구치부 매복은 1% 이내로 드문 편이지만 저위 교합, 반대 악궁 치아의 정출, 인접치 경사로 인한 공간 부족 등의 문제가 발생하게 된다. 구치부 치아는 교합 및 악골 발육에 관여하는 기능적으로 중요한 치아이므로 초기 혼합 치열기부터 지속적인 방사선 사진을 찍어 매복 여부를 관찰해야 한다. 매복의 원인에는 치근 발육의 저하, 과잉치, 낭종, 치아종, 유착 등이 있다. 첫 번째 증례에서는 초진 시 치근 발육이 1/2이하라서 자발적 맹출을 기대하였지만 2/3이상 발육 시에도 맹출 하지 못하여 외과적 노출 및 교정적 견인을 시함하였다. 두 번째 증례는 낭종을 포함하는 매복 구치를 주소로 내원하였고 낭종 크기가 작고 인접 부위에 주요한 해부학적 구조물이 없어 완전한 적출술을 시행 후 교정적 견인을 하였다. 교정적 견인 시 해당 치아에 직접 어태치먼트(attachment)를 붙인 후 악하고 지속적인 힘을 적용하는 것이 유착, 외흡수, 치주 부착 소실 등의 부작용이 나타나지 않는다. 또한 고정식 장치를 사용하면 환자의 협조도가 필요 없고 맹출 공간을 확보하면서 호선상에 매복치를 견인 할 수 있다. 본 증례에서는 매복 구치를 외과적 노출 및 교정적 견인을 통해 교합상으로 배열을 하여 양호한 결과를 얻었기에 보고하는 바이다.
Purpose: The purpose of this study was to preliminarily evaluate the influence of diabetes mellitus (DM) on periodontal tissue without establishment of periodontitis. Methods: Seven-week-old db/db mice were used for the diabetic experimental group and systematically healthy mice of the same age were used as controls. After 1 week of acclimatization, the animals were sacrificed for hard and soft tissue evaluation. The pattern of bone destruction was evaluated by stereomicroscope evaluation with alizarin red staining and radiographic evaluation by microscopic computerized tomography images. Histological evaluation was performed with hematoxylin and eosin stain for evaluation of soft tissue changes. Results: In both stereomicroscope evaluation and radiograph image analysis, aggressive form of bone destruction was observed in diabetic animals when compared to the systematically healthy controls. In histological evaluation, apical migration of junctional epithelium with slight inflammatory cell infiltration was observed with disarrangement of connective tissue fibers. Conclusions: Within the limits of this study, diabetic animals presented distortion in periodontal attachment and an aggressive bone loss pattern when compared to the healthy controls, suggesting that DM has an independent effect on periodontal tissue destruction irrespective of the presence or absence of periodontal disease.
Given the recent surge of interest towards utilization of renewable distributed energy resources (DER), in particular in remote areas, this paper aims at designing an optimal hybrid system in order to supply loads of a village located in Esfarayen, North Khorasan, Iran. This paper illustrates the optimal design procedure of a standalone hybrid system which consists of Wind Turbine Generator (WTG), Photo Voltaic (PV), Diesel-generator, and Battery denoting as the Energy Storage System (ESS). The WTGs and PVs are considered as the main producers since the site's ambient conditions are suitable for such producers. Moreover, batteries are employed to smooth out the variable outputs of these renewable resources. To this end, whenever the available power generation is higher than the demanded amount, the excess energy will be stored in ESS to be injected into the system in the time of insufficient power generation. Since the standalone system is assumed to have no connection to the upstream network, it must be able to supply the loads without any load curtailment. In this regard, a Diesel-Generator can also be integrated to achieve zero loss of load. The optimal hybrid system design problem is a discrete optimization problem that is solved, here, by means of a recently-introduced meta-heuristic optimization algorithm known as Lightning Attachment Procedure Optimization (LAPO). The results are compared to those of some other methods and discussed in detail. The results also show that the total cost of the designed stand-alone system in 25 years is around 92M€ which is much less than the grid-connected system with the total cost of 205M€. In summary, the obtained simulation results demonstrate the effectiveness of the utilized optimization algorithm in finding the best results, and the designed hybrid system in serving the remote loads.
Purpose: Rheumatoid arthritis (RA) is a chronic multi-systemic disease that causes damage to the bone and connective tissues. This study was conducted in order to accurately measure the correlation between RA and periodontitis, and to obtain an unbiased estimate of the effect of RA on periodontal indices. Methods: In this historical cohort study, which was conducted from February to May 2011 in Hamadan city, Iran, 53 exposed people (with RA) were compared with 53 unexposed people (without RA) in terms of clinical periodontal indices (the outcomes of interest) including 1) plaque index (PI), 2) bleeding on probing (BOP), and 3) clinical attachment loss (CAL). Results: A sample of 106 volunteers were evaluated, 53 rheumatoid versus 53 non-rheumatoid subjects. There was a statistically significant correlation between RA and BOP (P<0.001) and between RA and CAL (P<0.001). However, there was no statistically significant correlation between RA and any of the periodontal indices. No correlation was seen between gender and any of the indices either. There was a strong positive correlation between age and all three periodontal indices (P<0.001). Conclusions: The present study indicated a potential effect of RA on periodontal indices. However, much more evidence based on a prospective cohort study is needed to support the cause and effect relationship between RA and periodontal indices.
The feasibility of using stress-wave technique in the transverse direction for the assessment of early stages of decay was investigated using compression test specimens having different annual ring orientations subjected to decay by Tyromyces palustris for various time intervals. Decay detection, quantitative assessment of decay, and the prediction of residual strength of decayed wood with less than five percent weight loss can be feasible using stress-wave parameters (wave velocity, wave impedance, and stress-wave elasticity) and their percent reduction due to decay, measured by stress-wave technique in the transverse direction. The use of stress-wave technique in the transverse direction for the application of this technique to structural members in service is desirable, when considering the easiness of attachment of accelerometers of stress-wave measuring device on the surface of members and also accurate detection of localized decayed areas. In stress-wave technique in the transverse direction, stress-wave parameters measured were different according to the angles between wave propagation path and annual ring, due to the anisotropy of wood structure. Therefore, it is recommended to use percent reduction in stress-wave parameters instead of stress-wave parameters. This evaluation method using percent reduction in stress-wave parameters is ideal when it is impossible to observe annual ring orientation on the transverse surface of wood.
Purpose: The aim of this study was to compare the clinical outcome of open flap debridement (OFD) with a biphasic calcium phosphate (BCP) graft to that of OFD without BCP graft for the treatment of intrabony periodontal defects (IBDs). Methods: The study included 25 subjects that had at least one intrabony defect of 2- or 3-wall morphology and an intrabony component ${\geq}4$ mm as detected radiographically. Subjects were randomly assigned to treatment with (BCP group, n=14) or without BCP (OFD group, n=11). Clinical parameters were recorded at baseline and 6 months after surgery and included the plaque index, gingival index, probing depth (PD), clinical attachment level (CAL), and gingival recession (REC). A stringent plaque control regimen was enforced for all of the patients during the 6-month observation period. Results: In all of the treatment groups, significant PD reductions and CAL gains occurred during the study period (P<0.01). At 6 months, patients in the BCP group exhibited a mean PD reduction of $3.7{\pm}1.2$ mm and a mean CAL gain of $3.0{\pm}1.1$ mm compared to the baseline. Corresponding values for the patients treated with OFD were $2.5{\pm}0.8$ mm and $1.4{\pm}1.0$ mm, respectively. Compared to OFD group, the additional CAL gain was significantly greater in the patients in BCP group (P=0.028). The additional PD reduction was significant for the BCP group (P=0.048). The REC showed a significant increase in both groups, and the amount of recession was significantly smaller in the BCP group than OFD group (P=0.023). In radiographic evaluation, the height of the bone fill in the BCP group was significantly greater than OFD group. Conclusions: The clinical benefits of BCP found in this study indicate that BCP may be an appropriate alternative to conventional graft materials.
The aim of the study was to investigate the effect of surgical therapy combined with the usage of metronidazole gel in the treatment of juvenile periodontitis by comparing clinical indices of flap operation along with application of metronidazole gel and flap operation only. Comparing clinical indices of the baseline, 3 months after surgery, 6 months after surgery statistically, the results are as follows; 1. Bleeding on probing (BOP) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). BOP increased 0.9% in the control group 6 months after surgery, while in the experimental group, decreased 4.7% (p>0.05). However, there was no statistically significant difference in these groups (p>0.05). 2. Pocket probing depth (PPD) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). PPD increased 0.lmm in the control group 6 months after surgery, while in the experimental group, no increase of the depth could be observed (p>0.05). However, there was no statistically significant and difference in these groups (p>0.05). 3. Loss of attachment level(LOA) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). LOA increased 0.2mm in the control group 6 months after surgery, while in the experimental group, increased 0.3mm However, there was no statistically significant difference in these groups (p>0.05). In conclusion, flap operation was effective on the treatment of juvenile periodontitis. However, combined therapy of metronidazole gel could not give rise to any significant adjunctive effect on the treatment outcome.
Purpose: The aim of this study was to investigate the efficacy and validity of subgingival bacterial sampling using a retraction cord, and to evaluate how well this sampling method reflected changes in periodontal conditions after periodontal therapy. Methods: Based on clinical examinations, 87 subjects were divided into a healthy group (n=40) and a periodontitis group (n=47). Clinical measurements were obtained from all subjects including periodontal probing depth (PD), bleeding on probing (BOP), clinical attachment loss (CAL), and the plaque index. Saliva and gingival crevicular fluid (GCF) as a subgingival bacterial sample were sampled before and 3 months after periodontal therapy. The salivary and subgingival bacterial samples were analyzed by reverse-transcription polymerase chain reaction to quantify the following 11 periodontal pathogens: Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythus (Tf), Treponema denticola (Td), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Pavimonas micra (Pm), Campylobacter rectus (Cr), Prevotella nigrescens (Pn), Eikenella corrodens (Ec), and Eubacterium nodatum (En). Results: Non-surgical periodontal therapy resulted in significant decreases in PD (P<0.01), CAL (P<0.01), and BOP (P<0.05) after 3 months. Four species (Pg, Tf, Pi, and Pm) were significantly more abundant in both types of samples in the periodontitis group than in the healthy group. After periodontal therapy, Cr was the only bacterium that showed a statistically significant decrease in saliva, whereas statistically significant decreases in Cr, Pg, and Pn were found in GCF. Conclusions: Salivary and subgingival bacterial sampling with a gingival retraction cord were found to be equivalent in terms of their accuracy for differentiating periodontitis, but GCF reflected changes in bacterial abundance after periodontal therapy more sensitively than saliva.
In this work, we proposed a facile method to fabricate the three-dimensional porous copper current collector (3D Cu CC) for a Si-dominant anode in a Li-ion battery (LiB). The 3D Cu CC was prepared by combining chemical etching and thermal reduction from a planar copper foil. It had a porous layer employing micro-sized Cu balls with a large surface area. In particular, it had strengthened attachment of Si-dominant active material on the CC compared to a planar 2D copper foil. Moreover, the increased contact area between a Si-dominant active material and the 3D Cu could minimize contact loss of active materials from a CC. As a result of a battery test, Si-dominant active materials on 3D Cu showed higher cyclic performance and rate-capability than those on a conventional planar copper foil. Specifically, the Si electrode employing 3D Cu exhibited an areal capacity of 0.9 mAh cm-2 at the 300th cycles (@ 1.0 mA cm-2), which was 5.6 times higher than that on the 2D copper foil (0.16 mAh cm-2).

Various options have been introduced for the selection of implant overdenture attachments. Attachment wear due to the repeated insertion and removal of dentures has caused problems such as decreased retention and the requirement for suprastructure remanufacturing. In these cases, a Locator bar system was applied using the drill and tapping technique to achieve total retrievability. In a 55-year-old female patient who showed three degrees of mobility in most of her teeth due to severe alveolar bone loss, a complete denture in the maxilla and an implant supported type overdenture in the mandible were planned after extracting all the remaining teeth. Six implants were placed from canine region to the distal molar region, and the locator was connected to the milled bar using the drill and tapping technique. For a 61-year-old female edentulous patient who complained of poor retention with old denture, a complete denture in the maxilla and an implant-tissue supported type overdenture in the mandible were planned. Four implants were placed in front of mental foramen, and the Locator was also connected to the Hader bar using the drill and tapping technique. With this technique, female parts can be easily replaced, and retention can be continuously maintained.
