Lateral plantar nerve entrapment combined with a chronic plantar fasciitis in a basketball player -A case report- (농구선수에서 발생한 만성 족저근막염이 동반된 외측 족저 신경 압박증 -증례 보고-)

  • In athletes, repetitive low-energy impacts in plantar lesion lead often to tendinitis, stress fracture, or overuse syndrome. The major cause of heel pain in athletes is plantar fasciitis. And it is most often attributable to repetitive low energy impact, but the vast majority patients with heel pain achieve symptomatic relief with conservative treatment and return to full activities. Not commonly, Nerve entrapment may be occurred from repetitive low energy trauma in athletes, and is not as easily diagnosed. The authors observed a basketball player who complained of chronic heel pain that do not respond to conservative treatment, he had the lesions both plantar fasciitis and lateral plantar nerve entrapment. The authors described an unusual mechanism of entrapment of the lateral plantar nerve combined with a chronic plantar fasciitis in a basketball player and reported with review of literature.

Subcutaneous Fascial Pedicled Lateral Supramalleolar Flap (피하 근막 혈관경을 사용한 외측 과상부 피판술)

  • Soft-tissue reconstruction of the foot and ankle has long been a challenge for reconstructive surgeons. Limitations in the available local tissue and donor-site morbidity restrict the options. In an effort to solve these difficult problems, the authors have begun to use a subcutaneous fascial pedicled lateral supramalleolar flap. This report presents the authors' experience with five patients treated with this flap. The patients’ ages ranged from 26 to 72 years; four of the patients were male and one was female. The cause of the soft-tissue defects involved acute trauma and malignant melanom. All flaps survived and provided satisfactory coverage of the defect. Compared with the classic lateral supramalleolar flap, when the perforating branch is interrupted in its course, it is possible to elevate this subcutaneous fascial pedicled flap. The distally based flap with a compound pedicle which is continuous with a vascular axis and a band of subcutaneous fascial pedicle has long pedicle. This procedure is valuable for remote defect of the foot. It is believed that this flap is versatile and effective and is a good addition to the available techniques used by reconstructive surgeons for coverage of the foot and ankle.

The Effects of Task-Oriented Training for Left Trunk Flexion Pattern Using Real-Time Ultrasound Imaging -A Single-Subject Experimental Study- (편측무시 환자에게 실시간 초음파 영상을 이용한 왼쪽 몸통 굽힘 패턴을 적용한 과제지향적 훈련의 효과 -단일 사례 연구-)

  • Purpose: This study investigates the effects of task-oriented training for a left trunk flexion pattern using real-time ultrasound imaging in a stroke patient with unilateral neglect symptoms. Methods: This study used the ABA experimental design, which is a single-subject research method among individual case research methods. For the ABA experimental design, changes in the degree of unilateral neglect, balance ability, and the thickness of the lateral abdominal muscle were visually analyzed during the baseline process, in the intervention period, and after the intervention. The experiments were performed 24 times in total for 8 times in each of the 3 periods. The unilateral neglect was measured using the Albert test, balance ability was measured using the Berg balance test, and the thickness of the lateral abdominal muscle was measured using ultrasound imaging. The subject was a 50-year-old male patient with unilateral neglect caused by right cerebral hemorrhage. He performed task-oriented training for a voluntary left trunk flexion pattern using real-time ultrasound imaging during the intervention period. Results: The result of comparing the data collected during the intervention period with the data point average of the baseline process showed that balance ability improved and the tendency line was above the baseline. The tendency line of unilateral neglect was below the baseline and showed a decreasing tendency. The thickness of the lateral abdominal muscle showed an increasing trend and the tendency line was above the baseline. Conclusion: The results of this study suggest that the task-oriented training for left trunk flexion pattern using real-time ultrasound imaging has a beneficial effect on balance ability, the degree of unilateral neglect, and the strength of the lateral abdominal muscle in unilateral neglect patients.

  • The current cadaveric report describes an unusual morphology of the sartorius muscle (SM), the biceps-bicaudatus variant. The SM had two (lateral and medial) heads, with distinct tendinous origins from the anterior superior iliac spine. The lateral head was further split into a lateral and a medial bundle. The anterior cutaneous branch of the femoral nerve emerged between the origins of the lateral and medial heads. SM morphological variants are exceedingly uncommon, with only a few documented cases in the literature, and several terms used for their description. Although their rare occurrence, they may play an important role in the differential diagnosis of entrapment syndromes, in cases of neural compressions, such as meralgia paresthetica, while careful dissection during the superficial inter-nervous plane of the direct anterior hip approach is of utmost importance, to avoid adverse effects due to the altered SM morphology.

Effect of Training Time on Quality and Yield of 'Sinsakigake-2' Pepper (정지시기에 의한 '신사기가께2호' 고추의 품질과 수량)

  • This study was conducted to investigate the effect of training time on the growth and yield of 'Sinsakigake-2' pepper (Capsicum annuum L.). Plants were either left untrained as control or trained at the third node leaving four shoots per plant. Plants were traind at 20, 45, 70, or 95 days after transplanting (DAT). The earlier the training time, the more the branch number of the plant. Fruit weight was not affected by training time. However, the percentage of marketable fruits and the number of marketable fruits per plant were the highest in plants trained at 70 DAT, and the lowest in the control. Early marketable yield was the highest in the control, and the lowest in plants trained at 45 DAT.

  • The behavior of moment resistant steel structures depends on both the beam-column connections and columns foundations connections. Obviously, if the connections can meet the adequate ductility and resistance against lateral loads, the seismic capacity of these structures will be linked practically to the performance of these connections. The shape memory alloys (SMAs) have been most recently used as a means of energy dissipation in buildings. The main approach adopted by researchers in the use of such alloys is firstly bracing, and secondly connecting the beams to columns. Additionally, the behavior of these alloys is modeled in software applications rarely involving equivalent torsional springs and column-foundation connections. This paper attempts to introduce the shape memory alloys and their applications in steel structural connections, proposing a new steel column-foundation connection, not merely a theoretical model but practically a realistic and applicable model in structures. Moreover, it entails the same functionality as macro modeling software based on real behavior, which can use different materials to establish a connection between the columns and foundations. In this paper, the suggested steel column-foundation connection was introduced. Moreover, exploring the seismic dynamic behavior under cyclic loading protocols and the famous earthquake records with different materials such as steel and interconnection equipment by superelastic shape memory alloys have been investigated. Then, the results were compared to demonstrate that such connections are ideal against the seismic behavior and energy dissipation.

  • The effects of past earthquakes have demonstrated the seismic vulnerability of confined masonry structures (CMSs) to earthquakes. The results of experimental analysis indicate that damage to these structures depends on lateral displacement applied to the walls. Seismic evaluation lacks an analytical approach because of the complexity of the behavior of this type of structure; an empirical approach is often used for this purpose. Seismic assessment and risk analysis of CMSs, especially in area have a large number of such buildings is difficult and could be riddled with error. The present study used analytical and numerical models to develop a simplified nonlinear displacement-based approach for seismic assessment of a CMS. The methodology is based on the concept of ESDOF and displacement demand and is compared with displacement capacity at the characteristic period of vibration according to performance level. Displacement demand was identified using the nonlinear displacement spectrum for a specified limit state. This approach is based on a macro model and nonlinear incremental dynamic analysis of a 3D prototype structure taking into account uncertainty of the mechanical properties and results in a simple, precise method for seismic assessment of a CMS. To validate the approach, a case study was considered in the form of an analytical fragility curve which was then compared with the precise method.

Soft Tissue Reconstruction of Extremity using Anteromedial Thigh Perforator Free Flap (전 내측 대퇴부 천공지 유리 피판술을 이용한 사지의 연부조직 재건)

  • Thigh perforator flaps are used popularly, because they have a small thickness, a long vascular pedicle and a low rate of donor site morbidity. Among thigh perforator flaps, anterolateral thigh perforator flaps are generally used, but it is not easy to dissect the vastus lateralis muscle to find the vasuclar pedicle. The authors have planed 11 anteromedial thigh perforator flaps that have septocutaneous perforators. We were able to find anteromedial thigh perforators in 8 cases(72.7%). The perforators originated from the medial descending branch of the lateral circumflex femoral artery in 2 cases and a branch to the rectus femoris muscle in 6 cases. Of this type of surgery, 7 flaps survived. However, 1 flap failed because of arterial insufficiency. We believe the anteromedial thigh perforator flap is a good option when the thigh region is decided as a donor site, but surgeons should keep in mind that the perforator of anteromedial thigh flap may be absent.

Local and global buckling condition of all-steel buckling restrained braces

  • Braces are one of the retrofitting systems of structure under earthquake loading. Buckling restrained braces (BRBs) are one of the very efficient braces for lateral loads. One of the key needs for a desirable and acceptable behavior of buckling-restraining brace members under intensive loading is that it prevents total buckling until the bracing member tolerates enough plastic deformation and ductility. This paper presents the results of a set of analysis by finite element method on buckling restrained braces in which the filler materials within the restraining member have been removed. These braces contain core as the conventional BRBs, but they have a different buckling restrained system. The purpose of this analysis is conducting a parametric study on various empty spaces between core and restraining member, the effect of friction between core and restraining member and applying initial deformation to brace system to investigate the global buckling behavior of these braces. The results of analysis indicate that the flexural stiffness of restraining member, regardless of the amount of empty space, can influence the global buckling behavior of brace significantly.

  • Four tetraploid mulberry lines, Sawonppon No.11, No.13 and No.14, were induced by the colchicine treatment on the shoot tips of seedlings originated from the cross between Yongchonppong and Kaeryanppong. The major characteristics of the tetraploid lines were as follows: Green tip sprouting stage was similar to Kaeryanppong, a medium budding variety. Leaves were medium- to large-sized cordate type. Leaf surface was rougher and stronger than that of the diploid parents. Leaf thickness, leaf area weight and leaf water content were higher than those of the diploid parents. Average branch length was shorter than that of the diploid parents. Internode length and number of lateral branches were similar to the average values of the two parents. Death atop rate of branch was 1.6%~2.5% indicating strong cold-hardiness of the tetraploid lines. Therefore, these lines could be used as sources of cold-hardiness in developing triploid lines.
