Efficacy of Alismatis Orientale Rhizoma on Obesity induced by High Fat Diet (고지방식이로 유발된 비만에 대한 택사의 항비만 효과)

  • Objectives : The researcher investigated the anti-obesity effect of Alismatis Orientale Rhizoma(AP) water extract in mice fed a high fat diet and focused on the analysis of local area adipose tissue. Methods : Male ICR mice were divided into three groups, which were fed either a normal AIN diet, a 45% high fat diet (CT group), or a high fat diet and orally administration with a concentrations of 100 mg/kg (AP100 group) and 300 mg/kg body weight (AP300 group) for eight weeks. Results : As compared with CT group, AP100 group showed significant reductions in absolute weight of liver. As compared with CT group, AP100 group and AP300 group showed significant reductions in weight gain and relative weight of total fat. AST, triglyceride, total-cholesterol levels from the AP 100 group and 300 group were significantly lower than those of the CT, and ALT, LDL-cholesterol levels from the AP 100 group was significantly lower than those of the CT. But serum HDL-cholesterol levels from the AP 100 group and 300 group were significantly higher than those of the CT. And serum adiponectin levels from the AP 100 group was significantly higher than those of the CT. In result of real time PCR, all mRNA expression(PEPCK, G6Pase, PGC-$1{\alpha}$ and $ERR{\gamma}$) of two experimental groups were significantly decreased compared to those of CT group. The treatment with AP on local abdominal area made a fat cell size lessen on the fat tissue in the abdominal cavity and subcutaneous area. Conclusions : These results suggest that AP has an anti-obesity effect and the effect is mediated by inhibition of fat gain.

The Herbal Composition GGEx18 from Laminaria japonica, Rheum palmatum, and Ephedra sinica Inhibits High Fat Diet-Induced Obesity by Regulating Appetite Genes

  • The herbal composition Gyeongshingangjeehwan 18 (GGEx18), which is composed of three herbs, Laminaria japonica Aresch (Laminariaceae), Rheum palmatum L. (Polygonaceae), and Ephedra sinica Stapf (Ephedraceae), has been used as an anti-obesity drug in Korean local clinics. Thus, we investigated whether GGEx18 regulates obesity by suppressing appetite in high fat diet-induced obese C57BL/6J mice. Administration of GGEx18 to obese mice for 9 weeks significantly decreased body weight gain, epididymal adipose tissue weight, and food efficiency ratio. GGEx18 also caused a significant decrease in the circulating levels of leptin, which were increased by about 450% in obese control mice compared with normal lean mice. Concomitantly, GGEx18 decreased mRNA levels of a potent appetite-stimulating hormone neuropeptide Y, but increased an appetite-suppressing hormone pro-opiomelanocortin mRNA levels. These results suggest that GGEx18 may prevent obesity through regulating appetite in nutritionally obese mice.

Evaluation of Obesity from BMI and Waist Circumference, and Its Relation with Cardiovascular Risk Factors (건강검진 수진자들의 비만유형과 관상동맥질환 위험인자와의 관련성)

  • Objectives: The objective of this study was held for prevention of coronary artery disease and improvement of health of local community residents by classifying the obesity types of their waist circumference and BMI, and appraising the coronary artery disease risk factors(CRF). Methods: We analyzed the data on the 1,914 adult cases (1,156 male and 758 female) during Nov, 2006 to Mar. 2007 on a general hospital in Daegu city, Korea. The obesity types in this research were classified into normal group, seeming obesity group, abdominal obesity group and obesity group. Also, CRF was classified by normal, mild, moderate, and severe, and each class was given the index from 0 to 3. The coronary artery disease risk factors index(CRFI) was defined as the sum of index, and we defined that if the sum is higher, higher chance of coronary artery disease risk exist. Results: According to the research, by the age group, normal group has higher percentage in age 30 to 40, and obesity group and abdominal obesity group has higher percentage in age 50 to 60. CRFI is increased by order of normal group, seeming obesity group, abdominal obesity group, and obesity group on both male and female groups, and male shows higher index than female on all four groups. CRFI will be influenced by order of obesity group, seeming obesity group, and abdominal obesity group on male, and obesity group, abdominal obesity group, and seeming obesity group on female according to the result of multiple regression between obesity type and CRFI. Conclusion: According to the result from our research, we have to have keen attention to not only seeming obesity group and obesity group, but also abdominal obesity group which has normal waist circumference influence to CRFI. So, those kinds of indexes have to be controled by controling their weight. Also, we believe that health behaviour can improve and CRF can be prevented by the early health care and early health education to those with no abnormal indication on clinical indicator but with abnormal BMI and waist circumference.

The Development of Obese Pattern Identification Questionnaire for Uzbekistan (우즈베키스탄 비만변증 설문지 개발)

  • Objectives: The purpose of this study was to develop Russian version of Korean obese pattern identification questionnaire (KOPIQ) and classify the pattern of Uzbekistan obese patients. Methods: This study was conducted from 10 September 2014 to 10 December 2014 in Korea-Uzbekistan Oriental Hospital. The KOPIQ was translated to Russian language with the help of local expert in Russia and Korean Medicine. The patients who visited obese clinic in hospital were guided to join this study and informed consent was obtained. The inclusion criteria was >$23kg/m^2$ in body mass index. The Cronbach's alpha was used for it's inter consistency reliability and the KOPIQ result was compared with the experts one. Results: The Russian version of KOPIQ was developed. The 103 patient (25 males, 78 females; average age 57.2 years) joined in this study. The Cronbach's alpha of questionnaire was 0.787~0.883 according to individual pattern. The agreement rate of pattern Identification between local expert and KOPIQ was 13%. This developed questionnaire was realized as web version, which could be easily used in Uzbekistan. Conclusions: The Russian version of KOPIQ is developed in this study with suitable reliability. Further study is needed for KOPIQ to be applied in Uzbekistan with high validity.

Study of Experimentations and Clinical Trials' Trends for Obesity Treatment using Pharmacupuncture (약침을 활용한 비만연구의 실험실적 및 임상적 연구동향 고찰)

  • Objectives The objective of this study was to contribute to the development of pharmacupuncture for obesity treatment by reviewing the studies of pharmacupuncture experimentations and clinical trials. Methods We searched the papers with keywords of 'obesity' and 'pharmacupuncture' in the search site, RISS, Oriental medicine advanced searching integrated system(OASIS), Korean traditional knowledge portal, the society of Korean medicine for obesity research, Korean pharmacupuncture institute, the Korean academy of oriental rehabitation medicine and Korean acupuncture & moxibustion society. Results 1. We reviewed 37 articles searched. 27 articles(73.0%) were animal experimentations, 5 articles(13.5%) were cell experimentations, 4 articles(10.8%) were clinical trials and 1 article(2.7%) was study analysis. 2. The herbs, using for animal experimentations, were atratylodes japonica, coix lachrymajobi, ephedra sinica, crataegus pinnatifida, wild ginseng and etc. Acupucture points were joksamni(ST36), zhongwan(CV12), gansoo(BL18), pungnyung(ST40), umnungchon(SP9), bisu(BL20), gokji (LI11), cheun-chu(ST25) and etc. 3. For cell experimentations, preadipocytes and adipocytes performed on cell cultures with using rats, 3T3-L1 preadipocytes and porcine skin including fat tissue were treated with fel ursi, bovis calculus, ephedrae herba, spirodelae herba, wild ginseng. 4. For clinical trials, Sangsik no.1, Bigiheo, ephedra, green tea and sweet bee venom were injected at the region where a lot of fat like zhongwan(CV12), xiawan(CV10), kwanwon(CV4), cheun-chu(ST25) and thigh. Conclusion Through animal and cell experimentations and clinical trials, the treatment of obesity using local acupuncture therapy was effective. For clinical use, however, it is considered that animal and cell experimentation and clinical trial's connection using one kind of herb and studies about more clinical trials and associated side effects are needed.

  • This study is to investigate the effects of demographic characteristics and lifestyles on obesity in the community. The nurse measured the obesity level of 143 residents living in one apartment in Seoul, Korea. Demographics and lifestyles were analyzed by frequency and percentage and obesity level was analyzed by logistic regression analysis. In particular, as the age increases, the probability of obesity increases by 1.109 times. These results show that the development and application of community health promotion programs for obesity are important. Through this study, we discovered that obesity is significantly related to the demographic characteristics and lifestyles of the individual. And also, it is expected that these findings will be used as the basic information to plan a tailored obesity program taking into account local community environment and personal characteristics.

Oral surgery under local anesthesia with dexmedetomidine sedation in a morbidly obese patient with aortic dissection

  • We report a case of a morbidly obese man with an aortic aneurysm, in whom dental surgery was performed before elective cardiac surgery. His aortic aneurysm required emergency surgery. However, because of his morbid obesity, elective cardiac surgery was planned. Considering the high risk of infective endocarditis, dental surgery was required. Our patient was at a high risk of aortic rupture caused by hypertension and breathing difficulty in the supine position. Dexmedetomidine (DEX) is an anti-anxiety, sedative, and analgesic medicine that can stabilize circulatory dynamics and minimize blood pressure fluctuations. We administered intravenous DEX for sedation of the patient in Fowler's position. In conclusion, our understanding of the risk factors of DEX enabled us to perform safe invasive oral treatment.

A Comparison of the Depressive Predisposition between Obese and Non-Obese Elderly (노년기 비만군과 정상 체중군 간의 우울 성향 비교)

  • Objective : Obesity has been considered to be associated with numerous physical, mental and psychological diseases. Depression, which is a major psychological factor affecting occurrence and treatment of obesity, can be a cause of obesity as well as can be triggered by obesity. This study aimed to find out the need of positive consideration to the existence of depressive mood in the management of obesity. Methods : A total of 101 subjects were enrolled among those who were 60 years old and over at one local clinic. Obese group (n=49) was defined as BMI${\geqq}$25 and normal weight group (n=52) as 20$5.2{\pm}2.5$) was significantly higher than that of the normal weight group ($3.8{\pm}1.7$)(p<0.05). In male subjects, there was no significant difference in the mean GDSSF-K score between both groups, but in female subjects, the mean GDSSF-K score of the obese group, $6.4{\pm}2.6$ was significantly higher than that of the normal weight group ($4.0{\pm}1.8$)(p<0.05). Conclusion : The elderly obese group showed higher depressive scale score than elderly normal weight group, especially in female elderly. Since the elderly obese female group had a depressive predisposition, comprehensive management including mental and psychological approach is required in obesity control programs.

Current Status and Improvements of Obesity Related Legislation (비만 관련 법령 현황 및 개선방향)

  • Great attentions have been given to obesity in Korea, while the obesity rate has rapidly risen up due to various reasons including the dietary change into the western style. Central and local governments have tried to reduce the rate through various policies and action plans. The governments' activities are frequently criticised to be more coordinated in order to achieve the aim of the activities. The purpose of this study was to develop the coordination strategies through the examination of the acts relating to obesity. At present, activities relating to obesity are based on 25 acts including 'Health Promotion Act', 'Special Act on Children Diet Management' and so on. Six ministries including the Ministry of Health and Welfare and the Ministry of Education, Science and Technology are in charge of these acts. As a result, a number of areas, especially dietary guidance, research, manpower and institutions, seem to overlap. In a short term, Ministerial Council on Obesity should be established in order to enforce the coordination of the policies and activities. In the mid-long term, it is recommended that functions of the Ministries should be more simplified through the reorganization.

The Effect of Nutrition Education on Weight Control in Male Workers by the Community Health Center (보건소에서 실시한 직장인 비만 영양교육의 효과)

  • The principal objective of this study was to determine the importance of obesity management nutrition programs for obese male workers after performing a 10-week nutrition program and to compare changes in the obesity index, health behavior, meal intake, and blood constituents. The study subjects included 44 individuals, all without chronic disease. After the program, the weight, body mass index (BMI), body fat percentage, and waist-hip ratio (WHR) of the subjects dropped. Dietary life behavior scores increased. In particular, more than 30% increases were detected in self-control of surfeit, control of eating speed, temperance, and regular exercise items. Total caloric intake and average drinking intake dropped. Though insignificant, systolic pressure, total cholesterol, LDL-cholesterol, and sGPT all dropped as compared to the levels measured prior to the program. Fasting blood sucrose, triglyceride, $\gamma$-GTP, and atherogenic index dropped significantly. HDL-cholesterol, though insignificant, increased. As the result of examining correlations between the subjects' health life behavior, caloric intake, and improvement in obesity, we noted a significant correlation between self-control of surfeit and temperance in the health life behavior item, along with significant correlations among total calorie intake, surfeit percentage, and daily average drinking intake. These results demonstrated that nutrition program for obese workers performed at the Community Health Center had the effect of reducing obesity, improving health behavior, and improving clinical levels. Thus, it is advisable that obesity management programs be achieved, linking the center of local societies' health promotion projects, community health centers, and workplaces, and various programs should be developed to maintain and continue changes in workers' behaviors, such that the positive effects of improvement in obesity could be maintained, even after the program.
