Molecular Pathology of Lung Cancer: Current Status and Future Directions

  • The rapid development of targeted therapies has enormously changed the clinical management of lung cancer patients over the past decade; therefore, molecular testing, such as epidermal growth factor receptor (EGFR) gene mutations or anaplastic lymphoma kinase (ALK) gene rearrangements, is now routinely used to predict the therapeutic responses in lung cancer patients. Moreover, as technology and knowledge supporting molecular testing is rapidly evolving, the landscape of targetable genomic alterations in lung cancer is expanding as well. This article will summarize the current state of the most commonly altered and most clinically relevant genes in lung cancer along with a brief review of potential future developments in molecular testing of lung cancer.

The Changes of Antioxidant Enzymes in the Lung of Alloxan-induced Diabetic Rats (알록산 유도 당뇨흰쥐의 폐에서 황산화계의 변화)

  • The present study was attempted to investigate the mechanism of oxidative cellular injuries which occur in diabetic rats by determining changes of antioxidant enzymes activity in the lung of alloxan-induced diabetic rats, the contents of glutathione in the lung, liver, blood samples, and ${\gamma}$-glutamylcysteine synthetase activities in the liver. Superoxide dismutase activities (SOD), including Cu, Zn-SOD and Mn-SOD, decreased in the lung of diabetic rats compared with those of normal control rats. However, activities of catalase and glutathione peroxidase(GPX) activities were not affected in the lung of diabetic rats. In diabetic rats, glutathione contents in the lung, liver, and blood samples, as well as the activities of ${\gamma}$-glutamylcysteine synthetase in the livers which is known to be the key enzyme of glutatione biosynthesis, decreased significantly. From these experimental results, it is thought that the decrease in SOD activities in the lung, glutathione contents and ${\gamma}$-glutamylcysteine synthetase activities in some tissues in alloxan-induced diabetic rats may be the crucial cause of vullnerability to oxidative cellular injuries.

Diagnosis of Pulmonary Tuberculosis and Nontuberculous Mycobacterial Lung Disease in Korea

  • The recovery of nontuberculous mycobacteria (NTM) from respiratory specimens and the number of patients with NTM lung disease have been rapidly increasing in Korea. An early differential diagnosis of NTM lung disease from pulmonary tuberculosis (TB) is important, as the therapeutic regimen differs from that of pulmonary TB, and it is not necessary to track the contacts of patients with NTM lung disease. However, differentiating NTM lung disease from pulmonary TB remains difficult, because the clinical presentations of the two diseases are similar and a definite diagnosis of NTM lung disease based on sputum culture takes time. This review focuses on the changing epidemiology, clinical and radiographic manifestation, and laboratory diagnosis of pulmonary TB and NTM lung disease in Korea.

  • The clinical and radiographic findings of lung cancer have been well established many journals. Even if the radiographic findings of lung cancer show a typical pattern, the specific cell type of lung cancer sometimes needs to be determined prior to a pathological diagnosis. For example, the usual finding of a squamous cell carcinoma is similar to other cancer types such as an adenocarcinoma or a small cell carcinoma but with a lower incidence. Therefore, it should not be used to make a diagnosis of the cell type prior to a pathological diagnosis. Many unusual findings of lung cancer, so called atypical pattern have been reported, but atypical findings are widely accepted. The more important thing is not to diagnose a specific cell type of cancer but to differentiate it from other benign conditions such as tuberculosis, fungal infections or organizing pneumonia. This paper presents typical information of the cell type of lung cancer along with the atypical radiographic findings.

  • Since lung cancer is a major causative for cancer-related deaths, the investigations for discovering biomarkers to diagnose at an early stage and to apply therapeutic strategies have been continuously conducted. Recently, long non-coding RNAs (lncRNAs) and microRNAs (miRNAs) are being exponentially studied as promising biomarkers of lung cancer. Moreover, supportive evidence provides the competing endogenous RNA (ceRNA) network between lncRNAs and miRNAs participating in lung tumorigenesis. This review introduced the oncogenic or tumor-suppressive roles of lncRNAs and miRNAs in lung cancer cells and summarized the involvement of the lncRNA/miRNA ceRNA networks in carcinogenesis and therapeutic resistance of lung cancer.

UBE2S promotes the proliferation and survival of human lung adenocarcinoma cells

  • Ubiquitin-conjugating enzyme E2S (UBE2S), a family of E2 protein in the ubiquitination process, is involved in development of various cancers. However, its role in lung adenocarcinoma, has not been well elucidated. In this report, we attempted to investigate expression and function of UBE2S in lung adenocarcinoma. Up-regulation of UBE2S at mRNA, and protein level, was observed in human cancer tissues and lung adenocarcinoma cells. Higher UBE2S expression correlated with poorer prognosis of lung adenocarcinoma patients. UBE2S expression was efficiently suppressed by lentivirus-mediated shRNA strategy in A549 cells, and UBE2S silencing led to reduced cell proliferation, colony formation, and enhanced apoptosis. Inverse results were observed, in UBE2S over-expressed H1299 cells. Microarray analysis indicated that a large number of genes were regulated by UBE2S, and p53 signaling pathway may be critical, to the role of UBE2S in cancer development. Together, UBE2S could be a potential target for lung adenocarcinoma.

The Histologic Type of Lung Cancer in Idiopathic Pulmonary Fibrosis : the Difference According to the Presence of Fibrosis at Cancer Location (특발성 폐섬유화증에서 발생한 폐암의 조직형의 특성 : 폐암 위치의 섬유화 유무에 따른 조직형의 차이)

  • Background : It is well known that the prevalence of lung cancer is higher in idiopathic pulmonary fibrosis (IPF) patients than in the general population. This high prevalence is explained by the concept of 'scar carcinoma'. There have been several reports on the prevalence of histologic typo of lung cancer in IPF with conflicting results. Despite of the high smoker rate in almost all previous reports, none considered the smoking history of patients. Therefore we performed a separate studies on fibrosis associated lung cancer and smoking associated lung cancer. The purpose of this study is to investigate the proportion of lung cancer in IPF that is fibrosis associated and to determine the most common histologic type in fibrosis associated lung cancer in IPF. Method : A retrospective review of medical records and radiologic studies was performed for cases of lung cancer with IPF. We investigated smoking history, sequence of diagnosis of lung cancer and IPF, histologic type of lung cancer and the cancer location, especially whether the location is associated with fibrosis. To evaluate the proportion of fibrous associated lung cancer, the lung cancer in IPF were categorized according to the presence of fibrosis at cancer location. Results : Fifty seven patients were subjects for this analysis. Six (11%) cases were diagnosed as lung cancer during follow-up for IPF, and both diseases were diagnosed simultaneously in the others. Ninety four percent of patients were smokers and the average smoking amount was 47.1$\pm$21.9 pack-year. Among the patients with IPF and lung cancer, 42(80.8%) cases were considered as "fibrosis associated". The remainder was "not fibrosis associated" and probably was due to smoking etc. Although the most frequent histologic type was squamous cell carcinoma as a whole, adenocarcinoma was the prominent histologic type in "fibrosis associated lung cancer." Conclusion : Considering the proportion of "fibrosis not associated lung cancer" in the patients with IPF and lung cancer, significant proportion of lung cancer in IPF may not be fibrosis induced. This may influence the distribution of histologic type of lung cancer in IPF.

Primary Lung Cancer Presenting Initially as Spontaneous Pneumothorax (폐암에 동반된 자연기흉)

  • Spontaneous pneumothorax is a rare manifestation of primary lung cancer and it is even more rare as an initial manifestation. Recently we have experienced three cases of lung cancer presenting initially as spontaneous pneumothorax. These three cases involved 2 men and one woman with an average age of 70 years [66 - 74years]. Lung cancer was discovered by explothoracotomy in two cases and by endoscopic biopsy in one case. In pathologic cell types, the one was alveolar cell carcinoma and the others were squamous cell carcinoma. We report these three cases of primary lung cancer presenting initially as spontaneous pneumothorax with review of the literatures.

Closed Digital Reduction of Posttraumatic Lung Hernia (외상 후 폐장탈출의 폐쇄손가락정복)

  • Postraumatic lung hernia is a rare occurrence. A number of cases reported in the literature have been treated with early thoracotomy to repair partial protruded lung and pleura to prevent strangulation and incarceration. We present a case of a 45-year-old patient of left posttrumatic lung hernia, in which closed digital reduction was successful. The strategy of the management approach could be established by further accumulated experience.
