Epidural steroid injection is a treatment for low back pain which allows smaller doses with less risk of side effects and longer duration of relief than systemic administration. From 1 June 1992 to 31 January, 1994, 1 mg/kg of triamcinolone diacetate in 8 ml of lidocaine 1% was administered 56 times to 33 patients who complained of low back pain. Results of epidural steroid injection provided effectiveness in treating various low back pain diseases except postlaminectomy syndrome. However there are no gains about repeated epidural steroid injection.

Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).

This study is aimed to verify effects of Walking in treating hospitalized patients with low back pain in the Korean medicine hospital. The method of this study is choosing patients whose chief complaints were low back pain in the Korean medicine hospital in Gyeonggi Province. People who agreed to take Walking treatment were classified into experimental group and control group. Both experimental group and control group were treated with Korean medicine. In addition, experimental group performed Walking program more often. Changes of temperature difference taken by digital infrared thermal imaging(DITI) were used as measuring tools. The summary of this study's result is as in the following. There was significant difference in the experimental group which performed Walking treatments but not in the control group resulting from examining the change of temperature difference between left and right Yongchon. As known from the results above, Walking program is meaningful in curing the low back pain.

This case study reports the clinical response to Korean Medicine (KM) treatments including capsaicin-containing (CP) pharmacopuncture in three patients with acute low back and hip pain. Three patients were diagnosed with acute lumbar sprain, and were treated with CP pharmacopuncture, and other KM treatments including acupuncture, cupping, and herbal medicine. Numeric Rating Scale (NRS), Oswestry Disablility Index (ODI), and Clinical Evaluation Grade (CEG) were used to evaluate symptom changes. After one or two treatments, acute pain with limited range of motion in the low back and thigh region improved with a decrease in NRS, ODI, and CEG. These results suggest that further studies on KM treatments including CP pharmacopuncture for acute low back and hip pain are warranted.

Bertolotti's syndrome is a congenital abnormality in which the expanded lower lumbar transverse process articulates with the ilium or sacrum. It is an important cause of low back pain in children and adolescents that is frequently misdiagnosed. We describe the case of a 17-year-old girl with low back pain who had a 4-year history of juvenile idiopathic arthritis. She subsequently underwent plain radiography and magnetic resonance imaging and was eventually diagnosed with Bertolotti's syndrome. She was managed conservatively with 6 weeks of physical therapy and an exercise program, and her pain subsided. This case demonstrates the importance of considering anatomical and structural variants when evaluating low back pain in adolescents.

Objectives: This analysis of prescription used for low back pain in the Yomun(腰門) chapter of ${\ll}$Donguibogam(東醫寶鑑); The Precious Mirror of Oriental Medicine${\gg}$ is designed to be helpful to practical use of clinics. Methods : Proscriptions used for low back pain in the Yomun(腰門) chapter of ${\ll}$Donguibogam(東醫寶鑑)${\gg}$ were classified and analyzed according to the frequency of proscriptions and the characteristics of each herbs in proscriptions(efficacy, used frequency, related organs etc.) Results and conclusions : After analysis, we obtained the following results : 1. The causes of low back pain are mainly eohyeol(瘀血), yangheo(陽虛), punghanseub(風寒濕). In care of low back pain, I suppose more efficiency that if Angelica gigas NAKAI(當歸) Cnidium officinale MAKINO(三芎) Prunus persica BATSCH(桃仁) is added when the cause is eohyeol(瘀血), or if Psoralea corylifolia L.(破古紙), Cinnamomum cassia PRESL(肉桂), Foeniculum vulare MILL(茴香), Eucommia ulmoides OLIV.(杜冲), Citrus unshiu MARKOVICH(陳皮) are added when the cause is yangheo(陽虛), or if Phellodendron amurense RUPR.(黃柏), Notopterygium incisum TING(羌活), Atractylodes Japonica KOIDZ.(蒼朮) are added when the cause is punghanseub(風寒濕).

Objectives : The aim of this study was to investigate the characteristic of ryodoraku in the chronic low back pain patients who visited department of oriental rehabilitation medicine. Methods : Subjects were 32 patients who visited department of oriental rehabilitation medicine(<60 years old). The low back pain patients were measured points of twelve meridians by ryodoraku diagnosis and checked the duration of low back pain. We compared the mean ryodoraku and the percentage of each points beyond the physiological range. Results : The score of H1(LU9), H2(PC7), H3(HT7), H5(TE4) and F5(GB40) had statistical differences compared with mean ryodoraku. And the correlation between duration of pain and mean ryodoraku showed statistically significant negative correlation(r=-0.353). Conclusions : These results suggest that ryodoraku maybe used to evaluate chronic low back pain patients.

Objectives : The Purpose of this report is to examine the effect of Remote Acupuncture Point Needling in the Low Back Pain patient of Acute Stage. Methods : From October 1st 2005 to May 31th 2006, 30 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took near acupuncture point needling, and the other group(test group) took near acupuncture point needling with remote acupuncture point needling. For evaluating change of pain, Visual Analog Scale(VAS) was checked before and after Treatment. Results : Both acupuncture therapy showed good effect on low back pain of acute stage. And test group showed better effect on decreasing pain than control group. It was proved by the difference between VAS checked before treatment and what checked after treatment. But it was not significant statistically. Conclusion : Near acupuncture point needling and remote acupuncture point needling can be recommended as a useful therapy to treat low back pain of acute stage.

Objectives : This study was performed to evaluate the effects of thoracolumbar junction's Hyeopcheok points(夾脊穴) for low-back pain in the thoracolumbar junction syndrome. Method : 1. Measurement : We investigated 26 patients with low-back pain. The unpleasantness of pain was measured by visual analogue scale(VAS), and the Oswestry low-back pain disability index(ODI). We compared the VAS and ODI scores of the two groups statistically. 2. Intervention : Use Acupuncture and bee venom injection in thoracolumbar junction's Hyeopcheok points that display main tender points. Also, herb medication and physical therapy was used. Results and Conclusion : As a result of treatment, thoracolumbar junction's Hyeopcheok points is very effective for low-back pain. However, more cases are required to oriental medicine in order to prove the availability.

In this study, we treated a patient with psychogenic chronic low back pain for 2 years with herbal medication treatment and psychotherapy. The case was a 17 years old female patient complained chronic low back pain for a long time. She received the spine operation, epidural injection, many kind of oriental medicine treatment about low back pain, but there was no improvement. We diagnosed her pain as a psychogenic pain disorder, and treated her with herbal medication for decrease her stress, and did a psychotherapy such as the progressive muscle relaxation, and counseled about trouble with her family. After treatment, her chronic low back pain was improved so much. We think that the herbal medication treatment and psychotherapy can be helpful to treat patients who have psychic and physical disorders.
