Effects of Kinematics and Kinetics of the Lower Extremities Joint during Drop Landing in Adult Women with Patellofemoral Pain Syndrome (슬개대퇴동통증후가 성인 여성의 드롭랜딩 시 하지 주요관절의 운동역학적 변화에 미치는 영향)

  • Objective: This study investigated the different in isokinetic peak strength of the knee joint, and kinetics and kinematics in drop landing pattern of lower limb between the patellofemoral pain syndrome (PFPS) patients and normal. Method: 30 adult females were divided into the PFPS (age: 23.13±2.77 yrs; height: 160.97±3.79 cm, weight: 51.19±4.86 kg) and normal group (age: 22.80±2.54 yrs, height: 164.40±5.77 cm, weight: 56.14±8.16 kg), with 15 subjects in each group. To examine the knee isokinetic peak strength, kinematics and kinetics in peak vertical ground reaction force during drop landing. Results: The knee peak torque (Nm) and relative strength (%) were significantly weaker PFPS group than normal group. In addition, PFPS group had significantly greater hip flexion angle (°) than normal group. Moreover, normal group had significantly greater moment of hip abduction, hip internal rotation, and left ankle eversion than PFPS group, and PFPS group had significantly greater moment of knee internal rotation. Finally, there was significant differences between the groups at anteroposterior center of pressure. Conclusion: The PFPS patients had weakened knee strength, and which can result in an unstable landing pattern and cause of more stress in the knee joints despite to effort of reduce vertical ground reaction force.

Joint Pain in Patients with Osteoarthritis: Based on the 5th KNHNES (우리나라 골관절염 환자의 관절통증: 2012년 국민건강영양조사 자료를 이용하여)

  • Purpose:The purpose of this study was to examine the degree of joint pain, limitation of daily activities, and frequency of outpatient visits. Methods: This secondary analysis study used the data from the fifth Korean National Health and Nutrition Examination Survey (KNHNES). The data about general characteristics, prevalence and treatment of osteoarthritis, limitation of activities of daily living (ADL) and frequency of outpatient visits and hospitalization in 424 osteoarthritis patients over 50 years old were derived from the database. Data were analyzed with complex samples in SPSS ver. 20.0. Results: Among people with knee joint pain, 79.3% reported they had more than 4 out of 10 points of the degree of pain. Meanwhile, 97.3% of people with hip joint pain reported that they had more than 4 out of 10 points of the degree of pain. People reporting 10 out of 10 point of pain were 21.4% of those with knee joint pain and 25% of those with hip joint pain. The mean of the degree of knee joint pain was 6.35, and the mean of the degree of hip joint pain was 6.89 out of 10 points. About 50% of people with osteoarthritis visited an outpatient clinic within the last 2 weeks, and 27% had limitation of ADL. Conclusion: It is necessary to develop an integrated intervention program to improve quality of life in patients with osteoarthritis.

Shoulder and Hip Joint Range of Motion in Normal Adults (정상 성인의 견, 고관절 가동범위에 대한 조사)

  • The purpose of this article is to know the standard figures of joint range of motion, in conjuction with age and sen, for normal adults. The results of assessment and analysis io shoulder and hip joint range of motion are as follows : 1) The average shoulder joint range of motion in normal adults are $160.5^{\circ}$ in flexion, $53.5^{\circ}$ in extension, $159.3^{\circ}$ in adduction, $62.3^{\circ}$ in internal rotation, $83.9^{\circ}$ in external rotation, The average hip joint range of motions are $116.8^{\circ}$ in flexion, $16.1^{\circ}$ in extension, $41.1^{\circ}$ in abduction, $33.8^{\circ}$ in abduction, $40.0^{\circ}$ in interne rotation, $41.2^{\circ}$ in external rotation. 2) There is no significant difference in shoulder and hip joint range of motion between male and female (p>0.05). 3) As to the inter-relation in age and range of motion, the left flexion and extension, internal rotation and right extension in shoulder joint is decreased gradually with increasing age, and left flexion (knee flexion, knee extension) and right flexion (knee extension) in hip joint is decreased with increasing age (p<0.01). 4) Relating to age and sex, the twenties male shows highest range of motion in shoulder and hip joint, with .the fifties female shows, lowest range of motion.

Development of a Knee Exoskeleton for Rehabilitation Based EMG and IMU Sensor Feedback (단계별 무릎 재활을 위한 근전도 및 관성센서 피드백 기반 외골격 시스템 개발)

  • The number of knee-related disease patients and knee joint surgeries is steadily increasing every year, and for knee rehabilitation training for these knee joint patients, it is necessary to strengthen the muscle of vastus medialis and quadriceps femoris. However, because of the cost and time-consuming difficulties of receiving regular hospital treatment in the course of knee rehabilitation, we developed knee exoskeleton using rapid prototype for knee rehabilitation with feedback from the electromyogram (EMG) and inertia motion unit (IMU) sensor. The modules was built on the basis of EMG and an IMU sensor applied complementary filter, measuring muscle activity in the vastus medialis and the range of joint operation of the knee, and then performing the game based on this measurement. The IMU sensor performed up to 97.2% accuracy in experiments with ten subjects. The functional game contents consisted of an exergaming platform based on EMG and IMU for the real-time monitoring and performance assessment of personalized isometric and isotonic exercises. This study combined EMG and IMU-based functional game with knee rehabilitation training to enable voluntary rehabilitation training by providing immediate feedback to patients through biometric information, thereby enhancing muscle strength efficiency of rehabilitation.

The Effects of Static Stretching and Evjenth-Hamberg Stretching on Range of Motion of Knee Joint (정적 스트레칭과 에비안스-함베르크 스트레칭이 슬관절의 관절가동범위의 변화에 미치는 영향)

  • The purpose of this study was to examine the effects of static stretching and Evjenth-Hamberg stretching the on range of motion (ROM) of the knee joint. The subjects were composed of twenty healthy males without weight-training experience. The ROM of the knee joint was measured by using an En-Knee. Three tests (performed on the 1st week, 4th week, and 8th week) were conducted to examine the change of each variable. Data were analyzed with a $2{\times}3$ analysis of variance (group${\times}$test) for repeated measures on last factor through SPSS 10.0. The data analysis revealed the change in the ROM was dependent on the stretching method. The results were as follows: The ROM was improved in both methods by each time, but the E-HS was more improved than the SS. In conclusion, this study indicated that the E-HS is more efficient than the SS on the ROM of the knee joint.

  • An orthopedic surgeon normally gets the operational parameters of total knee arthroplasty from medical images(CT, MRI). Anatomical axis, mechanical axis, the width and height of femur, or tibia are the most important parameters related with accomplishment of TKA. This paper presents a methodology of simulation that virtually operates TKA according to 2D medical images. Using this simulator, some important parameters for operation can be achieved before hand. The simulator provides the 3D computational model of a knee joint and then derives the proper size of implant corresponding to the joint. The whole process of TKA can be simulated such as clipping a knee joint, assembling the joint and its implants, visualizing all the operation steps, deriving some crucial parameters such as anatomical axis and cutting thickness, and predicting the result of TKA. Some examples are given and discussed to validate the methodology.

  • Spontaneous osteonecrosis of the knee was first described by Ahlbck et al in 1968 as the spontaneous and sudden onset of severe pain, usually on the medial side of the knee joint, in old age(>60 years) with no specific etiologic factors. They differentiate the disease from osteochondritis dissecans, osteoarthritis, fracture, infection, neuropathic joint and secondary osteonecrosis of the knee joint. So far many investigators described this disease but had never observed cases developed in young patient with familial occurrence. The report presented here is a case of spontaneous osteonecrosis of both medial and lateral femoral condyles involving the bilateral knee joint that were treated by arthorscopic loose body removal and multiple drilling.

Effect of Contact Pressure on the Variations in Coefficients of Friction Between Porcine Knee Joint Cartilage and Co-Cr Alloy in a Repeat Pass Sliding Motion (반복 회전운동에서 코발트 크롬 합금과 미끄럼 접촉하는 돼지 무릎 관절연골의 접촉압력이 마찰계수 변화에 미치는 영향)

  • In this study, the influence of contact pressure on the variation in coefficients of friction between porcine knee joint cartilage and Co-Cr alloy in a repeat pass sliding motion was investigated. Flat-ended cartilage pin specimens(9 mm diameter, 8 mm long) were prepared from porcine(6 months old) knee joints by a drill-type punch. Friction tests were conducted by using a pin-on-disk type friction tester for an hour in PBS lubricated condition under the contact pressures of 0.5, 1 and 2 MPa with 50 mm distance per a cycle at ambient condition. As a result, coefficients of friction increased as the test duration increased for all contact pressures. The maximum coefficients of friction were 0.082, 0.06 and 0.098 for 0.5, 1, and 2 MPa, respectively. It showed that coefficients of friction of porcine knee joint cartilage against Co-Cr alloy depended on the level of contact pressure and related to squeeze film lubrication mechanism.

The Effect of Manual Joint Mobilization on Pain, ROM, Body Function and Balance in Patients with Knee Osteoarthritis (슬관절 골관절염환자에서 도수 관절가동술이 통증, 관절가동범위, 신체기능과 균형능력에 미치는 효과)

  • PURPOSE: The purpose of this study was to investigate effect of the manual joint mobilization to the patients with knee osteoarthritis and to determine the effect of pain, range of motion, body function and balance after applying it. METHODS: The thirty participants who complained the knee pain were randomly assigned to control (Con) group (n=15) that received the general physical therapy and experimental (Exp) group (n=15) that received the applied the manual joint mobilization and the general physical therapy three times per week, 30 minutes per day for four weeks. It measured the visual analogue scale (VAS), the range of motion (ROM), body function (WOMAC) and balance (TUG). RESULTS: It showed the significantly different between the control group and experiment group in VAS, ROM and WOMAC. After 4 weeks, the experiment group was significantly different from other group in VAS, ROM and WOMAC. But the measurement of balance did not show the significantly difference within group and between groups. CONCLUSION: This results suggest that Manual joint mobilization was effective in pain, ROM, function in patient with knee osteoarthritis.
