Oral Health Status of Deaf and Mute Children Attending Special School in Anand-Wan, Warora, India

  • Aim : This study was conducted to assess the oral health status of deaf and mute children attending special school. Materials and Methods : A cross-sectional descriptive survey was conducted among 137 deaf and mute children with ages ranging from 7 to 18 years. A total of 76 males (55.47%) with mean age of $14.2{\pm}4.5$ and 61 females (44.53%) with mean age of $13.8{\pm}4.2$ years and studying in a school for deaf and mute children in Warora were considered. Data were collected using a standard method recommended by WHO for the oral health survey in 1977. Oral health status was assessed using OHIS, Loe and Sinless, and CPI Index along with DMFT and DMFS Index. Gingival position was considered for measuring attachment loss. Statistical analysis was performed using the SPSS software package (version 17.0). Results : The mean DMFT was found to be $2.53{\pm}1.72$, and mean DMFS, $3.37{\pm}3.16$. The prevalence of dental caries was pegged at 35.32%, with mean OHIS score at $1.49{\pm}0.76$. Overall gingival index among deaf and mute children was $0.81{\pm}1.4$, whereas that for the upper arch and lower arch was $0.92{\pm}0.84$ and $1.19{\pm}0.95$, respectively. The mean score for the CPI Index among deaf and mute children was found to be $0.42{\pm}0.32$. Gingival clinical attachment loss was found to be $0.26{\pm}0.15mm$. Conclusion : These findings suggest that children with hearing disabilities can also have good oral hygiene comparable to normal individuals of the same age group. These results may be attributed to the fact that the study sample was taken from a single school of a private organization with a well-equipped dental setup.

Wear, microleakage and plastic deformation of an implant-supported chair-side bar system

  • PURPOSE. This in-vitro study was designed to evaluate retention forces, microleakage and plastic deformation of a prefabricated 2-implant bar attachment system (SFI-Bar, Cendres+$M{\acute{e}}taux$, Switzerland). MATERIALS AND METHODS. Two SFI implant-adapters were torqued with 35 Ncm into two implant analogues. Before the tube bars were finally sealed, the inner cavity of the tube bar was filled with liquid red dye to evaluate microleakage. As tube bar sealing agents three different materials were used (AGC Cem (AGC, resin based), Cervitec Plus (CP; varnish) and Gapseal (GS; silicone based). Four groups with eight specimens each were tested (GS, GS+AGC, AGC, CP). For cyclic loading, the attachment system was assembled parallel to the female counterparts in a chewing simulator. The mean retention forces of the initial and final ten cycles were statistically evaluated (ANOVA, ${\alpha}{\leq}.05$). RESULTS. All groups showed a significant loss of retention forces. Their means differed between 30-39 N initially and 22-28 N after 50,000 loading cycles. No significant statistical differences could be found between the groups at the beginning (P=.224), at the end (P=.257) or between the loss of retention forces (P=.288). Microleakage occurred initially only in some groups but after 10,000 loading cycles all groups exhibited microleakage. CONCLUSION. Long-term retention forces of the SFI-Bar remained above 20 N which can be considered clinically sufficient. The sealing agents in this study are not suitable to prevent microleakage.

차동 임피던스 분석을 사용한 SATA 커넥터의 신호 전달 특성 개선 (Signal Transmission Properties Improvement of Serial Advanced Technology Attachment Connector Using Analysis of Differential Impedance)

  • 본 논문에서는 SATA(Serial Advanced Technology Attachment) 커넥터의 차동 임피던스 계산 방법을 적용하여 설계 변경 방법을 제안하고 신호 전달 특성을 개선하였다. 3차원 FEM(Finite Elements Method) 전자기장(Electromagnetic Field) 시뮬레이터를 이용하여 SATA 커넥터의 차동 모드 S-파라미터를 계산하고, 신호 전달 특성을 분석한다. 차동 임피던스는 Odd mode 임피던스를 이용하여 계산되므로 인덕턴스, 커패시턴스, 상호 인덕턴스, 상호 커패시턴스 값이 필요하다. 따라서 시뮬레이터를 이용하여 SATA 커넥터의 각각의 값을 추출하여 차동 임피던스를 계산하였고, 이 값은 $107.3{\Omega}$으로 설계 사양을 만족하지 못하였다. 신호 전달 특성 개선을 위해서 SATA 커넥터의 핀을 $d_x$, $d_y$ 방향으로 설계 변경하고, 각 경우의 신호 전달특성과 차동 임피던스를 분석하였다. $d_y=0.1mm$의 경우에서 신호 전달 특성이 가장 우수하게 나타났고, 차동 임피던스가 $98.7{\Omega}$으로 정합되었다. 이때, 반사손실은 1.5 GHz에서 15 dB 개선되었다.

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. To achieve periodontal regeneration, various kinds of methods have been investigated and developed, including guided tissue regeneration and bone graft. Bone graft can be catagorized into autografts, allografts, xenografts, bone substitutes. And materials of all types have different biological activity and the capacity for periodontal regeneration, but ideal graft material has not been developed that fits all the requirement of ideal bone graft material. Intensive research is underway to identity, purify, synthesize a variety biologic modulators that may enhance wound healing and regeneration of lost tissues in periodontal therapy. The present study evaluates the effects of ABM/P-15 on the periodontal regeneration in intrabony defects of human. We used thirty four 2-wall or 3-wall osseous defects in premolars and molars of chronic peridontitis patient that have more than 5mm pockets and more than 3mm in intrabony defect. 12 negative control group underwent flap procedure only, 11 positive control group received DFDBA graft with flap procedure, and 11 experimental group received ABM/P-15 graft with flap procedure. The changes of probing pocket depth, loss of attachment and bone probing depth following 6months after treatment revealed the following results: 1. The changes of probing pocket depth showed a statistically significant decrease between after scaling and 6months after treatment in negative control(2.0${\pm}$0.9mm), positive control(3.0${\pm}$0.9mm), and experimental group (3.4${\pm}$1.5mm) (P<0.01). Significantly more reduction was seen in experimental group compared to negative control group (P<0.05). 2. The changes of loss of attachment showed a statistically significant decrease between after scaling and 6months after treatment in positive control(2.0${\pm}$0.6mm), and experimental group (2.2${\pm}$l.0mm) except negative control group(0.1${\pm}$0.7mm) (P<0.01). Significantly more reduction was seen in both experimental and positive control group compared to negative control group(P<0.05). 3. The changes of bone probing depth showed a statistically significant decrease between after scaling and 6months after treatment in positive control(2.7${\pm}$l.0mm), and experimental group (3.4${\pm}$1.3mm) except negative control(0.l${\pm}$0.9mm) (9<0.01). Significantly more reduction was seen in both experimental and positive control group compared to negative control group (P<0.05). The results suggest that the use of ABM/P-15 in the treatment of periodontal intrabony defects can reduce loss of attachment and bone probing depth more than flap operation only. It suggests that ABM/P-15 may be an effective bone graft material for the regeneration of periodontal tissue in intrabony defects.

Bioactive glass의 장기 임상적 평가 (Clinical Long-term Assessment of Bioactive Glass Graft)

Recently, bioactive glass that has been utilized in plastic surgery is being investigated for application in dental practice. But, there has not been any long-term assessment of bioactive glass when used in periodontal intrabony defects. The present study evaluates the long-term effects of bioactive glass on the periodontal regeneration in intrabony defects of human and the effect of plaqu control on long term treatment results after dividing patients into those who underwent 3-month regular check-up and those who didn't under go regular check-up The clinical effect on 74sites from 17 infrabony pockets of 11 patients were analyzed 36months after treatment. 51 sites which underwent regular check up were classified as the Follow-up group(F/U group), and 23 sites which did not undergo regular check up were classified as Non Follow-up group(Non F/U group). After comparing the probing depth, attachment loss, bone probing depth before and 36months after treatment, the following results could be concluded. 1. The changes of probing pocket depth showed a statistically significant decrease between after baseline and 36 months after treatment in F/U group(1.79${\pm}$0.68mm) and did no show astatistically significant decrease between after baseline and 36months after treatment in Non F/U group(0.61${\pm}$0.54mm) (P<0.05). 2. The changes of loss of attachment showed a statistically significant decrease between after baseline and 36 months after treatment in F/U group(1.44${\pm}$0.74mm) and did no show astatistically significant decrease between after baseline and 36months after treatment in Non F/U group(1.18${\pm}$1.54) (P<0.05). 3. The changes of bone probing depth showed a statistically significant decrease between after baseline and 36 months after treatment in both F/U(1.35${\pm}$0.28) and Non F/U group(0.78${\pm}$0.55mm) (P<0.05). The results suggest that treatment of infrabony defects with bioactive glass resulted in significan reduction of attachment loss and bone probing depth 36months after the treatment. The use of bioactive glass in infrabony defects, combined with regular check-up and proper plaque control generally shows favorable clinical results. This measn that bioactive glass could be a useful bone substitute.

임플란트 피개의치를 위한 개별유지형 어태치먼트의 유지력에 관한 연구 (A Study on the Retentive Force of Stud Attachments for Implant Overdenture)

  • 이번 연구에서는 두 종류의 어태치먼트 시스템인 Kerator와 O-ring의 다양한 유지력을 가지는 matrix를 사용한 반복적인 착탈 후 평가를 통해 다음과 같은 결론을 얻었다. 초기 유지력은 KP군($62.09{\pm}6.00N$)에서 높았으며, 이어서 KR군($32.83{\pm}4.40N$), KB군($12.69{\pm}3.32N$), OO군($9.55{\pm}1.33N$), 그리고 OR군($9.35{\pm}0.71N$) 순으로 나타났다. 유지력 소실량은 KP군($22.26{\pm}8.02N$)에서 가장 높았으며, KR군($6.23{\pm}3.04N$), OO군($6.03{\pm}1.24N$), OR군($5.81{\pm}0.73N$), 그리고 KB군($0.87{\pm}1.46N$) 순으로 나타났다. 각 군 내의 초기 유지력과 2,500회의 착탈 후의 최종 유지력을 비교하였을 때, KB군을 제외한 모든 군에서 유의한 차이를 보였다(p<0.05). 어태치먼트 표면 마모양상은 Kerator와 O-ring의 matrix에서 눈에 띄는 변형과 손상이 관찰되었다. 각 patrix에서는 유지력 소실에 기여할 만한 변화는 관찰되지 않았다. 결론적으로 어태치먼트의 종류, 재료 그리고 초기 유지력 차이에 따라 마모양상이나 유지력 소실에 차이가 있음을 확 인하였고, 유지력이 보존되는 것처럼 보여도 SEM으로 관찰하였을 때 어태치먼트의 내부가 상당히 변형된 경우가 많았다. 따라서 적절한 유지력을 가진 어태치먼트를 선택하고 각 어태치먼트 시스템 별 적절한 교체시기를 숙지하여 그에따른 사용기간의 준수가 필요할 것으로 생각된다.

Videodensitometer를 이용한 치은연한소파술후 치간골 골밀도의 변화 (THE CHANGE OF INTERPROXIMAL BONE DENSITY ASSESSED BY VIDEODENSITOMETER AFTER SUBGINGIVAL CURETTAGE)

  • The purpose of this study was to evaluate the changes of interproximal bone density by means of videodensitometer and to examine the clinical applicability of videodentitometer to assess the periodontal disease activity.Twelve interproximal sites, with periodontal pockets deeper than 5mm and vertical loss of bone on standard dental radiograph, were treated by subgingival curettage. The papilla bleeding index, the plaque index, the degree of mobility, the depth of pockets, and the level of attachment were measured. Standardized reproducible radiographs were taken by using the occlusal stent with parallelling film holder. The density of the interdental bone was measured on the radiographs by a videodensitometer at three levels: the most 'superficial' level; the 'deep' level, arbitrarily 1.5mm below: and the 'apical' level, where no bony changes were to be expected. The clinical parameter and the radiographical change were measured at initial, and 1 month, 3 months, and 6 months after treatment.The results were as follows :1. The papilla bleeding index and the degree of mobility decreased significantly until 3 months after subgingival curettage and showed the Same level in the remaining experimental periods. 2. The pocket depth mainly decreased due to the gingival recession until 1 month after treatment, but to the attachment gain after 1 month. 3. The density of the interdental bone did not show a significance increase until 1 month after treatment, but showed a steady increase throughout the 6 months of observation. 4. The close relationships were shown between the decrease in pocket depth and the gain of attachment and the improvement of bone density at 6 months after treatment.

하악 2급 치근 이개부 병소에서 키토산 나노 차폐막을 이용한 치주조직 재생의 임상적 효과에 관한 비교 연구 (A comparative study of the clinical effects of chitosan nanofiber membrane in the treatment of mandibular class II furcation defects)

  • The purpose of this study was to evaluate the clinical efficacy of guided tissue regeneration(GTR) technique using chitosan nanofiber membrane and to compare it to the clinical efficacy following GTR using PLA/PLGA(copolymer of polvlactic acid and polylacticglycolic acid) membrane in mandibular class II furcation defects in human. The chitosan nanofiber membranes were applied to the mandibular class II furcation defects of 13 patients(test group) and PLA/PLGA membranes were applied to those of 11 patients(control group). Probing pocket depth, clinical attachment level, gingival recession, plaque index and gingival index were measured at baseline and 3 months postoperatively. Vertical and horizontal furcation defect depth were measured at surgery. Both groups were statistically analyzed by Wilcoxon signed Ranks Test and Mann-Whitney Test using SPSS program. The results were as follows: 1. Probing pocket depth, clinical attachment loss and gingival index were significantly reduced at 3 months postoperatively compared to values of baseline in both groups(p<0.05). 2. Gingival recession and plaque index were not significantly decreased at 3 months postoperatively compared to values of baseline in both groups. 3. No significant difference between two groups could be detected with regard to changes of probing pocket depth, gingival recession, clinical attachment level, plaque index and gingival index at 3 months postoperatively. In conclusion, chitosan nanofiber membrane is effective in the treatment of human mandibular class II furcation defects and a longer period study is needed to fully evaluate the outcomes.

Implant-supported overdenture with prefabricated bar attachment system in mandibular edentulous patient

  • Implant-supported overdenture is a reliable treatment option for the patients with edentulous mandible when they have difficulty in using complete dentures. Several options have been used for implant-supported overdenture attachments. Among these, bar attachment system has greater retention and better maintainability than others. SFI-Bar$^{(R)}$ is prefabricated and can be adjustable at chairside. Therefore, laboratory procedures such as soldering and welding are unnecessary, which leads to fewer errors and lower costs. A 67-year-old female patient presented, complaining of mobility of lower anterior teeth with old denture. She had been wearing complete denture in the maxilla and removable partial denture in the mandible with severe bone loss. After extracting the teeth, two implants were placed in front of mental foramen, and SFI-Bar$^{(R)}$ was connected. A tube bar was seated to two adapters through large ball joints and fixation screws, connecting each implant. The length of the tube bar was adjusted according to inter-implant distance. Then, a female part was attached to the bar beneath the new denture. This clinical report describes two-implant-supported overdenture using the SFI-Bar$^{(R)}$ system in a mandibular edentulous patient.

식편압입의 발현에 관한 임상적 연구 (A CLINICAL STUDY ON THE OCCURRENCE OF FOOD IMPACTION)

  • The purpose of this study was to investigate the causes of food impaction and to explore solutions as well. For this study, 39 patients with food impaction were selected. 77 contact areas in these patients were investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone loss, proximal caries, marginal ridge distance and occlusal relationships. The results were as follows ; 1. Teeth without distal support were found to be the most frequent site of food impaction (41.6%). Food impaction was found to be more frequent in the upper teeth (65.2%) than the lower teeth (33.8%). 2. Food impaction was found in tight contact cases (71.4%). Alveolar bone loss was not found in the early stage of food impaction (83.1%) 3. The distance between the marginal ridges of food impaction sites (mean=0.48mm) was short-er than that of the control group. (mean=0.77mm) (p<0.001) 4. In 18.2% of the cases, proximal carries were found at the food impaction site. 5. Food impaction affected patient's occlusion with the following frequencies, cusp to marginal ridge relationship (72.7%), cusp to fossa relationship (3.9%) and stepped relationship (23.4%).
