The Effects of EEG Power and Coherence on Cognitive Function in Normal Elderly, Non-Demented Elderly With Mild Cognitive Impairment, and Demented Elderly During Working Cognition Task

  • The purpose of this study was to find out the effects of electroencephalograph (EEG) power and coherence on cognitive function in normal elderly, non-demented elderly with mild cognitive impairment, and demented elderly during working cognition tasks. Forty elderly women (19 demented elderly, 10 non-demented elderly with mild cognitive impairment, 11 norma1 elderly) participated in this study, All subjects performed working cognition tasks with Raven's CPM while EEG signal was recorded, EEGs were measured continuously at rest and during the working cognition task. EEG power and coherence was computed over 21 channels: right and left frontal, central, parietal, temporal and occipital region. We found that there were more correct answers among normal elderly women than in other groups Owing the working cognition task, ${\Theta}$ wave at Fp1, Fp2 and F8, a wave at Fp2, ${\beta}$ wave at Fp1, Fp2. F4 and F8 of the frontal region was increased significantly in the demented elderly group. On the other hand. ${\Theta}$ wave at Fp1, Fp2 and F7, ${\beta}$ wave at Fp1, Fp2, F3 and F7 of the frontal region was increased significantly in the group of non-demented elderly with mild cognitive impairment. In contrast. in the normal elderly group, all of the ${\Theta}$ wave and ${\beta}$ wave at Fp1, Fp2, F3, F4, F7 and F8 of the frontal region (except ${\beta}$ wave at F3) was increased significantly, These results suggest that the nerves in prefrontal and right hemisphere regions were most active in the demented elderly group during problem solving, and the nerves in the prefrontal and left hemisphere lobe were most active in the group of non-demented elderly with mild cognitive impairment. In contrast, me majority of nerves in the frontal region were active in the normal elderly group.

말티즈견에서 괴사성 뇌막뇌염 증례 (A Case of Necrotizing Meningoencephalitis in a Maltese Dog)

  • A 18-month-old, male Maltese dog with acute seizure was referred to Veterinary Medical Teaching Hospital, Kyungpook National University. Abnormal neurologic findings included a tendency to fall to the left, turn and circle to the right, negative bilateral menace responses and pupillary light reflexes, and diminished responses to facial sensation testing on both sides. Radiographic signs were normal, except for occipital dysplasia. Magnetic resonance imaging of the brain revealed bilateral asymmetric ventriculomegaly, which is more on left side. Cerebral edema was also suspected. The dog died after 9 days from the onset of seizure, and a necropsy was performed. Histopathologically, the brain lesions in the meninges and the cerebral cortex of this dog was presented. There was severe diffuse nonsuppurative inflammation in perivascular area of the meninges and the cerebral white matter. Based on the results, this case was diagnosed as necrotizing meningoencephalitis in Maltese dog.

우울과 불안의 뇌 기능 - EEG, ERP, Functional Neuroimaging, HRV 소견을 중심으로 - (Neurophysiological and Neuroimaging Characteristics of Depression and Anxiety)

  • The purpose of this review was to investigate the neurophysiological and neuroimaging characteristics of patients with depression and anxiety reported in previous studies. A literature search was conducted using Medline and psychiatric textbooks. "Electroencephalography (EEG)", "Event Related Potentials (ERP)", "functional neuroimaging", "heart rate variability (HRV)" and "depression or anxiety" were used as key words. A physiological finding indicated that there was a higher degree of relativity with regards to prefrontal dysfunction in patients with depression. Right prefrontal lobe hyperactivity and left prefrontal hypoactivity were consistently observed, and abnormalities were observed in other regions (ACC, hippocampus, amygdala, etc.). Therefore, dysfunctions in these areas are related to depressive symptoms. In patients with anxiety disorder, each emotional condition showed specific activation patterns in different brain regions, such as the prefrontal cortex, occipital lobe, temporal lobe, hippocampus, and limbic system, including the amygdala. However, in the majority of patients with anxiety disorder, the degree of activation was higher in the right hemisphere than in the left hemisphere. The current data supports that there is a difference in brain dysfunction characteristics between depression and anxiety and that the different activations of various brain regions would play a significant role in the pathophysiology of depression and anxiety disorder.

Transcranial Direct Middle Meningeal Artery Puncture for the Onyx Embolization of Dural Arteriovenous Fistula Involving the Superior Sagittal Sinus

  • A 66-year-old woman presented with intermittent paraparesis and generalized tonic-clonic seizure. Cerebral angiography demonstrated dural arteriovenous fistula (AVF) involving superior sagittal sinus (SSS), which was associated with SSS occlusion on the posterior one third. The dural AVF was fed by bilateral middle meningeal arteries (MMAs), superficial temporal arteries (STAs) and occipital arteries with marked retrograde cortical venous reflux. Transfemoral arterial Onyx embolization was performed through right MMA and STA, but it was not successful, which resulted in partial obliteration of dural AVF because of tortuous MMA preventing the microcatheter from reaching the fistula closely enough. Second procedure was performed through left MMA accessed by direct MMA puncture following small decortications of cranium overlying the MMA using diamond drill one week later. Microcatheter could be located far distally to the fistula through 5 F sheath placed into the MMA and complete obliteration of dural AVF was achieved using 3.9 cc of Onyx.

내·외적인 자극을 받는 뇌파의 비선형 동력학적 특징 (Nonlinear Characterization of EEG Under the Internal and External Stimuli)

  • Backgrounds and objective : EEG reflect dynamic changes of continuous neuronal activities by internal and external stimuli. The aim of this study is to quantify nonlinearly the local dynamic differences among EEG data corresponding to different states of brain. Methods : EEG was recorded from twelve healthy normal subjects(mean age, 29.7 years; 8 men and 4 women) using digital EEG machine. 18-channel EEG data were selected during eyes closed(EC), eyes open(EO), and mental arithmetic(MA) in each subject. Correlation dimension(D2) and largest Lyapunov exponent(LLE) were calculated from three states and average value was mapped 2 dimensionally and compared with each other. Results : The distribution of D2 was relatively symmetric and its value was higher in frontal than in parieto-occipital region during EC. These findings were reversed during EO. Bilateral centro-temporo-parietal region showed high D2 value in MA compared with those in EC, which was more prominent in left side. LLE was larger than zero in all state and showed significant differences among EC, EO and MA(p=0.000). Conclusion : These results suggest that nonlinear analysis of EEG can quantify dynamic state of brain.

후대뇌동맥 경색으로 발생한 시각 및 인지장애 환자 치험 1례 (Case Report on a Posterior Cerebral Artery Infarction Patient with Visual and Cognitive Abnormalities)

  • The posterior cerebral arteries supply the temporal and occipital lobes of the left cerebral hemisphere and the right hemisphere. Clinical symptoms associated with occlusion of the posterior cerebral artery are visual abnormalities including opposite visual field defects, hallucination, visual amnesia and a variety of other symptoms, including confusion, cognitive disorders, thalamic syndrome, Weber's syndrome, contralateral hemplegia. This case report is about a patient with visual and cognitive abnormalities caused by posterior cerebral artery infarction. He was regarded as Soyangin(少陽人) in constitution and was treated with Yangkyuksanhoa-tang(凉膈散火湯) and other treatments. Improvement in his general symptoms was observed.

A case of chronic cerebral paragonimiasis westermani

  • We report a chronic cerebral paragonimiasis from a 41-year-old Korean man who complains a headache and weakness of left motor neuron components. Magnetic resonance images of the brain revealed conglomerates of multiple ring-like enhancements in tempore-occipital and frontal lobes of the right hemisphere. An intradermal test for paragonimiasis westermani was positive. The patient was born near an endemic area of paragonimiasis and used to eat boiled or grilled freshwater crayfish in his childhood. Nodules in the brain were resected through craniotomies. The eggs of P. westemani were identified pathologically and parasitologically in the calcified necrotic lesions. Examinations on sputum and fecal specimens for the eggs of P. westemani were shown to be negative and a chest radiograph was normal. It is presumed that the brain lesions were formed by P. westemani approximately 30 years ago.

Brain Areas Subserving Torrance Tests of Creative Thinking: An Functional Magnetic Resonance Imaging Study

  • Background and Purpose Torrance Tests of Creative Thinking (TTCT) is a well-known and commonly used measure of creativity. However, the TTCT-induced creative hemodynamic brain activity is rarely revealed. The purpose of this study is to elucidate the neural correlates of creative thinking in the setting of a modified version of the figural TTCT adapted for an functional magnetic resonance imaging (fMRI) experiment. Methods We designed a blocked fMRI experiment. Twenty-five participants (11 males, 14 females, mean age $19.9{\pm}1.8$) were asked to complete the partially presented line drawing of the figural TTCT (creative drawing imagery; creative). As a control condition, subjects were asked to keep tracking the line on the screen (line tracking; control). Results Compared to the control condition, creative condition revealed greater activation in the distributed and bilateral brain regions including the left anterior cingulate, bilateral frontal, parietal, temporal and occipital regions as shown in the previous creativity studies. Conclusions The present revealed the neural basis underlying the figural TTCT using fMRI, providing an evidence of brain areas encompassing the figural TTCT. Considering the significance of a creativity test for dementia patients, the neural correlates of TTCT elucidated by this study may be valuable to evaluate the brain function of patients in the clinical field.

섬유 애착물의 종류에 따른 남녀 뇌파 신호 차이 분석 (Analysis of EEG Signal Differences in Gender according to Textile Attachments)

  • This study investigated the effects of textile attachments on electroencephalogram using 20 persons (10 males and 10 females). Four types of attachment cushions were manufactured by changing the shell fabric (cotton and microfiber) and interlining (synthetic loose fiber and buckwheat). This was done using BIOS-S8 (BioBrain Inc., Korea), an 8-channel polygraph for multi-body signal measurement, to measure EEG. Data were analyzed using the SPSS 24.0 statistical program. EEG values were significantly activated according to gender, particularly when the subjects' eyes were open. For the male cases, 'RT', 'RAHB' values were highly activated and for the female cases, 'RA', 'RB', 'RG', 'RFA', 'RST', 'RLB', 'RMB', 'RST', 'RMT' values were highly activated. Examining the differences in EEG according to type of attachment indicated no significant difference in both sexes. However, in cases of females with their eyes closed, the 'RSA' index was quite different in the left occipital lobe (O1), and when their eyes were open, the 'RFA' in the right frontal lobe (F4) showed a significant difference. However, there was no obvious correlation between the activation of EEG and the subjective preference of textile attachments.

군발 두통 환자의 한방 치료 치험 1례: 증례 보고 (Korean Medicine Treatment for a Patient with Cluster Headache: A Case Report)

  • Objectives: The purpose of this study was to report the effect of Korean medicine (KM) treatment on a patient diagnosed with cluster headache. Methods: The patient included in this study was diagnosed with cluster headache who received KM therapy (herbal medicine, acupuncture, moxibustion, cupping, and chu-na treatment). The main symptom of the patient was pain in the left occipital region and orbital region. It was diagnosed as a pattern of liver depression and spleen deficiency by oriental medicine. During the hospitalization period, treatment effect was evaluated by NRS. HIT-6. BDI, and STAI were additionally performed to assess depression and anxiety. Results: After 24 days of treatment, the intensity of headache decreased from NRS7 to NRS3. During follow-up at two weeks, there was no pain. Normal daily activities were possible. Conclusions: Korean medical approach might be useful for pain relief and restoration of daily living ability for patients with cluster headache.
