The Texture Classification of Liver Parenchyma Using the Fractal Dimension and the Fourier Power Spectrum (프랙탈 차원과 퓨리에 파워스펙트럼을 이용한 간조직 분류)

  • In this paper, we proposed the 2-stage ultrasound liver image classifier which uses the fractal dimensions obtained from the original image and its 1/2 subsampled image, and the Normalized Fourier Power Spectrum. The fractal dimension based on Fractional Brownian Motion (FBM) is calculated from the variance of the same scale pixels instead of the mean of them. Since the actual ultrasound. liver images does not fully match the FBM, to get the fractal dimension, we use the scale vectors which satisfy the FBM model. In 2-stage classifier, we first classified normal and diffuse liver and then classified the fat liver and cirrhosis from the diffuse liver. For the test liver images. 70% of normal liver and 80% of fat liver and 90% of cirrhosis is classified classified with our 2-stage classifier.

Case report on the Improvement of Liver Functions by Mountain Cultivated Wild Ginseng Pharmacopuncture (산삼약침의 간 기능 개선효과에 대한 증례보고)

  • Mountain cultivated wild ginseng(MCWG) pharmacopuncture, which is known to have anti-cancer and anti-oxidation actions, was administered for clinical usage and changes in manifestations were observed on three patients suffering from pain attributed from liver dysfunctions. In two cases of hepatitis due to poisoning by drugs and one case of liver cirrhosis, long-term administration of MCWG pharmacopuncture yielded significant changes in AST and ALT, which are important indices for liver functions. Improved liver functions after the administration of MCWG pharmacopuncture suggest a need for investigation on future usage of MCWG pharmacopuncture on liver dysfunctions, hepatitis, liver cirrhosis, or liver cancer. Further studies on treating aforementioned disorders by MCWG pharmacopuncture are needed immediately.

Gene Regulations in HBV-Related Liver Cirrhosis Closely Correlate with Disease Severity

  • Liver cirrhosis (LC) is defined as comprising diffuse fibrosis and regenerating nodules of the liver. The biochemical and anatomical dysfunction in LC results from both reduced liver cell number and portal vascular derangement. Although several studies have investigated dysregulated genes in cirrhotic nodules, little is known about the genes implicated in the pathophysiologic change of LC or about their relationship with the degree of decompensation. Here, we applied cDNA microarray analysis using 38 HBsAg-positive LC specimens to identify the genes dysregulated in HBV-associated LC and to evaluate their relation to disease severity. Among 1063 known cancer- and apoptosis-related genes, we identified 104 genes that were significantly up- (44) or down- (60) regulated in LC. Interestingly, this subset of 104 genes was characteristically correlated with the degree of decompensation, called the Pugh-Child classification (20 Pugh-Child A, 10 Pugh-Child B, and 8 Pugh-Child C). Patient samples from Pugh-Child C exhibited a distinct pattern of gene expression relative to those of Pugh-Child A and B. Especially in Pugh-Child C, genes encoding hepatic proteins and metabolizing enzymes were significantly down-regulated, while genes encoding various molecules related to cell replication were up-regulated. Our results suggest that subsets of genes in liver cells correspond to the pathophysiologic change of LC according to disease severity and possibly to hepatocarcinogenesis.

Prognostic Value of $^{99m}Tc-Phytate$ Liver Scintigraphy in Patients with Liver Cirrhosis ($^{99m}Tc-phytate$ 간스캔을 이용한 간 경변증의 예후 평가)

  • To evaluate the prognostic significance of $^{99m}Tc-phytate$ liver scintigraphy in patients with cirrhosis, we measured the grade of extrahepatic uptake (EHU) of $^{99m}Tc-phytate$, from 0 (absent EHU) to 5 (important EHU), according to the relative distribution of the radiotracer among liver, spleen and bone marrow. The results were as follows: 1) EHU score was correlated to the Pugh score (r=0.64) and to survival. 2) The ROC curve on an observed status of death was superior for EHU score. In conclusion, $^{99m}Tc-phytate$ liver scintigraphy nay be a useful prognostic method in patients with liver cirrhosis.

  • In this paper, I proposed a classifier of liver cirrhotic step using T1-weighted MRI(magnetic resonance imaging) and hierarchical neural network. The data sets for classification of each stage, which were normal, 1type, 2type and 3type, were obtained in Pusan National University Hospital from June 2001 to december 2001. And the number of data was 46. We extracted liver region and nodule region from T1-weighted MR liver image. Then objective interpretation classifier of liver cirrhotic steps in T1-weighted MR liver images. Liver cirrhosis classifier implemented using hierarchical neural network which gray-level analysis and texture feature descriptors to distinguish normal liver and 3 types of liver cirrhosis. Then proposed Neural network classifier teamed through error back-propagation algorithm. A classifying result shows that recognition rate of normal is 100%, 1type is 82.3%, 2type is 86.7%, 3type is 83.7%. The recognition ratio very high, when compared between the result of obtained quantified data to that of doctors decision data and neural network classifier value. If enough data is offered and other parameter is considered, this paper according to we expected that neural network as well as human experts and could be useful as clinical decision support tool for liver cirrhosis patients.

  • Hepatic fibrosis/cirrhosis is characterized by increased production and deposition of collagen, noncollagenous glycaproteins, and proteoglycans, which mainly compose the extracellular matrix (ECM). Recently, activations of macrophages, myofibroblasts and mast cells are thaught ta be associated with the accumulation of ECM. In order to investigate the kinetics of macrophages, myofibroblasts and mast cells and the relationship between these cells and the accumulation of ECM in carban tetrachloride (CCl$_4$)-injected rat liver, we induced liver cirrhosis of rat by an injection of CCl$_4$ for 14 weeks. A case study on the effect of musical experience through Korean music, Samulnori with abdominal breathing on liver cirrhosis

  • The purpose of this article is to argue that the joy of musical experience through Samulnori with abdominal breathing plays significant role in improving liver cirrhosis. Samulnori is one of the most popular programs in music education in Korea. It helps fully express the player's emotions. The data are based on interview and observation drawn from field experience. Interviewee has continued to practice in charity performances of Samulnori in nursing homes once a month since 2011. Through learning and practicing, musical experience of Samulnori produces a large measure of physical health and psychological satisfaction. This article concludes in-depth interview, a patient with liver cirrhosis that has shown gradual improvement through Samulnori with abdominal breathing from 2011 to 2014.

  • Non-alcoholic fatty liver disease (NAFLD) is typically associated with obesity and insulin resistance. Non-alcoholic steatohepatitis (NASH) is a more serious form of NAFLD. Although fibrosis is common in pediatric NASH, cirrhosis has been rarely reported. Patients with hypothalamic or pituitary dysfunction are at risk for obesity and insulin resistance with subsequent development of NAFLD. We report a case of NAFLD progressing to cirrhosis in an obese 16 year-old boy with hypopituitarism.

Clinical Observation of Liver Scintigram (간(肝) Scintigram의 임상적(臨床的) 고찰(考察))

  • Although primary application of radioisotope scanning technics to the liver has been of use in the detection of the intra-hepatic space occupying lesion from the normal functioning liver parenchyme, there has been on increasing awareness of its use in evalution of Liver function. In this study, the diseases of the liver were classified into group A,B,C and D by the liver scanning findings, conventional liver function tests and clinical findings. Following were the results: 1. The colloidal radiogold liver scan appeared normal in the group A, also the albumin in serum, alkaline phosphatase activity and prothrombin time were within normal levels in this group. 2. In the group B, there were acute hepatitis 24(48%), chronic hepatitis 5(10%), toxic hepatitis 3(6%), subacute hepatic necrosis 3(6%), typhoid liver 4(8%), hepatic tuberculosis 2(4%), diabetes mellitus 3(6%) and others 3(6%). In this group, SGOT and SGPT were increased predominantly as compared with group A, and the liver scan showed small amount of mottling of activity and faintly visualized spleen. 3. In the group C, there were postnecrotic liver cirrhosis 30(60%), Laennec cirrhosis 10(20%), cardiac cirrhosis 1(2%), cholangiocarcinoma 1(2%), chronic active hepatitis 6(12%), hepatic milliary tuberculosis 1(2%) and gall bladder cancer 1(2%). In this group, the albumin in serum and prothrombin time were lowered significantly and the liver scan showed severe mottling of activity with extra-hepatic uptake in the spleen and bone marrow along the vertebral column. 4. In the group D, there were primary hepatoma 26(52%), hepatoma with liver cirrhosis 7(14%), metastatic liver cancer 5(10%), liver abscess 10(20%), multiple liver cyst 1(2%) and cystic duct adenoma 1(2%), In this group, the alkaline phosphatase activity was elevated with single or multiple intrahepatic space occupying lesion in the radiogold colloid liver scan.
