  • Background : Complementary and alternative medicine has bee used to cardiovascular diseases. In recent years, many hospitals have tried to integrate complementary and alternative medicine(CAM) with conventional medical approaches for patients with chronic diseases. Recently, the prevalence of the use of traditional Korean medicine(TKM) among patients with chronic diseases, expecially, hemiplegia patients after cerebrovascular accident is increasing in Korea. To date, however, there were only few studies addressing the patients' attitudes, and utilization of TKM, compared to the well-documented escalating use of TKM among consumers in Korea. Objectives : The objective of this study was to analyze the prevalence of TKM use among hemiplegia patients after cerebrovascular accident and to determine what factors affect to use CAM among the patients. The study also aims to provide information on TKM and assist therapy selection among various CAM therapies for hemiplegia patients after cerebrovascular accident within health care system in which both practitioners of TKM and practitioners of modern Western medicine coexisted. Methods : The design of the study was descriptive cross-sectional, and data were collected using a 32-items questionnaire. The subjects were one hundred fifty nine patients with chronic diseases who visited or admitted to health care facilities in a hospital in Seoul Metropolitan city, Korea. Data were analyzed using 'SPSS Statistics 18.0 Network Version(on release 18.0.1 of PASW Statistics)' program. Various statistical methods were used to obtain a profile for participants and the therapies most frequently used by hemiplegia patients of TKM. Logistic regression analysis was employed in order to determine the predicting variables of TKM use. Results : The prevalence of the use of TKM was 51.6%. The most common TKM therapies used by the patients included acupuncture(93.2%), herbal medicine(64.8%), and cupping(37.5%). Results of logistic regression analysis revealed that the variables significantly related with TKM use were gender, marital status, job, No. of visiting health care facilities/week. Conclusions : This study shows that the use of TKM among the hemiplegia patients is relatively high in Korea, this topic should be taken into account in the development of a holistic approach for patients with chronic diseases and an efficient chronic disease management system in Korea.

  • The purpose of this study was to examine the relationship between the self-expression level of dental hygiene students related to communicative competence and their stress during clinical practice and what affected their stress. The subjects in this study were 125 dental hygiene students in W college, on whom a survey was conducted from September 18 through 30, 2006. After the collected data were analyzed with SPSS WIN 10.0 program, the following findings were acquired: 1. When a factor analysis was made to evaluate the self-expression of the students, there appeared three different categories of self-expression: voice/content, facial expression/attitude and sentiment. The three made a 58.1% prediction of their self-expression. As for overall reliability, they turned out highly reliable(Cronbach'a = .881). 2. The dental hygiene students got a mean of 3.58 out of possible five points in self-expression, which indicated that they expressed themselves relatively well. Concerning connections between their general characteristics and self-expression level, those who were inactive during clinical practice got a mean of 3.28, whereas the others who were active got a mean of 3.85. It implied that those who took a more active attitude to clinical practice expressed themselves better(p < .01). The person with whom they found it hard to get along made a statistically significant difference to their self-expression(p < .05). The students who didn't fare well with dental hygienists got the best score(3.70). The second best group(3.53) didn't get along with dentists, followed by assistant nurses(3.46) and patients/caregivers(3.31). As for the impact of the field of dream job, the students who hoped to work or study overseas(4.21) excelled in self-expression those who wanted to be hired in a general hospital, to go onto a school of higher grade and to work in a public dental clinic(p < .05). Among the general characteristics, satisfaction level with major, health status and motivation of choosing dental hygiene made no statistically significant differences to their self-expression. 3. Regarding relations between self-expression level and stress about clinical practice, those who didn't express themselves properly in terms of sentiment scored higher in stress level(3.65). Their stress was statistically significantly different according to self-expression level (p < .05). 4. As for the influence of self-expression and general characteristics on stress with clinical practice, sentiment was selected from among the self-expression categories as a decisive factor to affect stress. Their stress varied statistically significantly with that(p < .05). • The problems of unemployment have been recognized as male problems even though the female unemployment problems are as significant as those of men since IMF. The women who have experienced insecure employment and lower wage than men in the labor market are having more serious economic difficulties after being unemployed. Especially economic problems of the unemployed female headed families are anticipated to be tremendous because the female heads do not have spouses who can act as a buffer seeking or beginning to work to reduce economic hardship. The purpose of this paper is to compare living conditions and utilization of the unemployment programs between male and female heads of the family who are unemployed and discouraged workers. It will examine the functionalism, human capital theory, and feminist perspectives on the cause of female unemployment in order to help understanding of their living situations after being unemployed. Then, it will compare the socio-demographic characteristics, their economic activities before being unemployed, their living conditions and changes in family life after being unemployed, their efforts to overcome unemployment and utilization of various government unemployment programs. It used the national data of National Unemployment Survey of Korean Institute of Health and Social Affairs conducted in 1998 and analyzed 283 female and 1,528 male family heads who are unemployed and discouraged workers. It reveals that the female family heads experienced lower economic status in the labor market before being unemployed, are having more serious economic hardship because of it, utilize more of the government programs such as public work and public assistance than the male family heads but less of the public loans for the unemployed. It recommends expansion of public work and public assistance, more vigorous advertisement of various unemployment programs, and active training of job creation technique for female family heads.

  • In a period of rapid change, transition and re-definition of care concept, this study reviewed social policies on care providers in the UK. In the face of care crisis, the British government has made a radical reform of the care system and enacted new legislations. In the UK social policy, care providers are classified into carers and care workers. Carers mean informal caregivers and care workers are those who are paid for providing care as part of a contract of employment. Recently, the United Kingdom has given carers recognition and reward. To enhance the status and right of carers, the Carers (Recognition and Services) Act 1995, the strategy document Caring about Carers 1999, Carers and Disabled Children Act 2000, and Carers (Equal Opportunities) Act 2004 have been enacted. At the heart of the policy for carers is the idea of active citizenship, carers-friendly employment and work-life balance etc. In case of paid care worker, government's focus seems to be on quality of employment. The government has established a new national infrastructure for quality. The five national bodies founded on Care Standards Act 2000 has been established. The UK government has realized care work would play an important part in job creation strategy. In this article, we have presented several criticisms and issues of current care policy in UK.

  • Korea has made a rapid economic development since the last three decades. This has helped Korea narrow the gap in health service differences in resource availability and in quality of care. However urban and rural differentials are still remarkable. This study has maintained that health status of rural residents is inferior to that of urban dwellers. Therefore, this study was carried out to develop policy measures for improving health services in rural areas. In order to achieve the objective of this study the authors collaborated closely and made field visits, interviews and conducted an extensive literature review regarding rural health services. The following policy options are recommended as a summary ; First, the quality of rural health personnel is a single most important factor influencing the level of rural health services. An innovative program for public health doctors to the internship and/or residency training program such as specialty board program of family medicine. Second, dissatisfaction regarding employment of public health doctors is problematic. More rational employment and deployment programs are needed to meet their personal desire. One way to do this is to make it wide open and competitive. Third this study shows how to increase physician productivity in the rural public health sector. Incentive system needs to be elaborated for the career development of rural health workers. University linked job opportunity as clinical professor is an example. Fourth, without straightening the function of health centers and subcenters, the future of rural health services is doomed to failure. Straightening primary health care is one way to enrich the program of public health facilities and reactivating the operation of health center/hospital is another. A close linkage of public facilities with private hospitals is a minimum requirement for the operation of health delivery system within a health district. Fifth, some measures are urgently required to enhance hospital services in medically underserved areas. Financial subsidy, tax exemption, long-term public loans and higher priority of health manpower deployment are some of them. Sixth, new health programs should be in tiated to meet changing needs of peoples in rural areas. Home health care program, hospice program, nursing home, residential program for the elderly are recommended.
