive, easy to apply and minimally invasive.

발육중인 생쥐 하악 과두에서 연골 및 골의 특이 유전자 발현 (Expression of mRNAs characteristic of cartilage and bone in the developing mandibular condyle of mice)

  • 하악과두 연골이 발생하고 분화하는 과정에서 나타내는 특성을 규명하기 위하여, 발생 16, 18일과 출생 1일, 5일, 10일, 20일 및 30일 후의 ICR생쥐의 하악과두를 형태학적으로 분석하고, 생쥐 I형, II형, X형 교원질, Indian hedgehog (IHH) 및 BMP-4 등의 mRNA 발현을 in-situ hybridization 방법으로 연구하였다. 1. 생쥐 I형 및 II형 교원질 mRNA는 하악과두의 발생 및 성장과정에서 모두 발현되었다. I형 교원질 mRNA는 휴지층과 증식층의 상부에서 관찰된 반면 II형 교원질은 휴지층과 증식층 그리고 비대연골층의 상부에서 관찰되었다. 2. 하악과두 연골은 성장에 따라 비대연골층이 계속 증가하는 소견을 보였으며, 비대 연골층의 세포들은 특징적으로 X형 교원질 mRNA의 발현을 보였다. 3. BMP-4 mRNA는 하악과두 연골 원기와 골화중인 하악골체에서 모두 발현되었다. 4. IHH mRNA는 하악과두의 발생과정에서 증식 연골층의 하부와 비대연골층의 상부에서 선택적으로 관찰되었다.

  • Purpose: After the surgical correction with sagittal split ramus osteotomy, the position of the mandibular condyle in the glenoid fossa and the proximal segment of the mandible change because of bony gap between proximal and distal segment, especially in case of mandibular setback asymmetrically. In this study, positional changes in the condyle and proximal segment after BSSRO were estimated in the mandibular asymmetry patient by analyzing the in submentovertex view and P-A cephalogram for identification of ideal condylar position during surgery. Patients and Methods: The 20 patients were selected randomly who visit Dankook Dental Hospital for mandibular asymmetry. Bilateral sagittal split ramus osteotomy with rigid fixation was performed and P-A cephalogram and submentovertex view was taken at the time of preoperative, immediate postoperative, 3 month postoperative period. Results: Intercondylar length and transverse condylar angle was increased due to inward rotation of proximal segment and anteromedial rotation of lateral pole of condyle head. The condylar position had a tendency to return to the preoperative state and after 3 months return up to about half of the immediate post-operative changes, and all the results showed more changes in asymmetry patient and deviated part of the mandible. Conclusion: Based on all these results above, surgeon should make efforts to have a precise preoperative analysis and to have a ideal condylar position during rigid fixation after BSSRO.

구강 접근과 트로카를 이용한 하악 과두하부 골절의 정복과 고정 (OPEN REDUCTION AND TRANSORAL FIXATION USING TROCHAR FOR MANDIBULAR SUBCONDYLE FRACTURE)

  • The mandibular condyle fracture occurs at 15-30% frequency of whole mandibular fracture. The treatment of choice is open reduction or closed reduction. In many cases, closed reduction is preferred for treatment of condylar fracture because it is hard to approach to condyle and there is risk of surgical complications, such as nerve damage in open reduction. Open reduction, however, has some advantages like possibility of anatomical reduction, occlusal stability and rapid functional recovery. Furthermore, it is possible to retain original ramal heights and to decrease deviation during mouth opening. There are many surgical approaches for open reduction of subcondyle fracture. At present, transoral approach using trochar device is tried for effective and minimally invasive method for open reduction of subcondyle fracture. And the authors report the cases of reduction of subcondyle fracture with transoral approach using trochar device.

Reduction of superior-lateral intact mandibular condyle dislocation with bone traction hook

  • Lateral and superior-lateral dislocations of the intact condyle are a rare complication, following traumatic insult to the mandible. We report an unusual case of a 54-year-old male patient who experienced both types of dislocations of the intact condyles with symphysis fracture following a road-traffic accident. Under general anesthesia, conventional manipulation was unsuccessful in relocating the condyles into the glenoid fossa. After applying a percutaneous traction force, using a bone traction hook placed at the sigmoid notch, the displaced intact mandibular condyles were repositioned, and the symphyseal fracture was finally reduced and fixed. The mouth opening was within normal limits, and favorable occlusion was confirmed one month postoperatively. To our knowledge, this is the first case of dislocation of both intact condyles--associated with symphysis fracture--being reduced with bone traction hook.

A large osteoid osteoma of the mandibular condyle causing conductive hearing loss: a case report and review of literature

  • Osteoid osteomas are benign skeletal neoplasms that are commonly encountered in the bones of the lower extremities, but are exceedingly rare in jaw bones with a prevalence of less than 1%. This unique clinical entity is usually seen in younger individuals, with nocturnal pain and swelling as its characteristic clinical manifestations. The size of the lesion is rarely found to be more than 2 cm. We hereby report a rare case of osteoid osteoma originating from the neck of the mandibular condyle that grew to large enough proportions to result in conductive hearing loss in addition to pain, swelling and restricted mouth opening. In addition, an effort has been made to review all the documented cases of osteoid osteomas of the jaws that have been published in the literature thus far.

  • Proliferative periostitis is a rare form of osteomyelitis that is characterized by new bone formation with periosteal reaction common causes of proliferative periostitis are dental caries, periodontitis, cysts, and trauma. While proliferative periostitis typically presents as a localized lesion, in this study, we describe an extensive form of proliferative periostitis involving the whole mandibular ramus and condyle. Because the radiographic findings were similar to osteogenic sarcoma, an accurate differential diagnosis was important for proper treatment.
