A LATERAL CONTROL ALGORITHM FOE ROLL-TO-ROLL WEB SYSTEM BASED ON BACK-STEPPING APPROACH

  • Roll-to-roll based manufacturing plays an important role in producing devices at high speed with lower production cost in printed electronics and publishing industry. Web lateral control is one of the most important factors in improving the quality of product and contributes a considerable point in making devices at micrometer-level accuracy. In recent years, most algorithms proposed for web lateral control base on the Shelton‘s model for designing the feedback control system using the PI controller. Experimental results showed that the existing models do not fully describe the characteristics of the lateral dynamics for some typical operating conditions and so result in poor control algorithms. In this paper, a new lateral control algorithm is proposed for web lateral control system based on back-stepping approach. The outcome of this study proves the reliability throughout simulation results in Matlab/Simulink and comparison with the algorithms based on the existing results.

족근동 접근법을 이용한 관절 내 종골 골절의 치료 (The Sinus Tarsi Approach for the Treatment of Intra-Articular Calcaneal Fractures)

  • Open reduction and internal fixation is currently considered as a gold standard of treatment in most of the intra-articular calcaneal fractures. Among various different approaches, extensile lateral approach is the most popular since it provides good exposure to the subtalar joint. However, wide skin incision followed by extensive soft tissue dissection leading to increased risk of wound breakdown is the most serious drawback. Sinus tarsi approach, a minimal invasive technique to approach the subtalar joint and reduce the intra-articular calcaneal fractures, provides good clinical outcome and less wound complications compared to the extensile lateral approach. This article introduces the surgical technique and review of the literature regarding the sinus tarsi approach.

전방 도달법과 경비골 도달법을 이용한 족관절 유합술의 결과 비교 (Comparison of the Outcomes between the Anterior and Transfibular Approaches in Ankle Arthrodesis)

  • Purpose: This study compared the clinical and radiology results of arthrodesis between the anterior and transfibular approaches with a lateral malleolar-saving procedure in ankle arthritis. Materials and Methods: This study was a retrospective study of 31 cases who underwent ankle arthrodesis with the anterior approach (14 cases) and trans-fibular approach (17 cases). The remnant lateral malleolus was fixed during the trans-fibular procedure. The patients included 17 females and 14 males with a mean age of 57.2 (range 41~73) years; the mean follow-up was 30.4 (range 15~68) months. The clinical and radiology outcomes, including the American Orthopedic Foot & Ankle Society (AOFAS) scores, visual analog scale (VAS), and union time, were recorded. The complications and subjective satisfaction degrees were also recorded and compared between the two groups. Results: Clinically, the preoperative mean AOFAS score and VAS in the anterior approach group were 39.3 and 7.4, respectively, which changed to 61.9 and 3.1 postoperatively. In the trans-fibular approach group, the mean AOFAS score and VAS increased from 36.6 to 64.2 and 7.1 to 2.4, respectively. On the other hand, no significant differences in the clinical results were observed between the two groups. The time to achieve union was 9.2 and 11.6 weeks in the anterior and transfibular approach groups, respectively. Three patients (21%) complained of tenderness and discomfort around the fibular tip in the anterior approach group, and seven patients (41%) showed a gap between the talus and remnant lateral malleolus in the trans-fibular approach group. Conclusion: There was no difference in the clinical and radiological results between the anterior and transfibular approaches with a lateral malleolar saving procedure in ankle arthrodesis. Careful selection of the approach method according to the patient's preoperative condition is needed to prevent remnant discomfort or nonunion around the lateral malleolus.

Aneurysm of the Posterior Inferior Cerebellar Artery: Clinical Features and Surgical Results

  • Objective: Aneurysms arising from the posterior inferior cerebellar artery(PICA) are uncommon. We review literature on that and surgical results on aneurysmal treatment by choice of surgical approach. Methods: On the basis of radiologic findings & charts, we review retrospectively the surgical results of 12 cases from Mar 1999 to Dec 2003. Results: The mean age of the 12 patients was 55.8(ranged from 36 to 71) and female was predominant (female:male = 8:4). Locations of PICA aneurysms revealed variously(vertebral artery - PICA junction: 8, lateral medullary segment: 2, PICA - anterior inferior cerebellar artery common trunk: 1, telovelomedullary : 1). Surgical approaches & treatments were attempted in 11 cases and embolization was done in 1 case(Far lateral transcondylar or supracondylar approach & clipping: 9, Far lateral transcondylar or supracondylar approach and trapping: 2, suboccipital approach & clipping: 1). The surgical result were 8 of 12 patients were good outcome, 1 of 12 was severely disabled and 3 of 12 were died. Conclusion: First, we choose surgical approach by the laterality of aneurysms and surgical or interventional treatment is attempted as soon as possible. The PICA aneurysm is regarded as having a relatively good surgical outcome without drilling of the posterior arch of the atlas.

편도와 접근을 통한 측방인두간극 이물제거의 치험 증례 (REMOVAL OF FOREIGN BODY IN THE LATERAL PHARYNGEAL SPACE VIA TRANSTONSILLAR APPROACH)

  • Lateral pharyngeal space is one of potential fascial planes of head and neck, that may become involved by various pathological processes, such as infection, inflammation and neoplasm. The calcified stylohyoid ligament with styloid process is also located in this space, so this space is more acquainted with Eagle's syndrome in oral and maxillofacial field. During the mandibular transbuccal fixation procedures of 29-year old female patient who had right condylar neck and left parasymphysis fracture, we had lost one 10.0 mm miniscrew. After confirming the location of the lost miniscrew from different angled plain skull radiographies, we tried to find it in the lateral pharyngeal space via transtonsillar approach at the time of plate removal operation. This case report is aimed to share our valuable experience of the effective approach way to the lateral pharyngeal space, which has many advantages, such as short operative time, minimal bleeding, fast post-operative recovery, and less morbidity. The related literature is also reviewed.

A Modified Approach of Percutaneous Endoscopic Lumbar Discectomy (PELD) for Far Lateral Disc Herniation at L5-S1 with Foot Drop

  • Foraminal or extraforaminal Far Lateral Disc Herniations (FLDH) extending into or beyond the foraminal zone have been recognized as between 7-12% of all lumbosacral disc herniations. Conventional posterior laminectomy may not provide good access to a herniation that lies far lateral to the lateral margin of the pedicle. Use of the endoscopic technique through a percutaneous approach to treat such FLDH patients can decrease the surgical morbidity while achieving better outcomes. We made an effort to utilize the advantages of percutaneous endoscopic lumbar discectomy (PELD) and to determine the appropriate approach for FLDH at the level between the 5th Lumbar and first Sacral vertebrae(L5-S1). The authors present a case of an endoscopically resected lumbar extruded disc of the left extraforaminal zone with superior foraminal migration at the level of L5-S1, which had led to foot drop, while placing the endoscope in the anterior epidural space without facetectomy.

Three-column reconstruction through the posterior approach alone for the treatment of a severe lumbar burst fracture in Korea: a case report

  • Generally, patients with severe burst fractures, instability, or neurological deficits require surgical treatment. In most cases, circumferential reconstruction is performed. Surgical methods for three-column reconstruction include anterior, lateral, and posterior approaches. In cases involving an anterior or lateral approach, collaboration with general or thoracic surgeons may be necessary because the adjacent anatomical structures are unfamiliar to spinal surgeons. Risks include vascular or lumbar plexus injuries and cage displacement, and in most cases, additional posterior fusion surgery is required. However, the posterior approach is the most common and anatomically familiar approach for surgeons performing spinal surgery. We present a case in which three-column reconstruction was performed using only the posterior approach to treat a patient with a severe lumbar burst fracture.

Damage detection for beam structures based on local flexibility method and macro-strain measurement

  • Many vibration-based global damage detection methods attempt to extract modal parameters from vibration signals as the main structural features to detect damage. The local flexibility method is one promising method that requires only the first few fundamental modes to detect not only the location but also the extent of damage. Generally, the mode shapes in the lateral degree of freedom are extracted from lateral vibration signals and then used to detect damage for a beam structure. In this study, a new approach which employs the mode shapes in the rotary degree of freedom obtained from the macro-strain vibration signals to detect damage of a beam structure is proposed. In order to facilitate the application of mode shapes in the rotary degree of freedom for beam structures, the local flexibility method is modified and utilized. The proposed rotary approach is verified by numerical and experimental studies of simply supported beams. The results illustrate potential feasibility of the proposed new idea. Compared to the method that uses lateral measurements, the proposed rotary approach seems more robust to noise in the numerical cases considered. The sensor configuration could also be more flexible and customized for a beam structure. Primarily, the proposed approach seems more sensitive to damage when the damage is close to the supports of simply supported beams.

항공기의 횡방향 접근 제어를 위한 축소차수 상태관측기 설계 (Reduced Order Luenberger State Observer Design for Lateral Direction Approach Control of Aircraft)

  • The availability of the GPS signal has been expanded greatly in the field of society overall through the development and construction of the GNSS(Global Navigation Satellite System). Furthermore, in the military, aviation and field of space, the GPS signal is applied widely through the combination of INS consisting of gyroscope and accelerometer, IMU, AHRS with the addition of magnetic sensor. Particularly, the performance of these equipments or sensors is very important with GPS and PAR(Precision Approach Radar) in the flight control of the aircraft. This paper deals with MATLAB simulation and ROLSO(Reduced Order Luenberger State Observer) design to reduce the load of system and realize the stable lateral direction approach control in an appropriate time for reduction of the horizontal error which is importantly considered while an aircraft lands instead of the FOLSO(Full Order Luenberger State Observer) using all measurement values. Consequently, ROLSO is expected to be used for the aircraft's attitude control in the aircraft landing causing the burden to the pilots.

Lateral arthroplasty along with buccal fat pad inter-positioning in the management of Sawhney type III temporomandibular joint ankylosis

  • Objectives: The objective of this study was to highlight the role of lateral arthroplasty along with interposition of the buccal fat pad (BFP) in the management of Sawhney type III temporomandibular joint (TMJ) ankylosis. Materials and Methods: Ten patients with TMJ ankylosis (7 unilateral and 3 bilateral, total of 13 joints) were treated with lateral arthroplasty and BFP interposition. The bony bridge of the ankylotic mass on the lateral aspect was resected, leaving a distance of 1.5 to 2.0 cm from the base of the skull to the neck of the condyle. The condyle was left intact. Coronoidectomy was performed on the ipsilateral side via the same approach in all cases. The inter-incisal opening was measured at that time, and if it was less than 35 mm, contralateral coronoidectomy was performed by using the intra-oral approach. After satisfactory inter-incisal mouth opening (${\geq}35mm$) was achieved, the TMJ surgical site was revisited, and BFP was retrieved and used to cover the lateral aspect of the medially placed condyle. Results: With lateral arthroplasty, the medially displaced condyle can be left in-situ to maintain the mandibular ramal height and function and to act as a growth center in children. Interposition of the BFP prevents reformation of the lateral bony bridge that was removed. Conclusion: Lateral arthroplasty along with interpositioning of the BFP is a novel technique for managing Sawhney type III ankylosis that achieves management goals while avoiding complex and advanced reconstructive surgical procedures.
