Intracavitary Carboplatin Chemotherapy in a Malignant Mesothelioma Dog (개의 악성중피종의 흉강내 Carboplatin 투여 증례)

  • A 10-year-old castrated male Siba dog was presented for signs referable to pleural effusion associated with neoplasm of the thoracic cavity. The pleural effusion fluid consisted of blood and tumor cells by thoracocentesis. Histopathological examination of the sedimentary tumor cells revealed malignant mesothelioma. Intracavitary carboplatin was administered at 300 mg/$m^2$ every 5 weeks for 3 treatment and pleural effusion was disappeared after 3 treatments. The dog had recurrence of pleural effusion at 515 days but intracavitary carboplatin chemotherapy had no effect. It would be thought that the intracavitary carboplatin treatment was quite a useful method to control a canine malignant mesothelioma with minimal toxicity.

  • A 3-year-old male Tosa was presented the severe dyspnea, emaciation and dehy dration. By echocardiograpy, right ventricle was found to be a mobile mass dynamically occluding the right ostium atrioventriculare in the systolic phase. At necropsy, 14 days after ultrasonography multiple tumor masses of various size were observed in the heart base, right ventricular lumen, myocardium, lung and liver. Histopathologically, the tumor cells, arranged in sheets or nests, were diagnosed as metastatic intracavitary cardiac aortic body tumor

  • This study, involving thirty-two patients with carcinoma of uterine cervix treated by high dose rate intracavitary irradiation using a remotely controlled afterloading system, compares the doss at point A and 8 with the Curie-minutes prescription. A linear least-square regression analysis was used to compare the two sets of date. Correlation coefficients between doses at points A and B and the Ci-min prescription are 0.92 (p<0.001) and 0.90 (p<0.001), respectively, and linear relationship is observed between these two system. The limitation and significance of the comparison of the two approaches to intracavitary dosimetry is discussed.

Treatment of Malignant Effusion with Intracavitary OK-432 Chemical Pleurodesis (악성흡수에 OK-432를 이용한 흉막유착술)

  • To determine the efficacy of OK-432 as pleural sclerosant, we examined the outcomes in 81 patients (age : 27 to 82 years) with malignant pleural effusion and the outcomes in 64 patients ecieving OK-432 3-10KE(1 Klinische Einheit unit) through a chest tube for malignant pleural effusions. Of 81 patients with malignant pleural effusion, 40 patients had lung cancer. Lung cancer is the most frequent cause of malignant pleural effusion in men and women, in which 57 oyo of it was adenocarcinoma. Eighty seven percent of patient had respiratory symptom. Of the 64 patients with intracavitary injection of OK-432 for malignant pleural effusion, 59 patients had a complete short-term response (no fluid reaccumulation during 1 month after intracavitary injection of OK-432). Five patiens of the non-responders had partial control of effusion, with improvement in respiratory symptoms and these patients underwent thoracentesis. Of the 51 patients who survived longer than 1 month, 48 patients did not have re- accumulation of the fluid during follow up. Fever after intracavitary injection of OK-432 was a majors side effect although but that was easily controlled with non-steroidal anti inflammatory drug therapy, Thus the efficacy of intracavitary OK-4)2 injection for malignant pleural effusion was very helpful.

Result of Radiation Therapy of the Cervix Cancer Stage IIIB (자궁경부암 IIIB 기의 방사선치료 성적)

  • From September 1985 through September 1989,56 patients with stage IIIB carcinoma of the cervix were treated with radiation therapy with curative aim. The overall survival at 5 year was $38{\%}$. The survival rate was better for patients treated with combined external radiotherapy and high dose rate intracavitary radiotherapy than with external radiotherapy alone. No significant survival difference was observed between the unilateral and bilateral parametrial extension of the tumor Seventeen patients experienced recurrence within the irradiated field with a loco-regional recurrence rate of $30{\%}$. Ten patients had complications ($18{\%}$). The complications were mild in three, moderate in four, and severe in three patients. A study was made on the relationship between the fraction numbers of intracavitary radiotherapy, vaginal packing and the complication rate, respectively. In this analysis author observed that the significant treatment factor influencing the survival of cervical cancer was the use of intracavitary radiation, and meticulous vaginal packing could decrease the late complication rate of radiotherapy of cervical cancer.

  • The intracavitary cones were designed which were made of stainless steel and have scratched inside cone to be generated electron scatter and designed to be attached easily to the LINAC collimator and controlled cones length to be contacted smoothly between the patient and the cone tip. Two types of intracavitary cones were designed. One is the straight end cones with circular opening on the distal end and the other is 30 degree beveled end cones with elliptical opening on the distal end. Each type of intracavitary cone ranged in daimeter from 2.5 cm to 3.5 cm and required a separate set of lower trimmer annulias cone diameter. The film phantom was designed with an internal cassette that accurately aligned the film edge with the film phantom surface. Film optical density data were measured by photodensitometer(Wellhofer 700i) Dosimetry measurements were made to commission the LINAC for 6 - 20 MeV electron using the intracavitary cones. Isodose curves were measured for all energy and cones combinations. Output is defined as the maximum dose per MU along the clinical central axis in water at 113 cm SSD. Calibration output, defined to be the output for the 15cm$\times$15cm diameter straight cone, was adjusted to 1.00 cGy/MU at each energy according to the TG-21 protocol.

  • The most common symptom associated with an pulmonary aspergilloma is hemoptysis, with estimates of frequency ranging from 50 to 85 percent of patients. Hemoptysis may be infrequent and minimal in amount or it may be severe with a fatal outcome. The major options available for the treatment of pulmonary aspergilloma include sugical resection of the lesion, a number of medical therapies, or simple observation of the patient for a time. Surgery is the treatment of choice but it is not feasible in some patients who have diffuse or advanced pulmonary disease that makes them poor candidates for thoracotomy. As an alternative to it, some categories of therapy including bronchial artery embolization and parenteral or endobronchial administration of antifungal drugs were tried without remarkable success. But percutaneous instillation of intracavitary amphotericin B for symptomatic aspergilloma has been reported with better result. The authors present a case of percutaneous intracavitary instillation of amphotericin B for the treatment of pulmonary aspergilloma and its successful result for the repetitive hemoptysis.

Intracavitary Radiation Therapy for Recurrent Cystic Brain Tumors with Holmium-166-Chico : A Pilot Study

  • Objective : Intracavitary injection of beta-emitting radiation source for control of cystic tumors has been tried with a benefit of localized internal radiation. The authors treated cystic brain tumor patients with Holmium-166-chitosan complex (Ho-166-chico), composed of a beta-emitting radionuclide Holmium-166 and biodegradable chit polymer, and evaluated the safety and effective measurement for response. Methods : Twenty-two patients with recurrent cystic brain tumor and/or located in a deep or eloquent area were enrolled in this pilot study. The cyst volume and wall thickness were determined on CT or MRI to assess radiological response. The activity of Ho-166-chico injected via Ommaya reservoir was prescribed to be 10-25 Gy to the cyst wall in a depth of 4 mm. Results : There was neither complications related to systemic absorption nor leakage of Ho-166-chico in all 22 patients. But, two cases of oculomotor paresis were observed in patients with recurrent craniopharyngioma. Radiological response was seen in 14 of 20 available follow-up images (70%). Seven patients of 'evident' radiological response experienced more than 25% decrease of both cyst volume and wall thickness. Another 7 patients with 'suggestive' response showed decrease of cyst volume without definitive change of the wall thickness or vice versa. All patients with benign tumors or low grade gliomas experienced symptomatic improvement. Conclusion : Ho-166-chico intracavitary radiation therapy for cystic tumor is a safe method of palliation without serious complications. The determination of both minimal effective dosage and time interval of repeated injection through phase 1 trial could improve the results in the future.
