An unusual presentation of osteoblastoma of the maxilla: A case report

  • This report presents a rare case of maxillary osteoblastoma in a 17-year-old female. The patient presented with dull pain and facial asymmetry inferior to the left zygoma. An intraoral examination found a painless swelling on the buccal gingival tissue in the left posterior maxilla. Panoramic radiographs and multidetector computed tomographic images revealed an ill-defined, non-corticated, mixed attenuating entity of osseous density located within the left posterior maxilla apical to the left maxillary molars. The entity exhibited a heterogeneous internal structure with a fine granular appearance, and the periphery showed a partial hypo-attenuating rim along the antero-medial aspect. Expansion of the left posterior maxilla accompanied with displacement of the left maxillary sinus floor was noted. External root resorption of the first and second molars was noted, as well as postero-superior displacement of the third molar. The histopathologic diagnosis of the biopsy was osteoblastoma. Complete excision of the tumor was performed.

Pulmonary Blastoma - A Case Report (폐아세포종;1례 보고)

  • Pulmonary blastoma is a very rare, primary, malignant neoplasm of the lung. The tumor typically is large, well-defined mass and arises in the periphery of the lung. Histologically, it resembles fetal lung tissue evoking the concept that these tumors are drived from the primitive pluripotential pulmonary blastema, but controversy also exit about the nature of this tumor. Clinical evaluation is inconsistant and unreliable as to the diagnostic determination of the tumor type and degree of malignancy. Therefore, diagnosis is usually based on histologic findings. The prognosis after adequate resection appears to be better than for other malignant tumors of the lungs. We experienced a case of pulmonary blastoma occurring in 56-year-old female. She was hospitalized for evaluation of an abnormal shadow in the left lower lung field which found incidentally on chest roentgenogram. We approached the lesion though left posterolateral thoracotomy and performed the left lower lobectomy. The microscopic findings of resected specimen revealed characteristic appearance of pulmonary blastoma. The postoperative course was uneventful without complication.

PDGFC, MARK3 and BCL2 Polymorphisms are Associated with Left Ventricular Hypertrophy in Korean Population

  • Left ventricular hypertrophy (LVH) refers to the expansion and the enlarged myocardium due to the increased resistance to ejection from the left ventricle to the aorta and/or the periphery, or the long-term burden imposed by the blood increase. Hypertension is a major risk factor that accounts for more than 50% of the causes of cardiovascular disease. If hypertension endure in the long term, the myocardium responds to abnormal heartbeat in the heart. Therefore, the prevalence of left ventricular hypertrophy also increases. As a result of genome-wide association study (GWAS) analysis for European people, PDGFC, MARK3, and BCL2 were related to blood pressures. In this study, the genetic polymorphisms of PDGFC, MARK3, and BCL2 were extracted and selected based on Korean genomic and epidemiologic data, and then logistic regression analysis was performed on LVH. As a result, one SNP (rs9307953) in PDGFC gene, four SNPs (rs6575983, rs17679475, rs2273703 and rs10141388) in MARK3 gene and two SNPs (rs17756073 and rs17070739) in BCL2 gene were statistically significant. The rs6575983 of the MARK3 gene showed the highest significance level ($P=7.2{\times}10^{-3}$) among the SNPs and the relative risk of 1.08 (95% confidence interval: 1.06 to 1.45). These results suggest that the polymorphisms of PDGFC, MARK3, and BCL2 not only affect European blood pressures but also correlate with LVH in Korean. These results suggest that increased understanding of the genetic correlations of the pathogenesis of LVH.

A STUDY OF THE BENIGN CEMENTOBLASTOMA (양성 백악모세포종에 관한 연구)

  • The benign cementoblastoma is a benign odontogenic tumor of cemental-forming tissue, which has an unlimited growth potential. The radiographic appearance of the lesion is pathognomonic and consists of a radiopaque mass surrounded by a thin radiolucent line. This mass is inseparable, radiologically, from the tooth root and is attached to it. Microscopic evaluation suggests that the lesion enlarges by peripheral growth, with the center of the neoplasm being more calcified and inactive than the periphery. We have observed two cases of beneign cementoblastoma{mature calcified stage) occured in the left mandibular first molar of 22-year-old man and 14-year-old man.

  • Fine needle aspiration cytology (FNAC) is an easy convenient non-invasive method in the diagnosis of superficial palpable masses. The cytologic findings by FNAC of reactive and neoplastic lesions in various organs including breast, lymph node, thyroid, salivary gland, etc., have been described, but, those of soft tissue lesions including proliferative fasciitis are relatively rare to find. We recently experienced a case of FNAC of proliferative fasciitis in the left back of a 72-year-old male. The FNAC smears were scant in cellularity and contained large cells with abundant basophilic cytoplasm, one to two nuclei lying at the periphery, and prominent nucleoli that resemble ganglion cells.

Flow Characteristics in Spin-Up of a Three-Layer Fluid

  • A numerical study is made of the spin-up from rest of a three-layer fluid in a closed, vertically-mounted cylinder. The densities in the upper layer $\rho_1$, middle layer $\rho_2$ and lower layer $\rho_3\;are\;\rho_3\;>\;\rho_2\;>\;\rho_1$, and the kinematic viscosities are left arbitrary. The representative system Ekman number is small. Numerical solutions are obtained to the time-dependent axisymmetric Navier-Stokes equations, and the treatment of the interfaces is modeled by use of the Height of Liquid method. Complete three-component velocity fields, together with the evolution of the interface deformations, are depicted. At small times, when the kinematic viscosity in the upper layer is smaller than in the middle layer, the top interface rises (sinks) in the central axis (peripheral) region. When the kinematic viscosity in the lower layer is smaller than in the middle layer, the bottom interface rises (sinks) in the periphery (axis) region. Detailed shapes of interfaces are illustrated for several cases of exemplary viscosity ratios.

A Clinical Study of cysts on Mandible (하악골에 발생한 낭종에 관한 임상적 연구)

  • Cystic lesions of the mandible are fairly common and usually a presumptive diagnosis is made readily, more often on the basis of roentgenographic appearance than by any other means. Occasionally, however, it is easy to fall into errors of diagnosis which may affect one's choice of treatment. The diagnosis in these situations are usually resolved by the histopathologic examination of material obtained by surgical exploration. The present discussion to the lesions which can be most easily confused in the roentgenographic interpretation, that is, the traumatic bone cyst, the dentigerous cyst, the radicular cyst. These lesions often grow to considerable size before they cause any subjective or objective symptoms. Less frequently, perhaps, the patient presents himself with the complaint of enlargement of the affected part, a discharge, or pain. On rare occasions the first sign is fracture through the cyst cavity. In any case, an adequate roentgenographic survey by means of extraoral films is essential. This is the primary means of diagnosis. The three lesions in mandible, reported here, resembled each other roentgenographically in that they were osteolytic lesions. The follicular and radicular cysts usually have a smooth periphery and may be surrounded by a white line. The follicular cyst, slow-growing lesions, is usually associated with an unerupted tooth. The radicular cyst, the most common type of the odontogenic cyst, is always associated with a nonvital tooth, or it may persist as a "residual" radicular cyst after the causative tooth has been extracted. The traumatic bone cyst can often be differentiated from the first two in that the periphery is less definite and is irregular. It is thought that because it does not occur in older people, the lesion is self-limiting and heals spontaneously if left alone.

Electroacupuncture of $GB_{43}$ Modulates Neuronal Activities in Medial Vestibular Nuclei of Rats (협계(俠谿)에 대한 전침자극이 흰쥐의 내측 전정신경핵 자발활동성에 미치는 효과)

  • Objectives: It is known that the vestibular imbalance leads to vestibular symptoms such as nausea, vomiting, vertigo and postural disturbance. Since the non-labyrinthine inputs from the limbs and viscera converge on the vestibular nucleus neurons receiving signal from peripheral vestibular endorgan, acupuncture to the periphery may influence the activities of vestibular nuclear neurons and produce a therapeutic effect on the vestibulacr symptoms. The present study was to examine a modification and characteristics of the static and dynamic activities of medial vestibular nucleus (MVN) neurons following electroacupuncture (EA) of GB43' acupoint. Methods: In 54 Sprague-Dawley adult male rats weighing 250${\sim}$300g, spontaneous firing discharges and dynamic responses induced by sinusoidal whole body rotation about vertical axis at 0.2 Hz were observed in MVN of rats during EA of GB43' acupoint, located between the left 4th and 5th toe, which is the territory of sural and peroneal nerves, with 0.2 ms, 40 Hz and 600${\pm}$200 ${\mu}A$. Results: EA of the left GB43' acupoint induced modifications of spontaneous firing rates in 45% of MVN neurons recorded, and the percentage of modified neurons was 44% in type I, 52% in type II and 46% in non-type neurons. The excitatory or inhibitory responses of spontaneous firing discharges were predominant in the ipsilateral MVN neurons during EA. The excitatory response was abolished after EA but the inhibitory response was prolonged after EA in the ipsilateral MVN. The neurons of MVN showing modified spontaneous firing discharges by EA showed lower frequency (${\geq}$10 spikes/sec) of mean spontaneous firing rates than non affected ones. Conclusion: These results suggest that the neuronal activities of MVN neurons were influenced by EA of GB43' acupoint and the effects of EA may be related to the convergence of the peripheral vestibular inputs and ascending somatosensory inputs on MVN.

EFFECTS OF ALCOHOL AND GLYCEROL INJECTION ON THE RAT INFRAORBITAL NERVE (백서 안와하신경에서 알콜 및 글리세롤 주입의 효과)

  • This study was performed to investigate the changes of nerve after the injection of alcohol and glycerol at the infraorbital nerve in rats. Using the eighteen Sprague-Dawley rats, weighing $200{\sim}250g$, 99% alcohol, pure glycerol, and sterile saline was injected to the epineurium of the infraorbital nerve. Glycerol injected rats were devided into 0.01ml, 0.03ml and 0.05ml groups. The alcohol and control group were injected 0.03ml at the left infraorbital nerve. The following results were obtained by histopathological examination after 1 week, 1 month, and 3 months. A few inflammatory cell infiltration and no signs of nerve degeneration were noted in control group. Total nerve degeneration was noted in the alcohol group and no regeneration was noted in 1month, and partial regeneration was noted at 3month. The nerve degeneration was noted at the periphery of nerve bundle in 0.01ml glycerol injection group. Total degeneration was noted in the 0.03ml and 0.05ml glycerol injection group and the degree was propotional to dose. These results suggest that injection of alcohol and glycerol are effective to nerve blockage by nerve degeneration, and nerve degeneration by glycerol injection is propotional to dose and nerve regeneration by glycerol injection is inversely propotional to dose.

An English Tough-construction Resolution: A Minimalist Account

  • Tough-construction is, undoubtedly, one of the most puzzling syntactic problems in the history of transformational grammar. Various approaches have been proposed including Deletion approaches (Akmajian, 1972; Lasnik and Fiengo, 1974) and movement approaches. Among the movement approaches, Chomsky (1977; 1981) argues for movement of null Operator, and Hornstein (2001) argues for a two-step movement equipped with Sideward movement. Most recently, Schueler (2004) and Hartman (2009) each have also argued for a kind of movement approach. With the development of the Minimalist syntax (Chomsky, 1995; Chomsky, 2000; Chomsky, 2001), tough-construction, an age old problem in the description of grammar, turns into another round toward to a more satisfactory answer. By examining the most recent competing analyses of tough-constructions, this paper defends and extends Schueler's (2004) analysis, rather than Hartman's (2009) two step movement approach. Furthermore, this paper proposes that tough-subject originates from the intermediate CP internal Spec-TopicP position rather than from the iterated CP layer (Authier, 1992). This approach has more descriptive power than it was originally argued for in Schueler (2004) and is a step closer toward Minimalism insofar as the conception of government is no longer utilized.
