The Effects of Diagnostic Radiology Image on Radiopharmaceutical Testing (방사성의약품 검사 시 진단(CT)영상에 미치는 영향)

  • This research attempts to qualitatively evaluate the intensity change by radiopharmaceuticals and obtain computed tomography using phantom injected with various nuclide. Cylindrical phantom is used for comparing and analysing the effect on diagnosis image during radiopharmaceuticals inspection. Inside of the phantom, water is injected and computed tomography image is scanned. During nuclear medicine invitro, frequently used radiopharmaceuticals, $^{99m}TcO_4$ 20 mCi and $^{18}F$ 14 mCi, is diluted in the water phantom and scanned in the same method. Traverse image obtained by CT scan is divided into six traverse image in the same slice of each scanned image. CT-number(HU) value of 10 measuring point is measured in 2 cm interval based on the center of the phantom. Measured HU value, based on the water phantom, is compared with the image after injecting $^{99m}TcO_4$ and $^{18}F$. Average scale of water is 2.8~1.6 HU, $^{99m}TcO_4$ is 3.0~1.6 HU and $^{18}F$ is 1.2~0 HU. Average of water is $2.3{\pm}0.17$ HU, $^{99m}TcO_4$ is $2.2{\pm}0.85$ HU and F-18 is $0.7{\pm}0.95$ HU. Based on water, reduced value of about 0.1 HU and about 0.5 HU is acquired from $^{99m}TcO_4$ and F-18. Radionuclide used in nuclear medicine inspection utilizes 100~200 KeV energy and obtains image through scintillation camera and PET-CT utilizes 511 KeV positron annihilation energy to obtain image. What we learned from this research is that gamma rays from these energies used in CT scan for diagnosis purpose or radioactive therapy plan can change the intensity of the image. The nuclear medicine inspection for reducing the effect of emitted gamma ray diagnosis image should be obtained after a period of time considering half-life which would be reduced distortion or changed in image.

The Optimization of Optical Current Transformer owing to Incident Polarization (입사편광에 따른 광섬유형 광 CT의 최적화)

  • The optical current transformer was developed for 170 kV GIS using optical fiber. The sensor optimized on the optical CT was wound 3 turns and twisted 4 times per a turn at the pipe with a diameter of 130 m. To optimize the optical CT, the output signal was measured according to the setting angle of polarizer and analyzer, The asymmetry and distortion of the output signals were improved when the parallel polarized light was incident to the fiber sensor and under the angle of analyzer was $45^{o}$. The measurement error for the linearity was only $\pm{0.42}\;\%$ to 1,000 A in the case of reflection type.

The evaluation of the feasibility about prostate SBRT by analyzing interfraction errors of internal organs (분할치료간(Interfraction) 내부 장기 움직임 오류 분석을 통한 전립선암의 전신정위적방사선치료(SBRT) 가능성 평가)

  • Purpose : To figure out if the treatment plan for rectum, bladder and prostate that have a lot of interfraction errors satisfies dosimetric limits without adaptive plan by analyzing MR image. Materials and Methods : This study was based on 5 prostate cancer patients who had IMRT(total dose: 70Gy) Using ViewRay MRIdian System(ViewRay, ViewRay Inc., Cleveland, OH, USA) The treatment plans were made on the same CT images to compare with the plan quality according to adaptive plan, and the Eclipse(Ver 10.0.42, Varian, USA) was used. After registrate the 5 treatment MR images to the CT images for treatment plan to analyze the interfraction changes of organ, we measured the dose volume histogram and the changes of the absolute volume for each organ by appling the first treatment plan to each image. Over 5 fractions, the total dose for PTV was $V_{36.25}$ Gy $${\geq_-}$$ 95%. To confirm that the prescription dose satisfies the SBRT dose limit for prostate, we measured $V_{100%}$, $V_{95%}$, $V_{90%}$ for CTV and $V_{100%}$, $V_{90%}$, $V_{80%}$ $V_{50%}$ of rectum and bladder. Results : All dose average value of CTV, rectum and bladder satisfied dose limit, but there was a case that exceeded dose limit more than one after analyzing the each image of treatment. After measuring the changes of absolute volume comparing the MR image of the first treatment plan with the one of the interfraction treatment, the difference values were maximum 1.72 times at rectum and maximum 2.0 times at bladder. In case of rectum, the expected values were planned under the dose limit, on average, $V_{100%}=0.32%$, $V_{90%}=3.33%$, $V_{80%}=7.71%$, $V_{50%}=23.55%$ in the first treatment plan. In case of rectum, the average of absolute volume in first plan was 117.9 cc. However, the average of really treated volume was 79.2 cc. In case of CTV, the 100% prescription dose area didn't satisfy even though the margin for PTV was 5 mm because of the variation of rectal and bladder volume. Conclusion : There was no case that the value from average of five fractions is over the dosimetric limits. However, dosimetric errors of rectum and bladder in each fraction was significant. Therefore, the precise delivery is needed in case of prostate SBRT. The real-time tracking and adaptive plan is necessary to meet the precision delivery.

  • Purpose: It was reported that CT-based measured attenuation correction (CT-MAC) produced radioactivity concentration values significantly higher than $^{68}Ge$-based segmented attenuation correction (Ge-SAC) in PET images. However, it was unknown whether the radioactivity concentration difference resulted from different sources (CT vs. Ge) or types (MAC vs. SAC) of attenuation correction (AC). We evaluated the influences of the source and type of AC on the radioactivity concentration differences between reconstructed PET images in normal subjects and patients. Material and Methods: Five normal subjects and 35 patients with a known or suspected cancer underwent $^{18}F-FDG$ PET/CT. In each subject, attenuation corrected PET images using OSEM algorithm (28 subsets, 2 iterations) were reconstructed by 4 methods: CT-MAC, CT-SAC, Ge-MAC, and Ge-SAC. The physiological uptake in normal subjects and pathological uptake in patients were quantitatively compared between the PET images according to the source and type of AC. Results: The SUVs of physiological uptake measured in CT-MAC PET images were significantly higher than other 3 differently corrected PET images. Maximum SUVs of the 145 foci with abnormal FDG uptake in CT-MAC images were significantly highest among 4 differently corrected PET images with a difference of 2.4% to 5.1% (p<0.001). The SUVs of pathological uptake in Ge-MAC images were significantly higher than those in CT-SAC and Ge-MAC PET images (p<0.001). Conclusion: Quantitative radioactivity values were highest in CT-MAC PET images. The adoption of MAC may make a more contribution than the adoption of CT attenuation map to such differences.

Evaluation of PET Image for Fluorine-18 and Gallium-68 using Phantom in PET/CT (PET/CT에서 Phantom을 이용한 Fluorine-18, Gallium-68 방사성 핵종의 PET 영상 평가)

  • The purpose of this study is to compare PET imaging performance with Fluorine-18 ($^{18}F$) and Gallium-68 ($^{68}Ga$) for influence of physical properties of PET tracer. Measurement were performed on a Siemens Biograph mCT64 PET/CT scanner using NEMA IEC body phantom and Flangeless Esser PET phantom containing filled with $^{18}F$ and $^{68}Ga$. Emission scan duration(ESD) was set to 1, 2, 3, 4 and 5min/bed for $^{68}Ga$ and 1min/bed for $^{18}F$. The PET image were evaluated in terms of contrast, spatial resolution. Under same condition, The percentage of contrast recovery measured in the phantom ranged from 16.88% to 72.56% for $^{68}Ga$ and from 27.51% to 74.43% for $^{18}F$ and The FWHM value to evaluate spatial resolution was 10.96 mm for $^{68}Ga$ and 9.19 mm for $^{18}F$. For this study, $^{18}F$ produces better image contrast and spatial resolution than $^{68}Ga$ due to higher positron yield and lower positron energy ($^{18}F$: 96.86%, 633.5 keV, $^{68}Ga$: 88.9%, 1899 keV), The physical properties of PET tracer effect on the PET image. $^{68}Ga$ image applying ESD of 3, 4, 5min/bed were showed similar to $^{18}F$ image with ESD of 1min/bed. This study suggests that increasing ESD for acquiring $^{68}Ga$ PET image seem to be similar to $^{18}F$ image.

Comparison of CT based-CTV plan and CT based-ICRU38 plan in Brachytherapy Planning of Uterine Cervix Cancer (자궁경부암 강내조사 시 CT를 이용한 CTV에 근거한 치료계획과 ICRU 38에 근거한 치료계획의 비교)

  • Purpose : In spite of recent remarkable improvement of diagnostic imaging modalities such as CT, MRI, and PET and radiation therapy planing systems, ICR plan of uterine cervix cancer, based on recommendation of ICRU38(2D film-based) such as Point A, is still used widely. A 3-dimensional ICR plan based on CT image provides dose-volume histogram(DVH) information of the tumor and normal tissue. In this study, we compared tumor-dose, rectal-dose and bladder-dose through an analysis of DVH between CTV plan and ICRU38 plan based on CT image. Method and Material : We analyzed 11 patients with a cervix cancer who received the ICR of Ir-192 HDR. After 40Gy of external beam radiation therapy, ICR plan was established using PLATO(Nucletron) v.14.2 planing system. CT scan was done to all the patients using CT-simulator(Ultra Z, Philips). We contoured CTV, rectum and bladder on the CT image and established CTV plan which delivers the 100% dose to CTV and ICRU plan which delivers the 100% dose to the point A. Result : The volume$(average{\pm}SD)$ of CTV, rectum and bladder in all of 11 patients is $21.8{\pm}6.6cm^3,\;60.9{\pm}25.0cm^3,\;111.6{\pm}40.1cm^3$ respectively. The volume covered by 100% isodose curve is $126.7{\pm}18.9cm^3$ in ICRU plan and $98.2{\pm}74.5cm^3$ in CTV plan(p=0.0001), respectively. In (On) ICRU planning, $22.0cm^3$ of CTV volume was not covered by 100% isodose curve in one patient whose residual tumor size is greater than 4cm, while more than 100% dose was irradiated unnecessarily to the normal organ of $62.2{\pm}4.8cm^3$ other than the tumor in the remaining 10 patients with a residual tumor less than 4cm in size. Bladder dose recommended by ICRU 38 was $90.1{\pm}21.3%$ and $68.7{\pm}26.6%$ in ICRU plan and in CTV plan respectively(p=0.001) while rectal dose recommended by ICRU 38 was $86.4{\pm}18.3%$ and $76.9{\pm}15.6%$ in ICRU plan and in CTV plan, respectively(p=0.08). Bladder and rectum maximum dose was $137.2{\pm}50.1%,\;101.1{\pm}41.8%$ in ICRU plan and $107.6{\pm}47.9%,\;86.9{\pm}30.8%$ in CTV plan, respectively. Therefore, the radiation dose to normal organ was lower in CTV plan than in ICRU plan. But the normal tissue dose was remarkably higher than a recommended dose in CTV plan in one patient whose residual tumor size was greater than 4cm. The volume of rectum receiving more than 80% isodose (V80rec) was $1.8{\pm}2.4cm^3$ in ICRU plan and $0.7{\pm}1.0cm^3$ in CTV plan(p=0.02). The volume of bladder receiving more than 80% isodose(V80bla) was $12.2{\pm}8.9cm^3$ in ICRU plan and $3.5{\pm}4.1cm^3$ in CTV plan(p=0.005). According to these parameters, CTV plan could also save more normal tissue compared to ICRU38 plan. Conclusion : An unnecessary excessive radiation dose is irradiated to normal tissues within 100% isodose area in the traditional ICRU plan in case of a small size of cervix cancer, but if we use CTV plan based on CT image, the normal tissue dose could be reduced remarkably without a compromise of tumor dose. However, in a large tumor case, we need more research on an effective 3D-planing to reduce the normal tissue dose.

Comparative Analysis of Bone Mineral Contents with Dual-Energy Quantitative Computed Tomography (이중에너지광자선의 전산화단층촬영술을 이용한 정량적 골무기물함량의 비교분석)

  • Purpose : The Dual-Energy Quantitative Computed Tomography(DEQCT) was compared with bone equivalent $K_2HPO_4$ standard solution and ash weight of animal cadaveric trabecular bone in the measurement of bone mineral contents(BMC). Method and Materials : The attenuation coefficient of tissues highly depends on the radiation energy density and effective atomic number of composition, The bone mineral content of DEQCT in this experiments was determined from empirical constants and mass attenuation coefficients of bone,fat and soft tissue equivalent solution in two photon spectra. In this experiments, the BMC of DEQCT with 80 and $120kV_p$ X rays was compared to ash weight of animal trabecular bone. Results : We obtained the mass attenuation coefficient of 0.2409 0.5608 and 0.2206 in $80kV_p$, and 0.2046, 0.3273 and $0.1971cm^2/g$ in $120kV_p$ X-ray spectra for water bone and fat equivalent materials, respectively. The BMC with DEQCT was acomplished with empirical constants $K_1=0.3232,\;K_2$=0.2450 and mass attenuation coefficients has very closed to ash weight of animal trabecular bone The BMC of empirical DEQCT and that of manufacturing DEQCT were correlated with ash weight as a correlation r= 0.998 and r= 0.996, respectively. Conclusion : The BMC of empirical DEQCT using the experimental mass attenuation coefficients and that of manufacture have showed very close to ash weight of animal trabecular bone.

Studies on effects of calibration methods and current lead position on the direct current potential drop method for crack length measurement (직류전압강하법에 의한 균열길이 측정에 미치는 도선의 위치 및 보정방법의 영향에 관한 연구)

  • The effective resolution of the direct current potential drop (DCPD) method for crack length determination is strongly affected by a number of factors including wire locations and calibration method. In the present study, the effects of wire locations, thermal EMF and reference probe locations on the accuracy of calibration methods, including Hicks-Pickard equation and Johnson's equation, were examined with the CT specimens which were nine times larger than the standard specimen. In light of experimental results, it was found that Hicks-Pickard equation can accurately represent the a/W-V/Vo relationship when current input wire is located at the load line. It was also found that the accuracy of DCPD method can be greatly improved with the thermal EMF calibration and the use of Vo value at a/W = 0.241. The use of reference potential was found to be impractical when current input wire is located at the load line.

Empirical Determination of a CT X-ray Spectra by Numerical Analysis using Transmission Data (투과선량의 수치해석에 의한 전산화단층영상장치 X선의 에너지 분포결정)

  • The knowledge of x-ray spectra is highly desirable in some investigation involves the differential penetrating power and absorption coefficient correction of various photon beam. The transmission data were obtained from the 80 kVp and 120 kVp of CT x-ray beam with the aluminium filter which is designed in a 30 cm of diameter and pipe-typed filter was prepared from 5.0 mm upto 92.3 mm of thickness. To obtain the reconstructed spectra of CT x-ray, the investigator used the iterative numerical analysis which has been extended to include the tungsten characteristics from experimental transmission data with energy interval of 2 keV. Comparison of the calculated transmission data from the reconstructed spectra with that of measurement shows good agreement in both 80 kVp and 120 kVp x-ray beams. This numerical analysis based on iteratively calculation of fractional exposure per energy interval shows the high potential of usefulness of determination the x-ray spectra from the attenuated beam in diagnostic energy range.

Drug susceptibility of bacteria and M pachydermatis isolated from canine external ear canals (개 외이도에서 분리한 세균 및 M pachydermatis의 약제감수성에 대하여)

  • The present work was conducted to investigate the drug susceptibility of microorganisms isolated from canine external ear canals. Antifungal susceptibility test of M pachydermatis (17 strains) was perfomed by agar dilution method, using 11 antifungal drugs including amphotericin B(A), nystatin(N), pimaricin(P), griseofulvin(G), bifonazole(B), clotrimazole(C), miconazole(M), econazole(E), ketoconazole(K), tolnaftate(T), 5-fluorocytosine(F). All isolates were highly sensitive to K, M, T(geometric mean MIC ; GM $MIC{\leq}0.16{\mu}g/ml$) but they weren't sensitive to P, F and G(GM $MIC{\geq}92.37{\mu}g/ml{\sim}{\geq}128{\mu}g/ml$). Antibacterial susceptibility test against 119 isolates of bacteria was performed by agar dilution method, using 9 antibacterial drugs including erythromycin(ET), chloramphenicol(CP), gentamycin(G), vancomycin(V), ampicillin(AP), amoxacillin(AX), chlortetracycline(CT), ciprofloxacin(CF), enrofloxacin(EF). All isolates of Staphylococcus spp(101 strains) were highly sensitive to EF, CF, G(GM MIC $0.33{\sim}1.47{\mu}g/ml$). In other gram positive cocci(4 strains), they were highly sensitive to EF, CF, V(GM MIC $1{\sim}4.76{\mu}g/ml$) and CT(GM MIC 1 UFL unit/ml). In gram positive rods(13 strains), they were highly sensitive to EF, CF, G(GM $MIC{\leq}0.19{\sim}1{\mu}g/ml$). In Pseudomonas aeruginosa(1 strain), it was highly sensitive to AX, EF, ET, CF(GM MIC $0.06{\sim}1{\mu}g/ml$) and CT(GM MIC 1 UFL unit/ml). All isolates weren't sensitive to AP(GM MIC 16~>$32{\mu}g/ml$).
