Problems and Ways of Improvement towards the Maldevelopment of Reserved Areas of Natural Environments, Ulleung Island (울릉도 자연환경자원보호구역에서의 난개발 문제점과 개선방안)

  • This study is to discuss the problems due to the maldevelopment occurred at the reserved areas of natural environments of the Ulleung Island, Gyeongsangbuk-do province, and it also suggests the ways of solving them The types of the maldevelopment practiced on the Ulleung Island consist of the change of land use, the opening of a round road, the destruction of mountains for the construction of harbor, the development of tourist areas, and the construction of military facilities. The decreased habitat of animals and the pollution of drinking water are occurred by the change of land use. The destruction of ecological system is due to the opening of a round road. The damage of natural landscape and the change of ocean eco-system is closely related to the construction of harbor and the destruction of mountains. Finally, the damage of rare plants and the pollution of drinking water are due to the development of tourist areas and the construction of military facilities. In the followings, the ways of solving the problems occurred by the various types of maldevelopment are suggested. First, the planning of pro-environmental development has to be established and practiced to preserve rare plants which are damaged due to the change of land use. Second, the destruction of natural landscape and resources has to be minimized by the implementation of environmental impact assesment when road and harbor are constructed. Third, the stones needed for the development of islands ought to be imported from mainland. Fourth, the established tourist areas are needed to rationally manage and the potential tourist areas has to be transferred to the reserved areas of natural environments. Fifth, military facilities should be constructed though the cooperation of autonomous local government. Finally, government officers' and residents' conscience of protecting and reserving natural environments is needed, and government has to give residents financial supports.

  • Artificial classification of anorectal malformation (ARM) is difficult because it is a spectrum of embryologic maldevelopment. However, the establishment of an ideal classification of ARM should be established in order to choose the adequate treatment and evaluate the prognosis. The classification system of ARM has followed the development of medicine, and the pediatric surgeons, who manage the ARM, should understand the change of this classification system.

  • Congenital diverticulosis of the left ventricle is an extremely rare maldevelopment. We report a 9 year old girl with probable isolated left ventricular diverticulum in whom the diagnosis was made by cross sectional echocardiography and by angiography.

  • Bring this analysis down to people-centered development perspective and looking through democratization in the Philippines, Thailand and Indonesia, we find similarities and differences among them related with the intensity of conflicts between development and human rights in the process of democratization in line with global transformation. Civil society in the Philippines criticized the developmental path in the Philippines which failed to implement land reform and eradication of poverty under the transition from 'patrimonial oligarchy' to democracy. In Thailand the coalition of military and the royalists had consolidated its power since Sarit military regime, which later paved the way 'hybrid oligarchy' era. Most Thai civil society organizations has regarded their developmental experience rather as 'maldevelopment' which disregarded economic and social rights. It has been especially believed by Thai localists that the stimulation of local markets and the building of autonomic community society will form the alternative economy without going against the conservative banner of nation, religion and king. Thaksin as a populist successfully took advantage of Thai localist ethos in favour of taking the seat of power. He projected himself as a modernizer focused on economic growth and cleaner politics. However Thaksin's procedural legitimacy was overthrown by counterattacking from military-royalist alliance, pretexting that Thaksin caused internal conflicts and lacked morality. Soeharto's New Order regime which can be called 'administrative oligarchy' had an antipathy towards notions of economic and social rights as well as civil and political rights. In spite of the fact that the fall of Soeharto opened the political space for democratic civil society organizations which had long struggled with development aggression and human rights abuses, there have been continuously a strong political and military reaction against human rights activists, NGOs and ethnic minorities such as Aceh and Papua. Nevertheless, Indonesian democracy is more promising than Philippine's and Thai democracy in terms of comparatively less pre-modern legacies.

Oral findings and dental management of a patient with Moebius syndrome: a case report

  • Moebius syndrome is a rare, congenital neurological disease involving facial paralysis and limitation of eye movements. It results from maldevelopment of the sixth and seventh cranial nerves. Dental features of this syndrome include micrognathia, microstomia, tongue deformity, cleft palate, hypoplasia of the teeth, and congenital missing teeth. A 7-year-old female with Moebius syndrome was referred from a local dental clinic for caries treatment. She presented with facial paralysis and microstomia. Oral findings included multiple caries with enamel hypoplasia, congenital missing teeth, and tongue deformity. Dental treatments including restorative and preventive procedures were performed. Oral findings and management aspects of Moebius syndrome for this case are discussed. Early evaluation and multidisciplinary care are needed for children with Moebius syndrome.

  • Urinary tract infection (UTI) is a common bacterial illness in children. Acute pyelonephritis in children may lead to renal scarring with the risk of later hypertension, preeclampsia during pregnancy, proteinuria, and renal insufficiency. Until now, vesicoureteral reflux (VUR) has been considered the most important risk factor for post-UTI renal scar formation in children. VUR predisposes children with UTI to pyelonephritis, and both are associated with renal scarring. However, reflux nephropathy is not always acquired; rather, it reflects reflux-associated congenital dysplastic kidneys. The viewpoint that chronic kidney disease results from renal maldevelopment-associated VUR has led to questioning the utility of any regimen directed at identifying or treating VUR. Despite the recognition that underlying renal anomalies may be the cause of renal scarring that was previously attributed to infection, the prevention of renal scarring remains the goal of all therapies for childhood UTI. Therefore, children at high risk of renal scar formation after UTI should be treated and investigated until a large clinical study and basic research give us more information.

  • A case of foregut cyst communicated with esophagus and lined by bronchial mucosa is reviewed and its embryologic base of maldevelopment is discussed. It is not always easy to distinguish between digestive and respiratory cyst in mediastinum. There is whole range of intermediate between a cyst with ciliated and one with squamous or columnar mucosa. Origin of this dysembryoplasia is difficult to determine when one consider that the esophagus is covered with ciliated epithelium until the eleventh week of fetal life and that ciliated growth are found on its wall until the sixth month of the fetal life. And we concluded, general agreement is that cysts which have gastric epithelium in whole or in part, represent a distinct type and should be classified as (gastro) enteric cyst, mediastinal cyst containing cartilage were considered definitely as respiratory(bronchial or bronchogenic) cyst.

  • Nitrofen [2,4-dichlorophenyl-P-nitrophenyl ether] is a diphenyl ether herbicide used for pre and post-emergent control of broad leafed weeds. This chemical was known to induce a variety of congenital cardiovascular anomalies with diaphragmatic hernia and hydronephrosis in the rate fetuses. The present study was conducted to produce congenital cardiovascular anomalies in the rat fetuses by oral nitrofen administration at the indicated doses and days of gestation, and to find the characteristics of nitrofen-induced cardiovascular anomalies. All the observed fetuses were removed from the pregnant Sprague-Dawley rats sacrificed on the twenty-first day of gestation. They were preserved in 10 per cent formalin and dissection for examination were carried out under a dissecting microscope using forceps and scissors. Following results and conclusion were based on dissecting microscopic findings on 482 offsprings. 1. The eleventh day of gestation was the most sensitive day for nitrofen induction of congenital cardiovascular anomalies in the rat. This incidence was dose-related in rats exposed on the eleventh day of gestation. 2. Ventricular septal defect was the most common single anomaly that represented more than half of the total cardiovascular anomalies, followed by aortic arch anomalies and tetralogy of Fallot. 3. Cardiac anomalies derived from infundibular maldevelopment such as tetralogy of Fallot and pulmonary atresia with ventricular septal defect were only observed in the eleventh gestation day treated group. 4. Aortic arch anomalies were found in high frequency and the great majority were characteristically anomalous right subclavian artery with left aortic arch. DENTAL TREATMENT OF PATIENTS WITH DOWN SYNDROME UNDER GENERAL ANESTHESIA (다운증후군 환자의 전신마취 하 치과치료)

  • Background: Down's syndrome, or trisomy 21, is the commonest congenital chromosome anomaly. With improvement in medical care, these patients increasingly reach adulthood in spite of their physical maldevelopment and mental retardation. And, the number of those who required general anesthesia for dental treatment is increasing. Methods: We reviewed the 26 cases of 22 patients with Down's syndrome who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 22 years. They all had severe mental retardation and some had congenital heart anomaly, epilepsy, hypothyroidism, acute leukemia, autism, cleft palate, and chronic renal failure. For anesthesia induction, 4 cases was needed physical restriction, but others showed good or moderate cooperation. Drugs used for anesthesia induction was thiopental (17 cases) and sevoflurane (9 cases). All patients received nasotracheal intubation and 3 cases needed difficult airway management. Mean total anesthetic time was $166{\pm}60$ min and staying time at PACU was $92{\pm}48$ min. There was no death or long term hospitalization because of severe complications. Conclusion: If general anesthesia is needed, pertinent diagnostic tests and workup about anomaly, and appropriate anesthetic planning are essential for safety.

  • Bronchogenic cyst is rare lesion that arises from maldevelopment of the primitive foregut, and is usually found in the lung and mediastinum. Cutaneous or subcutaneous bronchogenic cyst is rare and occur unusually in the shoulder region. We report here a case of 20-month-old boy with a bronchogenic cyst on his left shoulder region. He underwent incisional biopsy and curettage for a tender cystic mass at a department of dermatology, diagnosed as a epidermal inculsion cyst. But, additional cystic mass was palpated during wound care. MRI showed a well-defined subcutaneous cystic mass. The excised cyst was lined with pseudostratified ciliated columnar epithelium with occasional goblet cells and diagnosed as a bronchogenic cyst.
