Mediastinal lipoma and liposarcoma (종격동 지방종 및 지방육종: 3례 보고)

  • The lipomatous of the mediastinum are fairly uncommon tumors. Since 1971, two patients with proven mediastinal lipoma and one primary liposarcoma of the mediastinum have been treated at the Seoul National University Hospital. This report reviews our experience with the review of literature. Case 1.: A 3 year old body revealed a huge round homogenous mass density in the posterior mediastinum in routine chest X-ray. The tumor mass, lipoma, was successfully removed and postoperative course was uneventful. Cases 2.: An asymptomatic 24 year old male was operated on with the preoperative diagnosis of mediastinal lipoma. His preoperative chest X-ray and CT films showed a huge anterior to middle mediastinal tumor, right, with fat density. There is no postoperative problem after successful removal of the tumor mass. Case 3. : A 24 year old female was hospitalized with the complaints of cough and chest pain. A mediastinal mass was removed, which proved to be a liposarcoma on pathologic examination. About one year later, she was found to have recurrent liposarcoma in the right chest area at the OPD follow-up. She was lost to follow-up since then.

Intramedullary Spinal Cord Lipoma without Spinal Dysraphism

  • Extradural lipomas have been frequently reported in the literature, but intramedullary lipomas are far rarer, constituting only approximately 2% of total intramedullary tumors. Intramedullary lipomas are also commonly associated with spinal dysraphism. Lipomas which are not associated with spinal dysraphism are present in only about 1% of spinal lipoma patients. Here, we report a rare case of a patient suffering from an isolated intramedullary lipoma without evidence of spinal dysraphism.

  • In this case report, we present a rare case of lipoma of the tendon sheath localized to the wrist which caused median entrapment neuropathy and was successfully treated with surgical excision. Dynamic examination using ultrasonography revealed the exact location of the lipoma. Electrodiagnostic study (EDX) was done before surgery to elucidate combined neuropathy, and surgery for ulnar neuropathy around elbow was also performed simultaneously. Diagnostic ultrasound can be used for dynamic examinations with real-time visualization.

  • Pediatric nondysraphic intramedullary lipoma is very rare, and only limited cases have been reported. In the present case, we present two infant patients with these pathologies who were surgically treated. Previous literature on 20 patients with these diseases who had undergone surgical treatments was analyzed. Surgical treatment should be considered in most symptomatic patients, and laminoplastic laminotomy and internal debulking of the lipoma under intraoperative neurophysiological monitoring are mostly recommended.

  • Lipomas, the most common soft-tissue mesenchymal neoplasms in adults, are characterized by the proliferation of mature white adipocytes without cytologic atypia. Lipomas are rarely observed in the head and neck region. We present a case of resection and orthognathic surgical removal of an intramuscular lipoma of the mandible with involvement of the mandibular ramus and condylar head and neck. An 18-year-old female patient was referred to our hospital for orthognathic surgery for the management of facial asymmetry and mandibular prognathism. The patient did not present with facial swelling, pain, or temporomandibular dysfunction; however, on radiographic examination, including cone-beam computed tomography and magnetic resonance imaging, an infiltrative fatty lesion was observed in the masticator space inside the right mandible, and the adjacent mandible exhibited bone thinning and deformity. Resection of the lipoma was performed along with orthognathic surgery, including a Le Fort I osteotomy for the maxilla and bilateral sagittal split ramus osteotomy (BSSRO). In this case, because the ramus was split using BSSRO, accessing the lipoma intraorally was easy. Consequently, aesthetic scarring was avoided, and no complications, such as unfavorable splitting or pathologic fracture, occurred. Although recurrence has not been observed about 1 year, long-term follow-up should be performed.

  • Lipoma is a common neoplasm in soft tissues. However, an intrapulmonary lipoma is a rare benign tumor. Patients with a bronchial lipoma might have a malignant potential related to their smoking history due to the case reports of lung cancer accompanied with lipoma. Endobronchial lipoma can cause irreversible parenchymal lung damage if not diagnosed and treated early. Therefore, it should initially be treated by fiberoptic bronchoscopy or surgery depending on the status of distal parenchymal lung damage. Bronchiolitis obliterans with organizing pneumonia (BOOP) is a pathological syndrome that is defined by the presence of buds of granulation tissue consisting of fibroblasts and collagen within the lumen of the distal air spaces. BOOP is caused by drug intoxication, connective tissue disease, infection, obstructive pneumonia, tumors, or an unknown etiology. We encountered a 58 year-old male patient with endobronchial lipoma, causing the collapse of the right middle and lower lobes, and BOOP due to obstructive pneumonia.

  • Mediastinal lipomas are fairly unusual tumors. They are usually lobulated, encapsulated and found in the anterior mediastinum. Although benign, they may grow to an huge size, but they no specific symptom. We experienced a case of large cervico-mediastinal lipoma, which successfully resected. This report reviews our experience with the review of literature.

Case Report: A Giant Lipoma from the Buccal Fat Pad Resembling Two Different Masses (증례 보고: 2개의 종괴를 모방하는 볼 지방 덩이에서 유래한 거대 지방종)

  • We report a surgically challenging case of a large lipoma located from the temple, across the zygomatic region to the buccal area, resembling two different masses. An 82-year-old man presented with two persistently growing soft masses at his left temple and cheek. A computed tomographic scan revealed a single large radiolucent mass extending from the submuscular layer of the left temple crossing beneath the zygomatic arch to the buccal region, rather than two individual masses. Excision was performed through upper gingivobuccal and temporal incisions. The mass was dissected through both incisions, cut in half, and extracted from both sides. No complications were observed. The biopsy result was consistent with a lipoma. Four months later, the scars were inconspicuous, and the patient was satisfied. Considering the size, shape, and location, this is a rare and intriguing case. The bi-directional approach allowed for successful total excision without any complications, leaving inconspicuous scars.

Subcutaneous Lipoma on the Forehead of a Slaughtered Cattle in Jeju (제주도 도축우의 이마에서 발생한 피하 지방종)

  • A dome-shaped subcutaneous nodular mass was detected on the forehead of 2-year-old mixed cattle encountered at Jeju slaughter house. The mass showed distinctive greasy appearance and float in 10% buffered formalin. Mass was well demarcated with other tissues, and firm in consistency and whit-yellowish color on cut surface. Histopathologically, subcutaneous mass was well circumscribed but unencapsulated. This mass was composed of larger lipocytes or more variability in size than normal mature lipocytes. The cytoplasmic lipid of tumor cells could be demonstrated using Oil red O staining. Based on the gross and histopathologic findings, this case was diagnosed as subcutaneous lipoma in cattle. In our best knowledge, this is the first case of bovine lipoma in Korea.

  • Endobronchial lipomas are rare benign tumors that arise from the lung. They partially or totally obstruct the bronchial lumen, producing a variable degree of collapse, irreversible bronchiectasis, and pulmonary damage. Although bronchoscope, CT and MR are reported to be helpful in establishing the diagnosis, CT is highly specific and sensitive in detecting fatty tumor. They may be removed by endoscope or thoracotomy or lobectomy. We present a case of endobronchial lipoma completely obstructing the right middle lobe and postobstructive irreversible pulmonary change with review of literatures.
