Development of a Screw-Crane System for Pre-Lifting the Sternal Depression in Pectus Excavatum Repair: A Test of Mechanical Properties for the Feasibility of a New Concept

  • Background: Pre-lifting of the sternum marked a major turning point in pectus excavatum repair. The author developed the crane technique in 2002 and successfully applied it to more than 2,000 cases using sternal wire stitching. However, blind sternal suturing limited the use of the wire-stitch crane. We propose a novel screw for sternal lifting as a new tool for the crane technique. Methods: We developed a screw system strong enough to withstand the pressure needed for sternum lifting. The screw was designed to have a broader thread to hold the bony tissue securely. The screw's sustaining power was tested using the torsion, driving torque, and axial pull-out tests in a polyurethane block and ex-vivo porcine sternum. Results: The screws were easily driven into the sternum, and the head of the screw was connectable to the table-mounted retractor. In the torsion test, the 2° offset torsional yield was 4.53 N·m (reference value, 1 N·m). In the polyurethane block driving torque test, the maximum torque was 0.98 N·m (reference value, 0.70 N·m). The axial pull-out test was 446 N (reference value, 100 N). The maximum pull-out resistance in the ex-vivo porcine sternum model was 1,516 N. Conclusion: The screw crane was strong enough to sustain the chest wall weight to be lifted. Thus, the screws could effectively replace the sternal wire stitching in crane pre-lifting of the sternum. We expect that application of the screw-crane will be easy and that it will improve the safety and success rate of pectus repair surgery.

역도 인상기술 향상을 위한 부분 동작 강화훈련법 개발 및 운동학적 평가 (Development and Kinematic Evaluation for Training Method to Strengthen Part Motion of Snatch in Weight Lifting)

  • The purpose of this study was to develop an innovative training model and method to improve the posture and motion, while pulling up during weight lifting by evaluating the problems of Korean weight lifters performing this motion. To investigate the effectiveness of the new training method substitute members of the Korean national weight lifting team performed both the original pull up technique and new pulling training technique while kinetics and kinematics were recorded. For this study, the first phase of the new training method is more appropriate than the original training with the pull up drop slow deadlift to the knee joint. For the second phase, the new training motion is deemed to be more effective than the current box deadlift motion. Also, this new motion corrects the posture as there is more anterior hip joint motion(about 10 cm) and the knee flexes to about 120 degrees. For the third phase, starting about 10cm above the knee the box snatch high pull up is identified as a more suitable training method. For the forth phase, the box top snatch method is judged to be a more effective training method than the original top snatch training method.

  • Human posture prediction and motion simulation methods try to solve inverse kinematic problems using the optimization technique based on the concept of minimum principle. It is very important to select a cost function which relfects the human posture acurately. In this study, lifting postures were predicted using the five biomechanical cost functions and compared with real human postures in order to evaluate the predictivities of the cost functions. The result showed that all the biomechanical cost functions used in this study could not predict lifting postures accurately. The cost function which minimizes the sum of joint moments showed the smallest mean prediction error, while the one which minimizes the MUR showed statistically better performance.

  • The use of osseointegrated implant has been reported that is an acceptable procedure for the restoration of totally or partially edentulous patient and that offers good predictability of long term success. It is difficult to get high success rate in edentulous maxillae with inadequate bony quality and quantity, and anatomic limitations such as pneumatic maxillary sinus and nasal floor. The various trials such as sinus lifting, bone grafting, guided bone regeneration, trabecular condensation with osteotome, and the use of wide-diameter implant have been introduced to solve these problems. This study was undertaken to assess the evaluation of clinical prognosis of the implant restorations with these various implantation techniques in the maxillary edentulous area. One hundred eight patients were treated with a total of 386 endosseous implants from March 1994 to January 1998 at Dept. of Dentistry, Korea Veterans Hospital in Seoul Korea. The various techniques for implantation in the edentulous maxillae were supplied to overcome the limitations of implant fixation. These techniques consist of sinus lifting, guided bone regeneration, onlay bone grafting, and osteotome trabecular condensation technique. The total success rate of implant restoration of this study was 93% in the maxillae. The success rate of implant restorations with conventional technique was 94.6%, with osteotome trabecular condensation technique was 94.1%, with guided bone regeneration technique was 93.3%, with bone grafting technique was 92.9%, with sinus lifting technique was 83.8%. The success rate on the maxillary anterior area was 95.2% and that on the posterior area was 91.9%. The failures were associated not only with surgical installation techniques but also bony quality and quantity, characteristics of implant, and stress distribution when in function.

Analysis of a Long Volumetric Module Lift Using Single and Multiple Cranes

  • Industrialized and modular construction is a growing construction technique that can transfer a large portion of the construction process to off-site fabrication yards. This method of construction often involves the fabrication, pre-assembly, and transportation of massive and long volumetric modules. The module weight keeps increasing as the modules become more complete (with infill) to minimize the work at the site and, as higher productivity can be achieved at the fabrication shop. Thus, a volumetric module delivery gets more challenging and risky. Despite its importance, past research paid relatively insufficient attention to the problem related to the lifting of heavy modules. This can be a complex and time-consuming problem with multiple lifting for transportation-and-installation operations both in fabrication yard and jobsite, and require complex crane operations (sometimes, more than one crane) due to crane load capacity and load balance/stability. This study investigates this problem by focusing on the structural perspective of lifting such long volumetric modules through simulation studies. Various scenarios of lifting a weighty module from the top using four lifting cables attached to crane hooks (either a single crane or double crane) are simulated in SAP software. The simulations account for various factors pertaining to structural indices, e.g., bending stress and deflection, to identify a proper method of module lifting from a structural point of view. The method can identify differences in structural indices allowing identification of structural efficiency and safety levels during lifting, which further allows the selection of the number of cranes and location of lifting points.

  • This paper proposes a technical requirements analysis of implementation of multi-trade prefabrication. Recently, there has been a rise in the use of prefabrication to minimize on-site work for time reduction to increase productivity. Prefabrication technique is evolved into multi-trade prefabrication combining other trades from single-trade prefabrication. For implementation of new technique, not only itself but complementary techniques have to be prepared. In this paper, MEP corridor rack, a major item of multi-trade prefabrication, was implemented in the test bed and its process was analyzed to find out technical requirements. As a result, comparatively high level of IT technique was required for efficient use of multi-trade prefabrication in design, lifting and construction phase. In design phase, component level of BIM library was needed for manufacturing; and in lifting phase, BIM-based site logistics process was required. Also in construction phase, laser scanning was implemented for gathering shape and geometry of the wall and slab that were attached to multi-trade prefabrication module.

역도 인상동작의 운동학적 분석 (A Kinematic Analysis of Snatch Technique in Weight-Lifting)

  • This study analyzed snatch technique in weight-lifting performed by female gold, silver, and copper medalists at the 75kg level in the 2002 Pusan Asian Games and obtained the following conclusions. 1. KSH used more extensor in the waist than those in ankles, knees, or hip joints during the second pull, So further training is required to manifest both each of joints and waist extensor effectively and thus for the line from shoulders through barbell to hip joints to form a smooth vertical pattern. 2. Contrary to those in other countries, Korean athlete KSH tended to be swift in starting but slow in the second pull section, showing less effectiveness in movement during the second pull; therefore, they must try to exert a swift movement in lockout. 3. KSH showed slowdown in the speed, which was a factor interfering with performance during second pull, despite the great maximum speed of moving barbell. It is important to use barbell sufficiently not to reduce the speed of vertical movement but rather to keep the speed increasing. 4. KSH, who kept doing inefficient movements such as failure to perform swift lockout after lifting the heel at the maximum angle of lower limbs, needs to reduce this meaningless extension of ankles, knees, or hip joints to exert energy effectively.

블록의 탑재 안전성을 위한 초기 평형 자세 탐색 방법 연구 (A Study on the Methods for Finding Initial Equilibrium Position of a Lifting Block for the Safe Erection)

  • In a shipyard, block lifting is an important process in the production of ships and offshore structures. Block lifting is a sensitive process because lifting blocks have to be erected with exact positions and orientations. If we use a numerical method for the process, it is important to find tensions of wires and positions of equalizers to maintain the initial equilibrium position of the block. At this time, equations of motion of the block should be solved to calculate the initial equilibrium position of the block. Because the solving technique changes according to the number of equalizers, a suitable equation for the corresponding problem is required. In this study, three types of equations are proposed to find the initial equilibrium position of the block according to the number of equalizers. The Newton-Raphson's method is used to solve nonlinear simultaneous equations and the optimization method is used to determine the appropriate solution to the undetermined problem. To evaluate the applicability of the proposed methods, the dynamic simulations are performed using the tensions calculated from the proposed methods, and the results are discussed. The results show that the proposed methods can be effectively used to determine initial equilibrium position of the block for the block lifting.

Histologic evaluation of low-intensity pulsed ultrasound effects on bone regeneration in sinus lift

  • Purpose: Many techniques have been described for achieving vertical augmentation of the maxillary sinus. The aim of this study is to evaluate the effect of low-intensity pulsed ultrasound (LIPUS) to enhance bone regeneration after sinus floor elevation. Methods: The sinus lifting technique was performed through a lateral approach on 8 different sites of 5 patients (3 males and 2 females) and their mean age was 45.7 years old. The sites were randomly assigned to the control or test groups. The control group had 4 sites that received lateral sinus lifting procedure only, while the test group had 4 sites that received LIPUS application after the lateral sinus lifting procedure. 24-32 weeks (an average of 29 weeks) postoperatively, new bone formation in the augmented sinus sites was evaluated through histologic and histomorphometric analyses of the biopsy specimens obtained during implant placement. Results: In the test group, the mean percentage of newly formed bone was $19.0{\pm}2.8$%. In the control group, the mean percentage of newly formed bone was $15.2{\pm}3.1$%. The percentage of newly formed bone was approximately 4% higher in those cases where the sinus was treated by LIPUS than the percentage in those cases where it was not used. The difference was statistically significant. Conclusions: Within the scope of this study, low-intensity pulsed ultrasound application after sinus lifting appeared to have a significant effect on the development of new bone formation.
