농흉의 임상적 고찰 (Clinical Evaluation of Empyema Thoracis)

  • One hundred forty patients with empyema thoracis were managed under the general anesthesia at the Seoul National University Hospital between 1980 and 1990. The patients, who were managed by thoracentesis or intercostal tube drainage alone, were excluded in this study. There were 92 males and 48 females, ranging from 8 to 80 years of age. Underlying pathologic lesions of empyema thoracis were primary bronchopulmonary infection [84%], postoperative empyema[11%], malignancy, paragonimiasis, spontaneous pneumothorax and so on. A single causal organism was isolated only in 17 patients[the most common being staphylococcus aureus, pseudomonas, & streptacoccus pneumoniae], multiple organism in 31, and no growth in 32. Surgical treatment modalities were decortication[75], pleuropneumonectomy[30], decortication with lobectomy[10], empyemectomy[9], open drainage[13], Clagett procedure [6], thoracoplasty with or without muscle transposition[9]. Hospital mortality were in 2 cases[1.4%], one empyema related and the other nonrelated. In this study, bacteriologic findings were nonspecific and often polymicrobial. We conclude that early thoracotomy can be lifesaving in the presence of a benign clinical course.

  • In this study, we have studied rail infrastructure related to emergency action to manage the risk when emergency caused by faults of facility or rail vehicle during operation happens. Especially we have compared the effect of emergency action with examining the structure of vehicle, tunnel, bridge and access road, etc which are related to emergency action. Also, we have tried to analyze effects of radio and communication equipment, lifesaving and refuge which could be used for rolling stock, station, control room, tunnel, bridge and etc, and we have presented the way of reporting the emergency to the train driver or crew, control room, outside networks which could be used by passengers in vehicle, station, railroad line. Based on these, we have analyzed the conduct of emergency action in length of time when emergency happens in railway and high-speed railway, and studied the method of which passengers could be guided safely and escape from the scene of the accident.

  • 재난지역의 영상 촬영 및 물품 조달 등 인간이 직접 수행하기 어려운 임무를 대신하기 위해 드론이 사용된다. 시중에 판매되고 있는 드론들은 비행과 영상촬영 기능을 모두 제공하지만 firmware 및 응용프로그램 소스코드가 제한적으로 공개되어 있어 일반 개발자가 드론의 firmware를 수정하거나 센서 및 카메라를 직접 제어할 수 없다는 단점이 있다. 하지만 개발자의 목적에 맞는 드론의 비행 시스템을 구축하기 위해서는 시스템 내부의 제어코드를 직접 수정할 필요성이 있다. Pixhawk는 firmware 및 관련 application의 개발에 대한 소스 코드가 공개되어 있고, 일반 개발자들의 접근이 상대적으로 용이하다는 장점이 있다. 본 프로젝트에서는 Pixhawk와 3D Robotics 사에서 제공하는 Drone-Kit Platform을 활용하여 조난 상황에 대한 인명 구조 시스템을 설계하고, 응급상황에 대한 드론 제어 시스템을 구현하였다.

Blind nasal intubation as an alternative to difficult intubation approaches

  • Airway difficulties are a major concern for anesthesiologists. Even though fiberoptic intubation is the generally accepted method for management of difficult airways, it is not without disadvantages-requires patient cooperation, and cannot be performed on soiled airway or upper airways with pre-existing narrowing pathology. Additionally, fiberoptic bronchoscopy is not available at every medical institution. In this case, we encountered difficult airway management in a 71-year-old man with a high Mallampati grade and a thick neck who had undergone urologic surgery. Several attempts, including a bronchoscope-guided intubation, were unsuccessful. Finally, blind nasal intubation was successful while the patient's neck was flexed and the tracheal cartilage was gently pressed down. We suggest that blind nasal intubation is a helpful alternative in difficult airway management and it can be a lifesaving technique in emergencies. Additionally, its simplicity makes it a less expensive option when advanced airway technology (fiberoptic bronchoscopy) is unavailable.

개심술후 출혈로 인한 응급개흉술의 임상적 고찰 (Emergency Reexploration for Bleeding after Open Heart Surgery wth Cardiopulmonary Bypass -A Report of 16 Cases-)

  • Bleeding after open heart surgery with cardiopulmonary bypass was a cause of concern. requiring reexploration of the chest in approximately 8 percent of patients who have had operations on the heart. From April., 1983 to October, 1991, 16 patients[2%] out of 777 patients who underwent open cardiac surgery had emergency reexploration with bleeding at the Department of Thoracic and Cardiovascular Surgery, Chungnam National University Hospital. In 12 cases[75%], reexploration was performed for continuous bleeding and the reminder[4 cases] were performed for suspected tamponade & hypotension. There were 9 cases[56%] of congenital heart disease and 7 cases of acquired heart disease. The mean blood loss were 997$\pm$472ml /sq. M in total cases and 1442$\pm$ 647ml /sq. M in cases repair of cyanotic heart disease. The mean interval till reoperation was 16.6hr [1hr~72hr] and 41 hr[12~72hr] in tamponade cases and 8.4hr[1hr~24hr] in continuous bleeding cases. The bleeding sites were identified in 7 cases: aortotomy site in 2 cases, ventriculotomy site, SVC, thymus, pleura and sternum wiring site in each other case. But no specific sites was found in the remaining 9 cases. The 8 cases had complications but all except 1 cases with hypoxic brain damage were recovered without sequale. We conclude that emergency thoractomy after open heart surgery may be lifesaving and-/or diminishing complications with bleeding if performed promptly with excessive bleeding, tamponade and unexpected hypotension.

해상추락 인명구조용 무선시스템에 관한 연구 (A Study on a Radio System for Lifesaving in Case of a Falling Accident of the Sea)

  • 해상에 추락한 선원을 구조하기 위해서는 사고 발생 정보를 신속하게 본선에서 알 수 있어야 한다. 이 연구에서는 보급이 용이하도록 형식검정을 받아 별도의 허가절차 없이 사용이 가능하도록 하기 위하여 전파법규에서 특정 소출력 무선국이라고 규정된 447 MHz대를 사용하였다. 이 장치는 송신기의 센서가 물에 접촉되면 작동한다. 500 Hz 구형파로 변조된 전파를 송신하고, 수신기에서 이를 판독하여 경보음을 울리는 시스템이다. 도달거리는 1.5 km이상이다. 또한, 야간 식별을 위하여 송신기는 일정 간격으로 섬광을 발사한다. 인명구조용으로 활용하는 것은 물론 2,400 bps 이하의 저속 데이터의 전송에도 사용이 가능하여 다른 목적의 시스템에도 사용할 수 있다.

  • 서울에는 지상 60층이 넘는 초고층 건물과 시하 8층 이상의 초심층 건축물. 지하철과 상가, 백화점, 호텔, 전시장등이 연결된 대규모 복합유통시설 지역 등이 있다. 이런 지역이나 시설에서 화재와 같은 재난사고가 발생시 진압작전을 위해 투입된 소방관의 위치를 제대로 파악할 수 없으면 투입된 소방관의 안전은 물론 효율적인 지휘통제가 제한되어 결국 더 많은 자체 피해와 더불어 화재 진압 및 시민구조에 제한을 받을 수밖에 없다. 이러한 대형재난에 유비쿼터스 기술을 활용하여 투입한 소방력의 효율적인 운영과 과학적인 진압작전을 구사하여 시민의 생명과 재산을 최대한 보호할 수 있는 소방대응지원 시스템 구축방안을 제시하였다.

  • 본 연구에서는 지하공간 안전체계 확립을 위하여 위험상황별 5 가지 안전관리요소(시설 구조, 피난, 인명구조 소화활동, 성능기반의 화재 안전계획 운영 관리)를 분류해 보았다. 분류된 안전관리 요소로 화재위험성 평가를 수행하기 위하여 평가항목을 분류하고, 평가항목에 대한 점수를 계산하여 최종적으로 화재위험성 평가방안을 제시하였다. 평가항목에 대한 배점은 중요도에 따라 3 단계로 분류하여 단계별로 차등 부여하였고, 평가항목별 점수 평가기준은 소분류의 평가항목 점검결과를 통해 4단계로 구분하여 평가할 수 있게 하였다.

국내 탐지견의 환경 및 위생 관리 현황 조사 (The Survey for Environmental Management of Korea Detector Dog)

  • This research was conducted to grasp the present condition of environmental management relating to Korea detector dogs that performed their duties such as detection and lifesaving. We directly conducted a survey throughout visit to five government agencies. In environmental management case, average length and width of kennels were 1.78 m and 3.50 m, respectively. Types of floor were searched as coating cement, tile, ucrete and concrete. Most of government agencies ventilated kennels using ventilation fan systems with natural ventilation. But there were differences between government agencies in frequencies of the sanitary management items. On the other side, major diseases of the German shepherd breed as a major detector dog were in order of arthritis, measles, earache, and dermatosis. And their daily training time was average 4 hours. Finally, it is thought that these results could be used as a basic data of standard breeding management manual of Korea detector dog.

병원내 심폐소생술 모형에서 환자와 구조자의 거리 및 위치에 따른 가슴압박의 질 비교 (Comparisons of the qualities of chest compression according to various positions of rescuer to patient at the in-hospital cardiopulmonary resuscitation model)

  • Purpose: The purpose of the study is to evaluate the distance and location of the rescuer to patient for the effective chest compressions qualities. Methods: The subjects were 42 students who earned the basic lifesaving technique and had informed consents to participate in the study from May 1 to 20 in 2013. The position of the rescuers included model-0(reference point), model-1(10 cm distance), model-2(20 cm distance), and model-kn(kneeling up). Results: The mean depth of compression was $50.6{\pm}6.6mm$ in Model-0, $48.7{\pm}8.2mm$ in Model-1, $44.2{\pm}10.4mm$ in Model-2, and $51.8{\pm}6.0mm$ in Model-kn. There were statistically significant differences between each Model(p<.001). Conclusion: The closer distance between rescuer and patient could provide more effective chest compressions. Kneeling on the bed stance provided the deeper chest compression consistently than the stool stance.
