The Effect of Clamping Angle of a Locker on the Clamping Force of the Wedge Type Rail Clamp (Locker 의 물림각이 쐐기형 레일클램프의 압착력에 미치는 영향 평가)

  • The wedge type rail clamp has the operating mechanism: First, the jaw pad clamps a rail with small clamping force. Next as the wind speed increases, the clamping force of the Jaw pad Is Increased by the wedge. The initial clamping force of a jaw pad was determined by the clamping angle of a locker. In this study, we carried out the finite element analysis to evaluate the relationship between the clamping angle of a locker and the clamping force of a jaw pad with respect to the design wind speed, such as 2, 4, 6, 8, and 10m/s, we adopted the wedge type rail clamp fur 50tons class container crane with the wedge angle of $10^{\circ}$.

  • The wedge type rail clamp has the operating mechanism: First, the jaw pad clamps a rail with small clamping force Next as the wind speed increases, the clamping force of the jaw pad is increased by the wedge. In order to design the wedge type rail clamp, we need to determine the proper wedge angle to minimize the sliding distance of a roller and the proper clamping angle of a locker to generate the initial clamping force of a jaw pad. The researches for the proper wedge angle have conducted, and in this study we conducted the investigation to determine the proper clamping angle of a locker in the rail clamp with wedge angle of $10^{\circ}$. Because the initial damping force of the jaw pad was determined by the clamping angle of the locker, in order to carry out the clamping force of a jaw pad, we measured the locking force applied to a locker with respect to the clamping angle of a locker, such as $3^{\circ},\;4^{\circ},\;5^{\circ},\;6^{\circ},\;$ using a pressure gauge, and compared the results with the FEA results.

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a disease characterized by jaw necrosis and delayed wound healing in patients who had received bisphosphonates. Buccal fat pad (BFP) can be used as a pedicled flap in the posterior region of the oral cavity. BFP pedicle flap needs simple surgical technique and it shows less donor site morbidity and aesthetic problem than other vascularized flap. BFP pedicled flap was fed by 3 arteries-facial, internal maxillary, and transverse facial artery. Osteomyelitis was generally related with poor blood supply. Thus, rich blood supply of BFP pedicle flap can have a potential advantage to BRONJ patients. In this case report, we presented 3 BRONJ patients treated by BFP pedicle flap after sequestrectomy.

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is defined as exposed necrotic bone without evidence of healing for at least 8 weeks in the maxillofacial area in a patient with history of bisphosphonate use. Obtaining complete coverage of the hard tissue by soft tissue in BRONJ patients is especially important. Therefore, managing the mucosa is one of the key factors in a successful outcome, but this is especially hard to achieve in BRONJ patients. Various applications of buccal fat pad in oral reconstruction-including the closure of surgical defects following tumor excision, repair of surgical defects following the excision of leukoplakia and submucous fibrosis, closure of primary and secondary palatal clefts, coverage of maxillary and mandibular bone grafts, and lining of sinus surface of maxillary sinus bone graft in sinus lift procedures for maxillary augmentation-have been studied. Eliminating all potential sites of infection and post-operative infection control is crucial in BRONJ. We present a case using the buccal fat pad pedicle for a stage 3 BRONJ defect. Uneventful total epithelialization of the buccal fat pad regardless of size was noted. In summary, the buccal fat pad has versatile application and various recipient sites for surgical utilization. It is an easy technique, with promising overall success rates. With careful selection and handling, buccal fat graft can resolve problems with soft tissue coverage in stage 2 or 3 BRONJ patients.

  • Many congenital and acquired defects occur in the maxillofacial area. The buccal fat pad flap (BFP) is a simple and reliable flap for the treatment of many of these defects because of its rich blood supply and location, which is close to the location of various intraoral defects. In this article, we have reviewed BFP and the associated anatomical background, surgical techniques, and clinical applications. The surgical procedure is simple and has shown a high success rate in various clinical applications (approximately 90%), including the closure of oroantral fistula, correction of congenital defect, treatment of jaw bone necrosis, and reconstruction of tumor defects. The control of etiologic factors, size of defect, anatomical location of defect, and general condition of patient could influence the prognosis after grafting. In conclusion, BFP is a reliable flap that can be applied to various clinical situations.

  • In this paper, we design a wedge type rail clamp which can protect container crane from a sudden strong blast with constant clamping force regardless of the operating period. When we design wedge type rail clamp, it is important to determine the angle of wedge and analyze a contact condition of roller and wedge so that we might develop a wedge type rail clamp for parking devices of port cargo working system with variable capacity. Therefore, this paper suggests a process to decide wedge angles within feasible range which could be obtained using load analysis and FEA of wedge type rail clamp

  • Osteoradionecrosis is one of the most serious complications of patients receiving radiation therapy. It is characterized by hypovascularity, hypocellularity, and hypoxia-inducing necrosis of bone and soft tissue following delayed healing. In this case, a 72-year-old man was referred to the Department of Oral and Maxillofacial Surgery complaining of trismus following extraction three months before first visit. He had a history of right tonsillectomy, radical neck dissection and radiotherapy performed due to right tonsillar cancer seven years prior. After the diagnosis of osteoradionecrosis on right mandibular body and angle, conservative antibiotic therapy was used first, but an orocutaneous fistula gradually formed, and extensive bony destruction and sequestrum were observed. Sequestrectomy, free particulated iliac bone and umbilical fat pad graft were performed via a submandibular approach under general anesthesia. Preoperative regular exams and delicate wound care led to secondary healing of the wound without vascularized free flap reconstruction.

A Histochemical Study of the Masseter Muscle Fibers of White Rat with Unilateral Edentulous Jaw (편측 무치악이 백서 교근 근섭유에 미치는 영향에 관한 조직화학적 연구)

  • The study of the muscle fiver composition and the muscle fiver type conversion during unilateral edentulous condition was undertaken in the rostral superficial masseter muscle of the whiter rat. 16 4-week-old male white rats weighting approximately 130gm that crowns of left upper and lower molare were cut intentionally, were divided into 4 groups (one control group and 3 experimental groups). After experimental groups were sacrificed by etherization in 6 days($E_1$), 18 days($E_2$) and 36 days($E_3$) separately, samples of the rostral superficial masseter muscle were obtained bilaterally and the proportion of type I, type IIA, type IIB, and type IIC fibers was determined and counted according to their histochemical activity of myosin ATPase (at pH 9.4, pH 4.6, and pH 4.2)and PAD staining. The obtained results were as follows : 1. The rostral superficial masseter muscle of the white rat contained approximately 47.5% type I fiber and 52.5% type II fiber. 2. Type I/ Type II ratio of molar-present side was increased significantly in the group E2 (18 days group) 3. Type IIA fiber was increased at molar-present side and decreased at molar-absent side in experimental groups.

  • The clamping force of a jaw pad is determined by the displacements of main part when two lockers are locked, after the clamping angle of a locker was set up in the wedge type rail clamp for a container crane. In this time, if the resistance of wedge frame generates due to several factors, the clamping angle of a locker to display the initial clamping force will be changed because of the reduction of displacement of extension bar. This resistance is determined by the eccentric distance between the roller and the wedge, and by the gap between the wedge frame and outer frame. In this study we measured the tensile force of both extension bar through the performance test of the prototype rail clamp in order to evaluate the effect of the resistance of wedge frame on the clamping force of the wedge type rail clamp.

  • The rail clamp is the device to prevent the crane slips along a rail from the wind blast as well as to locate a container crane in the set position during an operating mode. In this study we conduct the research for determining the proper position of supporter to prevent the overload applied to the rail clamp with respect to the wedge angle in the wedge type rail clamp. The friction force between the jaw pad and the rail to prevent that the crane slips along a rail, when the wind blows, is generated fly the rail-directional wind load. Accordingly the proper position of the supporter to prevent the overload is determined fly analyzing the forces applied to the rail clamp in the wedge working stage. In order to analyze the effect of the wedge angle on the position of supporter, 5-kinds of wedge angles, such as 2, 4, 6, 8, $10^{\circ}$, were adapted as the design parameter, and the wind speed of 40m/s was adapted as the design wind speed criteria.
