Determinants of Intent to Leave among Workers at Long-Term Care Institutions (노인의료복지시설 근무자들의 이직의도 결정요인)

  • The purpose of this study is to investigate the determinants of intent to leave among workers at long-term care institutions in South Korea. The model represents a revision of the Price(2001) model which extends previous research by providing a comprehensive review of the economic, psychological, sociological literature. This study also examined the moderating effects of values toward independent variables on the job satisfaction and organizational commitment. The sample used in this study consisted of 400 workers from 9 long-term care hospitals and facilities nationwide. Data were collected with self-administered questionnaires and analysed using multiple regression analysis and path analysis. The main results of the study were as follows. The variance of job satisfaction and organizational commitment explained by the independent variables used in the study are 74.5% and 76.3% respectively. The results of this study revealed that the following variables have significant negative effects on intent to leave among workers; organizational commitment, job satisfaction, positive affectivity, supervisor support, job significance, job security, job growth. And the following variables have significant positive effects on intent to leave among workers; job opportunity, negative affectivity. This result is same when the moderating role of values were considered in the analysis. Among demographic variables, only age was found to have a significant positive effect on the intent to leave among workers.

  • Purpose: The purpose of this study was to explore the experiences of family caregivers who care for the elderly under Long-term Care Insurance. Methods: Data were collected using focus group interviews and analyzed using a phenomenological approach. The four focus groups consisted of eight caregivers, two social workers and three nurses in B city, Korea. Results: Five themes were identified: 'Obtaining a care-helper certification for employment', 'Taking care of the elderly in their homes', 'Difficulties due to life changes', 'Difficulties due to reduced wages' and 'Dissatisfaction with the Long-term Care Insurance operating system'. Conclusion: The results of this study demonstrate that the long-term care system for family caregivers faces many systematic challenges in providing care for the elderly harmoniously in their home. To help them succeed in their tasks, Long-term Care Insurance system must offer respite and support programs to family caregivers.

Demand Prospect of Professional Workers in Execution of the Elderly's Long-Term Care Insurance -Nurse, Social Welfare, Care Probation Professional workers- (노인 장기요양 보험제도 실시에 따른 전문직종사자의 수요 예측 -간호사, 사회복지사, 요양보호사를 중심으로-)

  • The purpose of this paper is to prepare the department creation and the staff increase of education institution by forecasting the demand of health and welfare workers according to execution of the elderly's long-terms treatment insurance system in July, 2008. We estimated confirmors of the elderly's long-terms treatment and the demand of professional workers of health and welfare field due to the increase of care insurance users in facilities treatment organization, care at home service facilities, and family allowance facilities for 2010-2030. The numbers of social welfare professional worker are estimated as 16,624 workers in 2020 and 24,688 workers in 2030. The numbers of nurses are forecasted as 11.287 in 2020 and 16,624 in 2030, we expect that the increase of the demand be accelerated. The demand of necessary care probation worker is 44,824 in 2009, but we already trained over around 500,000 workers in 1,078 education institutions through one year in Aug. 31, 2009, which excesses over the numbers of workers demanded as much as 10 times.

The Effect of Managerial Leadership on the Care Burden of Care Workers -Focus on the Leadership of Top-leader and Leadership of Mid-level Leader- (관리자의 리더십이 요양보호사의 케어부담감에 미치는 영향 -시설장의 리더십과 중간관리자의 리더십을 중심으로-)

  • The purpose of this study was to investigate the influence of the leadership of the top-leader and the mid-level leader on the care burden of care workers. A survey was conducted on 400 care workers working in long-term care facilities in Seoul and Gyeonggi area. We conducted a hierarchical regression analysis to grasp the effect of the leadership of top-leader and mid-level leader on the care burden of care workers. Monthly income of care workers was significantly associated with care burden. In next step, the leadership of the top-leader affected the care burden of care workers. But, In last model 3, only the leadership of mid-level leader affected the care burden with monthly income. This findings suggested that it is urgent to stabilize the salary level of care workers, and to develop the leadership skills of mid-level leader. The improved leadership of Mid-level Leader will help to reduce jop chaos for care workers.

Effect of the ADDIE Model-based Distance Infection Control Education Program on Infection Control Performance of Care Workers

  • This study examined the effect of the distance Infection Control Education Program (ICEP), developed based on the ADDIE model, on infection control knowledge, attitude, and performance among care workers in long-term care facilities nationwide. The program, developed based on the ADDIE model, was applied to 173 care workers directly responsible for nursing care of elderly residents in lomg-term care facilities. The distance ICEP for care workers was conducted through the website and lasted 30 minutes for each of the eight topics. To determine the effectiveness of the education, infection control knowledge, attitude, performance, and satisfaction were surveyed before and four weeks after the program. Differences in infection control knowledge, attitude, and performance before and after the distance ICEP were assessed by a t-test. A significant difference was observed in knowledge and infection control performance after the distance ICEP was administered to care workers. In the sub-domains of infection control performance, overall understanding of infection, regular infection control education, infection control by special pathogen (multidrug-resistant bacteria, tuberculosis, tick-borne infectious diseases), and detailed infection control education by infection site (pressure ulcers and urinary tract infections) were significantly improved. Infection control knowledge and performance improved through the distance ICEP applied to care workers. Satisfaction also displayed high scores on most items and indicated that it was helpful for infection control in facilities, confirming the effectiveness of infection control education. Based on the survey of care workers nationwide, the infection education program can be effectively used for care workers in the future.

Acceptance Measure of Quality Improvement Information System among Long-term Care Workers: A Psychometric Assessment (장기요양인력의 질 향상 정보시스템 수용 측정도구: 신뢰타당도 평가)

  • Purpose: We evaluated the psychometric properties of a questionnaire on the acceptance of the quality improvement information system (QIIS) among long-term care workers (mostly nurses). Methods: The questionnaire composes of 21 preliminary questions with 5 domains based on the Technology Acceptance Model and related literature reviews. We developed a prototype web-based comprehensive resident assessment system, and collected data from 126 subjects at 75 long-term care facilities and hospitals, who used the system and responded to the questionnaire. A priori factor structure was developed using an exploratory factor analysis and validated by a confirmatory factor analysis; its reliability was also evaluated. Results: A total of 16 items were yielded, and 5 factors were extracted from the explanatory factor analysis: Usage Intention, Perceived Usefulness, Perceived Ease of Use, Social Influence, and Innovative Characteristics. The five-factor structure model had a good fit (Tucker-Lewis index [TLI]=.976; comparative fit index [CFI]=.969; standardized root mean squared residual [SRMR]=.052; root mean square error of approximation [RMSEA]=.048), and the items were internally consistent(Cronbach's ${\alpha}=.91$). Conclusion: The questionnaire was valid and reliable to measure the technology acceptance of QIIS among long-term care workers, using the prototype.

  • This study compared policies of elderly abuse of long-term care facilities on WHO, Main countries for improvement of Korean policy. So, This study reviewed policies of elderly abuse of long-term care facilities on WHO, Main countries. So, This study reviewed risk factors that may increase the potential for abuse of an older person can be identified at individual level, care-woker levels, socio-cultural levels and facilities levels. The results of the study were as follows: First, The openness of long-term care facilities are emphasized on many countries. There are a need for a policy considerations such as policies of Ombudsman, Adult guardianship for visiting facilities, external monitoring. Second, There was a difference of facilities infrastructure and workers of treatment condition long-term care facilities each other countries. So It is important to improve facilities infrastructure and workers of treatment condition. Third, It is necessary to consider traits of elderly in long-term care facilities. Especially, it is required to manage dementia elderly and elderly on night time in long-term care facilities. Finally, implications and future directions of policies of elderly abuse of long-term care facilities were discussed based on the finding of the study.

  • Purpose: The purpose of this study was to understand the experiences of those who take care of the elderly at a long-term care facility during the COVID-19 pandemic. Methods: Colaizzi's phenomenological research method was applied to describe the meaning and nature of the experiences of eight workers in an elderly care facility during the COVID-19 crisis. Results: Studies have shown that these workers experienced 'fear from infectious disease' and had the opportunity to 'reflect upon life changed by infectious disease.' They also worked together to overcome the crisis by 'protect nursing home with the nurses at COVID-19 sites' and felt 'safety from the nursing home,' and experienced 'a strong sense of calling for the job' through the crisis. Conclusion: This study is the first of its kind conducted in Korea on workers in an elderly care facility. The results indicate that it is necessary to supports these workers with practical useful education, counseling, and emotional support during the epidemic.

  • Currently in Germany, there is talk of 'state of emergency care', which is addressed to the problems of quality assurance in aged care and shortage of aged care workers. In order to solve this problem in the aging German society, the federal government has set itself the goal of providing high qualified care givers through a systematic and on a high level of professional training opportunities. Various projects and measures have been carried out to improve the reputation of the primarily care profession in society and for the purpose of attracting especially young trainees for the aged care professions. The present work considers training and qualifications in the long-term care sector in Germany; it points out both the characteristics of aged care education and the learning content in the aged care education and attempts to highlight what roles and perception of tasks contained therein.

A Relationship of Care Time with Functional Status and Patients Characteristics among Patients in Long-term Care Hospitals (장기요양환자에서 환자 특징 및 기능상태와 환자돌봄 시간과의 관련성)

  • Objectives : The aim of this study was to investigate the functional status variables related to the care time of health professionals for patients in long-term care facilities. Methods : The functional stati of 1001 patients in 8 long-term care hospitals were examined by the Resident Assessment Instrument for Long-term Care Facility Version 2.0. The care time of health professionals for patients was calculated using data from a self-reported task survey by nurses, auxiliary nurses, private aides, doctors, physiotherapists and social workers. Results : The average care time per diem was 240.6 minutes. The care time by doctors, nurses and private aides were 11.0, 71.0 and 139.5 minutes, respectively. The lower the function of activities of daily living (ADL) and the greater the symptoms of extensive services, special care and clinical complexity, the more care time was served. On the contrary, the greater the symptoms of nursing rehabilitation, depression, cognitive disorder, behavior problem and psychiatry/mood disorder, the less care time was served. Age and gender were not significantly related to the care time. Conclusions : Developing a case mix classification system for elderly long term care patients may be helpful for both of patients and health care providers. The ADL, extensive services, special care and clinical complexity of variables should be considered in the development of a case mix system for the long term care of patients in Korea.
