The Lateral Middle Phalangeal Finger Flap for Pulp and Palmar Defects of the Finger (수지 첨부 및 수장부 연부 조직 결손에서 시행한 외측 중위 수지 피판술)

  • Purpose: The aim of this study was to evaluate the efficacy of lateral middle phalangeal finger flap for pulp and palmar defect of the finger. Materials and methods: We performed the lateral middle phalangeal finger flap in thumb pulp defect 4 cases and the palmar defect of other finger 3 cases. Mean age was 38(25-53) years old and there were male 6 cases and female 1 case. Sensate flap was performed in 4 cases of thumb pulp defects. Mean follow-up period was 14(7-22) months. Results: All flaps were survived. Mean static two-pint discrimination of sensate flap 4 cases was 8(6-10) mm. The sensation of donor finger tip was normal in all cases. Limitation of range of motion of the donor fingers was absent. Patients complained of transient cold intolerance 1 month after surgery but didn't complain of that in all cases at last follow-up. Conclusions: The advantages of the lateral middle phalangeal finger flap are the preservation of the ipsilateral palmar digital nerve, good sensory reconstruction of the fingertip, well maintained donor finger mobility with minimal exposure of the extensor tendon, cosmetically good appearances of donor finger, and easy raising as a large flap. So we suggest that this flap is versatile for reconstructing of relatively large pulp defect of the thumb and the palmar defects involving the joint of finger.

Study on Characteristics of Lactobacillus Isolated from Hen′s Cecum (산란계 맹장 유산균의 특성에 관한 연구)

  • Preset study was carried out to evaluate characteristics of lactic acid producing bacteria(LAB) in hen's cecum as probiotics value. Distribution of LAB in intestinal tracts was investigated using 5∼25 weeks - old hens. So, 12 strains to LAB with different morphology were isolated purely. Acid tolerance of LAB tested at pH 1, 2, 3, and 4, and bile resistant also tested at 0, 0.3% and 0.5% bile salt concentration. Growth pattern of LAB observed to 60h. All strains of cecal LAB couldn't survive at pH 1, and decreased linearly survival colony after incubation at pH 2 although some strains could survive for 2h. Most of LAB maintained constant number at pH 3 and 4. The bacterial action could increase linearly at 0% bile salt concentration in all of tested strains. However, only one strain could multiply at 0.3% bile salt, others were influenced by bile salt. That tendency was similar at 0.5% bile salt. Growth was peaked at 12 to 18 h after innoculation. After peak, the decreasing pattern of colony was different to strains which some strains decreased rapidly or maintained for long time. The LAB of hen's cecum was similar to intolerance acidity, but different to resistant to bile salt and growth pattern by strain. So, we choose three strains which have probiocs value, and identified as Lactobacillus amylovorus LLA7, Lactobacillus crispatus LLA9 and Lactobacillus vaginalis LLA11.

Tricuspid valve dysplasia(TVD) in an American cocker spaniel dog (American cocker spaniel dog에서 발생한 삼첨판 이형성 증례)

  • A 2-year-old, female, American cocker spaniel dog presented for a 1-year history of severe ascites, exercise intolerance, tachypnea. At that time, she was in an emergency state. First, the dog was stabilized with oxygen therapy. A diagnosis of cardiac problem was made from history, auscultation, radiograph, ECG, and echocardiography. Jugular pulsation was palpated and a harsh, systolic murmur of tricuspid regurgitation was prominent at the right cardiac apex. Tricuspid valve dysplasia (TVD) was confirmed with echocardiography, accompanying enormous myocardial hypertrophy. The clinical signs had been improved for 8 months with careful therapy and periodic abdominocentesis, and ascites was well controlled. The situation, however, became worse quickly in a week because the client did not follow our management schedule. Finally, she died due to dyspnea and shock. After the spontaneous death, necropsy and histopathological examination were performed and when we opened the thorax, a significantly large heart was observed. On histopathological findings, grossly myocardium appeared pale initially, then progressed to yellow and white. Microscopically, there was an extensive hemorrhage along with loss of myocardial striations. Interstitial fibrosis and various degenerative alterations in myocytes were also present.

Application of new external total ring prostheses made by drip chamber of intravenous administration set in normal dogs (정상 견에서 수액세트 점적통을 이용한 새로운 기관 외부 보철링의 적용)

  • The purpose of this study was to compare easiness of application of polypropylene external total ring prosthesis (PPTRP, Group A) with that of polyvinylchloride external total ring prosthesis (PVCTRP, Group B), which were used for surgical correction of tracheal collapse in dogs. PPTRP was made from 3 ml syringe and PVCTRP from the drip chamber of intravenous administration set. Prostheses of group A (n=5) and B (n=5) were placed to cervical trachea in clinically normal 10 dogs, respectively weighing between 4 kg and 6 kg. There were mild coughing and swelling in one to three dogs of both groups for 3 days after surgery. No exercise intolerance was observed in both groups after surgery. There were no different results of clinical signs and radiographic views after surgery between group A and group B. The time (mean${\pm}$SD) to make total ring prosthesis, group A took $23.2{\pm}1.9$ minutes which was remarkably longer than that $(4.6{\pm}0.3)$ of group B. Also, the time to place around trachea, group A required $61.8{\pm}8.8$ minutes and group B $38.4{\pm}8.0$ minutes. Conclusionly, PVCTRP was timesaving and easier to make, fix, and suture than those of PPTRP. PVCTRP may be used alternatively to PPTRP for the tracheal collapse to treat in dogs.

A Review of Cigarette Smoking-related Behaviors and Health Problems among University Students (대학생 흡연 관련 행태 및 흡연에 의한 건강문제)

  • Objectives: This article reviewed research on smoking behaviors and smoking-related health problems among university students to suggest evidences for developing smoking control strategies for them. Methods: Domestic research papers were screened through Research Information Sharing Service (RISS), Koreanstudies Information Service System (KISS), Medical Library Information System (MEDLIS), Korean Medical Database (KMbase), and digital national assembly library. International papers were searched mainly via PubMed. Results: Smoking prevalence among male college students were found to be over 50% in majority of Korean studies. While studies on smoking-related health problems were scant in Korea, several recent studies overseas showed possible effects of smoking on health among young adults, including decreased lung function, respiratory symptoms, cardiovascular problems, glucose intolerance, buccal disease, depressive symptoms, and so forth. Besides, smoking behaviors were closely related with other risk behaviors including drinking, and several psychosocial factors. Conclusions: To explore the smoking problem among college students, the first step should be a national representative survey with scientific methods. More research should be focused on the smoking-related problems among college students. To prevent smoking among college students, smoking prevention education, and smoking cessation counseling, and the initiatives of smoke free campus are needed.

The Short-Term Effects of Soft Pellets on Lipogenesis and Insulin Sensitivity in Rats

  • The purpose of the present study was to investigate the short-term effects of a 12-day, soft pellet (SP) diet with a 3-h restricted feeding schedule on caloric intake, body weight, lipid metabolism, and insulin sensitivity. Glucose and insulin levels were measured pre-, mid-, and post-feeding. The SP rats exhibited postprandial hyperglycemia compared to rats fed control pellets (CP). The insulin response of SP rats during a meal was significantly higher than that of CP rats. There were no significant differences in the hepatic triacylglycerol contents and lipogenesis gene mRNA levels of SP and CP rats. However, the hepatocytes of SP rats were slightly hypertrophic. In addition, histological analysis revealed that the pancreases of SP rats had more islet areas than those of CP rats. This study demonstrated that feeding an SP-only diet for 12 days induces glucose intolerance, suggesting that the consumption of absorbable food, like a soft diet, may trigger glucose metabolism insufficiency and lead to life-threatening diseases.

The First Neonatal Case of Neonatal Argininosuccinic Aciduria in Korea

  • Argininosuccinic aciduria (ASAuria) is a rare autosomal recessive urea cycle disorder. Neonatal presentation of ASAuria is the most common form. It is characterized by lethargy, feeding intolerance, decreased consciousness, and coma after 24 to 72 hours of birth. We describe a rare case of ASAuria in a female neonate who presented with severe hyperammonemia, a typical characteristic of urea cycle disorders. This patient's diagnosis was confirmed by biochemical analyses, and we found that the patient had a point mutation of the argininosuccinate lyase gene, which was homozygous for a novel 556C>T substitution. We have never seen the neonatal form of ASAuria in Korea. Therefore, this is the first report of neonatal onset ASAuria in Korea.

  • Religious intolerance has become a common feature of many countries in recent times. Studies have revealed a worldwide increase in government regulations and social hostilities against religious beliefs and practices. The stifling impact of both government and society on the market for religion, warrants closer scrutiny. This study examines the relationship between government regulations and social hostilities towards religious beliefs and practices, for the period of 2001-2011 for a sample of 45 European countries. The Generalized Method of Moments dynamic panel estimation technique is employed to analyze the micro panel dataset of 45 European countries, to establish the possible relationships that may exist between these variables. The theoretical framework for this study is based on the Religious Economies Theory and the Supply Side Theory of Religion. The results of this study show evidence of the positive relationship between government regulations and social hostility. Interestingly, the study also revealed that the impact of social hostility on the level of government restrictions is smaller in magnitude compared to the reverse impact of government restrictions on social hostilities, indicating the dangerous role played by governments in inciting social hostilities, when they regulate or restrict religious beliefs and practices.

One of effective approach to health maintenance and disease control is the use of dietary bacterial and carbohydrate supplements. This comprises use of probiotics and prebiotics. Probiotics mean the live microorganisms, which when administered in adequate amounts confer a health benefit on the host. Prebiotics mean a nondigestible food ingredient that beneficially affects the host by selectively stimulating the growth and/or activity of one or a limited number of bacteria that can Improve the host health. Especially, probiotics has the relation which is close with innate immunity and adaptive immunity. And probiotics has the clinical value with many disease like lactose intolerance, constipation, acute gastroenteritis, food hypersensitivity and allergy, atopic dermatitis, crohn's disease, rheumatoid arthritis, pelvic radiotherapy, intestinal inflammation and chemical exposure, colon cancer, inhibitory effect of Helicobacter pylori and lowering the level of cholesterol. We use jointly korean medicine and probiotics and it has the more therapeutic effect in the many disease.

Activation of the renin-angiotensin system in high fructose-induced metabolic syndrome

  • High fructose intake induces hyperglycemia and hypertension. However, the mechanism by which fructose induces metabolic syndrome is largely unknown. We hypothesized that high fructose intake induces activation of the renin-angiotensin system (RAS), resulting in hypertension and metabolic syndrome. We provided 11-week-old Sprague-Dawley rats with drinking water, with or without 20% fructose, for two weeks. We measured serum renin, angiotensin II (Ang II), and aldosterone (Aldo) using ELISA kits. The expression of RAS genes was determined by quantitative reverse transcription polymerase chain reaction. High fructose intake increased body weight and water retention, regardless of food intake or urine volume. After two weeks, fructose intake induced glucose intolerance and hypertension. High fructose intake increased serum renin, Ang II, triglyceride, and cholesterol levels, but not Aldo levels. High fructose intake increased the expression of angiotensinogen in the liver; angiotensin-converting enzyme in the lungs; and renin, angiotensin II type 1a receptor (AT1aR), and angiotensin II type 1b receptor (AT1bR) in the kidneys. However, expression of AT1aR and AT1bR in the adrenal glands did not increase in rats given fructose. Taken together, these results indicate that high fructose intake induces activation of RAS, resulting in hypertension and metabolic syndrome.
