Onlay technique에 의한 매복중절치의 교정적 치험예 (A CASE OF PALATALLY IMPACTED INCISOR TREATED BY ONLAY TECHNIQUE WITH SURGICAL EXPOSURE)

  • The patient, a girl of 19 years in good health, had a class I malocclusion. The maxillary left centra1 incisors and both lateral incisors had already erupted. But the space for the right central incisor was partially closed by the mesial drifting of the neighboring teeth. The caused a shift in the midline and a cross-bite relation on the incisors. X-ray examination revealed the presence of the right central incisor in the alveolar bone and odontoma just above the crown of the right central incisor. After enough space for the impacted incisor was created in the dental arch with a open-coil spring the rectangular incision was made. Removing the odontoma uncovered the flat surface of the labial aspect of the incisor. During the tooth had erupted of its own accord, any unnecessary force had been imposed on the tooth. When it was decided that the tooth should be brought out by the mechanical device, the gold cast onlay with hook was used and run a light elastic between this hook and the main arch wire. Finally the tooth was brought down to the arch level. The result was excellent. Fortunately the esthetic problem and any detrimental effects on the psychological make-up could be avoided.

Age, extraction rate and jaw surgery rate in Korean orthodontic clinics and small dental hospitals

  • Objective: This study aimed to investigate the current data regarding age, sex, and Angle Classification of Korean orthodontic patients and influence of these factors on the tendency to undergo extraction and orthognathic surgery. Methods: The recent trends of Korean orthodontic patients were assessed using questionnaire survey. The questionnaires were e-mailed to orthodontists who met the study criteria; 58% of the orthodontists opened the e-mails, and 27.7% replied to the e-mails. In all, the medical records of 11,340 patients who underwent orthodontic treatment at private clinics and small dental hospitals in Korea were analyzed. Results: The percentage of female patients in the study sample was 69.6%, and the average age of the patients was 19.87 years. The percentage of patients who were older than 19 years was 50.2%. Class II and Class III malocclusions were noted in 33.6% and 23.6% of patients, respectively. Extraction and orthognathic surgery were performed in 60.4% and 6.9% of patients, respectively. Conclusions: The results showed that there were a high percentage of adult, Class II malocclusion and extraction patients in private practices and small dental hospitals during the study period. Further, a relatively high proportion of adult patients opted to undergo orthognathic surgery.

Management of acquired open bite associated with temporomandibular joint osteoarthritis using miniscrew anchorage

  • This article reports the orthodontic treatment of a patient with skeletal mandibular retrusion and an anterior open bite due to temporomandibular joint osteoarthritis (TMJ-OA) using miniscrew anchorage. A 46-year-old woman had a Class II malocclusion with a retropositioned mandible. Her overjet and overbite were 7.0 mm and -1.6 mm, respectively. She had limited mouth opening, TMJ sounds, and pain. Condylar resorption was observed in both TMJs. Her TMJ pain was reduced by splint therapy, and then orthodontic treatment was initiated. Titanium miniscrews were placed at the posterior maxilla to intrude the molars. After 2 years and 7 months of orthodontic treatment, an acceptable occlusion was achieved without any recurrence of TMJ symptoms. The retropositioned mandible was considerably improved, and the lips showed less tension upon lip closure. The maxillary molars were intruded by 1.5 mm, and the mandible was subsequently rotated counterclockwise. Magnetic resonance imaging of both condyles after treatment showed avascular necrosis-like structures. During a 2-year retention period, an acceptable occlusion was maintained without recurrence of the open bite. In conclusion, correction of open bite and clockwise-rotated mandible through molar intrusion using titanium miniscrews is effective for the management of TMJ-OA with jaw deformity.

고정식 교정 장치물 장착후 타액내 미생물과 면역 글로블린 A의 변화 (A Longitudinal Study on Salivary Microorgnisms and Immunoglobulin A after Delivery of Fixed Orthodontic Appliances)

  • Fixed orthodontic appliances for the treatment of malocclusion has iatrogenic side effect such as demineralization of enamel, gingivitix and gingival hyperplasia. The purpose of this study is to longitudinally investigate the salivary microorganisms and immunoglobulin A after delivery of fixed orthodontic appliances for 10 months. Eight orthodontic patients were included in this study and the author has investigated the numbers of general bacteria, Streptococcus mutans Staphylococcus aureus and concentration of immunoglobulin A from unstimulated whole saliva. The author examined these parameters at prebracketing, 1 month after, 4 months after, 7 months after and 10 months after delivery of fixed orthodontic appliances. The obtained results were as follows : There were significant increases in the number of salivary general bacteria, Streptococcus mutans and Staphylococcus aureus after delivery of fixed orthodontic appliances The numbers of general bacteria were significantly increased at 1 month after (p<005), 4 months after (p<0.05), 7 months after (p<0.01), compared with prebracketing. However it showed no difference at 10 month after compared with 7 months after bracketing. The Numbers of Staphylococcus aureus were significantly increased at 1 month after (p<0.05), 4 months after(p<0.01), 7 month(p<0.01), compared with prebracketing. However it showed decreasing pattern at 10 months after compared with 7 months after bracketing. There was no significant difference in the concentration of immunoglobulin A after delivery of fixed orthodontic appliances.

Application of New Appliances for Management of Growing Class III Malocclusion Child: Comparazation Case Reports

  • The purpose of this comparison of case reports is to introduce the results of the application of new devices for the management of growing Class III malocclusions in children. Two 8-year-old boys had a chief complaint of anterior crossbite. Anterior crossbite correction using a tandem traction bow appliance (TTBA) or a Carriere Motion 3D Class III appliance with a Transforce appliance was planned. By comparing cephalometric radiographs before and after treatment, changes in skeletal growth and incisor inclination to the Frankfort horizontal (FH) plane could be measured. Both devices increased SNA and ANB angles, N-I Pg-A, U1 to SN, and U1 to FH. Both appliances improved facial features and resolution of anterior crossbite. The TTBA and Carriere Motion 3D Class III appliance had similar effects when applied as early treatment for growing mesio-occlusions and anterior crossbite in two boys. However, long-term outcome assessments and well-designed comparative studies are still required.

하악골 과두하부골절 정복술에서 관혈적 정복술과 비관혈적 정복술의 비교 연구 (Comparison Study of Open Reduction and Closed Reduction in Treatment of Mandibular Subcondylar Fractures)

  • Purpose: The choice of open versus closed reduction for mandibular subcondylar fracture is a debatable issue. To evaluate the advantage of open approach to closed method with IMF(intermaxillary fixation), we conducted a retrospective study to compare the outcomes of each method. Methods: From 2002 to 2006, 29 patients with mandibular subcondylar fractures were treated by open or closed reduction. 17 patients were treated by open reduction and 12 patients by closed reduction and IMF. Each group was assessed for duration of mandibular immobilization, incidences of buccal palsy, malocclusion, TMJ(temporomandibular joint) pain, and deviation of the mandible on mouth opening. Results: All cases showed accurate reduction in anatomical position, no significant displacement and no deviation on mouth opening during the follow-up period. IMF period is statistically shorter in open reduction (p<0.05). Differences in incidence of other complications were not significant statistically. Conclusion: As there are significant independent morbidities associated with IMF which requires postoperative rehabilitation, prolonged temporomandibular immobilization should not be overlooked. Some patients with poor compliances will not tolerate IMF in nonsurgical treatment. In the aspect of patient's convenience and early recovery by short IMF period, open reduction would be recommended as a better treatment method.

하악 시상골 절단술 후 고정 방법에 따른 회귀 성향에 대한 비교 연구 (Comparative Study of Skeletal Relapse According to the Fixation Method after BSSRO for Mandibular Setback.)

  • Objective : To compare two different methods of rigid fixation in postoperative stability after mandibular setback. Material and Methods : 28 patients with Class III malocclusion were treated by bilateral sagittal split ramus osteotomy(BSSRO) and mandibular setback were selected for this study. Group A(n=14) had the bone segments fixed with monocortical miniplate on the lateral side of the mandibular body and Group B(n=14) had three noncompressive bicortical screw inserted at the genial area through a transcutaneous approach. Cephalograms were taken preoperatively, postoperatively within 1 weeks and at a follow-up period (mean 8.9 months after surgery) and the amount of setback and postoperative change were measured. Results : Postoperative relapse between two groups was minimal in setback of the mandible. Statistical analysis showed no significant difference in postoperative relapse. Conclusion : This study suggests that both methods of skeletal fixation investigated give comparable postoperative stability and their use in mandibular setback appears to be a fairly stable clinical procedure .

하악상행지 시상분할골절단술 시 하악후퇴량의 방사선학적 예측 (Prediction of Amount of Mandibular Set Back with 3 Plain Radiographs in Mandibular Sagittal Split Ramus Osteotomy)

  • Purpose: The present study examined the reproducibility of an operation plan by comparing the jaw position of STO with the postoperative mandibular set back measurement in sagittal split ramus osteotomy. Methods: Thirty patients with class III dental and skeletal malocclusion and who were treated with BSSRO were reviewed. Three plain radiographs such as the panoramic view, the lateral cephalogram and the submentovertex view were taken before and after operation. Also, paper surgery for STO and model surgery were used to evaluate the amount of mandibular set back. Results: On the panoramic view, the amount of mandibular set back in STO was similar to the postoperative results of model surgery, but the amount of mandibular set back on the lateral cephalogram was smaller than the postoperative result of model surgery and then the amount of set back on submentovertex view was similar to the postoperative result of model surgery. Conclusion: Precise tracing and paper surgery should be performed for a combined expected STO in order to predict the exact amount of preoperative mandibular set back.

  • This study was undertaken to evaluate the cephalometric changes of the soft tissue and skeletal profile subsequent 10 the rapid palatal expansion in 25 Angle's Class III cases, ranging in age from six to fifteen years, with cross-bite of the anterior teeth, underdevelopment of maxilla and facial disharmony Following results were obtained: 1. ANS moved downward, Point A presented forward & downward movement increasing SNA and Point B presented backward & downward movement decreasing SNB. 2. Mandible was rotated to backward & forward and maxilla moved forward & downward with the bite opening and improvement of anterior teeth cross-bite. 3. Soft tissue on mandible was rotated to backward & forward following hard tissue changes causing the decrease of facial convexity angle and backward & downward rotation of Point B', Pog'. 4. Response of the upper lip was more significant in downward than forward direction, and correlated with the upper central incisor and mandible rotation. 5. Response of the lower lip was more significant in downward than backward direction, and correlated with the mandible rotation. 6. There was a rather high degree of correlation between skeletal profile and soft-tissue profile, 1 : LS, $\bar{1}$:Pog', Pog:LS, Pog:LI, Pog:Pog' in horizontal measurements and $\bar{1}$:Pog', Pog:LI, Pog:Pog' in vertical measurements.

  • The ankylosis of temporomandibular joint occured by several causes directly developes TM joint disfunction, In cases with prolonged condylar ankylosis, especially at growing age the condylar ankylosis evokes malfunction of growth center area, and then developes the abnormal facial morphology and malocclusion. Therefore it must be cured. Almost authors have agreed to the necessity of surgical correction of the TMJ anylosis. but they did not decide the one surgical method to get the best result. The Tx. method suggested by many authors are the using interposition after resection of condyle to remain a lever of 3rd class in Mn. kinetics, the autogenous condylar graft and the alloplastic condylar graft. Some authors have got the satisfied results only with the condylectomy of the involved TMJ. This study also operated only the detachment of fibrous adhesion on ankylosed condylar side and then established occlusion in the case with the unilateral TMJ ankylosis and fibrous joint adhesion and facial asymmetry evoked after the fracture of condylar head at early age. This study got a improved mouth opening and a stable postsurgical result after 1 year. Also, this study reviewed many author's study about the chanracteristics, etiology, diagnosis and Tx. method for the ankylosis of TMJ.
