실험적으로 유발한 말 감염성 관절염의 치유경과에 대한 체열학적 평가 (Thermographic Evaluation of Beating Process of Experimentally Induced Infectious Arthritis in Horses)

  • 말의 근골격계 질환의 진단과 치유 경과 평가를 위한 체열촬영술의 유용성을 검토코자 실험적으로 유발한 감염성 관절염의 치료 과정을 조사하였다. 실험적으로 완관절에 감염성 관절염을 유발하여 항생제와 봉독으로 처치한 후, 치료 과정별로 임상검사, 실험실 검사, 방사선 검사와 병행하여 실시한 체열촬영술은 전반적으로 모든 진단 결과와 유사하게 나타났으며, 진단 시 조직에 대한 비접촉성, 비침습성 및 무통성 특성과 함께 치유경과를 시각화, 객관화하므로 효과적인 것으로 판단되었다. 따라서 말의 근골격계 질환의 체열학적 평가는 기존의 검사나 진단 영상 장치와 더불어 말의 근골격계 질환의 진단 및 치유경과 평가에 있어서 유용할 것으로 사료된다.

개의 어깨 관절에서 박리성 골연골염의 진단과 치료를 위한 탐색적 관절내시경의 적용 (Exploratory Arthroscopic Treatment to Diagnose and Treat Osteochondritis Dissecans of the Shoulder Joint in a Dog)

  • 6개월 된 수컷 Great Pyrenees (35 kg)가 왼쪽 앞다리에 2주 동안 지속된파행으로 전북동물의료센터에 내원하였다. 파행은 병원 내원 3일 전부터 더욱 심해졌다고 한다. 신체검사 상에서 환자는 왼쪽 어깨 부분에 통증을 보였고, 보행 검사에서 파행을 보이고 있었다. 방사선 검사와 신경계 신체검사에서는 유의적인 소견을 보이지 않았다. 의학적 기왕력과 보행검사를 토대로 박리성 골연골염을 의심하였다. 어깨관절에 대한 확실한 진단을 위해서, 탐색적 관절내시경 수술이 시행되었다. 미미한 두갈래근건초염과, 활막염, 그리고 상완골 머리 후내측에 위치한 박리성 골연골염 병변이 관찰되었다. 연골편의 제거, 연골하골의 변연절제가 관절내시경과 전기 버(electric burr)를 사용하여 실시되었다. 환자는 수술 다음 날에 퇴원하였다. 2주 후, 환자는 술 부를 핥아서 생긴 염증과 피하조직에 생긴 장액종으로 인해 내원하였다. 항생제 처치와 무균적 붕대가 적용되었고, 장액종은 수술적으로 제거되었으며, 관절내시경을 실시한 11주 후 환자의 보행은 정상이었다. 관절내시경은 관절낭 안쪽의 구조물 관찰이 가능하기 때문에 진단이 어려운 관절질환에 유용하게 사용될 수 있을 거라 사료된다.

선천성 슬개골탈구를 지닌 자견의 조기 진단법 및 외과적 수술법 확립 (Establishment of Early Diagnosis and Surgical Operative Method in Puppies with Congenital Patellar Luxation)

  • Medial patellar luxation in dogs is one of the most common patellar problems presented to the veterinary practitioner. It is observed in toy and miniature breed and the majority of cases is a congenital form. Because of extensor mechanism's instability, it causes deformity and disorder in the growth of the affected limb when the luxation is left without treatment As lameness is not easily detectable in puppies, early diagnosis and correction are essential for therapy. Up to now, there has not been any reports refering to the diagnostic methods and the optimal age for correction in young dogs. Thirteen 45-90 days old puppies, have grade I and/or II medial patellar luxation. Only by palpation, all 13 dogs were diagnosed of patelar luxation. Skyline radiographic view was useful to interpret patellar morphology and depth of trochlear groove only above 60 days old. However, it was difficult to make definite diagnosis patellar luxation. The caudocranial and lateral radiographic view as well as ultrasonographic skyline view were not showed of patellar luxation. 2 puppies had unilateral patellar luxation and 11 puppies had bilateral patellar luxation which more serious on the left than on the right. Only 3 puppies among 11 puppies with bilateral patellar luxation were observed of lameness degree 1. Regardless of grade of patellar luxation and lameness, we performed trochlear chondroplasty using a U-shape sculpture blade to minimize cartilage injury, transposition of tibia tuberosity with No. 1 Supramid to align extensor mechanism and lateral imbrication. After surgery, we examined the operated animal daily for 10 days and on 15, 30 and 60 days after surgery respectively. After operation, pain and fever became normal on 7 days, swelling on 10 days, respectively. On 10 days after surgery, dogs showed normal standing position, and normal walking was observed in 15 days after surgery. In force plate analysis, the operated legs were normal weight bearing at 30 days after operation. After surgery, not only patellar luxation and clinical signs have been gradually reduced but also bone growth have become normal without showing growth physeal plate injury. The survival rate of puppies over 62 days old was 100%, while 42-45 days old 37.5%. The above results suggest that optimal age for surgical correction of congenital medial patellar luxation is recommended over 60 days old. In conclusion, combination of trochlear chondroplasty, transposition of tibia tuberosity, and lateral retinacular imbrication is appropriate for over 60 days old puppies to efficiently correct patellar luxation.

  • Legg-Calve-Perthes diseases were diagnosed in 2 dogs showing signs of lameness. In radiographs, the abnormal size and shape was due to the collapse of the left capital bone and enlarged femoral neck was seen at case I. The completely deformed left femoral head resulted from the earlier collapse of the osteolytic bone of the caput femoris at case II. In treatment, femoral head ezcision was performed in one dog.

  • 중간정도의 파행과 열감을 가진 3세된 더러브렛 암말에서 방사선 및 발굽 검사를 통해 우전지 발굽이 백선병으로 진단되었다. 장제요법 (제벽 절제, 변형 히트바 편자 장착)을 적용한 결과 4개월 후 파행이나 열감이 없었으며 8개월 후에는 조교와 경주에 출주하였다. 국내에서 사육중인 말에서 발생되고 있는 백선병 치료에 장제요법이 좋은 치료법임을 확인하였다.

Computed Tomographic Findings of Navicular Syndrome in a Horse

  • An 18-year-old warmblood gelding was presented to Jeju National University Equine Hospital with chronic bilateral forelimb lameness. Navicular syndrome was suspected based on clinical findings, the hoof test, palmar digital nerve block, and radiographic results. Computed tomography (CT) was performed under general anesthesia. Bone cysts, enlarged vascular channels, sclerosis, and enthesophytes were identified in the navicular bone on CT images. Mineralization in the deep digital flexor tendon was also observed. CT can be a useful diagnostic tool for identifying lesions of the navicular bone and adjacent structures in horses. The horse was treated with an intra-bursal injection of triamcinolone and gentamicin. Lameness started to improve two days later and the horse was sound after two months of the injection. CT enabled us not only to diagnosis of navicular syndrome but also to determine the degree and extent of the lesions.

  • Seven patients with signs of metabolic bone disease clinically seen at the Veterinary Teaching Hospital of Seoul National University from May 1990 to February 1992 were evaluated retrospectively. 1. Clinical signs include bowed leg, deformity of spine, digestive disturbance, constipation, retarded growth, lameness and hindlimb paralysis. 2. Radiographical sings include generalized skeletal demineralization, decreased radiological contrast between skeleton and soft tissues, thinning of bone cortex and pathological fractures. The diagnosis of nutritional secondary hyperparathyroidism resulting in metabolic bone disease caused by mineral imbalance was made based upon history, clinical signs, physical examination and radiographical signs.

새끼고양이에서 발생한 고독성 칼리시바이러스(VS-FCV) 감염증 (Virulent Systemic Feline Calicivirus Infection in a Kitten)

  • 5개월령 코리안 숏헤어 새끼 고양이(몸무게 1.7 kg)가 상부호흡기 증상, 얼굴과 발/발바닥의 궤양과 부종 및 파행을 주증으로 내원하였다. 실험실 검사상, 백혈구감소증, 림프구 감소증, 췌장염 및 칼리시바이러스 감염증이 확인되었다. 진단은 고독성 칼리시바이러스 감염증에 대한 특이 임상증상과 PCR을 통한 칼리시바이러스 동정 및 다른 상부호흡기 감염을 배제함으로써 내려졌다. 상부호흡기 증상완화와 이차 감염에 대한 처치 및 보조요법을 약 한 달간 실시하였고, 환자는 이러한 치료를 통해 임상증상이 크게 개선되었다. 본 증례는 고독성 고양이 칼리시바이러스에 대한 첫 번째 발병증례이다.

늑골에서 발생한 개연골육종의 증례 (A case of canine chondrosarcoma from the rib)

  • We report large tumors of the left thoracic cavity, which arose from the ribs, were diagnosed as chondrosarcoma in a 4-year-old male mixed dog. The dog was presented with swelling in the left side of the chest wall and lameness. The masses were found to be circumscribed, whitish-grey, large and firm. The cut surface revealed whitish-grey lobules of varying size with cartilaginous consistency. Those were subsequently metastasized to the lung and mandibular lymph node. Histologically, the tumors were made up of clusters of chondroid cells having pleomorphic hyperchromatic nuclei with occasional mitosis. A special and immunohistochemical staining was performed to confirm the diagnosis. Chondroid matrix in tumor showed a positive reaction for alcian blue-periodic acid-Schiff staining. Immunoreactivity to S-100 proteins was present in the cytoplasm and/or nucleus of chondrocytes in tumor. The final diagnosis was grade III chondrosarcoma in the rib, considering histological features in grading criteria.

Arthroscopic Removal of Large Cartilage Fragment in a Dog with Osteochondritis Dissecans of Shoulder Joint

  • An 11-month-old, 19.5 kg, intact male Border collie was referred with intermittent left forelimb lameness to the Gyeongsang Animal Medical Center. The symptom was first discovered about 6 months ago, and it has gotten worse for the last 10 days with non-weight bearing on the left forelimb. During the physical examination, the patient showed painful reaction when the left shoulder was abducted. On radiographic assessment, a radiolucent line and some osteophytes were found in both humeral heads. Based on patient's clinical signs and radiographic findings, osteochondritis dissecans (OCD) was very suspicious. So, we decided to perform an arthroscopic surgery on left shoulder for definitive diagnosis and treatment because the right forelimb revealed no clinical signs. During arthroscopic technique, we found a large OCD flap on the caudo-central area of humeral head, and observed severe synovitis over a wide range on posterior area of the articular capsule. The large OCD flap was removed by a grasping forceps, and many joint mice were removed either. Curettage was performed using a curette on the articular surface until hemorrhage occurred, and articular capsule flushed with a lactated-ringer's solution. The patient was discharged on the same day without any specific abnormal status. Antibiotic, anti-inflammatory and analgesic drugs were administered. Mild lameness on left forelimb was observed in 2 weeks after surgery, but after 4 weeks, the patient showed complete normal gait without any lameness. Although surgical removal of OCD flap with arthroscopic was previously reported, We would like note that a large OCD flap can also be removed by arthroscopic surgery in this report.
