The Timing of Femur Fracture Fixation is an Important Factor for Prolonged Mechanical Ventilation

  • Purpose: Mechanical ventilation (MV) is an essential life-saving modality for severely injured patients. However, the long-term use of MV is a major risk factor for late mortality. The surgical correction of long bone fractures plays a critical role not only in improving functional outcomes, but also in reducing physiological derangements, including MV duration. This study investigated the factors affecting prolonged MV (PMV) in severely injured patients with femur fractures. Methods: We retrospectively evaluated all severely injured patients (injury severity score >15) with femur fractures who were taken to the emergency department within 12 hours of the causative accidents between January 2016 and December 2018. PMV was defined as MV lasting for ≥7 days. We analyzed the factors affecting PMV. Results: In total, 35 patients were enrolled and 21 (33.3%) were included in the PMV group. The PMV group required more red blood cell (RBC) transfusions within 7 days RBC (7dRBC) (12.8 vs. 6.8 units; p=0.03) and the time to femur fracture fixation (TFFF) was longer (7.9 vs. 2.7 days; p=0.018). The area under the curve (AUC) for TFFF was 0.740 (95% confidence interval [CI]: 0.572-0.908; p=0.018) and the AUC for 7dRBC was 0.718 (95% CI: 0.546-0.889; p=0.031). Conclusions: This study indicates that TFFF is an independent risk factor for PMV. Early fixation of femur fractures might prevent PMV and its associated complications.

  • The release of iPhone by Apple in 2009 has changed the life pattern of an individual tremendously. That is, with the emergence of a smart phone, various services including voice/video call, camera, receiving and sending of e-mail, and web browsing have been realized. However, the broader the scope of the use of a smart phone has become, the greater the need for companies to introduce an MDM solution for protecting important documents has become. However the MDM solution may have a problem in that all data such as contacts, pictures, and memos saved in the smart phone can be accessed unlimitedly. For this reason, there is a risk that unwanted violation of privacy may happen to smart phone users. This paper proposed a plan to protect a personal privacy file of smart phone users, which disables access by others except for related smart phone users by enabling a person in charge of security or an MDM manager in a company to have access only to the file which was allowed by smart phone users to be disclosed and by saving non-disclosed files in a virtual disk.

  • The present study provides structural design and analysis of main wing and horizontal tail of a small scale WIG(Wing in Ground Effect) vehicle which has been developed as a part of the high speed maritime transportation system for the future of Korea. Weight saving as well as structural stability could be achieved by skin-spar with foam sandwich design and with wide application of carbon/epoxy composite material. A commercial FEM code, NASTRAN, was utilized to confirm the structural safety and stability through sequential design modifications to meet the final design goal. In addition, each wing and the fuselage were fastened together by eight insert bolts with high strength to accomodate easy assembling and disassembling as well as to guarantee a service life longer than 20 years.

A study for innovation of Adult nursing assessment tool (간호정보조사지 개선을 위한 연구)

  • This study tried to suggest the basic materials that can be efficiently applied in clinical cases by understanding problems through a content analysis of an adult nursing assessment tool and opinion agreement about nurse's practical usage presently used in the hospital. The study was carried out in 36 attached hospitals in nationwide universities from May to December, 1999, the two hundred and twenty five reports were for analysis. The contents of the collected nursing assessment tool were analyzed. It was found that the tool had been used with various names and content and there were instances of partial omission of a number of items, such as documentation and time records. Other results revealed that they mostly had a systematic classification of items, formation of details a form of a check list, and the effect of saving time. In spite of the adult nursing assessment tool, it was suggested that its style be subdivided according to the specialized of a department and that standardized style be amended and supplemented. The respondents also answered that there had to be education about continuous and sufficient health assessment skills on the physical examination record. The most frequently suggested items to be added were: past history, marital status, patient and caregiver's address and telephone number. It was found that a patient's education career, economic status, religion, hygienic practice, sexual life and hobby were the most frequently omitted items on record. The reason given wes because the items were associated with his/her privacy. These results highlight the importance of analyzing the content with an complete data collection, supplying basic content for a more accurate nursing record, computerization, sharing information and standardization of the form.

Surgical Management of Massive Cerebral Infarction

  • Objective : The aim of this study was to analyze the treatment results and prognostic factors in patients with massive cerebral infarction who underwent decompressive craniectomy. Methods : From January 2000 to December 2005, we performed decompressive craniectomy in 24 patients with massive cerebral infarction. We retrospectively reviewed the medical records, radiological findings, initial clinical assessment using the Glasgow Coma Scale, serial computerized tomography (CT) with measurement of midline and septum pellucidum shift, and cerebral infarction territories. Patients were evaluated based on the following factors : the pre- and post-operative midline shifting on CT scan, infarction area or its dominancy, consciousness level, pupillary light reflex and Glasgow Outcome Scale. Results : All 24 patients (11 men, 13 women; mean age, 63 years; right middle cerebral artery (MCA) territory, 17 patients; left MCA territory, 7 patients) were treated with large decompressive craniectomy and duroplasty. The average time interval between the onset of symptoms and surgical decompression was 2.5 days. The mean Glasgow Coma Scale was 12.4 on admission and 8.3 preoperatively. Of the 24 surgically treated patients, the good outcome group (Group 2 : GOS 4-5) comprised 9 cases and the poor outcome group (Group1 : GOS 1-3) comprised 15 cases. Conclusion : We consider decompressive craniectomy for large hemispheric infarction as a life-saving procedure. Good preoperative GCS, late clinical deterioration, small size of the infarction area, absence of anisocoria, and preoperative midline shift less than 11mm were considered to be positive predictors of good outcome. Careful patient selection based on the above-mentioned factors and early operation may improve the functional outcome of surgical management for large hemispheric infarction.

  • A smart card based on the existing Java card platform executes and installs an application only when the power is supplied for a minute. And preparing for unexpected power outrage, the execution state of an application and all the data that are modified during execution are saved in the heap. This kind of frequent data update of an EEPROM data is a main cause of reducing the life-cycle of a smart card. This is because the smart card has been developed not considering the current situation that the power is always supplied, and by this time it has continuously kept its old architecture. This paper explains the high performance Java card system free power restriction. The system improves not only application saving mechanism, but memory architecture. In special, we deploy RAM for running an applet, as well as EEPROM for downloading an application. Through proposed mechanism, we can find out performance evaluation that the creation speed of an applet and the execution speed of a method increase up to 58% and 33% respectively.

Efficacy of the Decompressive Craniectomy for Acute Cerebral Infarction : Timing of Surgical Intervention and Clinical Prognostic Factors

  • Objective : Acute cerebral infarction is often accompanied by transtentorial herniation which can be fatal. The aim of this study is to determine the timing of surgical intervention and prognostic factors in patients who present with acute cerebral infarction. Methods : We reviewed retrospectively 23 patients with acute cerebral infarction, who received decompressive craniectomy or conservative treatment from January 2002 to December 2004. We divided patients into two groups according to the treatment modalities [Group 1 : conservative treatment, Group 2 : decompressive craniectomy]. In all patients, the outcome was quantified with Glasgow Outcome Scale and Barthel Index. Results : Of the 23 patients, 11 underwent decompressive craniectomy. With decompressive craniectomy at the time of loss of pupillary light reflex, we were able to prevent death secondary to severe brain edema in all cases. Preoperative Glasgow Coma Scale and loss of pupillary light reflex were significant to the clinical outcome statistically. With conservative treatment, 9 of the 12 patients died secondary to transtentorial herniation. The clinical outcomes of remaining 3 patients were poor. Conclusion : This study confirms the value of life-saving procedure of decompressive craniectomy after acute cerebral infarction. We propose that the loss of pupillary light reflex should be considered one of the most important factors to determine the timing of the decompressive craniectomy.

A Terminological and Etymological Study about 'Safety' (안전'에 대한 용어적 및 어원적 연구)

  • The terminological definition of 'safety' is difficult because the word has an meaning and the usage is too wide and various in itself. There could be various meanings of 'safety' according to the specific area on which the term is used. Even though the variety is permitted, a definition on the term of 'safety' is necessary not only to raise the understanding of consistent terminological meaning but also to make assure the perfectness on all of the applications. The term of 'safety' can be understood clearly by the comparison with the antonym and the synonym, and the limitation of the usage can be established through the comparative studies. Comparing with the antonym such as 'risk', 'safety' implies the recognition and prevention of the danger before an accident or a disaster occurs. Comparing with another antonym of 'disaster', 'safety' comprehends all the saving activities for the disaster and the measures to prevent the spread of collateral damages. The comparison with the synonym, 'security' shows that 'safety' can include the prevention of intentional mishaps. The Korean word of 'safety' might be selected by a scholar who was well educated to understand Chinese language and certain canonical texts written in Chinese. The etymological study of 'safety' was concentrated on I-ching which has been believed that could foretell a future and show a measure to prevent bad luck by performing good virtue in the life. The Korean word of 'safety' might come from Xicizhuan(Great treatise) which is known as one of ten commentaries for I-ching. The texts from Xicizhuan describes that all of the explanations about safety which could be translated to the principles of safety nowadays.

A Study on "Hyang Yak Gu Geup Bang" (향약구급방(鄕藥救急方)에 대한 연구)

  • Characteristics Regarding Xiang Yao (Hyang Yag) Jiu Ji Fang Life Saving Local Botanical prescriptions in Medical Research Xiang Yao Jiu Ji Fang is Korea's most ancient extant medical manuscript. It was first published between 1232 and 1251 during the Goguryeo period (Gao Zong) by the (Jianghuadao Da Cang Du Jian) Great Storage Depository Under Capital Supervision. The entire work is divided into three scrolls named the front, middle and back parts. There is also an appendix titled Fangzhong Xiang Yao Mu Cao Bu 'Catalogue of Medicines in the Local Botanicals prescriptions' in one volume. The contents comprise discussion of internal medicine diseases, external medicine diseases, commentaries on sexually transmitted diseases, diseases caused by parasites and bugs, diseases of the five orifices, gynecological diseases and pediatric diseases, in all totaling fifty three types of disease. The prescriptions record 180 types of xiangyao or hyang yag-local botanicals representing the beginning of an independent Korean path of development. Owing to the development during the Goguryeo period of medical material left to us by history, investigation and research in the area of hyang yag local botanicals has bequeathed us methods used in the contemporary period. Through the related comprehensive annotated explanatory notes and documents, much analysis and discussion is taking shape.

A Study on Comparison and Analysis of Revision Building Rating System for Environment-friendly Residential Building (국내 친환경 공동주택의 활성화를 위한 개정 건물 성능 평가제도 비교 연구)

  • As design tools, building performance certification systems can be applied to provide adequate guidelines on design process to create environment-friendly buildings. Domestic certification systems of residential building took effect by doing supply to designers and contractors from affiliated organization of governments in the early 2000s. As a result, Building Energy Rating System, Apartment Performance Rating System, Green Homes and other means to promote green designs have been operated. International trends of applying certification systems were started in the early 1990s as forms of LEED in USGBC, BREEAM in BRE, GBTool Canada. These systems aim to evaluate building performance in line with the Climatic Change Convention and realize sustainable building design. In 2009, residential buildings accounted for the largest portion of the internal real estate market with 67.9 percent according to the National Statistical Office data. And for 18 years since 1991, apartments among constructed residential buildings have ranked top taking up 77.7% as of 2009. Apartment performance evaluations accordingly are to promote to constitution of improving tenant quality of life, the residential environment and saving energy and resources in the internal building market. The purpose of this study is to compare and analyze valuation bases of each sector in evaluation systems of residential buildings at home and abroad to upgrade current systems through reflecting the characteristics of residential buildings. Implementation of this study basically include comparison of valuation bases and partial analysis on properties of rating systems to suggest requisites for improvement in building performance certification.
