Outcome of Conservative Treatment of the Zone I, II 5th Metatarsal Base Fracture under Early Weight-Bearing (제5 중족골 제1, 2 구역 골절의 조기 체중부하의 비수술적 치료 결과)

  • Purpose: To determine how the location, displacement, intra-articular involvement, comminution of a 5th metatarsal base fracture affect results of early weight-bearing treatment. Materials and Methods: From January 2013 to July 2017, 34 cases of 34 patients diagnosed with a fracture of the zone I and II 5th metatarsal base were enrolled. The mean follow-up period was 13 months (6-15 months). One patient was excluded as a refracture during the follow-up period, and 33 patients underwent conservative treatment. Anteroposterior, lateral, and simple oblique radiography and computed tomography of the foot were performed to evaluate the location and displacement of the fracture, the degree of joint involvement, and comminution. In all 33 patients, a short leg cast or boot brace was selected immediately after the injury, tolerable weight bearing was allowed. If the pain disappeared, full weight bearing was performed after wearing a plain shoe or postoperative shoe. As a clinical result, the American Orthopedic Foot and Ankle Society (AOFAS) score was evaluated at the final follow-up. During outpatient follow-up, a simple radiograph of the foot was taken to confirm the time of radiological bone union and return to work. Results: Nine males and 24 females, with an average age of 48.7 years, were enrolled in the study. Twenty-four patients had zone I fractures, and nine patients had zone II fractures. Twenty-two out of 33 patients had a fracture displacement of 2 mm or more. Nine and five patients had joint involvement and comminution, respectively. There was a statistically significant return to work from zone I to zone II. The AOFAS score was excellent at the final follow-up and there was no significant difference. When classifying and comparing the degree of fracture displacement, joint involvement, and comminution, there were no significant differences in the radiological union time and return to work. In all cases, satisfactory results were obtained at the final follow-up. Conclusion: Satisfactory clinical results can be obtained by allowing early weight-bearing regardless of the fracture location, displacement, joint involvement, or comminution in zone I and II 5th metatarsal base fractures.

  • The copula-based models have been successfully applied to hydrological modeling including drought frequency analysis and time series modeling. However, uncertainty estimation associated with the parameters of these model is not often properly addressed. In these context, the main purposes of this study are to develop the Bayesian inference scheme for bivariate copula functions. The main applications considered are two-fold: First, this study developed and tested an approach to copula model parameter estimation within a Bayesian framework for drought frequency analysis. The proposed modeling scheme was shown to correctly estimate model parameters and detect the underlying dependence structure of the assumed copula functions in the synthetic dataset. The model was then used to estimate the joint return period of the recent 2013~2015 drought events in the Han River watershed. The joint return period of the drought duration and drought severity was above 100 years for many of stations. The results obtained in the validation process showed that the proposed model could effectively reproduce the underlying distribution of observed extreme rainfalls as well as explicitly account for parameter uncertainty in the bivariate drought frequency analysis.

Seismic Performance Evaluation of Unreinforced and ECC-jacketed Masonry Fences using Shaking Table Test (진동대실험을 사용한 비보강 및 ECC 자켓 보강 조적담장의 내진성능평가)

  • In this study, the efficacy of Engineered Cementitious Composite(ECC) jacket for masonry fences subjected to lateral dynamic load was experimentally verified through a shaking table test, comparing it with the performance of an unreinforced masonry(URM) fence. Firstly, dominant frequencies, modal damping ratios and deformed shapes were identified through an impact hammer test. URM and ECC-strengthened fences with heights of 940mm and 970mm had natural frequencies of 6.4 and 35.3Hz, and first modal damping ratios of 7.0 and 5.3%, respectively. Secondly, a shaking table test was conducted in the out-of-plane direction, applying a historical earthquake, El Centro(1940) scaled from 25 to 300%. For the URM fence, flexural cracking occurred at the interface of brick and mortar joint(i.e., bed joint) at the ground motion scaled to 50%, and out-of-plane overturning failure followed during the subsequent test conducted at the ground motion scaled to 30%. On the other hand, the ECC-jacketed fence showed a robust performance without any crack or damage until the ground motion scaled to 300%. Finally, the base shear forces exerted upon the URM and ECC-jacketed fences by the ground motions scaled to 25~300% were evaluated and compared with the ones calculated according to the design code. In contrast to the collapse risk of the URM fence at the ground motion of 1,000-year return period, the ECC-jacketed fence was estimated to remain safe up to the 4,800-year return period ground motion.

The Effect of Cycle Ergometer Exercise Inducing Movement of the Affected Side on Knee Joint Function after Total Knee Arthroplasty (환측사용유도 고정식자전거운동이 무릎관절 기능에 미치는 영향: 무릎인공관절수술 환자를 대상으로)

  • Purpose: The purpose of this study was to examine the effect of cycle ergometer exercise inducing movement of the affected side on knee joint function after total knee arthroplasty (TKA). Methods: The primary experiment was conducted on 19 members of the cycle ergometer exercise group to measure the muscle activity of the rectus femoris, hamstring, tibialis anterior, and gastrocnemius muscles during cycle ergometer exercise that induced the affected side's movement. In the second experiment, after receiving physiotherapeutic intervention for 30 min, the general bicycle exercise group and cycle ergometer exercise group performed the corresponding exercise for 15 min, 5 times per week, for 2 weeks. The ROM, muscle strength, pain, and balance were then measured and compared between the two groups. Results: In the results of the primary experiment, cycle ergometer exercise inducing movement of the affected side showed a significantly larger increase in the activity of leg muscles (rectusfemoris, hamstring, tibialis anterior, gastrocnemius) on the affected side than the general bicycle exercise (p <0.05). In the second experiment, the cycle ergometer exercise group showed a significantly larger increase in range of movement of affected side knee flexion and muscle strength of affected side knee flexion, knee extension, and plantarflexion than the general bicycle exercise (p <0.05). No significant between-group difference was observed in pain and balance before or after the intervention (p >0.05). Conclusion: Cycle ergometer exercise inducing movement of the affected side increases use of the muscles around the affected side knee joint after TKA more than general bicycle exercise and produces better effects for enhancing muscle strength. The application of cycle ergometer exercise inducing movement of the affected side is expected to reduce the patients' unbalanced use during the early postoperative period and help them to quickly return to normal daily life through rapid muscle strength recovery.

The effects of functional movement recovery of physical therapy after ACL reconstruction with MCL injury (물리치료가 슬관절 내측측부인대 손상을 동반한 전방십자인대 재건술 후 운동기능 회복에 미치는 영향)

  • This is the study of the knee joint injured patients at the orthopaedic surgery clinic where is located in Daejon, who has MCL combine injured ACL reconstruction caused by sport activity and accident during the period from Jan. 2001 to Oct. 2001. By comparing with groups between 7th case of I-group for MCL combined stitch and II-group for ACL reconstruction since 6weeks cast. We have been concluded with that following results. 1. Range of motion for the knee was not limited at 5th case(37%) of I-group, 6th case(42%) of II-group and the cases of Flexion deficit less then 10 -degree were 2nd case(13%) of I-group and II-group 1st case(8%) with no extension deficit more then 5 -degree. 2. The level of activity that tells you whether you are capable of exercise for six month after operation. It han been divided by 3 levels. The case of capable of doing low risk exercise(swimming, cycling, etc.) was 5th case of I-group, the case of capable of doing medium risk exercise(jogging, etc.) was 3rd case of I-group and 4th case of II-group and the case of capable of doing high risk exercise(football, etc.) were 3rd case of I-group and 3rd case of II-group. 3. The timing of the return to their job were average 6.4 weeks for I-group and average 22.9 weeks for II-group(P<.05, statistical difference). 4. There was no statistical difference between I-group and II-group for the timing of the return to their job(P>.05). 5. By using VAS to compare them there was no statistical difference between I-group and II-group of clinical results according to Lysholm scale.

Evaluation of Flood Severity Using Bivariate Gumbel Mixed Model (이변량 Gumbel 혼합모형을 이용한 홍수심도 평가)

  • A flood event can be defined by three characteristics; peak discharge, total flood volume, and flood duration, which are correlated each other. However, a conventional flood frequency analysis for the hydrological plan, design, and operation has focused on evaluating only the amount of peak discharge. The interpretation of this univariate flood frequency analysis has a limitation in describing the complex probability behavior of flood events. This study proposed a bivariate flood frequency analysis using a Gumbel mixed model for the flood evaluation. A time series of annual flood events was extracted from observations of inflow to the Soyang River Dam and the Daechung Dam, respectively. The joint probability distribution and return period were derived from the relationship between the amount of peak discharge and the total volume of flood runoff. The applicability of the Gumbel mixed model was tested by comparing the return periods acquired from the proposed bivariate analysis and the conventional univariate analysis.

Study on Evaluating Displacement Tolerance of Sky-bridge in Tall Buildings (고층 스카이브리지의 변위 허용치 산정에 대한 연구)

  • The new method for evaluating the displacement tolerance of sky-bridges with pin-roller type supports was proposed considering both return period of phase difference between connected buildings and geometrical characteristics of skybridge. Because displacement tolerance is relative value, which is most affected by the phase difference of the connected buildings, the dynamic response of these building with time history analysis should be evaluated. However, the initial phase could not be specified, so the result of displacement tolerance would be varied with respect to initial value. Thus, the tolerance can be reasonably evaluated SRSS calculation with design displacements based on statistical approach and of each building. In addition, the geometrical characteristics of sky-bridge should be considered because the transverse displacement of sky-bridge span causes the shear deformation of the bridge and longitudinal displacement tolerance cannot release the shear deformation. Therefore, the some pin-end support in sky-bridge should have longitudinal displacement tolerance to accommodate the shear deformation. By resolving this shear deformation, it is possible not only to accommodate transverse displacement, but also to avoid the complicated joint details such as both pot bearing and guided supports with shear key.

The 4+5th Extensor Compartmental Artery- Pedicled Vascularized Bone Graft in Lichtman Stage III Kienbock's Disease (Lichtman 제 III기 Kienbock 병에서 시행한 제 4+5 신전구획동맥 유경 생골 이식술)

  • Purpose: The purpose of this study was to evaluate the clinical results of the 4+5th extensor compartmental artery pedicled vascularized bone graft in advanced Lichtman stage III Kienbock's disease. Materials and Methods: Eight patients with advanced Lichtman stage III Kienbock's disease who underwent the 4+5th extensor compartmental artery pedicled vascularized bone graft and followed up more than 1 year were analyzed retrospectively. There were 3 men and 5 women. The mean age was 43.6 years old. Two patients were Lichtman stage IIIA and six patients were IIIB. The clinical outcomes were evaluated with radiocarpal joint pain, range of motion, grip strength, carpal-height ratio, radioscaphoid angle, return to daily living activity and/or work. The mean follow up period was 38.5 months (range from 12 to 86 months). Results: On last follow up, the pain was disappeared in 6 patients, and mild occasional pain was remained in 2 patients. Mean radiocarpal joint flexion and extension were 55 degrees and 60 degrees, 87% and 88% of the normal side, respectively. The carpal-height ratio was maintained or improved in 6 patients and slightly decreased in 2 patients. Radioscaphoid angle were improved or maintained in 7 patients. Mean grip strength was 67 lb, 93% of the normal side. All 8 patients returned to daily living activities and/or their previous works. Conclusion: The 4+5th extensor compartmental artery pedicled vascularized bone graft prevented the progression of disease and provided clinical improvement even in advanced Lichtman stage III Kienbock's disease.

Reliability-Based Design Optimization for a Vertical-Type Breakwater with an Emphasis on Sliding, Overturn, and Collapse Failure (직립식 방파제 신뢰성 기반 최적 설계: 활동, 전도, 지반 훼손으로 인한 붕괴 파괴를 중심으로)

  • To promote the application of reliability-based design within the Korean coastal engineering community, the author conducted reliability analyses and optimized the design of a vertical-type breakwater, considering multiple limit states in the seas off of Pusan and Gunsan - two representative ports in Korea. In this process, rather than relying on design waves of a specific return period, the author intentionally avoided such constraints. Instead, the author characterized the uncertainties associated with wave force, lift force, and overturning moment - key factors significantly influencing the integrity of a vertical-type breakwater. This characterization was achieved by employing a probabilistic model derived from the frequency analysis results of long-term in-situ wave data. The limit state of the vertical-type breakwater encompassed sliding, overturning, and collapse failure, with the close interrelation between wave force, lift force, and moment described using the Nataf joint probability distribution. Simulation results indicate, as expected, that considering only sliding failure underestimates the failure probability. Furthermore, it was shown that the failure probability of vertical-type breakwaters cannot be consistently secured using design waves with a specific return period. In contrast, breakwaters optimally designed to meet the reliability index requirement of 𝛽-3.5 to 4 consistently achieve a consistent failure probability across all sea areas.

Arthroscopic Management of Septic Arthritis of the Elbow (화농성 주관절염의 관절경적 처치)

  • Purpose : This study was to evaluate the effectiveness of arthroscopic management for septic elbows. Materials and Methods : The subjects were 7 patients ranged in age from 6 to 32 years. All patients were diagnosed as having septic arthritis of the elbow after arthrocentesis. Emergency arthroscopic lavage, debridement, and selective synovectomy for infective and necrotic tissue were performed. All patients had a follow-up period of more than 12 months by checking leukocyte count, ESR, CRP and range of motion. Results : For children, we found a return to normal of laboratory tests for infection after an average of 8.4 days while for adults, it required 12.3 days. After 12 months all patients showed normal elbow function as well as normal blood tests. Conclusion : We found arthroscopic management for septic arthritis of the elbows made it possible to visualize the pathologic findings directly and protect further articular damage. In conclusion, arthroscopic management is one of the efficient methods for controlling the joint infection.
