변형된 Fish-Mouth Technique을 이용한 새로운 기관공성형술식 (Management of Tracheostomal Stenosis : Modified Fish-Mouth Technique)

  • Tracheostomal stenosis after total laryngectomy is brothersome problem to surgeon and patient. To prevent tracheostomal stenosis, tension around the tracheostoma must be released. Advancement flap, V-Y inset and Z-plasty were suggested as solution to tracheostomal stenosis. But such methods need elevation of superior flap and it is very difficult when the patients received radiotherapy and has high change of developing pharyngocutaneous fistula. We suggested new stomaplasty technique which named "Modified Fish-Mouth Technique" that does not need to elevate superior skin flap.

  • Previously, laryngectomized patients can achieve speech with tracheoesophageal device. Tracheoesophageal speech is currently one of the best methods of postlaryngectomy voice rehabilitation. But, tracheoesophageal speakers have some troublesome with digital occlusion of tracheostoma in every speaking time. Therefore, the tracheostoma valve has been introduced since 1982, and this valve could eliminate the need of tracheostoma digital occlusion. Currently, couple of tracheostoma valves has been introduced and developed. In this article, we introduced the 3 representative tracheostoma valves. Also, we discussed their use and characteristics. Traditionally the tracheostoma valve is used in total laryngectomized patients. We introduced one case using a tracheostoma valve after near total laryngectomy. Advantage and disadvantage of Blom-Singer valve and BIVONA II valve during using in this patients are discussed.

후두암에 동반된 SIADH (Syndrome of Inappropriate ADH Secretion Associated with Laryngeal Carcinoma, Cured by Total Laryngectomy)

  • The syndrome of inappropriate ADH secretion(SIADH) is a clinical disorder in which there is continual release of antidiuretic hormone(ADH) unrelated to plasma osmolality. It may occur usually in association with malignant tumors below the clavicle and other pulmonary disorders which are capable of synthesizing, storing and releasing ADH into the circulation but rarely with the head and neck tumors yet. We experienced a patient with unexplained hyponatremia associated with laryngeal carcinoma. The clinical and laboratory diagnostic studies suggested that patient's sodium deficiency was secondary to the SIADH. Subsequent resection of the neoplasm, total laryngectomy led to resolution of hyponatremia, suggesting that a tumor associated humoral factor, such as vasopressin or vasopressin like substance, was responsible for the electrolyte disturbance.

기관인두발성과 기관식도발성에 대한 이해도 및 공기역학적 검사 (Intelligibility and Aerodynamic Study of Tracheopharyngeal and Tracheoesophageal Speechs)

  • Selected characteristics were compared in the speech of three tracheoesophageal, five tracheopharyngeal and ten normal laryngeal adult speakers. Tracheoesophageal speakers use Blom-Singer voice prosthesis after total laryngectomy and tracheopharyngeal speakers use tracheopharyngeal myomucosal shunt after near-total laryngectomy. Intelligibility judgement was based on standard Korean monosyllabic and bisyllabic word lists of 50 items. Aerodynamic study was composed of maximum phonation time, phonaton quotient. phonation pressure and mean air flow rate. Results indicate that intelligibility of tracheopharygeal speech is more similar to normal laryngeal speech than tracheoesophageal speech using Blom-Singer voice prosthesis.

방사선 치료 환자에서의 Provox 사용 (The effect of Radiation Therapy on the Use of Proven Prosthesis in Laryngectomees)

  • Provox is now widely used for voice rehabilitation for total laryngectomized patient because of its low airway resistance and easiness for phonation. This study was designed to reveal the influence of radiation therapy on Proven complications. Forty-four patients who underwent total laryngectomy were grouped into group A (no radiation), group B(radiation and then Provox insertion), group C(Proven insertion and then radiation). Provox complications were leakage, granulation tissue formation, malfunction and infection. The average survival time of Provox was longer in group C (9.2 me) than group A(8.6 m) or group B (7.3 me), but no statistical significance was found. The first time of Provox change was 10.2, 8.6 and 9.7 months respectively. The incidence of complication was not significantly different among groups. The cases of Provox remeval due to shunt failure were 5, 4 and 2 respectively. Even though a significant statistically difference was not found partialy due to the small numbers of patients, a special caution should be exercised in inserting Provox to prevent a serious complication for the patients who had a history of previous radiation.

지연성 후두방사선괴사에 대한 고찰 (Delayed Radionecrosis of the Larynx)

  • Radiation therapy is an effective treatment modality for malignant disease of the head and neck, but it is not without risk and complication. Response of the larynx to radiotherapy varies from mild erythema to severe inflammation with edema and induration. possibly leading to necrosis of cartilage. These changes are due to an inflammatory reaction characterized by infiltration of polymorphonuclear leukocytes, vascular thrombosis, and obliteration of lymphatic channels. Late changes consist of telangiectasia of the skin, alopecia, loss of subcutaneous fat, degenerative changes in the connective tissues. But, radiation necrosis of laryngeal cartilage is an uncommon complication and it is a devastating process for which further necessitates surgical treatment. It is generally agreed that the only treatment for patient not responding to conservative measures is a total laryngectomy. We experienced 4 cases of delayed radionecrosis of the larynx who underwent radiation therapy for glottic cancer and hypopharyngeal cancer. We report these cases with review of literature.

후두 전적출술 후 기관루 재발에 대한 종격동 청소술 치험 1례 (A Case of Mediastinal Dissection for Tracheostomal Recurrence after Total Laryngectomy)

  • Sternal recurrence has been defined as a diffuse infiltrate of neoplastic tissue at the junction of the amputated trachea and the skin. The overall prognosis is poor, resulting from progressive tracheostomal obstruction or massive hemorrhage due to erosion of major vessels. Neither radiation therapy nor chemotherapy has demonstrated any efficacy in controlling these sternal recurrence. Surgery, especially mediastinal dissection, may benefit only an occasional patient. Recently authors experienced one case of mediastinal dissection for sternal recurrence after total laryngectomy for laryngeal cancer. We report our case with a brief review of literature.

상윤상후두부분적출술 후 Modified Barium Swallow를 이용한 연하 재활 (Swallowing Rehabilitation with Modified Barium Swallow after Supracricoid Partial Laryngectomy)

  • Backgroud and Objectives : Supracricoid partial laryngectomy(SCPL) has showed good functional and oncological results since it was introduced by Laccourreye in 1990. But loss of laryngeal functions, especially glottic sphincteric one, due to a wide resection of laryngeal structures is a major problem and needs a active and effective rehabilitation postperatively. Modified barium swallow(MBS) is a videofluoroscopy designed to define the etiology of the aspiration or dysphagia and simultaneously provide the therapeutic and rehabilitative method eliminating etiology of the aspiration. And we examined the effectiveness of the MBS in swallowing rehabilitation of the SCPL Patients. Materials and Methods : We reviewed the medical records of the 52 Patients who received SCPL for laryngeal squamous cell carcinoma according to the description of Laccourreye in our clinic from 1993 to 2001. Among them 21 patients were performed MBS(MBS(+) group) postoperatively and remaining 31 were not(MBS(-) group). During MBS, we selected 12 patients who showed aspiration and trained them with a swallowing rehabilitation maneuver which was identified as the most effective one eliminating the aspiration and remaining nine without aspiration were able to feed orally immediately after MBS without rehabilitation. In MBS(-) Uoup, they were received the traditional rehabilitation training with a supraglottic swallow. Results : The mean postoperative day(POD) of decannulation was earlier in MBS(+) group ($12.6{\pm}4.7$ POD) than in MBS(-) group ($19.5{\pm}11.0$ POD) (p =0.012), especially in patients showing aspiration (MBS(+) ; $12.9{\pm}5.2$ POD, MBS(-) : $22.3{\pm}9.9$ POD (p =0.008)). No significant difference was found in the mean POD of oral feeding between MBS(+) and (-) group, but in patients showing aspiration the time of oral feeding was earlier in MBS(+) group than in MBS(-) by average 10 days though it was not statistically significant. The incidence of aspiration pneumonia was lower in MBS(+) group (1/12cases) than in MBS(-) (7/12cases). Conclusions : In SCPL Patients, the Swallowing rehabilitation introduced to eliminate the aspiration during MBS after SCPL is very helpful for some patients to resume the safe oral intake more rapidly.

Definitive Closure of the Tracheoesophageal Puncture Site after Oncologic Laryngectomy: A Systematic Review and Meta-Analysis

  • Tracheoesophageal puncture (TEP) and voice prosthesis insertion following laryngectomy may fail to form an adequate seal. When spontaneous closure of the fistula tract does not occur after conservative measures, surgical closure is required. The purpose of this study was to summarize the available evidence on surgical methods for TEP site closure. A comprehensive search across PubMed, Web of Science, SCOPUS, and Cochrane was performed to identify studies describing surgical techniques, outcomes, and complications for TEP closure. We evaluated the rate of unsuccessful TEP closure after surgical management. A meta-analysis with a random-effect method was performed. Thirty-four studies reporting on 144 patients satisfied inclusion criteria. The overall incidence of an unsuccessful TEP surgical closure was 6% (95% confidence interval [CI] 1-13%). Subgroup analysis showed an unsuccessful TEP closure rate for silicone button of 8% (95% CI < 1-43%), 7% (95% CI < 1-34%) for dermal graft interposition, < 1% (95% CI < 1-37%) for radial forearm free flap, < 1% (95% CI < 1-52%) for ligation of the fistula, 17% (95% CI < 1-64%) for interposition of a deltopectoral flap, 9% (95% CI < 1-28%) for primary closure, and 2% (95% CI < 1-20%) for interposition of a sternocleidomastoid muscle flap. Critical assessment of the reconstructive modality should take into consideration previous history of surgery or radiotherapy. Nonirradiated fields and small defects may benefit from fistula excision and tracheal and esophageal multilayer closure. In cases of previous radiotherapy, local flaps or free tissue transfer yield high successful TEP closure rates. Depending on the defect size, sternocleidomastoid muscle flap or fasciocutaneous free flaps are optimal alternatives.

조기 성문암의 방사선 치료 결과 (Radiotherapy Results of Stage I Glottic Cancer)

  • 목적 : 조기 성문암의 방사선 치료 후 생존율, 실패 양상, 그리고 예후 인자에 대해 분석하였다. 대상 및 방법 : 1985년 6월부터 1992년 11월까지 방사선 치료를 위해 의뢰된 57례의 성문암환자를 대상으로 후향적 분석을 실시하였다. 연령 분포는 17세부터 71세까지 광범위했고 (중앙값 39세), 남자 55례와 여자 2례로 남성에서 호발하였다. 방사선 치료는 6 MV X-ray를 이용하여 1회 선량 2 Gy씩, 주 5회, 총선량 66 Gy를 조사하였다. 결과 : 방사선 치료에 대한 반응은 완전 관해 89.5% (51/57), 부분 관해 혹은 무반응 10.5%(6/57) 였다. 51례의 완전관해 환자 중에서 7례의 재발이 관찰되었고 7례 모두 수술로 구제되었다. 부분 관해 혹은 무반응 6례 중 4례에 대하여 구제수술을 시행하였고 2례에서는 수술을 시행하지 못하였다. 구제수술을 시행한 4례 중 3례가 구제되었고 1례는 다시 국소재발하였다. 3년 및 5년 무병생존율은 각각 72.9% 및 63.8%였다. 병소의 위치에 따른 5년 무병생존율은 후방인 경우 85.5%, 전방인 경우 61.6%였다 (P<0.05). 방사선치료에 대해 부분 관해 혹은 무반응을 보인 5례를 포함하여 13/57 (22.8%) 례의 치료 실패가 관찰되었고 그 중 10/13 (76.9%) 례가 수술로 구제되었다. 최종 5년 국소제어율은 92.3%였고 전체적으로 47/57 (82.5%) 례에서 음성을 보존할 수 있었다. 결론 : 높은 음성 보존율을 고려하면 초기 성문암에서 방사선치료가 우선적으로 고려되어야 한다.
