Recently electric shock accidents constantly occurs caused by the street lamps. Especially the chance of electric shock accident is high when the street lamp submerges by heavy rainfall. Electric shock accident occurs mostly on the low voltage facilities of 220V, but the awareness of its danger is insufficient. The electric shock accident by street lamp voltage of 220V is very dangerous because it is installed in the street which is easily in contact with people. But there are insufficient investigation concerning the affect to hwnan body of underwater electric potential distribution as the distance changes from the leakage object in case of short circuit. In this thesis, the analysis will be made on the affect of underwater Earth leakage to human body and electric potential distribution in underwater, and to draw a comparison between electric shock channel and electric shock mechanism by experimenting on the affect to human body of underwater electric shock as the distance changes from the leakage object.
The leakage of hazardous materials due to the defect in storage tank foundations is likely to cause tremendous fire disasters in the industry cluster area. Thus, adequate design and construction of the tank foundation is required for preventing tank leakage. In this study, four types of typical tank foundations were classified and modeled for 3D FEM analysis to perform stability evaluation on tank foundations. Furthermore, numerical analysis indicated that stress concentration just below the tank shells is 40 times that at the tank center. The settlement influence zone is about the tank radius and tank diameter in the horizontal and vertical directions, respectively. Thus, the appropriate guidelines for the design and construction of tank foundations were suggested via a comparison assessment of the numerical analysis results on the stress distribution and displacement of the tank foundations.
Background: To prevent small leakage accidents, a real-time and direct detection system for small leaks with a detection limit below that of existing systems, e.g. $0.5gpm{\cdot}hr^{-1}$, is required. In this study, a small-size beta detector, which can be installed inside the reactor containment (CT) building and detect small leaks directly, was suggested and its feasibility was evaluated using MCNPX simulation. Materials and Methods: A target nuclide was selected through analysis of radiation from radionuclides in the reactor coolant system (RCS) and the spectrum was obtained via a silicon detector simulated in MCNPX. A window was designed to reduce the background signal caused by other nuclides. The sensitivity of the detector was also estimated, and its shielding designed for installation inside the reactor CT. Results and Discussion: The beta and gamma spectrum of the silicon detector showed a negligible gamma signal but it also contained an undesired peak at 0.22 MeV due to other nuclides, not the $^{16}N$ target nuclide. Window to remove the peak was derived as 0.4 mm for beryllium. The sensitivity of silicon beta detector with a beryllium window of 1.7 mm thickness was derived as $5.172{\times}10^{-6}{\mu}Ci{\cdot}cc^{-1}$. In addition, the specification of the shielding was evaluated through simulations, and the results showed that the integrity of the silicon detector can be maintained with lead shielding of 3 cm (<15 kg). This is a very small amount compared to the specifications of the lead shielding (600 kg) required for installation of $^{16}N$ gamma detector in inside reactor CT, it was determined that beta detector would have a distinct advantage in terms of miniaturization. Conclusion: The feasibility of the beta detector was evaluated for installation inside the reactor CT to detect small leaks below $0.5gpm{\cdot}hr^{-1}$. In future, the design will be optimized on specific data.
Many sorts of fatal accidents like explosion or fire caused by gas leakage have become a social issue with the increasing use of harmful chemicals in laboratories in universities and enterprise-affiliated research institutes in Korea. Importance of safety management has been emphasized and it made Act on the Establishment of Safe Laboratory Environment enacted not only to protect lives and bodies of people working in laboratories in universities and enterprise-affiliated research institutes but also to make pleasant experimental atmosphere. Safety management system has been built and periodical checkups and safety diagnosis have been implementing in universities and enterprise-affiliated research institutes to prevent such accidents. However, in spite of those actions, continuous accidents make analysis of root cause essential. This study will show results of analysis on incidents and accidents occurred in laboratories in universities and enterprise-affiliated research institutes using Root Cause Analysis Method and propose the direction of safety management.
A large number of underground pipelines in the Ulsan National Industrial Complex has been constructed to improve the productivity of chemical products and tackle transportation problems. Now, the total of 1,293km of underground pipelines around 62 companies has been installed and operated. Many of underground pipelines have been installed outside of factories. For a past three years, five gas leakage accidents have occurred and the emergency response took up to 8 hours or more. Due to these delay in accidents, second serious accidents might occur and lead to occur damages to adjacent residents. In this study, it is assumed that emergency valve systems are installed under a ground and the efficacy of these is verified. Consequence analysis program was employed to evaluate the mitigation impact of emergency valve systems. The results show that these valve systems are economical and their performances for a mitigation are excellent. The results indicate that the installation of emergency valve systems for underground pipelines should be urgently legislated and performed.

With the increased demand for membrane structures in recent years, there have been many studies of their mechanical properties, to the extent that such structures have become recognized as independent structures with a level of safety and durability comparable to those of other general structures. But in reality, the study for the maintenance of membrane structures has not been as active. In particularly, the study of watertightness from the perspective of maintenance has been very limited. Accordingly, a study on securing watertightness performance and the guidelines for maintenance is necessary. In this study, through a case study of water leakage accidents in membrane structures overseas, causes of leakage were selected from the membrane material itself, joint parts and open door of roof part in membrane structure. The water leakage and deterioration elements were analyzed from those leakage causes. The degree of importance of the water leakage and deterioration index was also designated using the AHP (Analytic Hierarchy Process) method. As a result, the basic technical index was suggested for the maintenance of the roofs of large-span membrane structures to prevent water leakage. This index will be used to make a guideline for the long-term maintenance of the roofs of large-span membrane structures.
Many state agencies and companies collect personal data for the purpose of providing public services and marketing activities and use it for the benefit and results of the organization. In order to prevent the spread of COVID-19 recently, personal data is being collected to understand the movements of individuals. However, due to the lack of technical and administrative measures and internal controls on collected personal information, errors and leakage of personal data have become a major social issue, and the government is aware of the importance of personal data and is promoting the protection of personal information. However, theory-based training and document-based intrusion prevention training are not effective in improving the capabilities of the privacy officer. This study analyzes the processing steps and types of accidents of personal data managed by the organization and describes measures against personal data leakage and misuse in advance. In particular, using Capture the Flag (CTF) scenarios, an evaluation platform design is proposed to respond to personal data breaches. This design was proposed as a troubleshooting method to apply ISMS-P and ISO29151 indicators to reflect the factors and solutions to personal data operational defects and to make objective measurements.

The Nuclear containment building is a main safety-related structure that performs shielding and conservation functions to prevent highly radioactive materials from leakage to the outside environment in the case of various environmental conditions and postulated accidents. The containment building contains a reactor, steam generator, pressurizer, tank, reactor coolant system, auxiliary system and engineering safety system, and is designed so that highly radioactive materials above the limits specified in 10 CFR 100 do not escape to the outside environment in the case of LOCA(Loss of Coolant Accident) for instance. The containment metal liner plate(CLP) is a carbon steel plate with a nominal plate thickness of 6 mm, which functions as a mold for the wall and dome of the containment building when concrete is filled, fulfills airtightness to prevent leakage of seriously radioactive materials. In recent years, backside corrosion was found on the liner plate in some domestic nuclear power plants. The main cause of backside corrosion was unfilled concrete. In this paper, an inspection technique of assessing filling suitability for CLP backside concrete is developed. Results show that the validity of inspection technique for CLP backside concrete using vibration sensor is successfully verified.
Most plants producing semiconductors use a lot of chemicals, hazardous materials, and explosive gases. Though those materials are hazardous too much, some works still have to be done manually by human workers. However, according to a historical survey, more than half industrial accidents of those plants resulted from human errors or malfunctions. Thus, this research aimed 1) to diagnose shifting hazardous materials of semiconductor plants, 2) to estimate failure probability of human workers through human reliability analysis, and 3) to find out the tasks on which educational emphasis should be put. Through personal interview and visiting working spots, shifting tasks were analyzed, and modelled into a 24-step work, and after that, THERP and ETA was applied. During the shifting work, estimated human failure probability under the assumption of independency, 2.3004E-05, underestimated that probability 8. l008E-05 which could be calculated under the assumption of dependency. And this analysis showed that gas leakage from an old cylinder occupies 78.27% in the case of independent failures whereas gas leakage from a new cylinder occupies 75.06% in the case of dependent failures. So it was concluded that dependency assumption may gloss real situations. In addition, confirming gauge of regulators and closing valves turned out to be the most important tasks than purge tasks.

As use of tunnels and subways increase there also are accidents proportionate to it. Daegu Subway Station fire, Hongjimoon tunnel fire led people to be conscious of disaster protection and as a result, there is a trend to adopt standards for fire protection. Accordingly, this thesis is focused on investigating various fire and water protection related issues for subaqueous tunnel under Ran river. The thesis developed evacuation and disaster prevention plan as fire level increases and have identified the suitability of disaster prevention through evacuation and fire simulation, countermeasure of a water leakage during construction and operation considering the subaqueous tunnel. And we selected EPB shield TBM equipment considering the ground condition and effect of boring hole, and accomplished reasonable water protection design through setting goals using event-tree method, as well as examining model test of boring hole and flooding in heavy rain. Also included structured total system consist of water leakage sensing system, water protection gate, pumping system and fire protection system to respond systematically in emergency.
