A 2-month-old female Abyssinian cat was presented with a severe ascites, cyanosis, and exercise intolerance. Diagnostic studies revealed V/VI holosystolic murmur, sinus tachycardia, generalized cardiomegaly with marked left atrial enlargement and shunt flow between left and right ventricles. Doppler study showed bi-directional shunts in rest and right-to-left shunt after exercise. Based on clinical signs and diagnostic findings, the cat was diagnosed as a reversed ventricular septal defect. The cat was treated with furosemide, nitroglycerine, dobutamine and oxygen supplement. Despite initial improvement of clinical signs after initiation of medical treatment, the cat died of sudden cardiac arrest. Necropsy revealed a perimembranous ventricular septal defect.
A 2-year-old female Pekingese dog was presented with primary complaints including exercise intolerance and neurological sign associated with hepatic encephalopathy. The major findings in clinical examination included an intermittent seizure, a slow heart rate with pulse deficit, leukocytosis and anemia in hemogram, elevated pre- and post-prandial serum bile acid and hepatic enzymes, hypoproteinemia, coagulopathy, ammonium urate crystaluria and bilirubinuria. Diagnostic tests revealed an intrahepatic portosystemic shunt complicated with a second degree atrioventricular block and QT prolongation. The case was successfully treated with a transvenous coil embolization. Clinical signs were gradually improved and cardiac bradyarrhythmia disappeared. This case is a rare case of intrahepatic portosystemic shunts complicated with a cardiac bradyarrhythmia in a small breed dog fixed by a transvenous coil embolization.
A 17-month-old intact male Cocker Spaniel was presented with primary complaints of severe ascites, exercise intolerance, and diarrhea. Diagnostic studies revealed tricuspid and pulmonic regurgitation on phonocardiogram, right ventricular enlargement on the electrocardiogram, typical right cardiac enlargement signs on the thoracic radiography and tricuspid valve malformation and marked enlargement of the right atrium and right ventricle on the echocardiography and tricuspid and pulmonary regurgitation on the color spectral echocardiography, suggesting tricuspid valve dysplasia and pulmonary hypertension. Using angiography and cardiac catheterization, pulmonary hypertension was ruled out. Further echocardiographic study revealed membranous valvular structures cranial to pulmonary annulus causing pulmonary regurgitation. Based on these findings on the diagnostic investigation, the case was diagnosed as tricuspid valve dysplasia complicated with pulmonic regurgitation. The dog was medically managed with furosemide, enalapril, nitroglycerine transdermal patch and pimobendan after the ascitic fluid removal.
An 8-year-old spayed female Maltese (2.5 kg of body weight) presented with the primary complaint of loud heart murmur and exercise intolerance. Diagnostic imaging revealed severe pulmonic stenosis (peak velocity 5.2 m/s) with right ventricular hypertrophy. The dog revisited after 2 years, at which time, diagnostic imaging revealed severe biventricular hypertrophy, dynamic left ventricular outflow tract obstruction, left atrial dilation and pulmonary hypertension with worsened pre-existing pulmonic stenosis. Postmortem investigation revealed hypertrophic cardiomyopathy and regional myocardial infarction. The case was diagnosed as hypertrophic cardiomyopathy secondary to severe right and left ventricular outflow tract obstruction.
A 6-month-old intact male Maltese dog (2.5 kg of body weight) was presented with the primary complaint of heart murmur and exercise intolerance. Diagnostic studies revealed severe pulmonic stenosis. Since the pressure gradient in stenotic pulmonic valve area was higher than 200 mmHg (maximum flow velocity = 7.1 m/s), the dog was underwent balloon valvuloplasty. With this procedure, the peak pulmonic velocity was reduced to 3.0 m/s (PG = 36 mmHg). Periodic re-evaluations performed in monthly interval revealed no further deterioration even after 4 month of procedure. This is the first case report of pulmonic stenosis fixed by non-surgical balloon valvuloplasty in Korea.
The prevalence of obesity in Korean children is estimated to be around 10%, and has increased significantly over the past 20 years. Metabolic syndrome, which includes central obesity, glucose intolerance, insulin resistance, dyslipidemia, and hypertension, is a well documented risk factor for cardiovascular diseases, but there is no general consensus concerning its definition for children. In this study, ablut 30-40% of overweight or obese children had metabolic syndrome compared to less than 10% in the general population. Dyslipidemia was the most common component and hyperglycemia was the least common component of metabolic syndrome. If the prevalence of childhood obesity continues to increase, it is highly likely that the prevalence of the metabolic syndrome will also increase. These findings emphasize the importance of preventing obesity in addition to the diagnosis and management of metabolic syndrome in Korean adolescents.
Postural orthostatic tachycardia syndrome (POTS) refers to the presence of orthostatic intolerance with a heart rate (HR) increment of 30 beats per minute (bpm) or an absolute HR of 120 bpm or more. There are sporadic reports of the autonomic nervous system dysfunction in migraine and fibromyalgia. We report a case of POTS associated with migraine and fibromyalgia. The patient was managed with multidisciplinary therapies involving medication, education, and exercise which resulted in symptomatic improvement. We also review the literature on the association between POTS, migraine, and fibromyalgia.
A 7-month-old 8.2 kg female Siberian Husky with history of exercise intolerance, delayed growth, and tachypnea was presented to Veterinary Medical Teaching Hospital, Kyungpook National University. A holosystolic murmur was identified in cardiac ausculation. In electrocardiography, thoracic radiographs, and ultrasonography, a right axis deviation, enlarged right ventricle, stenotic main pulmonary artery, large ventricular septal defect, marked right ventricle hypertrophy, and overriding aorta with dilation were identified. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect and the blood flow streams converging from the right and left ventricles into the aorta. The dog was diagnosed as tetralogy of Fallot. The current state of the dog is well-tolerated without any specific medication.
A 4-year-old, American Paint horse gelding with recurrent episodes of exertional rhabdomyolysis was diagnosed with equine polysaccharide storage myopathy (EPSSM). The common clinical signs were muscle weakness, loss of muscle mass, exercise intolerance, difficulty holding up limbs for farrier, sensitivity of grooming, cramping with reluctance to move and the inability to rise. Through histological examination of skeletal muscle specimens, this case was confirmed of EPSSM.
Subaortic stenosis in a 2.9kg, 3-month-old pug dog and a 11 kg, 4-month-old Rott -weiler dog showing signs of dyspnea, tachypnea, anorexia, exercise intolerance and depression was diagnosed with electrocardiography, thoracic radiography and ultrasonographic evaluation. In electrocardiography, PR interval widening, R wave amplitude increase in lead II in case 2, ST segment depression and left axis deviation in case 1 and 2 were identified. In lateral view, left ventricular enlargement, secondary dorsal deviation of trachea and post-stenotic dilation of aorta were seen. Also, edematous pulmonary changes were shown in peri-hilar area. In ultrasonographic examination, left ventricular dilatation, pericardial effusion and subaortic valvular ridge were seen with real time B-mode in right parasternal long axis view. Aortic valve regurgitation was identified in color-Doppler mode.
