Objectives: This study aims to develop a community care model in traditional Korean medicine (TKM) by developing a community care participation model for the health of the elderly and deriving tasks to implement it. Methods: This study implemented a group interview with experts. A fact-finding survey was conducted targeting 16 local governments that are implementing a leading project to identify the status of TKM service provision and welfare service linkage in all regions. An expert group interview (FGI) targeted public and private sector experts for each job role, the former represented by those in charge of the central government's health care policy and administrative delivery system, and the latter by professors majoring in social welfare, professors majoring in health, and local TKM societies. After forming the expert groups, three expert group interviews were conducted. Results: Through collective interviews with experts, a model for providing TKM and welfare services in community integrated care was derived by dividing it into local and central government levels. The strategies and tasks for promoting TKM-oriented health welfare services were derived from 3 strategies, 8 tasks, and 20 detailed tasks. Conclusion: The core direction of the TKM health care model is the region-centered provision of TKM and welfare services. To this end, policy support for the use and linkage of health care service resources is required at the central government level, and linkage and provision of health welfare services centered on TKM are necessary through linkage and convergence between service subjects and between government health care projects.
Objectives: In Indonesia, 61 million adults smoked in 2018, and 59 million were exposed to secondhand smoke at offices or restaurants in 2011. The Presidential Decree 109/2012 encouraged local governments to implement a smoke-free policy (SFP), and the city of Jayapura enacted a local bill (1/2015) to that effect in 2015. This study aimed to evaluate compliance with this bill and to explore challenges in implementing it. Methods: We conducted a mixed-methods study. Quantitatively, we assessed compliance of facilities with 6 criteria (per the bill): the presence of signage, the lack of smoking activity, the lack of sale of tobacco, the lack of tobacco advertisements, the lack of cigarette smoke, and the lack of ashtrays. We surveyed 192 facilities, including health facilities, educational facilities, places of worship, government offices, and indoor and outdoor public facilities. Qualitatively, we explored challenges in implementation by interviewing 19 informants (government officers, students, and community members). Results: The rate of compliance with all 6 criteria was 17% overall, ranging from 0% at outdoor public facilities to 50% at health facilities. Spatial patterning was absent, as shown by similar compliance rates for SFP facilities within a 1-km boundary around the provincial and city health offices compared to those outside the boundary. Implementation challenges included (1) a limited budget for enforcement, (2) a lack of support from local non-governmental organizations and universities, (3) a lack of public awareness at the facilities themselves, and (4) a lack of examples set by local leaders. Conclusions: Overall compliance was low in Jayapura due to many challenges. This information provides lessons regarding tobacco control policy in underdeveloped areas far from the central government.
Objectives: This paper reviews the integrated health promotion program in health center to analyse the problems and to provide the future directions aiming to health promotion block grants. Methods: The guidebook for the integrated health promotion program published by the Minister of Health and Welfare were reviewed in this study. And also the empirical results and theoretical considerations were used in making future directions. Results and Conclusion: This paper suggested four improvement directions as follows; first, the legal review committee is needed to approve when the central government change the way of health promotion program in health center. Second, the required programs should be replaced by an integrated outcom evaluation indicators. Third, the central government should strengthen the capacity of local government. And finally, visiting training should be conducted each health center.

Objectives: Based on the concept of environmental justice, we developed an evaluation model for setting and adopting the direction of environmental health policy using environmental health indicators and statistics from the local governments of Seoul, including environmentally susceptible populations. Methods: We selected a total of 20 variables based on data officially released from national and local governments. After the classification of these variables into the five components of environmental pressure, environmental status, environmental disease, sensitive population, and socioeconomic status, a basic model was constructed to calculate the relative scores of the local governments. Results: The cumulative impact scores for assessment of environmental health status were similar to those of the environmental pressure and status components. The highest five cumulative impact scores fell between 114 and 147. Local government A reported the highest scores and had high environmental pressure, environmental status and socioeconomic status. In addition, the evaluation of the other four local governments indicated that they exhibited above average scores for environmental pressure and environmental status, two did so for environmental disease, and four for sensitive population. Conclusion: We constructed a model to evaluate the environmental health status of the local governments of Seoul based on cumulative impact scores under the concept of environmental justice. As an approach for studying environmentally vulnerable areas through relative ranking, this model was feasible for policy-setting. In addition, this approach would be an analytically useful tool for decision-makers.

Objectives: In this study, we analyzed vision statements of the 6th community health plan of local government in Korea. Methods: We examined vision statements letters, missions and strategy plans, and long-term missions of 6th community health plans of 229 local government in Korea. We also analyzed the numbers of vision letters, sentence examination, word frequency, each vision statement with frequency analysis, chi-square test, and one-way ANOVA. Results: Among 229 local government, 172(75.1%) of local government had the number of letters (Korean) less than 17 of vision statements, and there were a significant differences according to type of community health centers (p<0.05). Figuration (37.1%) were the most used in an expression of vision statement sentence, and special characters (43.2%) were the most used language except Korean. The most commonly used words of vision statement in order of frequency were 'health', 'happiness', 'with', 'citizen', 'city', '100 years old' etc. Chungcheong provinces and Daejeon metropolitan city had a highest score in directionality on phrase evaluation, and there were a significant differences according to regional classes of local government (p<0.01). Gyeongsang provinces, Ulsan, Daegu, and Busan metropolitan cities had a highest score in future orientation and sharing possibilities on phrase evaluation, and there were a significant differences according to regional classes of local government (p<0.01). Conclusions: Vision is one of the most important component of community health plan. We need more detailed 'vision statement guideline' and the community health care centers of local government should effort to make more clear and complete their vision.

This study is intending to look into the status of the hitherto established local governments light pollution control ordinance, and to use the ordinance status as basic data of ordinance enactment related to light pollution in time of its establishment and revision in accordance with regional characteristics of each local government for the time to come. This study conducted this research in a total of 3-phase research methods. 1) This study inquired into the direction of the research plan through the theoretical consideration and analysis of the preceding researches after setting up research objectives according to research background and purpose. 2) Targeting 11 local governments which are enforcing the hitherto established ordinance, this study surveyed the status of the light pollution control ordinance. 3) This study carried out the contents analysis of the ordinance in the method of doing a comparative analysis of each provision of the ordinance according to local governments. It's hoped that on the basis of this research, local governments across the nation, which have not yet established the same ordinance, could minimize the damage to natural eco-system due to artificial lighting by promoting the urgent ordinance establishment suited for each region, and create human-oriented light environment by modifying various disorderly lighting environment.
This study aims to compare and analyze the contents of the ordinance related to disability awareness improvement education enacted by the local government to derive problems and propose improvement measures. To this end, as of December, 2021, a content analysis was conducted on the ordinances of 17 municipal school boards across the country on supporting education to improve disability awareness. As a result of the analysis, the objectives, the responsibilities of the superintendent, and the establishment of the basic plan in the normative system of the ordinance met to some extent the requirements of the ordinance in accordance with the requirements of the ordinance, but in the effective system, the matters concerning the establishment of a board of education, the degree of professional personnel regulation, and the supervision of guidance were insufficient. Accordingly, in this study, based on the results of these studies, we proposed ways to improve the Disability Awareness Improvement Ordinance.
Objectives: The distribution of hospitals in Korea is unbalanced in terms of accessibility. Many local public health centers (PHCs) exempt out-of-pocket payments (OOPs) based on local government laws to increase coverage. However, this varies across administrative regions, as many make this exemption for the elderly, while others do not. This study aimed to evaluate the effects of the OOP exemption at local PHCs among elderly individuals. Methods: This study used online data on Korean national law to gather information on individual local governments' regulations regarding OOP exemptions. Individual-level data were gathered from the 2018 Community Health Survey and regional-level data from public online sources. Results: The study analyzed 132 regions and 44 918 elderly people. A statistical analysis of rate differences and 2-level multiple logistic regression were carried out. The rate difference according to whether elderly individuals resided in areas with the OOP exemption was 1.97%p (95% confidence interval [CI], 1.07 to 2.88) for PHC utilization, 1.37%p (95% CI, 0.67 to 2.08) for hypertension treatment, and 2.19%p (95% CI, 0.63 to 3.74) for diabetes treatment. The regression analysis showed that OOP exemption had an effect on hypertension treatment, with a fixed-effect odds ratio of 1.25 (95% CI, 1.05 to 1.48). Conclusions: The OOP exemption at PHCs can affect medical utilization in Korea, especially for hypertension treatment. The OOP exemption should be expanded to improve healthcare utilization in Korea.
