Effect of High-frequency Diathermy on Hamstring Tightness

  • Background: The hamstring is a muscle that crosses two joints, that is the hip and knee, and its flexibility is an important indicator of physical health in its role in many activities of daily living such as sitting, walking, and running. Limited range of motion (ROM) due to hamstring tightness is strongly related to back pain and malfunction of the hip joint. High-frequency diathermy (HFD) therapy is known to be effective in relaxing the muscle and increasing ROM. Objects: To investigate the effects of HFD on active knee extension ROM and hamstring tone and stiffness in participants with hamstring tightness. Methods: Twenty-four participants with hamstring tightness were recruited, and the operational definition of hamstring tightness in this study was active knee extension ROM of below 160° at 90° hip flexion in the supine position. HFD was applied to the hamstring for 15 minutes using the WINBACK device. All participants were examined before and after the intervention, and the results were analyzed using a paired t-test. The outcome measures included knee extension ROM, the viscoelastic property of the hamstring, and peak torque for passive knee extension. Results: The active knee extension ROM significantly increased from 138.8° ± 9.9° (mean ± standard deviation) to 143.9° ± 10.4° after the intervention (p < 0.05), while viscoelastic property of the hamstring significantly decreased (p < 0.05). Also, the peak torque for knee extension significantly decreased (p < 0.05). Conclusion: Application of HFD for 15 minutes to tight hamstrings immediately improves the active ROM and reduces the tone, stiffness, and elasticity of the muscle. However, further experiments are required to examine the long-term effects of HFD on hamstring tightness including pain reduction, postural improvement around the pelvis and lower extremities, and enhanced functional movement.

Can Knee Joint Flexion Position of the Raised Lower Limb Affect Trunk Muscle Activation During Bird Dog Exercise in Subjects With Chronic Low Back Pain?

  • Background: Bird dog exercise (BDE) is one of the lumbar stabilization exercises that rehabilitate low back pain by co-contraction of the local and global muscles. Previous studies have reported the effect of various type of BDEs (for example, practicing the exercises on various surfaces and changing the limb movement) for muscle co-contraction. Objects: This study aimed to investigate the effect of knee joint flexion position of the raised lower limb on abdominal and back muscle activity during BDE in patients with chronic low back pain (CLBP). Methods: Thirteen males participated in this study (age: 32.54 ± 4.48 years, height: 177.38 ± 7.17 cm). Surface electromyographic (SEMG) data of the internal abdominal oblique (IO), external abdominal oblique (EO), lumbar multifidus (MF), and thoracic part of the iliocostalis lumborum (ICLT) were collected in two knee joint flexion positions (90° flexion versus 0° flexion) during BDE. The SEMG data were expressed as a percentage of root mean square mean values obtained in the maximal voluntary isometric contraction. Results: Greater muscle activity of the IO (p = 0.001), MF (p = 0.009), and ICLT (p = 0.021) of the raised lower limb side and the EO (p = 0.001) and MF (p = 0.009) of the contralateral side were demonstrated in the knee joint flexion position compared to the knee joint extension position. Greater local/global activity ratios of the abdominal muscle (i.e., IO and EO) of the raised lower limb (p = 0.002) and the back muscle (i.e., MF and ICLT) of the contralateral side (p = 0.028) were also noted in the knee joint flexion position. Conclusion: BDE with a knee joint flexion position might be recommended as an alternative lumbar stabilization exercise to enhance muscle activity in both the raised lower limb and the contralateral sides of the trunk for individuals with CLBP.

  • The primary purpose of a TKA is to restore normal knee function Therefore, ideally, a TKA should: (a) maintain the natural leverage of the knee joint muscles to ensure generating adequate knee muscle moments to accomplish daily tasks such as rising from climbing stairs; (b) provide adequate knee joint stability. A 16-channel MyoResearch XP EMG system was used to collect the differential input surface electromyography signals VM, VL, RF, BF, ST during climbing/descending stair tests. A Peak Motion Measurement System was used to collect the kinematic and kinetic data. AKIN-COM Ill isokinetic dynamometer was used for EMG of VM, VL, RF, BF and ST during maximal voluntary contraction. I Quadriceps EMG results for the VM of the passed 1year group limb demonstrated significant less RMS EMG than that of the passed 3year group limb $60^{\circ}-15^{\circ}$ of knee flexion(p<0.05). The VL of the passed 1year group limb also demonstrated significants less RMS EMG than that of the passed 3year group limb from $60^{\circ}-45^{\circ}$ of knee flexion(p<0.05). Similar to the VM and VL, the RF of the passed 1year group limb showed less RMS EMG than that of the passed 3year group limb from $60^{\circ}-30^{\circ}$ do knee flexion(p<0.05). Hamstring EMG results for the BF of the passed 1year group limb demonstrated less RMS EMG than that of the passed 3year group limb from $75^{\circ}-15^{\circ}$ of knee flexion(p<0.05). The passed 1year group limb tended to have less ADD displacement(p<0.071) than that of the passed 3year group limb. There was no significant difference of the ABD displacement between the passed 1year group and the passed 3year group limbs(p<0.73). The passed 3year group used compensatory adaptation movement strategies to compensate for the strength deficit of passed 3year group limbs. The passed 3year group limb also increased the quadriceps muscle activation level to produce more knee extension moment to compensate for the short quadriceps moment arm. The passe 3year group limb might have an unstable knee joint in the medio-Iateral direction during the climbing/descending by showing a tendency of more ADD displacement and greater hamming co-activation EMG than the passed 1year group limbs. The TKA design was not able to help the knee joint to produce adequate knee extension moment with less quadriceps muscle effort. I think that old man needs continuous exercise for muscle strength.

근위 경골 절골술 후 한방 재활치료 프로토콜을 적용한 재활치료의 임상적 효과: 증례 보고 (A Rehabilitation Protocol for High Tibial Osteotomy in Korean Medicine: A Report of 3 Cases)

  • In this study, we propose a rehabilitation protocol involving Korean medicine for patients after high tibial osteotomy (HTO) and report its effectiveness. Three patients underwent HTO using the rehabilitation protocol involving Korean medicine. We estimated the outcome evaluating physical examination findings of the knee joint, numeric rating scale, pain disability index, Lysholm knee score, and walking state. After the treatment, we observed that knee movement and muscular strength were improved. In addition, the numeric rating scale, pain disability index, Lysholm knee score, and walking state of patients were improved. Therefore, a rehabilitation protocol involving Korean medicine can be effectively used for patients after HTO. The limitation of this study was the insufficient number of cases and the difficulty of follow-up because of the characteristics of this operation and the long periods of time that are needed for each phase of the protocol. Further studies are needed to design a rehabilitation protocol involving Korean medicine.

보행 시 여성 노인의 하지 관절 근력과 국부 동적 안정성과의 관계 (Relationship between Strengths of the Lower Extremity's Joints and Their Local Dynamic Stability during Walking in Elderly Women)

  • Objective: The objective of the present study was to analyze the relationship between strength of the lower extremity's joints and their local dynamic stability (LDS) of gait in elderly women. Method: Forty-five elderly women participated in this study. Average age, height, mass, and preference walking speed were 73.5±3.7 years, 153.8±4.8 cm, 56.7±6.4 kg, and 1.2±0.1 m/s, respectively. They were tested torque peak of the knee and ankle joints with a Human Norm and while they were walking on a treadmill at their preference speed for a long while, kinematic data were obtained using six 3-D motion capture cameras. LDS of the lower extremity's joints were calculated in maximum Lyapunov Exponent (LyE). Correlation coefficients between torque of the joints and LyE were obtained using Spearman rank. Level of significance was set at p<.05. Results: Knee flexion torque and its LDS was negatively associated with adduction-abduction and flexion-extension movement (p<.05). In addition, ratio of the knee flexion torque to extension and LDS was negatively related to internal-external rotation. Conclusion: In conclusion, knee flexion strength should preferentially be strengthened to increase LDS of the lower extremity's joints for preventing from small perturbations during walking in elderly women.

  • The purpose of this study is to evaluate the possibility of identifying joint damping property through commercially available isokinetic ergometer (BIODEX). The proposed method is to estimate the damping torque of the knee joint from the difference between the external joint torque for maintaining isokinetic movement and the gravity torque of the lower leg. The damping torque was estimated at various joint angular velocities, from which the damping property would be derived. Measurement setup was composed of the BIODEX system with an external force sensor and Labview system. Matlab was used in the analysis of the damping property. The experimental result showed that the small variation in angular velocity due to acceleration and deceleration of the crank arm resulted in greater change of inertial torque than the damping torque, so that the estimation of damping property from the isokinetic movement is difficult.

발과 족관절의 운동학적 분석 (Kinetic analysis of the foot and ankle)

  • Purpose : To describes the important aspects of the foot and ankle movement and function used when git and balance strategy. Method : The foot and ankle was a very important roles in the lower limb movement and gait. This study summarizes the physiologic movement of knee to the PNF lower extremity patterns. Result : The ankle joint composed of the talocural joint, the subtalarl joint, transverse tarsal joint, talocalcaneonavicular joint. The onset of dorsiflexion muscle activity starts in pre swing gait patterns. First contract muscle is the extensor hallucis. Activity of tibialis anterior and extensor digitorum longus quickly follows in mid swing gait phase. During stance phase, the soleus and gastrocnemius muscle provided plantar flexor torque, which muscle reacts quickly to restrain ankle dorsiflexion, and contributes modulated control of the ankle motion in gait patterns. Conclusions : The understanding of ankle kinematics, could provide a good therapeutic approach for improving gait patterns in patients with various pathological condition.

근전도 바이오피드백을 적용한 엉덩관절 움직임 조절 운동이 엉덩관절 돌림근에 미치는 영향에 대한 융합적 연구 (A convergence study of the effect of movement control exercise of hip joint using visual EMG biofeedback on hip rotators)

  • 본 연구의 목적은 근전도 바이오피드백 장비를 사용하여 움직임 조절운동을 수행할 때 엉덩관절 근육에 미치는 영향에 대해 알아보고자 하였다. 본 연구는 유사실험설계 단일집단 검사전-검사후 설계이다. 건강한 성인 21명이 본 연구에 참여하였다. 모든 대상자는 근전도 바이오피드백 장비를 사용하여 엉덩관절의 움직임 조절 운동을 총 20분간 수행하였다. 적은무릎 구부리기 검사 수행동안 엉덩관절의 근활성도 변화를 확인하기 위해 중간볼기근, 큰볼기근, 넙다리근막긴장근, 넙다리 곧은근의 활성도를 측정하였다. 본 연구 결과에서 중재 후 큰볼기근의 유의한 변화를 확인하였다. 이러한 결과는 근전도 바이오피드백 장비를 적용한 엉덩과절 움직임 조절운동이 엉덩관절의 안쪽돌림을 조절하고 엉덩관절 가쪽돌림근을 활성화시키는 효과적 중재방법임을 확인할 수 있었고, ICT 융합기술을 접목한 재활운동이 임상에서 효과적인 중재가 될 수 있음을 보여주었다.

승마 속보 시 등자 길이에 따른 체간기울기와 양측 하지의 협응성 비교분석 : 비대칭 지수 및 전체이동지수 알고리즘 개발 (Analysis of the Coordination of the Trunk Tilting Angle and Bilateral Lower Limbs According to the Stirrups Length during Trot in Equestrian: Asymmetric Index Development of Overall Movement Index Algorithm)

  • Purpose : The purpose of this study was to analyze the coordination of the trunk tilting angle and bilateral lower limbs according to the stirrups length during trot in equestrian. Methods : Participants selected as subject were consisted of adult male(n=7, mean age: $45.00{\pm}3.78yrs$, mean height: $172.50{\pm}2.44cm$, mean body mass: $76.95{\pm}4.40kg$, mean, mean leg length: $97.30{\pm}2.60cm$). They were divided into 3-types of stirrups lengths(67 cm, 72 cm, 77 cm) during trot. The variables analyzed were consisted of the trunk front-rear angle, lower limb joint(Right Left hip, knee, ankle), overall movement index(OMI) of the lower limbs(thigh, shank, foot) and asymmetry index(AI%) during trot. Results : The average angle in hip and knee joint showed more extended posture according to the increase of stirrups lengths and ankle angle showed more plantarflexion posture according to increase of stirrups length during 1 stride in trot. Also, average angle showed more extended posture in right hip and ankle joint than that of left. The angle of knee joint didn't show significant difference statistically between right and left. Also asymmetric index in average angle of hip, knee and ankle joint didn't show significant difference statistically in between lower limbs, but hip joint showed higher asymmetric index in stirrup length of 77 cm and ankle joint showed higher asymmetric index in stirrup length of 67 cm than that of the others respectively. The FR angle in trunk of horse-rider showed relative backward leaning motions at stirrup length of 67 cm and 77 cm than that of stirrup length of 72 cm during stance and swing phase. OMI in thigh, shank, and foot limbs didn't show significant difference statistically according to the stirrups length of right and left lower limbs, but left lower limbs showed higher index than that of right lower limb. Stirrup length of 72 cm in shank and foot limbs showed higher index than that of stirrup length of 67 cm and 77 cm. But stirrup length of 72 cm showed higher asymmetric index than that of stirrups length of 67 cm and 77 cm. Conclusions : When considering the above, 72 cm(ratio of lower limb 74.04%) stirrup lengths could be useful in posture correction and stabilization than 67cm(ratio of lower limb 68.69%) and 77 cm(ratio of lower limb 79.18%) stirrup lengths during trot in horse back riding.

피부길이변화를 고려한 3차원 다리보호대 모델링 (3D Modeling of Safety Leg Guards Considering Skin Deformation and shape)

  • During a design process of a protective equipment for sports activities, minimizing movement restrictions is important for enhancing its functions particularly for protection. This study presents a three-dimensional(3D) modeling methodology for designing baseball catcher's leg guards that will allow maximum possible performance, while providing necessary protection. 3D scanning is performed on three positions frequently used by a catcher during the course of a game by putting markings on the subject's legs at 3cm intervals : a standing, a half squat with knees bent to 90 degrees and 120 degrees of knee flexion. Using data obtained from the 3D scan, we analyzed the changes in skin length, radii of curvatures, and cross-sectional shapes, depending on the degree of knee flexion. The results of the analysis were used to decide an on the ideal segmentation of the leg guards by modeling posture. Knee flexions to 90 degrees and to $120^{\circ}$ induced lengthwise extensions than a standing. In particular, the vertical length from the center of the leg increases to a substantially higher degree when compared to those increased from the inner and the outer side of the leg. The degree of extension is varied by positions. Therefore, the leg guards are segmented at points where the rate of increase changed. It resulted in a three-part segmentation of the leg guards at the thigh, the knee, and the shin. Since the 120 degree knee-flexion posture can accommodate other positions as well, the related 3D data are used for modeling Leg Guard (A) with the loft method. At the same time, Leg Guard (B) was modeled with two-part segmentation without separating the knee and the shin as in existing products. A biomechanical analysis of the new design is performed by simulating a 3D dynamic analysis. The analysis revealed that the three-part type (A) leg guards required less energy from the human body than the two-part type (B).
