A Case of New Surgical Correction of Angular Limb Deformities Using One Screw Implant & Periosteal Transection in a Thoroughbred Foal (Thoroughbred 망아지에서 단일나사못 장착 및 골막박리를 이용한 지세교정술)

  • Angular limb deformities (ALD) are common in foals. A 30-days-old Thoroughbred foal was presented for the evaluation of severe ALD of the both forelimbs. On radiographic examination, both distal radiuses were diagnosed as valgus angular limb deformities. But the degree of deviation of right forelimb was so severe that we tried to correct one after the other. We tried new surgical correction method combination of one screw implant on medial aspect for growth retardation and periosteal transection on lateral aspect of the right forelimb. 40 days later, successfully corrected and then removed the screw. After the right forelimb correction, the periosteal transection on left forelimb was performed. We did the inhalation anesthesia using isoflurane. There were no complications such as fibrosis over the screw heads, and overcorrection that produces an opposing deformity identified. These results suggest that combination of one screw implant and periosteal transection technique is able to be a safe and effective method to correct severe ALD in the foal.

Atropine Induced Paradoxical Atrioventricular Conduction Block in a Jindo Dog (진도종 개에서 아트로핀에 의해 발생한 역설적 방실 전도 차단)

  • A 6-month-old intact male Jindo dog was underwent surgery for hip fracture caused by hit by a car. Routine laboratory tests performed prior to surgery found no significant abnormalities, which might increase risks associated with general anesthesia. The dog was premedicated with atropine, induced general anesthesia with thiopental sodium and maintained with isoflurane. Forty minutes after surgery, the dog was suddenly bradycardic. Atropine (18 ug/kg) was slowly infused intravenously to normalize heart rate. However, paradoxically the dog showed slower heart rate with intermittent atrioventricular block ($2^{nd}$ degree type I) after atropine infusion. The dog's rhythm was returned to normal rate 7 minutes after ephedrine was infused. This is a rare case of paradoxical atrioventricular block induced by high dose of atropine in a dog.

  • Background and Objectives : Sphincteric function of the larynx, essential to lower airway protection, is most efficiently achieved through strong reflex adduction by both vocal cords. We hypothesize that central facilitation is an essential component of a bilateral adductor reflex and that its disturbance could result in weakened sphincteric closure. Materials and Method : Seven adult 20kg dogs underwent evoked response laryngeal electromyoraphy under 0.5 to 1.0 MAC isoflurane anesthesia. The internal branch of superior laryngeal nerve was stimulated through bipolar platinum-iridium electrodes and recording electrodes were positioned in the ipsilateral and contralateral thyroaryteonoid muscles. Results : Consistent threshold responses were obtained ipsilaterally under all anesthetic levels. However, contralateral reflex responses disappeared as anesthetic levels approached 1.0 MAC. Additionally, at 0.5 MAC, late responses (R2) were detected in one animal. Conclusion : Alteration of central facilitation by deepening anesthesia abolishes the crossed adductor reflex, predisposing to a weakened glottic closure response. Precise understanding of this effect may improve the prevention of aspiration in patients emerging from prolonged sedation or under heavy psychotropic control.

Diaphragmatic Hernia in a Two-month Old Cat

  • A 2 months old female Korean domestic shorthair cat weighing 1.2 kg was come to hospital because of respiratory discomfort and lethargy. Heart sounds was more intense and clear on the right side than the left. On radiographic views, loss of the normal diaphragm line, undistinguishable shadow of heart, shadow of gas-containing intestines could be observed in thoracic cavity. Diaphragmatic herniorrhaphy was performed by using propofol 8 mg/kg IV and isoflurane without any complication. On 7th day after the operation, almost all the clinical signs and radiographs including diaphragmatic line, cardiac silhouette, liver and small intestines were turned to normal.

  • A 10-year-old, female mixed breed dog and a 3-year-old, female chihuahua which had swelling below lower eyelid were refered to Veterinary Medical Teaching Hospital, Seoul National University. The plain maxillary oblique view revealed radiolucency at periapical area in maxillary fourth premolar. The 2 cases were diagnosed as periapical abscess by clinical sign and radiographs, and apicoectomies were performed for resetting the lesions and preserving the teeth. Under general anesthesia with isoflurane, periapical abscess was resected by burs and the defect was filled with zinc phosphate cement. Radiograph was perfomed at 1 week and 1 year after apicoectomy and there was no evidence of recurrence.

  • Authors introduce a case of successful orthopedic surgery for humeral fracture of Common buzzard (Buteo buteo). The bird, juvenile male, was rescued due to illegal gun shot in Yeoncheon-Gun, Kyonggi-Do. The external skeletal fixation (ESF) with intramedullary pinning (Tie-in method) using Imex clamps was chosen for this fracture case. The common buzzard was anesthetized with hand made face mask and Isoflurane. In particular, the rescued bird was needed rehabilitating procedures, such as flight training or physical exercise for their survivals in wild habitats. The injured buzzard is successfully recovered without any damage to ligaments and nerves after 6 weeks.

  • To evaluate method for monitoring anesthetic depth with quantitative electroencephalography (q-EEG), we recorded processed EEG (raw EEG) and pain score till 100 minutes in beagle dogs anesthetized for 60 minutes with propofol (n = 5, PRO group), isoflurane (n = 5, ISO group) and propofol-ketaminefentanyl (n = 5, PFK group). Raw EEG was converted into 95% spectral edge frequency (SEF) by fast Fourier transformation (FFT) method. We investigated anesthetic depth by comparing relationship (Pearson's correlation) between q-EEG (95% SEF) and pain score. Pearson's correlation coefficients are +0.2372 (p = 0.0494, PRO group), +0.79506 (p < 0.001, ISO group) and +0.49903 (p = 0.0039, PFK group).

  • Under General anesthesia with isoflurane, we insert a chicken's esophageal catheter into the near the left atrium. 1MHz radio wave was added to electrocardiogram electrodes of the esophagus, and the change of impedance (phase) was obtained by amplitude synchronous detection technique. At the same time, a thin tube is surgically inserted into the axillary artery to continuously measure blood pressure. The correlation between impedance (phase) and blood pressure was obtained. Both showed a very high correlation (R2=0.9665). It was also observed the waveform flowing from the left atrium into the left ventricle. When an individual infected with the avian influenza virus develops, the cytokine storms lead to hypotension earlier than the test for antigen-antibody reaction. In order to detect this, in the future, this impedance technique will be useful for screening individuals infected with avian influenza virus by measuring the blood pressure of chickens in cages in a non-contact manner using microwaves.

  • Moyamoya disease is a disorder in which certain arteries in the brain are constricted. Blood flow can be blocked by the constriction and blood clots. The patients frequently experience transient ischemic attacks (TIA), cerebral hemorrhage, or may not experience any symptoms at all. It is reported that they have a higher risk of recurrent stroke and a distinct underlying pathophysiology. A 3-year-8-month old boy with moyamoya disease experienced cerebral infarctions five times, and he underwent a cerebrovascular anastomosis surgery four years ago. He showed swallow disturbance, general delayed development, hemiplegia, and strabismus. Also he had hypocalcified teeth with or without multiple caries lesions in all dentitions. Dental treatment under general anesthesia using sevoflurane was performed due to his lack of cooperation. Moyamoya disease is associated with various medical conditions requiring a thoughtful deliberation and a careful examination before and during dental treatment. Pain and anxiety control during dental treatment is important because hyperventilation induced by crying has been seen to trigger TIA. Both isoflurane and sevoflurane are commonly used in patients with MMD, but dynamic autoregulation is better preserved during sevoflurane than isoflurane anesthesia. So sevoflurance general anesthesia may be recommendable to manage dental patients having multiple caries with moyamoya disease.
