노후된 배관은 예기치 못한 누수나 균열을 발생시킨다. 이를 방치하거나 늦게 대응하면 지속적인 가스자원, 수자원 등의 막대한 손실을 발생시킨다. 본 논문에서는 배관의 결함을 검출하기 위해 음향방출 신호를 사용하는 배관 조기 결함 검출 방법과 진단 알고리즘을 제안한다. 배관의 결함으로 인해 변형이 생길 경우 배관의 고유진동수가 변화하므로 이를 관찰함으로써 배관의 이상 유무를 판단할 수 있다. 배관 조기결함 검출 방법은 정상상태의 스펙트럼과 취득된 신호의 스펙트럼을 주파수 성분의 크기에 대해 비교함으로써 배관의 결함 유무를 판단한다. 배관 조기 결함 진단 알고리즘은 정상상태와 결함상태를 기계학습 알고리즘인 서포트 벡터 머신(SVM)으로 학습하고 실제 취득된 배관 음향방출 신호를 입력하여 배관 상태를 진단한다. 실험에서는 제작된 배관 테스트베드를 사용하여 정상상태, 5mm 균열 상태, 10mm 균열 및 파공 상태를 가공하여 제안 방법을 테스트하였다. 실험 결과에서는 제안한 검출 방법 및 진단 알고리즘의 배관 조기 결함 검출 성능의 우수성을 검증하였다.
Background: One of the most important reasons for the high mortality rates of cancer is the low level of awareness, which can lead to a late diagnosis and treatments starting too late. Therefore, it is necessary for individuals, especially at younger age, to gain awareness and integrate taking the necessary precautions into their lifestyle in order to prevent cancer and ensure early diagnosis. The aim of this study was to assess the levels of awareness of factors for major cancers among students studying in two different campuses of a university in the Western Black Sea Region. Materials and Methods: This descriptive/cross-sectional study was performed between January-March 2014. Students studying in the 2013-2014 academic year in a university in the Western Black Sea Region in two separate campuses were the subjects. Results: The mean age was $21.01{\pm}3.63$. While female students (51.2%) were the majority in the health sciences campus (74.8%), male students (48.8%) were the majority in the social sciences campus (76.5%). Some 9.6% of the students from the health sciences campus and 12.4% of the students from the social sciences campus thought that cancer was communicable, while most of the students from both campuses knew that smoking caused cancer. It can also be seen that the rate of answering questions regarding cancer correctly was higher among students studying in the health sciences campus. Conclusions: It was determined that students who do not study at the health sciences campus have insufficient information on cancer, cancer symptoms, and the possibility of breast cancer in males and approximately half of them regarded obesity as a risk factor for cancer.

Purpose: To examine breast cancer (BC) incidence trends in relation to mammographic screening and risk factor prevalence in South Australia (SA). Materials and Methods: Trends in annual BC incidence rates were calculated using direct standardisation and compared with projected incidence derived from Poisson regression analysis of pre-screening rates. Annual percentage change and change time points were estimated using Joinpoint software. Biennial mammography screening participation rates were calculated using data from BreastScreen SA. Trends in overweight/obesity, alcohol use and hormone replacement therapy (HRT) use were examined using 1991-2009 Health Omnibus Survey data. Trends in total fertility were examined using data from the Australian Bureau of Statistics. Results: BC incidence increased around the time BreastScreen commenced and then stabilised in the mid-1990s. However rates have remained higher than projected, even though the proportion and age distribution of first time screening attendees stabilised around 1998. A decrease in BC incidence was observed among women aged 50-59yrs from the late-1990's but not among older women. Obesity and alcohol use have increased steadily in all age groups, while HRT use declined sharply from the late-1990s. Conclusions: BC incidence has remained higher than projected since mammography screening began. The sustained elevation is likely to be due to lead time effects, though over-diagnosis cannot be excluded. Declining HRT use has also impacted incidence trends. Implications: Studies using individual level data, which can account for changes in risk factor prevalence and lead time effects, are required to evaluate 'over-diagnosis' due to screening.

Introduction: Worldwide, breast cancer is the commonest cause of cancer death in women. However, the survival rate varies across regions at averages of 73%and 57% in the developed and developing countries, respectively. Objective: This study aimed to determine the survival rate of breast cancer among the women of Malaysia and characteristics of the survivors. Method: A retrospective cohort study was conducted on secondary data obtained from the Breast Cancer Registry and medical records of breast cancer patients admitted to Hospital Kuala Lumpur from 2005 to 2009. Survival data were validated with National Birth and Death Registry. Statistical analysis applied logistic regression, the Cox proportional hazard model, the Kaplan-Meier method and log rank test. Results: A total of 868 women were diagnosed with breast cancer between January 2005 and December 2009, comprising 58%, 25% and 17% Malays, Chinese and Indians, respectively. The overall survival rate was 43.5% (CI 0.573-0.597), with Chinese, Indians and Malays having 5 year survival rates of 48.2% (CI 0.444-0.520), 47.2% (CI 0.432-0.512) and 39.7% (CI 0.373-0.421), respectively (p<0.05). The survival rate was lower as the stages increased, with the late stages were mostly seen among the Malays (46%), followed by Chinese (36%) and Indians (34%). Size of tumor>3.0cm; lymph node involvement, ERPR, and HER 2 status, delayed presentation and involvement of both breasts were among other factors that were associated with poor survival. Conclusions: The overall survival rate of Malaysian women with breast cancer was lower than the western figures with Malays having the lowest because they presented at late stage, after a long duration of symptoms, had larger tumor size, and had more lymph nodes affected. There is an urgent need to conduct studies on why there is delay in diagnosis and treatment of breast cancer women in Malaysia.
Purpose: Pompe disease (PD) is an autosomal recessive disorder caused by a deficiency of acid alpha-glucosidase resulting from pathogenic GAA variants. This study describes the clinical features, genotypes, changes before and after enzyme replacement therapy (ERT), and long-term outcomes in patients with infantile-onset PD (IOPD) and late-onset PD (LOPD) at a tertiary medical center. Methods: The medical records of 5 Korean patients (2 male, 3 female patients) diagnosed with PD between 2002 and 2013 at Samsung Medical Center in Seoul, Republic of Korea were retrospectively reviewed for data, including clinical and genetic characteristics at diagnosis and clinical course after ERT. Results: Common initial symptoms included hypotonia, cyanosis, and tachycardia in patients with IOPD and limb girdle weakness in patients with LOPD. Electrocardiography at diagnosis revealed hypertrophic cardiomyopathy in all patients with IOPD who showed a stable disease course during a median follow-up period of 10 years. Patients with LOPD showed improved hepatomegaly and liver transaminase level after ERT. Conclusion: As ERT is effective for treatment of PD, early identification of this disease is very important. Thus, patients with IOPD should be considered candidates for clinical trials of new drugs in the future.
The medical book Yosandangsinjipeuibangkeumnangjibo (『樂山堂新集醫方錦囊至寶』, below as Yosandang, 『樂山堂』) is housed in the Handok museum of Medicine and Pharmacy. At the end of the Yosandang, written in manuscript, contains a rhymes of herbal medicine (藥性歌). The rhymes of herbal medicine is a record of song form about herbal medicine. Song-forms were widely used because they were easy to sing and memorize. In particular, there are many records of song forms in Korean medical books, which have been used in various fields such as herbal medicines, acupuncture points, and diagnosis. Although Yosandang is not a widely known medical book, it is meaningful in that it shows a cross section of late Joseon Korean medicine. So, this paper considers rhymes of herbal medicine in Yosandang. Yosandang is a medical book of the late Joseon period written by doctor Byun Gwangwon in 1806. This book consists of 6 books and 14 volumes, 13 of which are rhymes of herbal medicine. The rhymes of herbal medicine in Yosandang is 7 words-2 phrases form for the first time in Korea. This is almost 80 years earlier than the same form of Bangyakhabpyeon (『方藥合編』). The first part of rhymes of herbal medicine in Yosandang is considered to refer to the rhymes of herbal medicine in Jejungsinpyeon (『濟衆新編』), and the last part seems to be based on the contents of Donguibogam (『東醫寶鑑』). In other words, rhymes of herbal medicine in Yosandang could be considered as the result of trying to contain herbal medicine knowledge as a new 7 words-2 phrases form based on the Jejungsinpyeon (『濟衆新編』) and Donguibogam (『東醫寶鑑』). Unlike the previous rhymes of herbal medicine made during the compilation of medical books led by the late Joseon government, the rhymes of herbal medicine in Yosandang is a new type of rhymes created based on individual efforts in the early 19th century. It has a medicine historical significance in that it can show some aspects of Korean medicine in the late Joseon period.

Study Design: Level III retrospective cross-sectional study. Purpose: To define and characterize the presentation, symptom duration, and patient/surgical risk factors associated with 'post-decompressive neuropathy (PDN).' Overview of Literature: PDN is characterized by lower extremity radicular pain that is 'different' from pre-surgical radiculopathy or claudication pain. Although it is a common constellation of postoperative symptoms, PDN is incompletely characterized and poorly understood. We hypothesize that PDN is caused by an intraoperative neuropraxic event and may develop early (within 30 days following the procedure) or late (after 30 days following the procedure) within the postoperative period. Methods: Patients who consented to undergo lumbar laminectomy with or without an instrumented fusion for degenerative lumbar spine disease were followed up prospectively from July 2013 to December 2014. Relevant data were extracted from the charts of the eligible patients. Patient demographics and surgical factors were identified. Patients completed postoperative questionnaires 3 weeks, 3 months, 6 months, and 1 year postoperatively. Questions were designed to characterize the postoperative pain that differed from preoperative pain. A diagnosis of PDN was established if the patient exhibited the following characteristics: pain different from preoperative pain, leg pain worse than back pain, a non-dermatomal pain pattern, and nocturnal pain that often disrupted sleep. A Visual Analog Scale was used to monitor the pain, and patients documented the effectiveness of the prescribed pain management modalities. Patients for whom more than one follow-up survey was missed were excluded from analysis. Results: Of the 164 eligible patients, 118 (72.0%) completed at least one follow-up survey at each time interval. Of these eligible patients, 91 (77.1%) described symptoms consistent with PDN. Additionally, 75 patients (82.4%) described early-onset symptoms, whereas 16 reported symptoms consistent with late-onset PDN. Significantly more female patients reported PDN symptoms (87% vs. 69%, p=0.03). Patients with both early and late development of PDN described their leg pain as an intermittent, constant, burning, sharp/stabbing, or dull ache. Early PDN was categorized more commonly as a dull ache than late-onset PDN (60% vs. 31%, p=0.052); however, the difference did not reach statistical significance. Opioids were significantly more effective for patients with early-onset PDN than for those with late-onset PDN (85% vs. 44%, p=0.001). Gabapentin was most commonly prescribed to patients who cited no resolution of symptoms (70% vs. 31%, p=0.003). Time to symptom resolution ranged from within 1 month to 1 year. Patients' symptoms were considered unresolved if symptoms persisted for more than 1 year postoperatively. In total, 81% of the patients with early-onset PDN reported complete symptom resolution 1 year postoperatively compared with 63% of patients with late-onset PDN (p=0.11). Conclusions: PDN is a discrete postoperative pain phenomenon that occurred in 77% of the patients who underwent lumbar laminectomy with or without instrumented fusion. Attention must be paid to the constellation and natural history of symptoms unique to PDN to effectively manage a self-limiting postoperative issue.

Spontaneous perforation of the esophagus after forceful vomiting is known as Boerhaave syndrome, a rare and life-threatening condition associated with a high rate of mortality. The management of Boerhaave syndrome is challenging, especially when diagnosed late. Herein, we report the successful management of late-diagnosed Boerhaave syndrome with T-tube drainage in a 55-year-old man. The patient was transferred to our institution 8 days after the onset of symptoms, successfully managed by placing a T-tube, and was discharged on postoperative day 46 without complications.
Radiation induced lung injury has long been considered a treatment limiting factor for patients requiring thoracic radiation. This radiation induced lung injury happens early as well as late. Radiation induced lung injury can occur in two phases viz. early (< 6 months) when it is called radiation pneumonitis and late (>6 months) when it is called radiation induced lung fibrosis. There are multiple factors that can be patient, disease or treatment related that predict the incidence and severity of radiation pneumonitis. Radiation induced damage to the type I pneumocytes is the triggering factor to initiate such reactions. Over the years, radiation therapy has witnessed a paradigm shift in radiation planning and delivery and successfully reduced the incidence of lung injury. Radiation pneumonitis is usually a diagnosis of exclusion. Steroids, ACE inhibitors and pentoxyphylline constitute the cornerstone of therapy. Radiation induced lung fibrosis is another challenging aspect. The pathophysiology of radiation fibrosis includes continuing inflammation and microvascular changes due to pro-angiogenic and profibrogenic stimuli resembling those in adult bronchiectasis. General supportive management, mobilization of airway secretions, anti-inflammatory therapy and management of acute exacerbations remains the treatment option. Radiation induced lung injury is an inevitable accompaniment of thoracic radiation.
This study was performed to evaluate the ultrasonographic findings of ethylene glycol intoxication. Ten healthy mongrel dogs which was administered with ethylene glycol, were evaluated in terms of clinical findings, hematological findings, blood chemistry, and ultrasonographic and histopathological findings of kidney. The results obtained through these experiment could be summarized as follows : 1. Typical clinical symptoms such as vomiting, initial apprehension, depression, thirst, dehydration, tremor, anorexia, hematuria, anuria, weakness, weight loss, flaccid paralysis, tachypnea, coma, and death, were revealed after administration of ethylene glycol. 2. Special symptom of bloody diarrhea was occurred by administration of ethylene glycol. 3. After administration of ethylene glycol, PCV was decreased continuously(p<0.01), and total leukocyte count was increased gradually, revealed the highest value at day 5 and thereafter decreased. 4. Remarkable changes of ultrasonographic findings such as high echo intensity of renal parenchyma and emergence of halo in corticomedullary junction, were revealed after administration of ethylene glycol. Early(hour 8) enlargement and late(day 3) enlargement were observed in kidney(p<0.01). Especially, late enlargement was observed concurrently with the elevation of BUN and creatinine values. 5. Calcium oxalate crystals, metabolites of ethylene glycol, were observed in histopathologic findings of kidney. Also, degeneration and necrotic exfoliation of epithelial cells were recognized in addtion to swelling of renal tubules.
