다발성 원발성 폐암 수술 치험 -2예 보고- (Surgical Treatment for Multiple Primary Lung Cancer -Report of 2 cases-)

  • 다발성 원발성 폐암은 동시성 원발성 페암과 이시성 원발성 폐암으로 나눌 수 있는데 두 가지 경우 모두 드물게 발견되는 폐암의 일종이다. 본원에서는 최근 동시성 원발성 폐암과 이시성 원발성 폐암을 각각 1예씩 수술 치험하였기에 보고하는 바이다.

흉부둔상에 의한 외상성 가성 폐기낭 (Traumatic Pseudocyst of Lung by Blunt Trauma)

  • Pseudocysts of lung by blunt chest trauma are rare lesion with 1 to 2 % incidence. The symptoms are nonspecific and misled or confused with lung abscess or congenital cystic disease of lung occasionally. Diagnosis is not difficult by radiographic findings and history of blunt chest trauma. There courses are benign and spontaneously absorbed and require no specific treatment mostly. Recently, we had the one case of 8 year-old female with traumatic lung cyst and its cavities filled the entire right lower lobe. The destruction of lung was severe, so, right lower lobectomy was done and recovered, discharged without event.

삼중 동시성 원발성 폐암 치험 1례 (Tripe synchronous primary lung cancer -one case report-)

  • Multiple primary lung cancer is not common and classified as a synchronous primary lung cancer and a metachronous primary lung cancer. We experienced one case of the triple synchronous primary lung cancer of different cell types. We conducted right pneumonectomy for preoperative diagnosed neuronendocrine tumor of the RUL and adenocarcinoma of the RLL. Pathologic examination revealed the carcinoid tumor of RUL bronchus, the squamous carcinoma of the RML and the adenocarcinoma of the RLL.

Donor Selection, Management, and Procurement for Lung Transplantation

  • Lung transplantation is a life-saving procedure in patients with end-stage lung disease. However, it inherently depends on the availability of donor organs. The selection of suitable lungs for transplantation, management of donors to minimize further injury and improve organ function, and safe procurement remain critical for successful transplantation. In this review, we provide an update on the current understanding of donor selection, management, and lung procurement.

AN EXPERIMENTAL STUDY ON TUMOR INHIBITORY EFFECT OF RED GINSENG IN MICE AND RATS EXPOSED TO VARIOUS CHEMICAL CARCINOGENS

  • This experiment was carried out to evaluate the effects of Korean ginseng extract on carcinogenesis induced by various chemical carcinogens. Red ginseng extract was used for this study and was administered orally to the experimental animals. Carcinogens that were injected in subscapsular region of ICR newborn mice within 24 hours after birth were 9,10-dimethyl-1,2-benzan-thracene (DMBA), urethane, N-2-fluorenylacetamide(AAF), aflatoxin $B_1$ and tobacco smoke condensate. N -methyl-N -nitroso-N'-nitroguani-dine(MNNG) was injected subcutaneously at the back of wistar rats. Experimental animals were autopsied in immediately after being sacrificed. All major organs were examined grossly and weighted. After fixation histopathological preparations were made for microscopical study. Following results were obtained. In DMBA group sacrificed at the 26th week after the treatment with DMBA, the incidence of lung adenoma was $77\%$ and the average number of the tumor was 17. However, in DMBA combined with red ginseng group, the incidence was $78\%$ and the average number of lung adenoma was 14.1. This indicates that ginseng extract had no effect on the incidence of lung adenoma but decreased the average number of lung adenoma by $17\%.$ In DMBA group sacrificed at the 48th week after the injection of DMBA, the lung adenoma incidence was $88\%.$ The average diameter of the largest lung adenoma was 3.5 cm, the incidence of diffuse pulmonary infiltration was $18\%$ and the average lung weight of male experimental mice was $528.2{\pm}469.1\;gm.$ On the other hand, in DMBA combined with red ginseng group sacrificed at the 48th week, the incidence of lung adenoma was $96\%.$ The average diameter of the largest adenoma was 2.7 cm, the incidence of diffuse pulmonary infiltration was $7\%$ and the average lung weight of male mice was $418.0{\pm}520\;gm.$ These observations show that ginseng extract did not have any inhibitory effect on the incidence of lung adenoma but decreased the average diameter of the largest lung adenoma by $23\%,$ the incidence of duffuse pulmonary infiltration by $63\%$ and the average lung weight of male experimental mice by $21\%.$ From these results we have found that the prolonged administration with ginseng extract showed no inhibitory effect on the incidence of adenoma but it had the inhibitory effect on the proliferation of lung adenomas induced by DMBA. In urethane group sacrificed at the 28th week after the injection of urethane, the incidence of lung adenoma was $94\%$ and the average number of lung adenoma was 8.6. In urethane combined with red ginseng group, the. incidence of lung adenoma was $73\%$ and the average number of adenoma was 6.0. These results indicate that there were $22\%$ decrease of the lung adenoma incidence and $31\%$ decrease of the average number of adenoma in urethane combined with red ginseng group. And in urethane group sacrificed at the 50th week, the incidence of lung adenoma was $98\%$ and the incidence of diffuse pulmonary infiltration was $14\%$. In urethane combined with ginseng group the incidence of lung adenoma was $85\%$ and the incidence of diffuse pulmonary infiltration was $12\%$. Therefore the ginseng administration resulted in $15\%$ decrease of the lung adenoma incidence and $14\%$ decrease of the diffuse pulmonary infiltration incidence. From these results we knew that the prolonged administration with ginseng extract inhibited the incidence and also the proliferation of the lung adenoma induced by urethane. Lung adenoma and hepatoma were induced in the experimental mice sacrificed at the 68th week but not in the experimental mice sacrificed at the 28th week after the injection of AAF. In AAF group sacrificed at the 68th week after the injection of AAF the incidence of lung adenoma was $18\%$ and the incidence of hepatoma was $27\%$. And in AAF combined with ginseng group the lung adenoma incidence was $12\%$ and the hepatoma incidence was $37\%$. So the ginseng seemed to decrease the lung adenoma incidence by AAF, but we were unable to conclude the significant inhibitory effect of the ginseng extract on the incidence of lung adenoma by AAF because the above incidence of lung adenoma were similar to that of control group which was $11\%$. And these experimental data revealed that ginseng extract didn't have any inhibitory effect on the incidence of hepatoma induced by AAF. In aflatoxin $B_1$ group sacrificed at the 56th week, the incidence of lung adenoma was $24\%$ and hepatoma was $11\%$. However in aflatoxin $B_1$ combined with ginseng group the incidence of lung adenoma was $17\%$ and hepatoma was $3\%$ These results indicate that there were $29\%$ decrease of the lung adenoma incidence and $75\%$ decrease of the hepatoma incidence in aflatoxin $B_1$ combined with ginseng group. In tobacco smoke condensate experimental group sacrificed at 67th week, no tumors were induced except just a few lung adenoma. The lung adenoma incidence both in tobacco smoke condensate group and in tobacco smoke condensate combined with ginseng group was $8\%$. And this incidence rate was similar to that of control group. These results indicate that the injection of 320 ug tobacco smoke condensate per ICR newborn mouse was unable to induce lung adenoma in our experiments. In MNNG group sacrificed at the 27th week the tumor incidence was $38.5\%$ and in MNNG combined with ginseng extract group was $37\%$. In MNNG group for investigation of the life span of tumor bearing rats the tumor incidence was $93\%$ and the average life span of tumor bearing rats was 318 days. And in MNNG combined with ginseng extract group the tumor incidence was $96\%$ and the average life span was 337 days. Tumor induced by MNNG was almost sarcoma. This indicates that there was no inhibitory effect of ginseng extract on the tumor incidence, but the extract prolonged the average life span of tumor bearing rats by approximately 19 days.

개흉 및 흉강경항폐생검의 전국실태조사 (The National Survey of Open Lung Biopsy and Thoracoscopic Lung Biopsy in Korea)

  • 연구배경: 개흉폐생검은 미만성 침윤성 폐질환의 감별진단, 면역억제 환자에서의 폐침윤의 원인 등 다양한 폐질환에서 확진에 이르기 위한 고전적인 진단술로서 충분한 크기의 검체를 제공할 뿐 아니라 폐생검시 생검사가 직접 병변부위를 관찰하고 생검장소를 결정할 수 있는 장점이 있는 반면 최근에는 흉강경하폐생검(video-assisted thoracoscopy, VATS)이 개발되어 늑막 및 폐질환의 진단에 이용되고 있다. 본 학회의 학술위원회에서는 폐질환에서 개흉 혹은 흉강경하 폐생검의 실시여부, 폐생검 실시 전에 시행할 진단절차, 실시시기 등을 결정하는데 필요한 기초자료를 수집하여 향후 임상연구 및 진료에 활용하고자 현재 한국에서의 개흉폐생검의 적응질환, 시행빈도, 진단성적, 치료에 미치는 영향, 합병증 등을 조사하였다. 방 법: 전국의 대학병원 및 400병상 이상의 종합병원에서 1994년 1월 1일부터 1996 년 12월 31일까지 3년간 개흉 및 흉강경하폐생검을 시행한 환자들에 대한 설문조사를 의뢰하였다. 본 실태조사에 응답한 37 개 병원의 511예를 대상으로 분석하여 다음의 결과를 얻었다. 통계 분석은 SAS 통계 프로그램을 이용하여 폐생검 전후의 비교는 paired t-test를 이용하였고 각 인자들에 따른 차이비교는 $x^2$-검정 혹은 t-검정법을 이용하였으며 P값은 0.05 이하인 경우를 유의하다고 판정하였다. 결 과: 1) 실태조사에 응답한 511 예 대상환자들의 평균연령은 50.2세(${\pm}15.1$세)이었으며, 남자 54.2%. 여자 45.9% 이었다. 2) 511 예 중에서 개흉폐생검은 313예(62%). 흉강경하폐생검은 192예(38%)에서 시행되었고, 미만성 폐질환자가 305명(59.7%), 국한성 폐질환자가 206명(40.3%)이었다. 3) 흉부 X-선 검사에서 이상소견이 발견되어 폐생검을 하기까지의 기간은 평균 82.4일 이었으며, 개흉폐생검은 평균 72.8일, 흉강경하폐생검은 평균 99.4일이 소요되었고, 폐생검 이전에 기관지내시경검사는 272예(53.2%), 기관지폐포세척술은 123예(24.1%), 경피적세침흡인술은 72예(14.1%)에서 시행되었다. 4) 대상질환은 간질성 폐질환이 230예(45.0%), 흉곽종양이 133예(26.0%), 결핵을 포함한 감염성 폐질환이 118예(23.1%), 선천성질환을 포함한 가타 폐질환이 30예(5.9%)이었다. 생검 방법에 따른 진단율이나 병변특성의 유의한 차이는 없었다. 5) 폐생검 이전의 임상적 진단과 폐생검 후의 최종 진단이 일치한 경우는 302예(59.2%) 이었고, 질환별로는 간질성 폐질환 66.5%, 흉곽종양 58.7%. 폐감염질환 32.7%, 결핵 55.1%. 선천성 폐질환을 포함한 기타 질환 62.5%에서 임상진단과 최종진단이 일치하였다. 6) 폐생검 전 및 1일 후에 시행한 동맥혈가스분석 검사상 $PaCO_2$$38.92{\pm}5.8mmHg$에서 $40.2{\pm}7.1mmHg$로 유의하게 증가하였고(P<0.05), $PaO_2/FiO_2$$380.3{\pm}109.3mmHg$에서 $339.2{\pm}138.2mmHg$로 유의하게 감소하였다(P=0.01). 7) 합병증은 10.1%에서 발생하였으며 개흉폐생검시 12.4%, 흉강경하폐생검시 5.8%로 흉강경하폐생 검시 유의하게 합병증이 적었다 (p<0.05). 합병증은 기흉 23예(4.6%), 혈흉 7예(1.4%), 사망 6예(1.2%), 기타 부정맥이나 타장기손상 15예(2.9%) 등이었다. 8) 폐생검으로 인한 사망은 개흉폐생겸후 5예, 기록이 불분명한 경우가 1예 등 모두 6예 이었으며 흉강경하폐생검 후 사망한 경우는 없었다. 사망한 사랑의 기저질환은 악성종양 3예(기관지폐포세포암 2예, 악성 중피종 1예), 전이성 악성종양 2예, 간질성폐질환 1예 이었다. 폐생검 후 사망까지의 기간은 평균 $15.5\pm9.9$일 이었다. 9) 개흉 또는 흉강경하폐생검으로도 진단할 수 없었던 경우는 19예(3.7%)이었다. 진단이 안된 원인은 병소 이외에서 조작검사를 시행한 5예, 조직의 크기가 너무 작은 3예 등 폐조직검사를 부적절하게 시행한 경우가 8예 이었고, 조직검사는 잘 되었으나 병리학적으로 진단이 불가능하였던 경우가 11예 이었다. 10) 개흉 또는 흉강경하폐생검이 진단에 결정적으로 기여한 경우가 334예(66.5%), 다소 기여한 경우가 140예(27.9%), 도움이 되지 않았거나 판단이 불가능한 경우가 28예(5.6%)이었다. 따라서 폐생검을 한 경우의 94.4%에서 확진에 도움을 주었다. 결 론: 개흉 및 흉강경하폐생검은, 기관지내시경 등 다른 술기로 진단이 안되는 폐질환의 확진에 도움이 되는 방법이며 환자상태가 위중한 경우에는 보다 안전하고 대등한 진단성적을 얻을 수 있는 흉강경하폐생검이 추천된다.

GPR78 promotes lung cancer cell migration and metastasis by activation of Gαq-Rho GTPase pathway

  • GPR78 is an orphan G-protein coupled receptor (GPCR) that is predominantly expressed in human brain tissues. Currently, the function of GPR78 is unknown. This study revealed that GPR78 was expressed in lung cancer cells and functioned as a novel regulator of lung cancer cell migration and metastasis. We found that knockdown of GPR78 in lung cancer cells suppressed cell migration. Moreover, GPR78 modulated the formation of actin stress fibers in A549 cells, in a RhoA- and Rac1-dependent manner. At the molecular level, GPR78 regulated cell motility through the activation of $G{\alpha}q$-RhoA/Rac1 pathway. We further demonstrated that in vivo, the knockdown of GPR78 inhibited lung cancer cell metastasis. These findings suggest that GPR78 is a novel regulator for lung cancer metastasis and may serve as a potential drug target against metastatic human lung cancer.

폐(肺)와 피부(皮膚)의 관계(關係)에 대(對)한 문헌적(文獻的) 고찰(考察) (The investigation of literature about relation of lung and skin)

  • The skin is related the lung as follows ; 1. The lung control skin and give a nutrition to skin. The skin and hair 'hab' (合) the lung. 2. As a damage of lung is a damage of skin, skin and hair 'chi-lag' (聚落) 3. A defensive energy of lung give a nutrition to skin, and strengthen the body surface. 4. Dryness damage the lung, and so damage the skin. According to the above findings, we know the body surface are closely related to the lung functionally.

망내계기능 항진이 폐장보존에 미치는 영향 (Effects of Reticuloendothelial Hyperfunction on Preservation of Lung)

  • The effect of reticuloendothelial hyperfunction on hypothermic preservation of lung was studied in dogs. In order to evaluate the viability after hemodynamic_ load in preserved isolated lung, observations were made on the rate of increase in weight, degree of edema,compliance and surface activity of lung. The results obtained as follows: l. In the group of activating of the reticuloendothelial system by injection of sodium thiosulfate intravenously before pneumonectomy and infusion of naphthionine through the pulmonary artery before hypothermic preservation of isolated lung the limit of preservation was eight hours whereas four hours in non-treated control group. 2.Therefore the method of activating of the reticuloendothelial system before and after pulmonary resection seems effective in preserving for prolonging the period of preservation of lung by means of inhibition of pulmonary edema. 3. Pulmonary surface activity is expected to be valuable as a method in evaluation of the viability of preserved lung along with compliance and rate of increase in weight of lung.
