Who Wrote Huangdi Neijing?: The Authors' Status, Class and Political Ideology (『황제내경』의 저자는 누구인가?: 그들의 신분·계급 그리고 정치적 이념)

  • Objectives : The purpose of this paper is to clarify the social characteristics of the authors of Huangdi Neijing such as status, class, and political ideology. Methods : We analyze the Neijing text and the social order and historical situations in the Han dynasty. Results : Some authors of the Neijing were the local medical officers whose salary was 100~400shi. Their positions were medical craftsmen(yigong) or chief medical craftsmen(yigongchang). They would have published the Neijing after the administrative reforms(146-145 BCE) that began after the suppression of the Rebellion of the Seven Kingdoms. The bureaucrat yigong(chang) would have expected to participate in the public health policy of the empire or kingdom as an acupuncture expert. They would have also expected to contribute to the welfare and health of the privileged intellectual group and the public, hoping to ascend in status and class. Conclusions : By investigating the social characteristics of the authors who composed the Neijing, its various aspects would be newly understood.

The Effect of Tienchu Acupoint Block, Occipital Nerve Block, and Trigger Point Injection for Treatment of Tension Type Headache (긴장형 두통 환자에서 천주점, 후두신경차단 및 통증유발점주사의 치료효과)

  • Background: Despite tension type headache is popular, the exact mechanism and method of treatment are not certain yet. So, we supposed the basic mechanism of tension type headache is myogenic, and did Tienchu acupoint block, occipital nerve block, and trigger point injection in tension type headache patients. Methods: Fifty-seven tension type headache patients were treated with local anesthetics and small dose of steroid. The intensity of pain and effect of every treatment was evaluated as Verbal Rating Score (VRS; 0~10) before and after every treatment. Evaluation of treatment was based on the time of treatment (1,2 times, 3 times, 4,5 times). Goal of treatment was VRS reaching below two point and it was considered as treated state. Results: Symptom improvement rates of each treatment were 90% (1, 2 times), 91% (3 times), 70% (4, 5 times) respectively. VRS reduction more than 50% rates were 60%, 64%, 60% respectively. Treated state (VRS<2) rates were 33%, 27%, 30% respectively. Conclusion: Tienchu acupoint block, occipital nerve block, trigger point injection were sorts of most effective and simply applicable modalities of treatment in tension type headache.

Comparative Study of Beijiqianjinyaofang and Sunzhenrenqianjinfang: Focused on the Third Chapter of Limb Diseases (손사막의 『비급천금요방』과 『손진인천금방』과의 비교연구: 「권삼십침구·사지제삼」편을 중심으로)

  • Objectives : The purpose of this study is to identify changes of texts by investigating similarities and differences of the third chapter of limb diseases section between Beijiqianjinyaofang(BJQJYF) and Sunzhenrenqianjinfang(SZRQJF). Methods : I reviewed the third chapter of limb diseases section both of BJQJYF and SZRQJF and analysed the changes of texts. Results : 1. Hand, shoulder and low back pains mentioned in the second chapter of glossopathy from SZRQJF were moved to the third chapter of limb diseases in BJQJYF. 2. Inappropriate indications were changed reasonably. 3. Contents related with treatment were revised, by addition or deletion of contents. 4. There were some contents which were worth clinically in SZRQJF. 5. The rule of choosing acupoints for hand, arm, leg, knee and limb disease was selection of local points, and for shoulder and low back disease was selection of distant points. Conclusions : Classification and contents of the third chapter of limb diseases were re-organized systematically through proofreading by medical printing authority. However, some contents deleted from SZRQJF were worth clinically, and more studies are necessary to identify the reason why the indication and selection of acupoints were changed by proofreading.

Evaluation of Significance & Development of Quantitative Measurement System for Acupoint Pigmentation by Cupping Therapy (부항요법에 의한 배수혈 혈색소 변화의 정량적 측정 시스템 개발 및 유의성 평가)

  • Objectives : The aim of this study is to develop the system and evaluate the optical analytical technique that reflects acupoint pigmentation and extravasated blood by cupping. Methods : We designed the system able to express XYZ coordinate on local skin color. To evaluate measurement-accuracy, we compared with 24 Color Checker Chart by standardized Commission Internationale de l'Eclairage. After confirming the performance of system, we experimented with cupping which was 80 kPa negative pressure for 1 minute on left/right BL13. The X, Y and Z values were converted to R, G and B, $L^*$, $a^*$ and $b^*$, Erythema Index (E.I.), and Melanin Index (M.I). We compared and analyzed two cases on before/after cupping. Results : The R, G and B values which were converted by X, Y and Z values had high linearity as a high level of R-square (R: 0.969, G: 0.996, and B: 0.992). Moreover, we confirmed that it was possible to quantitatively analyzed the change in skin color by cupping using R, G, B, $L^*$, $a^*$, $b^*$, E.I., and M.I. Conclusions : Therefore, we proposed the new analytical technique for objectifying the oriental medical diagnostic method using cupping and optical sensing technique.

The Stomach's Communication with Its Related Acupoints, and the "Intelligent Tissue" Hypothesis

  • Background: The intelligent tissue hypothesis holds that an organ's states (be it normal function or stressed states) are reflected in real time at its related acupuncture points (acupoints), causing physical, real-time changes in the local tissue. The experiment was devised to test this. Methods: The patient drank chilled water while the impedance at 6 stomach-related acupoints was monitored in real time (sample rate of 1 kHz). Any changes in the local tissue at the acupoints ought to be reflected in changes in the impedance. Results: The impedance at every test acupoint showed a response to the chilled water being ingested. Also, the duodenal pacesetter and the stomach's slow waves were clearly visible in the impedance pattern at all the acupoints. Conclusion: Hence, many separate details of the stomach's function were reflected at these acupoints. The duodenal features were consistent with the traditional indications for these acupoints, which are noted to be able to treat intestinal conditions. Therefore, the results were consistent with the hypothesis and also provided a possible explanation for how the use of these acupoints is able to treat intestinal conditions.

Spatial and Temporal Electrodynamics in Acuzones: Test-Induced Kinematics and Synchronous Structuring. Phenomenological Study

  • Background: So far there is no confidence in the basics of acupoint/meridian phenomena, specifically in spatial and temporal electrical manifestations in the skin. Methods: Using the skin electrodynamic introscopy, the skin areas of 32 × 64 mm2 were monitored for spectral electrical impedance landscape with spatial resolution of 1 mm, at 2 kHz and 1 MHz frequencies. The detailed baseline and 2D test-induced 2 kHz-impedance phase dynamics and the 4-parameter time plots of dozens of individual points in the St32-34 regions were examined in a healthy participant and a patient with mild gastritis. Non-thermal stimuli were used: (1) (for the sick subject), microwaves and ultraviolet radiation applied alternately from opposite directions of the meridian; and (2) (for the healthy one) microwaves to St17, and cathodic/anodic stimulation of the outermost St45, alternately. Results: In both cases, the following phenomena have been observed: emergence of in-phase and/or antiphase coherent structures, exceeding the acupoint conditional size of 1 cm; collective movement along the meridian; reversible with a reversed stimulus; counter-directional dynamics of both whole structures and adjacent points; local abnormalities in sensitivity and dynamics of the 1 MHz and 2 kHz parameters indicating existence of different waveguide paths. Conclusion: It is assumed that these findings necessitate reconsideration of some basic methodological issues regarding neurogenic/acupuncture points as spatial and temporal phenomena; this requires development of an appropriate approach for identifying the acuzones patterns. These findings may be used for developing new approaches to personalized/controlled therapy/treatment.

A Literature Study On Physiology, Pathology and Clinical Applications for TCM Cosmetology (중의미용(中醫美容)의 생리병리, 임상응용에 관한 문헌고찰)

  • Objectives : This study was performed to investigate physiology, pathology and clinical applications for cosmetology in Traditional Chinese Medicine. We hope most clinicians practicing cosmetic acupuncture to understand theoretical background well and to treat cosmetic diseases more diversely by this paper. Methods : We collected useful informations from 35 papers in Chinese at website www.cnki.net about TCM treatments including acupuncture, moxibustion and herbal medicine. And we could see important factors connected with physiology, pathology and clinical applications within this field. Conclusions : Liver, lung, stomach, mental disorders and yin-deficiency constitution were connected each other in cosmetic physiology. Blood stasis and congestion were thought highly in cosmetic pathology. In clinical applications, wet cupping therapy on the acupoints located in the back line of Governor Vessel(GV), ear-acupuncture, acupuncture using local facial acupoints, pharmacopuncture, moxibustion were used diversely for each case with cosmetic disorders. Especially, local acupoints and remote acupoints were selected at the same time for wrinkle care. And studies on plastic surgery sequelae, dark skin, acne and melasma were more published than those on wrinkle care.

The Analysis of the Muscle Fatigue Recovery Effect on LR9 by Compound Stimulation of the PEMFs and LED (음포혈(陰包, LR9)에서 펄스형 전자기장 및 가시광 LED 복합 자극이 대퇴부 근피로 회복에 미치는 영향 분석)

  • Objectives : The aim of this study was to develop the combined medical stimulus system consisted of the PEMFs (Pulsed electromagnetic fields) and LED which are able to stimulate local point such as acupoints and trigger points. Methods : To evaluate the therapeutic effect on the musculoskeletal disorders and the possibility of alternative method on manual acupuncture, we compared the fatigue recovery of two groups by analyzing the EMG and peak torque (non-stimulation and, stimulation group) after strenuous knee exercise. We chose the LR9 as a stimulation point. Results : The median frequency (MF) and fatigue index (F.I) of the stimulation group were recovered faster than those of the non-stimulation group. Also the peak torques of both groups were not restored until after 20 minutes. However, the peak torque of the stimulation group was regained higher than that of the non-stimulation group. Conclusions : We confirmed that the proposed combined stimulus system had useful effects as treatment instrument of musculoskeletal disorder using non-invasive method of PEMFs and LED.

Acupuncture treatment of torticollis in a foal

  • A 6-month-old thoroughbred filly foal had torticollis and circled towards the right side. A local veterinarian treated her twice using dexamethasone for 1 week but there was little clinical improvement. Needles were inserted into Ting points of both hind limb and one ocular acupoint (shang jiao area from GB01). Injection acupuncture (dexamethasone, twice/week) was used at GV16, GB20, BL10 and LU07, for the neck and head. Acupoint GB34 was added to those points at session 4. In addition, a trigger point in the left neck was injected with 0.2 ml ($200{\mu}g$ of apitoxin) of bee-venom diluted with 1 ml of 2% lidocaine from session 1 to session 3. At session 2, the symptoms had ameliorated a little. At session 3, they were much improved; the right ear was completely normal and the neck could be moved about 60% of normal range. At session 4, nodding was possible and stiffness of the neck was much improved, having returned to about 80% of normal range. Three days after session 4, the symptom of torticollis had disappeared completely and the foal could walk in a straight line. In conclusion, the present patient was a case with equine torticollis which showed favourable therapeutic response by combination of needle-acupuncture plus injection-acupuncture with dexamethasone and apitoxin.

Effect of Moxibustion on Peripheral Facial Paralysis According to Selection Method of Acupoints (말초성(末梢性) 안면신경마비(顔面神經麻痺)의 취혈부위(取穴部位)에 따른 구치료(灸治療) 효과(效果) 비교(比較))

  • Objectives : This study was to compare the effect of moxibustion therapy on peripheral facial para- lysis according to location of acupoint. Methods : We investigated 41 cases of inpatients with Peripheral Facial Paralysis, and divided them into two groups, One group was treated by moxibustion on local point(ML), and the other was treated by moxibustion on distant point(MD). We evaluated the effect of moxibustion in each group by using Gross Grading System of House-Brackmann(HB score) and Yanagihara's Unweighed Grading System(Y score). Results : 1. In ML and MD, compared with baseline, at final, HB score was significantly decreased and Y score was significantly increased. 2. At final, ML showed significant decrease on HB score and significant increase on Y score compared with MD. Conclusions : These results suggested that ML should be more useful for improving symptoms related with peripheral facial paralysis than MD.
