EEG responses in highschool students during visual attention task (정신집중시 고교생의 EEG 반응)

  • 본 연구에서는 EEG를 이용하여 정신집중 시, 뇌 기능의 변화를 알아보았다. 실험은 건강한 남녀 고교생 9명을 대상으로 EEG는 Fp1. Fp2, F3, F4, P3, P4, T5, T6, O1, O2의 10위치에 대하여 정신집중 작업 진의 3분과 작업 시 30분간을 측정하였다. 작업은 PC를 이용하였으며 모니터 상의 움직임에 집중하고 목표자극을 탐지하였을 때 신속하게 적절한 반응키를 누르도록 하였다. 각 주파수 대역은 theta2와 (6.0-8.0Hz), alpha1파 (8.0-10.0Hz), alpha2파(10.0-12.0Hz)로 구분하였으며 비대칭지수를 이용하여 분석하였다. 그 결과, 정신집중 작업 시 theta1, theta2, alpha1대역에서 두정엽의 우반구과 측두엽 및 후두엽의 좌반구영역이 우세하였다. 이들 영역은 시갈, 지각, 움직임 기능을 수행하며 theta2파가 주의, alpha1파가 각성 관련이다. 이상의 결과에서 두정엽의 우반구와 측두엽 및 후두엽의 좌반구영역의 theta1파, theta2파, alpha1파가 정신집중 작업 시 중요한 역할을 담당하는 것으로 나타났다.

The Aneurysmal Subarachnoid Hemorrhage following Stellate Ganglion Block -A case report- (성상신경절 차단후 발생한 동맥류성 지주막하출혈 -증례 보고-)

  • Stellate ganglion block(SGB) is frequently performed to relieve a patient from headache of various. We experienced a rare case of subarachnoid hemorrhage by aneurysmal rupture after SGB. A 46-year-old female patient diagnosed with tension headache, and normal MRI finding consulted our pain clinic. We performed right SGB in combination with greater occipital nerve block. The next day, we performed left SGB with 6 ml of 0.25% bupivacaine. She had no evidence of subarachnoid block or intravascular injection. 15 minutes after injection, she abruptly developed convulsion and loss of consciousness. She was given artificial respiration with oxygen. The diagnosis of ruptured left posterior communicating aneurysm was confirmed by 4-vessels angiography.

Infantile Dural Arteriovenous Fistula of the Transverse Sinus Presenting with Ocular Symptoms, Case Reports and Review of Literature

  • Dural arteriovenous fistula (DAVF) of the transverse sinus with ophthalmic manifestations in young children are rare. We reviewed two cases of direct AVF of the transverse sinus with ocular manifestations managed at our institution. The first, a 2.5 years old male child presented with left exophthalmos. Angiography revealed AVF between the occipital artery and the transverse sinus. The second, a 2 years old female child, complained of left exophthalmos. Imaging studies showed bilateral direct AVFs of the transverse sinus with bilateral dysmaturation of the sigmoid sinus. Transarterial embolization was done in both cases. Clinical and radiological follow up revealed complete cure. This report suggests that DAVF of the transverse sinus supplied by the external carotid branches can present with ophthalmic manifestations especially if there is distal venous stenosis or obliteration involving sigmoid sinus. Transarterial embolization using coils and liquid embolic agents could be safe and feasible to obliterate the fistula.

Papillary Meningioma with Leptomeningeal Seeding

  • A 43-year-old male presenting with headache and dizziness underwent craniotomy and gross total resection of an extraaxial tumor was achieved via left occipital interhemispheric approach. The tumor was diagnosed as papillary meningioma arising from the left falcotentorium with such pathologic characteristics of bronchoalveolar adenocarcinoma. At postoperative day 40, he developed generalized tonic clonic seizure and then progressed to a status epilepticus pattern. Brain magnetic resonance imaging showed irregular leptomeningeal enhancement with a significant peritumoral area. Through a cerebrospinal fluid (CSF) study, we identified the meningioma cells of the papillary type from the CSF. At the postoperative day 60, he fell into semicomatose state, and the computed tomography imaging showed low density on both cerebral hemispheres, except the basal ganglia and cerebellum, with overall brain swelling and an increased intracranial pressure. He died on the following day. We experienced a rare case of a papillary meningioma with leptomeningeal seeding.

Intra-Rater and Inter-Rater Reliability of Brain Surface Intensity Model (BSIM)-Based Cortical Thickness Analysis Using 3T MRI

  • Purpose: Brain surface intensity model (BSIM)-based cortical thickness analysis does not require complicated 3D segmentation of brain gray/white matters. Instead, this technique uses the local intensity profile to compute cortical thickness. The aim of the present study was to evaluate intra-rater and inter-rater reliability of BSIM-based cortical thickness analysis using images from elderly participants. Materials and Methods: Fifteen healthy elderly participants (ages, 55-84 years) were included in this study. High-resolution 3D T1-spoiled gradient recalled-echo (SPGR) images were obtained using 3T MRI. BSIM-based processing steps included an inhomogeneity correction, intensity normalization, skull stripping, atlas registration, extraction of intensity profiles, and calculation of cortical thickness. Processing steps were automatic, with the exception of semiautomatic skull stripping. Individual cortical thicknesses were compared to a database indicating mean cortical thickness of healthy adults, in order to produce Z-score thinning maps. Intra-class correlation coefficients (ICCs) were calculated in order to evaluate inter-rater and intra-rater reliabilities. Results: ICCs for intra-rater reliability were excellent, ranging from 0.751-0.940 in brain regions except the right occipital, left anterior cingulate, and left and right cerebellum (ICCs = 0.65-0.741). Although ICCs for inter-rater reliability were fair to excellent in most regions, poor inter-rater correlations were observed for the cingulate and occipital regions. Processing time, including manual skull stripping, was $17.07{\pm}3.43min$. Z-score maps for all participants indicated that cortical thicknesses were not significantly different from those in the comparison databases of healthy adults. Conclusion: BSIM-based cortical thickness measurements provide acceptable intra-rater and inter-rater reliability. We therefore suggest BSIM-based cortical thickness analysis as an adjunct clinical tool to detect cortical atrophy.

Effect of Transcranial Direct Current Stimulation on Concentration of Attention (경두개직류전류자극이 정상인의 주의집중력에 미치는 영향)

  • Purpose : Study on the effects of attention(working memory) for normal adults by applying transcranial Direct Current Stimulation(tDCS). Methods : There are two groups of 10 random people aged between 20 to 30, one is an experimental group(stimulation) and the other is a comparison group(simulate stimulation). For the 1st da y, pre-stimulation test is executed, and for the 2nd day to the 4th day, tDCS is applied stimulated both on the right prefrontal lobe and the left occipital lobe through the electrode. The stimulation lasts for 15 minutes with voltage power of 1mA for each day. The post-stimulation test is executed on the day of 5th. In this paper, a FAIR attention test is used for measuring the attention. Results : As the result of the experiment, there are significant gaps both on the value of P, the ability of selective attention, and the value of C, the consistent ability after the stimulation on right prefrontal and left occipital lobe. And there is no significant gap on the value Q, the quality(Control). However, all the P, Q, and C values are increased according to the average value between the pre-stimulation test and the post-stimulation. Conclusion : Finally, ability of selective attention and self-control, and consistent attention is progressed by applying the tDCS to the normal people.

  • The author studied EEG differences among normal, children, children with emotional behavioral problems determined by Rutter's questionnaire and children with attention-deficit hyperactivity disorder according to DSM-III-R. The results are as follows ; There were no differences in incidence and pattern of abnormal EEG between normal control children and children with emotional behavioral problems. But children with attention-deficit hyperactivity disorder tend to show lower average amplitude, and less wave percentage time of alpha wave in occipital area than normal did, furdermore wave percentage time of alpha wave appeared more in left occipital area than in right occipital area, These results demonstrated that emotional behavioral problems in elementary school children may not be related to EEG abnormality, Where as children with attention deficit hyperactivity disorder showed underdeveloped alpha waves as compared with normal control children.

Foramen Magnum Decompression with Adipose Tissue Grafting for Caudal Occipital Malformation Syndrome in a Maltese Dog

  • A 7-year-old intact female Maltese dog was referred to the hospital with a history of paresis in the hind limbs, left head turn, and a loss of balance that persisted for 2 weeks. Her condition was initially managed with steroids, prescribed by the referring veterinarian, but her neurological symptoms were not alleviated. Physical and neurological examinations, radiography, computed tomography, and magnetic resonance imaging were performed. Based on the findings on these examinations, caudal occipital malformation syndrome (COMS) with syringohydromyelia was diagnosed. Medical treatment was not effective in the previous trial; therefore, foramen magnum decompression, durotomy, and free autogenous adipose tissue grafting were performed. After 3 days, an improvement was observed in the clinical symptoms and was maintained for 8 months postoperatively. Based on the results, it is suggested that the decompression method with a fat graft may be considered an effective surgical treatment for the management of COMS that did not respond well to previous medical treatment.

Associations between Brain Perfusion and Sleep Disturbance in Patients with Alzheimer's Disease

  • Background and Purpose Although sleep disturbances are common and considered a major burden for patients with Alzheimer's disease (AD), the fundamental mechanisms underlying the development and maintenance of sleep disturbance in AD patients have yet to be elucidated. The aim of this study was to examine the correlation between regional cerebral blood flow (rCBF) and sleep disturbance in AD patients using technetium-99m hexamethylpropylene amine oxime single-photon emission computed tomography (SPECT). Methods A total of 140 AD patients were included in this cross-sectional study. Seventy patients were assigned to the AD with sleep loss (SL) group and the rest were assigned to the AD without SL group. SL was measured using the sleep subscale of the Neuropsychiatric Inventory. A whole-brain voxel-wise analysis of brain SPECT data was conducted to compare the rCBF between the two groups. Results The two groups did not differ in demographic characteristics, severity of dementia, general cognitive function, and neuropsychiatric symptoms, with the exception of sleep disturbances. The SPECT imaging analysis displayed decreased perfusion in the bilateral inferior frontal gyrus, bilateral temporal pole, and right precentral gyrus in the AD patients with SL group compared with the AD patients without SL group. It also revealed increased perfusion in the right precuneus, right occipital pole, and left middle occipital gyrus in the AD with SL group compared with the AD without SL group. Conclusions The AD patients who experienced sleep disturbance had notably decreased perfusion in the frontal and temporal lobes and increased rCBF in the parietal and occipital regions. The findings of this study suggest that functional alterations in these brain areas may be the underlying neural correlates of sleep disturbance in AD patients.

Effects of an Agro-Healing Activity Program on the Physiological Condition of Adults with Chronic Metabolic Diseases

  • This study was conducted to find out the effects of agro-healing activities and strolling on the physiological conditions of patients with chronic metabolic diseases. A total of 11 subjects participated in agro-healing activities, and their average age was 49.6±11.3. Changes in their salivary cortisol were compared and analyzed before and after participating in the agro-healing activity program, and it was found that the stress hormone decreased after participating in the program. Differences in brain wave between before and after participating in agro-healing activities and strolling were measured, and the ratio of alpha to high beta (RAHB) of subjects increased in most of the measured points after participating in agro-healing activities. In addition, the increase in the value in the temporal lobe (T3) that controls mortor skills and occipital lobe (O2) that controls visual functions indicates that the activities were effective in improving relaxation and stability. The relative low beta (RLB) power spectrum of subjects also statistically significantly decreased in the right occipital lobe (O2) that controls visual functions, the relative mid beta (RMB) and relative high beta (RHB) power spectrum decreased in the left temporal lobe (T3) and the right occipital lobe (O2). These results indicate that the activities were effective in improving relaxation under no stress and tension. The systolic and diastolic blood pressure of subjects after participating in agro-healing activities decreased from the prehypertension level to the normal level. These results indicate that horticultural activities and visual elements in healing farms, such as agro-healing activities and strolling, have positive impacts on patients with chronic metabolic diseases accompanied with diabetes or high blood pressure by increasing the brain activity and psycho-physiological conditions of participants.
