Lip and perioral soft tissue changes after bracket bonding using 3-D laser scanner (브라켓의 접착 전후 입술 주위 연조직의 3차원적 변화)

  • Objective: The aim of this study was to evaluate the lip and perioral soft tissue changes after bracket bonding. Methods: The soft tissue changes in 45 adult patients (age greater than 18 years and less than 29 years) without severe skeletal discrepancy were evaluated using three-dimensional images acquired with a laser scanner before and after bracket bonding was performed using 4 types of labial orthodontic brackets. Results: Among the statistically significant changes in distance observed for the landmarks, the biggest change was observed in forward movement. The landmarks on the lateral sides also showed significant changes. While the landmarks on the upper lip showed significant upward movement, those on the lower lip showed significant downward movement. However, the changes were smaller for the landmarks on the upper lip (average, 0.87 mm) than for the landmarks on the lower lip (average, 1.21 mm). The type of bracket used did not significantly affect the soft tissue changes. Conclusions: These findings will help predict soft tissue changes after bracket bonding for orthodontic treatment.

3-D FEA on the intrusion of mandibular anterior segment using orthodontic miniscrews (교정용 미니스크류를 이용한 하악 전치 함입 시 변위양상의 3차원 유한요소분석)

  • Objective: The purpose of this study was to analyze the stress distribution and the displacement pattern of mandibular anterior teeth under various intrusive force vectors according to the position of orthodontic miniscrews and hooks, using three-dimensional finite element analysis. Methods: A three-dimensional finite element model was constructed to simulate mandibular teeth, periodontal ligament, and alveolar bone. The displacement of individual tooth on three-dimensional planes and the von Mises stress distribution were compared when various intrusion force vectors were applied. Results: Intrusive forces applied to 4 mandibular anterior teeth largely resulted in remarkable labial tipping of the segment according to the miniscrew position. All 6 mandibular anterior teeth were labially tipped and the stress concentrated on the labiogingival area by intrusive force from miniscrews placed mesial to the canine. The distointrusive force vector led to pure intrusion and the stress was evenly distributed in the whole periodontal ligament when the hook was placed between the central and lateral incisors and the miniscrew was placed distal to the canine. Conclusions: Within the limits of this study, it can be concluded that predictable pure intrusion of the 6 anterior teeth segment may be accomplished using miniscrews placed distal to the canine and hooks located between the central and lateral incisors.

The Structure of Korean Consonants as Perceived by the Japanese (일본인이 지각하는 한국어 자음의 구조)

  • Twelve Japanese students living in South Korea have been examined for their perceptual identification of an initial consonant in Korean syllables with or without a white noise. A confusion matrix was then subject to analyses of additive clustering, individual difference scaling, and probability of information transmission, the results of which were also compared to those of South Koreans. The Japanese in the present experiment confused /다/and/타/ most frequently, followed by /가/ and /카/, /자, 차, 짜/, /타/ and /따/, and so on. The results of additive clustering analysis of the Japanese significantly differed from those of the South Koreans. Individual difference scaling revealed dimensions of sonorant, aspiration and coronal. While South Koreans showed binary values on aspiration and tenseness dimensions, the Japanese did continuous values on such dimensions. An information transmission probability analysis revealed that the Japanese participants could not perceive very well such larynx features as tenseness and aspiration compared to the South Korean participants. The former group, however, perceived very well place of articulation features such as labial and coronal. The present results suggest that an approach dealing with structures of base representations is important in understanding the phonological categories of languages.

A STUDY ON THE DISTORTION OF THE COPINGS FOR CERAMOMETAL CROWNS DURING REPEATED FIRING (도재전장금관을 위한 코핑의 변형에 관한 연구)

  • Ceramometal crowns are common restorations in fixed prosthodontics because of their casting accuracy, the high strength properties of the metal, and the cosmetic appearance of porcelain. However, deterioration of the initial fit of the metal coping has been observed after the porcelain firing cycle. The distortion due to repeated firing makes it difficult to fit crown margin and elicits microleakage. The major causes of distortion are the residual stress that accumulate during wax-up, casting, cold work and the induced stress caused by the mismatch of porcelain-metal thermal contraction. This study examined the marginal fit changes of metal copings in relation to repeated firing and the effects of heat treatment that reduce the distortion resulted from residual stress. The marginal changes of the copings that were treated with conventional method and those treated with heat before repeated firing, were evaluated. The metal die which represented preparations of a maxillary central incisor was fabricated, and 45 wax patterns were cast with nonprecious metal alloys. The heat treatment of each group was performed as follows. Group 1(control) : Casting - Devesting - Cold work - Firing Group 2 : Casting - Heat treatment - Devesting - Cold work - Firing Group 3 : Casting - Devesting - Cold work - Reinvesting - Heat treatment - Devesting - Firing The copings were fired 3 times. After each firing, the marginal fit changes were measured with inverted metallurgical microscope at the 4 reference points located at labial, lingual, and both proximal surface. Measurements were compared, and statistically analyzed. The results were as follows ; 1. In all groups, the highest value of marginal fit changes of the copings studied were found after the first firing cycle. 2. When the distortion of each experimental group at the first firing cycle were compared, group 1 exhibited the greatest changes($20-27{\mu}m$), followed by group 2($9-13{\mu}m$), and group 3($8-10{\mu}m$). 3. The copings treated with heat before devesting(group 2) revealed significantly smaller marginal fit changes than the copings treated with conventional method(group 1). (p<0.01) 4. The copings treated with heat after reinvesting(group 3) revealed significantly smaller marginal fit changes than the copings treated with conventional method(group 1). (p<0.01) 5. No siginificant differences in marginal fit changes were found between the copings treated with heat before devesting(group 2) and the copings treated with heat after reinvesting(group 3). (p>0.01)

  • The purpose of this study was to determine the effect of finish line design, amount of incisal reduction, and loading condition on the stress distribution in anterior all-ceramic crowns. Three-dimensional finite element models of an incisor all-ceramic crown with 3 different finish line designs : 1) shoulder with sharp line angle 2) shoulder with rounded line angle 3) chamfer : and 2 different incisal reductions : 2mm and 4mm were developed. 300 N force with the direction of 45 degree to the long axis of the tooth was applied at 3 different positions : A) incisal 1/3, B) incisal edge, C) cervical 1/5. Stresses developed in ceramic and cement were analyzed using three-dimensional finite element method. The results were as follows : 1. Stresses were concentrated in the margin region, which were primarily compressive in the labial and tensile in the lingual. 2. Stresses were larger in the area near line angle than on the crown surface of the margin region. In case of shoulder with sharp line angle, stresses were highly concentrated in the porcelain near line angle. 3. At the interface between porcelain and cement and at the porcelain above the margin on crown surface, stresses were the highest in chamfer, and decreased in shoulder with sharp line angle and shoulder with rounded line angle, respectively. 4. At the interface between cement and abutment on crown surface, stresses were the highest in shoulder with sharp line angle, and decreased in shoulder with rounded line angle and chamfer, respectively. 5. The amount of incisal reduction had little influence on the stress distribution in all-ceramic crowns. 6. When load was applied at the incisal edge, higher stresses were developed in the margin region and the incisal edge than under the other loading conditions. 7. When load was applied at the cervical 1/5, stresses were very low as a whole.

A study on the O'Leary index and dental plague index of dental patients (치과내원 환자의 올리어리(O'Leary) 지수와 개별치아의 치면세균막 지수에 관한 연구)

  • The purpose of this study was to examine the relationship of the general characteristics of dental patients to their total O'Leary index and individual plaque indexes in a bid to check the oral hygiene state of citizens and evaluate their plaque management ability. The subjects in this study were 288 patients at dental hospitals and clinics in Busan and South Gyeongsang Province. A survey was conducted from September to November 2008. The findings of the study were as follows: 1. Overall, the O'Leary index of the patients investigated stood at 72 percent. By age and gender, the O'Leary index was highest among those in their 30s(83%) and among the women(75%)(p<.05). As to the influence of toothbrushing frequency, the patients who did it twice or more a day had the highest O'Leary index(75%)(p<.001). By the use of oral hygiene supplies, the patients who used oral hygiene supplies had a higher O'Leary index(76%) than the others who didn't(p<.01). 2. As for plaque index for each tooth, 28 percent of their teeth were given zero point, and 52 percent were given one mark. 13 percent were given two marks, and 6.3 percent were given three marks. Thus, the teeth that were given one mark were largest in number(p<.001). 3. As for plaque index for labial posterior mandibular, 43 percent of their teeth were given one point, and 36 percent were given zero mark. 14 percent were given three mark, and 5 percent were given three marks(p<.01). 4. As for plaque index for buccal posterior maxilla, 57 percent of their teeth were given one point, and 20 percent were given zero mark. 16 percent were given two mark, and 6.3 percent were given three marks(p<.01). 5. As for plaque index for lingual posterior mandibular, 56 percent of their teeth were given one point, and 27.8 percent were given zero mark. 9 percent were given three mark and 6 percent were given three mark(p<.001).

Rhinoplasty using Various Autogenous Tissues (다양한 자가조직을 활용한 비성형술)

  • Purpose: In Korean rhinoplasty, alloplasts such as silicone rubber have been used for dorsal and tip augmentations. However, alloplasts have produced complications such as deviation, exposure, and infection. Although autogenous tissue is an ideal material, the amount of the cartilage in Korean is not sufficient. Therefore, the authors developed a method to harvest the maximum amount of the septal and conchal cartilage. The authors first performed a complete septal extension graft for short and retruded columella and then performed nasal augmentation using various autogenous tissues. Methods: These surgical techniques were performed on 11 patients. Their average age was 27 years. An open approach with a V-shaped columellar labial incision was performed. At first, the complete septal extension graft was fixed on the entire caudal margin of the septum, and the alar cartilage was suspended to the septal extension graft to make the tip projected and the columella advanced caudally. A temporal fascial ball was used for radix augmentation. For dorsal augmentation, a batten-shaped septal cartilage graft wrapped with or without the temporal fascia or a diced cartilage graft wrapped with the temporal fascia was performed. For nasal tip augmentation, a shield graft and a cap graft or a folded conchal cartilage graft was performed.Results: The mean follow-up period was 11.2 months. The overall results were natural and satisfactory. Two patients underwent slight absorption and caudal displacement. With the exception of these cases, no complications were observed. Conclusion: Korean rhinoplastic surgeons are accustomed to using the alloplasts despite of serious complications. When rhinoplasty using autogenous tissue was performed, some complications occurred such as the displacement of the diced cartilage graft wrapped with the fascia, but many of the complications of alloplastic rhinoplasty were avoided. Therefore, we propose that by this time Korean rhinoplastic surgeons need to change their preference from alloplastic rhinoplasty to autogenous rhinoplasty.

A study on practice behavior of toothbrushing in preschool children by observing survey (미취학 아동의 잇솔질 실천 행동에 관한 관찰조사 연구)

  • Objectives : The purpose of this study was to examine the toothbrushing practice behavior of preschool children by observing children's actual toothbrushing behavior and further to offer basic data available for being reflected in direction and goal establishment of the systematic and organized oral health education in consideration of individual characteristic. Methods : It targeted 225 preschool children from May 6, 2010 to June 8, and analyzed by having the survey results of totally 208 people, except 17 who are noncooperative with the observation survey. As for the collected materials, it carried out frequency analysis and $x^2$-test in order to grasp subjects' general characteristics and toothbrushing practice behaviors and to examine correlation, by using SPSS 12.0 for window. Results : 1. A grasp of toothbrush was indicated 47.1% for "proper" and 52.9% for "improper." The position of holding a toothbrush was indicated to be the highest with 59.6% for "the center in grip." 2. Toothpaste amount was indicated to be 49.0% for" tip in toothbrush, "followed by 38.9% for "half in toothbrush" and 12% for "every side of toothbrush" in order. Significant difference was shown according to age($x^2$=19.125. p<.05). 3. Average toothbrushing time per once was surveyed to be $106{\pm}56$ seconds. 4. As for a toothbrushing method by region, the labial and the baccal surface were indicated to be horizontal scrub with 63% and fones method 11.5%. Significant difference was shown according to gender($x^2$=10.275. p<.05). The anterior lingual surface wasn't washed with 72.6%. Significant difference was shown according to appearance of education($x^2$=6.056. p<.05). 5. A case of requiring exchange because of being widened toothbrush was indicated to be 59.2%. The toothbrush replacement in the highest ratio was needed in kindergarten teachers. Conclusions : The actual toothbrushing practice attitude of preschool children was surveyed to be not right as a whole such as toothbrushing time, toothbrushing method, toothpaste amount, and replacement of toothbrush. Accordingly, the oral health education will need to be performed in order for a change into positive attitude and for improvement in practice level. In addition, it is considered to be likely necessary for development in substantial oral health educational program available for enhancing knowledge level of oral health care in mothers and kindergarten teachers.

EXPRESSION OF DSPP AND BSP MRNAS DURING ODONTOBLAST DIFFERENTIATION IN THE NFI-C (-/-) MICE (NFI-C 결손 생쥐의 상아모세포 분화과정에서 DSPP와 BSP mRNA의 발현)

  • Nuclear factor I (NFI) exists in the odontoblast and osteoblast. NFI-C null mice demonstrated aberrant odontoblast differentiation, abnormal dentin formation, and molar lacking roots. The purpose of this study was to examine phenotype of the aberrant odontoblast in NFI-C null mice and to evaluate the expression of DSPP and BSP mRNAs in NFI-C null mice with in-situ hybridization. The results were as follows: 1. In the NFI-C (-/-) mice, the crown dentin of molar showed normally formation, but there was no root dentin. 2. In the NFI-C (-/-) mice, the labial dentin of mandibular incisors showed relatively a lot of dentin formation, but the lingual dentin showed defect. 3. In the NFI-C (-/-) mice, the odontoblast of mandibular incisors revealed abnormal shape and trapped in osteodentin-like mineralized tissue. 4. In the NFI-C (-/-) mice, the odontoblast in the crown dentin of molars showed strong expression of DSPP, the odontoblast in the root dentin of molars was not expression of DSPP. In the NFI-C (-/-) mice the odontoblast in the mandibular incisors showed weekly expression of DSPP 5. In the wild mice, the odontoblasts of mandibular incisors were not expression of BSP, but in the NFI-C (-/ -) mice the odontoblast of mandibular incisors showed strong expression of BSP These results suggest that odontoblast in the NFI-C (-/-) mice changes the phenotype into osteoblast.

TREATMENT OF CROWN-ROOT FRACTURE USING FIBER-REINFORCED POST: A CASE STUDY (섬유강화형 포스트를 이용한 치관-치근 파절의 치료: 증례 보고)

  • The crown-root fracture is defined as a fracture of tooth that contains enamel, dentin and cementum with or without pulp exposure. Generally the fracture lines place obliquely from labial surface, between incisal edge of the crown and marginal gingiva, to palatal surface subgingivally. If the fracture line is located supragingivally, the removal of tooth fragment and supragingival restoration can be performed. In subgingival fracture line, the surgical exposure, orthodontic eruption or surgical eruption can be considered. If the fracture line is too deep to restorate, extraction or decoronation can be selected. In children and adolescents, the extraction should be the last option. Another option to select before extraction is the restoration using fiber-reinforced post and the reattachment of tooth fragment. The fiber-rainforced post enhances the retention and the durability of tooth fragment. The reattachment of crown fragment using resin adhesive system is considered minimal invasive treatment biologically. This case reports the treatment of crown-root fracture using the reattachment of crown fragment and the insertion of fiber-reinforced post.
