신장 조직의 방사선 손상과 천연물질의 방어기전 연구 (Protection Effect of Natual Matter and Radiation Damage on Kidney Tissue)

  • 본 연구는 방사선조사 후 신장 조직의 손상과 방어기전을 알아보고자 생쥐 모델을 이용하여 미세구조를 관찰하였다. 광학현미경(LM)의 관찰 결과; 5Gy 조사 후 10일된 신장 조직에서는 정상조직과 비교하여 사구체가 위축되었으며, 10Gy 관찰에서는 곱슬세관(convoluted tubules)의 막이 파괴되어 세포질이 유출되었다. 투과전자현미경(TEM) 관찰 결과; 5Gy 조사에서는 사립체의 막이 파괴되거나 함몰된 것으로 관찰되었으며, 내부 크리스테의 형태도 소실되었다. 또한 일부에서는 핵막의 파괴도 확인되었다. 10Gy 조사한 조직에서는 기저막의 파괴가 뚜렷하게 관찰되고, 용해소체도 비후 된 것을 관찰하였다. 하지만 프로폴리스 섭식한 실험 군에서는 소기관의 형태가 뚜렷하게 관찰되었고, 핵막과 염색질도 선명하게 관찰되어 방어효과를 확인할 수 있었다.

Protective effect of ginsenoside Rh3 against anticancer drug-induced apoptosis in LLC-PK1 kidney cells

  • Background: Ginsenosides are active components of Panax ginseng that exert various health benefits including kidney protection effect. The medicinal activity of ginsenosides can be enhanced by modulating their stereospecificity by heat processing. Ginsenosides Rk2 and Rh3 represent positional isomers of the double bond at C-20(21) or C-20(22). Methods: The present study investigated the kidney-protective effects of ginsenosides Rk2 and Rh3 against cisplatin, a platinum based anticancer drug, induced apoptotic damage in renal proximal LLC-PK1 cells. Results: As a result, ginsenoside Rh3 shows a stronger protective effect than that shown by Rk2. Cisplatin-induced elevated protein levels of phosphorylated c-Jun N-terminal kinase (JNK), extracellular signal-regulated kinase (ERK), p38, and cleaved caspase-3 decreased after cotreatment with ginsenoside Rh3. The increase in the percentage of apoptotic LLC-PK1 cells induced by cisplatin treatment also significantly reduced after cotreatment with ginsenoside Rh3. Conclusion: These results demonstrate that inhibition of the JNK and ERK mitogen-activated protein kinase signaling cascade plays a critical role in mediating the renoprotective effect of ginsenoside Rh3.

민들조개(Gomphina melanaegis)에서 방사성 동위원소의 조직내 분포와 결합단백질에 관한 연구 (Tissue Distribution and Binding Proteins of Radionuclides in Bivalve, Gomphina melanaegis)

  • 민들조개에 의해 흡수된 방사성 동위원소의 조직내 분포와 생화학적 거동을 알아 보기 위해 민들조개를 $^{65}Zn,\;^{54}Mn,\;^{137}Cs$ 용액에 7일간 노출시킨 후 조직을 잘라내서 방사능을 측정하고 간과 콩팥을 균질화시켜서 Sephadex G-75 유리판을 통한 젤투석 실험을 하였다. 방사성 동위원소의 조직내 분포는 방사성 동위원소에 따라 서로 다른 조직내 축적경향을 나타냈으며 특히 다른 조직에 비해 간과 콩팥에 아주 높게 농축되었다. 방사능이 농축된 간과 콩팥에서 방사성 동위원소와 결합하고 있는 단백질을 조사하기 위해서 간과 콩팥을 균질화시킨 상등액을 Sephadex G-75 column chromatography로 분리 한 결과 $^{65}Zn,\;^{54}Mn,\;^{137}Cs$가 서로 다른 유출 패턴을 나타냈고 또한 $^{65}Zn$$^{54}Mn$은 간과 콩팥에서 서로 다르게 유출되었다. 본 실험의 결과 방사성 동위원소의 종류와 조직에 따라서 결합하고 있는 단백질의 종류가 서로 다른 것을 알 수 있었다.

신장 이식후 발생한 해리성 흉복부 대동맥류의 외과적 치험 (Thoracoabdominal Aortic Aneurysm Repair after Renal Transplantation)

  • 20개월 전에 신장이식을 시행한 환자에 있어서 해리성 흥복부 대동맥류를 성공적으로 절제하였다. 이 식된 신장은 정상 신장에 비하여 허혈에 보다 민감하므로 동맥혈 차단후 이식된 신장의 관류를 위하 여 대퇴 동-정맥으로 심페기를 연결하였다. 138분간의 대동맥 교차 차단중에 신장으로의 관류는 관류압이 (19-)1) 27 minHg이고 관류량은 0.53에서 0.83 Limin 였고, 32mm 크기의 헤마쉴드(길이 22cm)가 동맥류 부위 에 대용되 었다. 수술후 특별한 합병증은 없었으며 심폐기를 이용한 대퇴동-정맥 우회술은 이와같은 수술시 척수와 신 장을 보호하는 간단하고 효율적 인 방법으로 사료된다.

  • Chronic Kidney Disease (CKD) is a major global health burden. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have demonstrated potential in slowing CKD progression. We evaluated the expanding role of SGLT2 inhibitors, emphasizing their renoprotective benefits in diabetic and non-diabetic CKD patients. We also investigated the underlying mechanisms, including the reduction of glomerular hypertension via modulation of tubuloglomerular feedback. Our study critically analyzed current indications for SGLT2 inhibitor therapy based on recent clinical trial data. To optimize patient outcomes, we present a comprehensive analysis of practical considerations for the prescription of SGLT2 inhibitors, including the potential initial decline in the estimated glomerular filtration rate and a review of adverse events.

  • Coronary artery disease is highly prevalent in chronic kidney disease (CKD) and is a risk factor for contrast-associated acute kidney injury (CA-AKI), a complication of cardiovascular procedures that require contrast administration (e.g., coronary angiography, percutaneous coronary intervention [PCI]). CA-AKI has a major impact on morbidity, mortality, and healthcare resource utilization. The incidence of CA-AKI is particularly high in patients with pre-existing CKD, advanced age and comorbidities that increase the likelihood of CKD. The focus of the present review is to provide a brief overview on the assessment of the risk for and prevention of CA-AKI in patients undergoing angiography and PCI, including recognition of the important patient- and procedure-related factors that may contribute to CA-AKI. Preventive and treatment strategies, the mainstay of which is volume repletion by normal saline, are briefly discussed. The main focus of the review is placed on technical details of contrast minimization techniques, including ultra-low contrast angiography and zerocontrast PCI. Operator competence in such techniques is important to ensure that procedural challenges in patients with CKD, like vessel calcification, multivessel disease and complex anatomical subsets, are effectively addressed by PCI while minimizing the risk of CA-AKI.

UV-B가 강낭콩(Phaseolus vulgaris L.)의 지방산 구성, 지질과산화 및 polyamine 함량에 미치는 영향 (Effect of UV-B on fatty Acid Composition, Lipid Peroxidation and Polyamine in Kidney Bean(Phaseolus vulgaris L.))

  • UV-B에 의한 강낭콩 식물의 피해양상, 지질과산화, 지질조성 변화와 UV-B에 대한 식물의 방어기작 등을 조사하기 위하여 3주간 UV-B 조사 실험을 수행하였다. UV-B 처리에 의해 초장이 약 22% 정도 감소하는 것으로 나타났다. MDA 함량은 UV-B 처리에 의해 약 50% 정도 증가하는 것으로 나타났다. Glutathione 및 ascorbate acid 함량은 UV-B 조사에 의해 산화형이 증가하고 환원형이 감소하는 것으로 나타났다. UV-B 조사에 의한 지방산 구성 변화를 조사한 결과 당지질 및 인지질 모두 UV-B조사에 의해 포화지방산이 증가하는 것에 반해, 불포화지방산이 감소하는 것으로 나타났다. 강낭콩 잎에는 주로 3종류의 polyamine이 존재하는 것으로 나타났으며, 3종류 모두 UV-B 조사에 의해 증가하는 것으로 나타났다. 본 실험 결과로 볼 때, UV-B 조사는 활성산소를 생성하여 생체막 지질에 직접적인 영향을 미치는 것으로 나타났으며, ascorbate acid, glutathione, polyamine 등의 항산화물질들이 이에 대한 피해를 최소화하기 위하여 작용하는 것으로 나타났다.

L-Arginine Ameliorates Kidney Function and Urinary Bladder Sensitivity in Experimentally-induced Renal Dysfunction in Rats

  • Effects of L-arginine and NG-nitro-L-arginine methyl ester (L-NAME) on the renal dysfunction that is induced by cisplatin (CDDP) were investigated. A single dose of CDDP (7.5 mg/kg i.p.) induced renotoxicity, which was manifested by increasing the sensitivity of isolated urinary bladder rings to acetylcholine (ACh), together with a significant elevation of serum urea and creatinine, and a severe decrease in serum albumin. Moreover, renal dysfunction was further confirmed by a significant decrease of enzyme activities, such as glutathione peroxidase, GSH-Px (E.C 1.11.1.9), catalase (E.C 1.11.1.6), as well as a significant increase in lipid peroxides that were measured as malondialdhyde (MDA) in kidney tissue homogenates. The administration of L-arginine (70 mg/kg/d p.o in drinking water 5 d before and 5 d after the CDDP injection) significantly ameliorated the renotoxic effects of CDDP, as judged by restoring the normal responses of isolated bladder rings to Ach, and also by an improvement in a range of renal function indices, which included serum urea and creatinine concentrations and kidney weight. In addition, L-arginine prevents the rise of MDA, as well as a reduction of GSH-Px and catalase activities in kidney tissues homogenates. On the other hand, the administration of L-NAME (4 mg/kg/d p.o) resulted in no protection against renal dysfunction that was induced by CDDP treatment. The findings of this study suggest that L-arginine can attenuate kidney injury that is produced by CDDP treatment. In addition, L-arginine may be a beneficial remedy for CDDP-induced renal toxicity, and could be used to improve the therapeutic index of CDDP.

Hot Water Extract of Triticum aestivum L. (Common Wheat) Ameliorates Renal Injury by Inhibiting Apoptosis in a Rat Model of Ischemia/Reperfusion

  • Objectives : Interruption and subsequent restoration of blood flow into the kidney result in renal injury. As an approach to preventing the renal injury, we determined the optimal conditions and the underlying mechanisms by which supernatant of hot water extract of ground Triticum aestivum L. (extract) attenuated ischemia/reperfusion (I/R) injury. Methods : One hour after administration of the extract (400 mg/kg) by intraperitoneal injection, renal I/R injury was generated by clamping the left renal artery in rats after surgical removal of the right kidney, followed by reperfusion. The maximal difference between the vehicle-treated and the extract-treated group under ketamine/xylazine or enflurane anesthetization was assessed at varying periods of ischemia (30-45 min) and reperfusion (3-48 hr), based on the renal function assessed with serum creatinine levels, tissue injury with hematoxylin/eosin staining, and apoptosis with terminal deoxynucleotidyltransferase-mediated dUTP nick-end labeling staining. Results : Enflurane anesthetization with 40 min of ischemia and 24 hr of reperfusion was identified to be the optimal condition, under which condition serum creatinine levels and tubular damage in the extract-treated group were significantly reduced compared with those in the vehicle-treated group ($1.3{\pm}0.2$ versus $2.7{\pm}0.3$ mg/dL, P < 0.01, and average score $1.8{\pm}0.1$ versus $3.5{\pm}0.3$, P < 0.01, respectively). These beneficial effects were mediated by inhibition of apoptotic cascades through attenuation of renal tissue malondialdehyde levels, Bax/Bcl-2 ratio and caspase-3 levels. Conclusions : The extract conferred renal protection against ischemia/reperfusion injury in rats by scavenging reactive oxygen species and consequently blocking apoptotic cascades, plausibly augmented by enflurane protection.

Loganin Ameliorates Acute Kidney Injury and Restores Tofacitinib Metabolism in Rats: Implications for Renal Protection and Drug Interaction

  • Tofacitinib, a Janus kinase (JAK) inhibitor used to treat rheumatoid arthritis, is metabolized through hepatic cytochrome P450 (CYP), specifically CYP3A1/2 and CYP2C11. Prolonged administration of rheumatoid arthritis medications is generally associated with an increased risk of renal toxicity. Loganin (LGN), an iridoid glycoside, has hepatorenal regenerative properties. This study investigates the potential of LGN to mitigate acute kidney injury (AKI) and its effects on the pharmacokinetics of tofacitinib in rats with cisplatin-induced AKI. Both intravenous and oral administration of tofacitinib to AKI rats significantly increased the area under the plasma concentration-time curve from time 0 to infinity (AUC) compared with control (CON) rats, an increase attributed to the decelerated non-renal clearance (CLNR) and renal clearance (CLR) of tofacitinib. Administration of LGN to AKI rats, however, protected kidneys from severe impairment, restoring the pharmacokinetic parameters (AUC, CLNR, and CLR) of tofacitinib to those observed in untreated CON rats, with partial recovery of kidney function, as evidenced by an increase in creatinine clearance. Possible interactions between drugs and natural components should be considered, especially when co-administering both a drug and a natural extract containing LGN or iridoid glycosides to patients with kidney injury.
