Novel computational approaches characterizing knee physiotherapy

  • A knee joint's longevity depends on the proper integration of structural components in an axial alignment. If just one of the components is abnormally off-axis, the biomechanical system fails, resulting in arthritis. The complexity of various failures in the knee joint has led orthopedic surgeons to select total knee replacement as a primary treatment. In many cases, this means sacrificing much of an other-wise normal joint. Here, we review novel computational approaches to describe knee physiotherapy by introducing a new dimension of foot loading to the knee axis alignment producing an improved functional status of the patient. New physiotherapeutic applications are then possible by aligning foot loading with the functional axis of the knee joint during the treatment of patients with osteoarthritis.

Radiologic Evaluation of Change of Ankle Joint after Total Knee Arthroplasty (인공 슬관절 치환술 후 족관절 변화에 대한 방사선학적 연구)

  • Purpose: To evaluate the incidence and describe radiologic pattern of ankle arthritis following change of mechanical loading axis by total knee arthroplasty. Materials and Methods: We reviewed radiographs of 419 cases, 243 patients underwent total knee arthroplasy from January 2002 to October 2006 retrospectively. We described radiologic parameters around the ankle joint and measured the amount of change of knee varus or valgus angle by comparing preoperative and postoperative anteroposterior standing lower extremities AP X-rays. We divided cases into two groups, one with radiologically arthritic change of the ankle joint and the other one without any radiologic change after surgery. We compared two groups in each parameters and analyzed statistically (SPSS v13.0). Results: Three hundred eighty one cases were divided into varus group and 38 cases in valgus group. 125 cases were divided into ankle arthritic change-positive subgroup among the varus group and 251 cases were in negative subgroup. The amount of varus angle correction by total knee arthroplasty showed significant difference between two subgroups. There was no significant difference in each parameters between subgroups within 38 valgus cases. Conclusion: Ankle arthritis can be aggravated after total knee arthroplasty because of the change of mechanical loading axis onto the ankle joint. Therefore it may be needed to evaluate symptoms and function of ankle joints before performing total knee arthroplasties especially in patients with huge varus deformities of knee joints.

Effect of Knee Stabilization Exercise on Balance and Walking Ability in Patients with Total Knee Replacement (무릎관절 전치환술 환자에게 적용한 무릎 안정화 운동이 균형과 보행능력에 미치는 영향)

  • Background: Degenerative arthritis accounts for a large portion of the elderly, causing a lot of inconvenience in daily life. Total knee replacement (TKR) are performed to relieve pain in the knee joint. The purpose of this study was to determine whether knee joint stabilization exercises are effective in improving balance and walking ability in degenerative arthritis patients who have undergone TKR. Methods: A total of 30 TKR patients participated in this study. They were assigned to two groups. The experimental group, the group that underwent knee stabilization exercises, joint mobilization and general physical therapy. On the other hand, and the controlled group, the group that underwent joint mobilization and general physical therapy. All exercises were conducted thrice a week, for four weeks. The main balance outcomes were evaluated using the posture balance training system, while walking ability was assessed using a wireless 3-axis accelerometer. Results: The experimental group had significantly higher scores in postural stability testing (PST), limits stability testing, and waling ability. The post-intervention evaluation, there were significant differences in all variables between the two groups. However, no significant difference was noted in the foam eyes closed test in PST. Conclusion: This study suggests that knee joint stabilization exercises effectively improve the balance and walking ability of TKR patients. Thus, it could be presented as useful in clinical practice.

An Experimental Studies on the alleviation effects of Daebangpoongtang(大防風湯) in LPS-induced arthritis (대방풍탕(大防風湯)의 LPS유발(誘發) 관절염(關節炎) 완화효과(緩和效果)에 관한 실험적(實驗的) 연구(硏究))

  • Purpose: This study is to investigate the alleviation effects of Daebangpoongtang in LPS induced arthritis in mice knee joint. Methods : Daebangpoongtang was chosen to treat the arthritis caused by injecting $300{\mu}g/kg$ LPS to mice knee joint. The control group had no treatment, while the LPS group was injected $300{\mu}g/kg$ LPS to mice knee joint and the DBP group was oral administrated of Daebangpoongtang. After injection of $300{\mu}g/kg$ LPS to mice knee joint, the alteration of synovial lining cell, vessel, fibrosis, distribution of collagen fiber, fibroblast, mast cell, infiltration of inflammation component cell and distribution of ICAM and VCAM was observed by light microscope(BX50). Results : In the DBP extract treatment group, the distribution of vessel, the enlargement of synovial lining cell layer, the synovial lining cells with filopodia, the fibrosis, the distribution of fibroblast in synovial membrane, the distribution of TCAM and VCAM on the knee joint was less than that of LPS group. Infiltrated lymphocyte into the apical surface had not observed in the DBP extract treatment group. The distribution of mast cell was as same as control group(no treatment group) and it showed granulated type. Conclusion: According to the above results, it might be considered that the administration of Daebangpoongtang has a curative effect on synovial membrane injury in arthritis by inhibiting increase of vessel, cell adhesion molecule(ICAM and VCAM) in LPS induced arthritis.

The Effect of Health Promotion Program on Health of the Clients with Arthritis in Primary Health Care Center (관절염 대상자를 위한 건강증진 프로그램의 효과 -보건진료소를 중심으로-)

  • A quasi-experimental study was conducted to investigate the effects of health promotion program on health of the clients with arthritis. In this study. the health promotion program consisted of self appointment and confirmation. discussion, health education. group counsel, and exercise. And the program focused on self - help group meeting. A total of 68 subjects was randomly assigned into either the control group(n = 24) or the intervention group(n =44). The results of the study analyzed using a SPSS win, were as follows: 1) In physical function of physical health. there was a significant improvement in flexibility of the shoulder joint(hold the hand upward and downward behind the back). sit and reach, extension of the knee joint in the intervention group, compared to the control group, while no difference in flexibility of the shoulder joint(raising the arm), flexion of the knee joint, and grip strength. There was a significant improvement in physical functional disability in the intervention group, compared to the control group, but no difference in fatigue and pain. 2) The health promotion program resulted in improvement in psychosocial health(e.g. increase of self-efficacy and decreases of social functional disability) in the intervention group, compared to the control group. It was concluded that the health promotion program(weekly session for 6 weeks) employed in this study was appropriate for the clients with arthritis in primary health care center and had a positive effect on health in general.

Inhibitory Effects of Soyeum Pharmacopuncture (SPP) on Rheumatoid Arthritis in Collagen II-induced Arthritis (CIA) Mice (소염약침액이 Collagen II 유발 관절염 mouse의 TNF-α, IFN-γ 생성 및 비장세포 증식에 미치는 영향)

  • Objective : The aim is to examine the effect of Soyeum Pharmacopuncture (SPP) on collagen-induced arthritis (CIA) in DBA/1OlaHsd mice. Methods : To determine the effect of SPP on chronic IFNlammatory joint disease, we induced CIA in DBA/1OlaHsd mice by immunization with bovine type II collagen. Animals were treated with intraperitoneal injection doses of 2 mg/kg of SPP, beginning 3 days before the expected onset of disease symptoms. Inhibitory Effects of SPP were observed by serum levels of TNF-${\alpha}$, and IFN-${\gamma}$,,, or cell proliferation in the spleen cell culture and histological examination of knee joint. Results : In the CIA Mice, serum levels of TNF-${\alpha}$, and IFN-${\gamma}$,, production in the spleen cell culture were reduced. At the histopathological examination of knee joint, chondropathy of cartilage in the synovial joint in the SP group was repaired while compared with control group. Conclusion : These results suggest that the SPP may be effective for the prevention and treatment of rheumatoid arthritis disease.

Effects of Ground Exercise for Arthritis Program in Person with Chronic Arthritis (만성관절염 환자에 대한 관절염체조의 효과)

  • The purpose of this study was to explore the effects of GEAP on pain, joint function, activities of daily living(ADL) and fatigue in chronic arthritis patients. The GEAP was held twice a week for 6 weeks for chronic arthritis patients at one university hospital in Seoul, Korea. Thirty four subjects completed the program, who were recruited at four times from September, 1999 to September, 2000. The effect of GEAP were evaluated as follows: Pain severity and number of painful joints ADL, fatigue were measured before and after the GEAP. In order to examine the joint flexibility and strengthening, the followings were measured: the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion (ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension, and the grip strength. Paired t-test and Wilcoxon signed rank test were used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the GEAP. After the GEAP, followings were found: 1. Pain severity and number of painful joints was significantly decreased. 2. The flexibility of both shoulders and arms, knee, both ankle were significantly improved. 3. The strengthening of both arms was significantly improved, but the strengthening of ankle was not changed. 4. ADL was significantly increased. 5. Fatigue was significantly decreased. In conclusion, GEAP used in this study was clearly proved to be an effective exercise program to reduce pain and fatigue, to enhance joint function and ADL in people with chronic arthritis. It is suggested that the GEAP should be recommended as one of the useful and appropriate nursing interventions for chronic arthritis patients.

The Effect of Bee Venom Pharmacopuncture Therapy on the Condition of Different Concentration in Rheumatoid Arthritis Rat Model (흰쥐의 류마티스 관절염 모델에서 봉약침의 농도별 처리 조건에 따른 치료 효과)

  • Objectives : The aim was to study the effect of bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model. Methods : We enforced a bee venom pharmacopuncture therapy with different concentration on rheumatoid arthritis rat model by the intradermal injection of chicken type II collagen emulsified. 14 days after the onset of the rheumatoid arthritis rat model, a fixed volume of bee venom was daily injected to ST-35 acupoint in the rat's knee joint for 2-3 weeks. The hind paw volume, arthritic index, arthritic flexion pain test, pain threshold, and serum analysis (CRP, $PGE_2$, ALT, AST) were analyzed, and the expression profiles of COX-2, c-fos, and substance-P at the dorsal horn region of the spinal cord and subchondral bone of the knee joint were also analyzed by using the immunohistochemistry. Results : After the treatment of rheumatoid arthritis rats with bee venom pharmacopuncture, the paw volume of edema of arthritic rats were almost restored to the level of normal group, and behavior tests were very effective. Also the evaluation on the blood serum analysis was remarkable. COX-2, c-fos, and substance-P positive cells in the immunohistological section of dorsal horn region of the spinal cord and subchondral bone of the knee joints were significantly decreased. also the bee venom pharmacopuncture was effective to alleviate their rheumatoid arthritic inflammation cytokine inhibition as regards to the behavior tests and joint histological appearance. Conclusions : Based on the results in this study, bee venom pharmacopuncture with concentrated treatment condition was very effective in low fixed quantity and progressive low increased quantity.

A study on related factors of knee joint pain in teachers (일선 교사들의 무릎관절 통증에 영향을 미치는 요인)

  • This study examined on the causes of chronic pain in the knee joint that was related to the aspects of teacher's job and daily lives. The subjects of investigation in this study include 237 of teachers who work at the elementary, middle and high schools. The chronic pain of knee joint of the subjects' is mainly related to their stress, and also to the vehicle that they normally use(p<0.05). This study aims at grasping the frequency of the pain which is caused by a knee joint's stress when teachers teach in a standing-up posture for many hours, looking at the aspects in the pain of knee joint that are related to job, being conducive to the development of the pain-cure, and, in addition, presenting an effective countermeasure to related agencies in order to prevent such diseases as arthritis.

  • Fungal arthritis is a rare joint infection that occurs in immunosuppressant patient, intravenous drug abuser and long term antibiotics user, and is especially rare in a healthy adult. Two case reports of fungal arthritis have been demonstrated in the country, but those in healthy adults have not been reported yet. Here, we experienced an antifungal treatment following successful arthroscopic debridement of fungal arthritis with a popliteal cyst in a healthy adult who got repetitive intraarticular steroid injections and report the case with review of relevant literatures. Surgeons should consider the possibility of fungal arthritis although it is rare and demonstrates nonspecific clinical features.
