호르몬수면상실이 에너지와 대사에 미치는 영향 (The Effect of Sleep Loss on Energy and Metabolism)

  • The release of hormones and the metabolism of human body are controlled by the circadian rhythm related to sleep-wake cycle. Growth hormone, prolactin, thyroid stimulating hormone, cortisol, glucose, and insulin-secretion rates fluctuate according to the sleep-wake cycle. In addition, sleep is related to the appetite regulation and carbohydrate and other energy metabolism. Hypocretin (orexin), an excitatory neuropeptide, regulates waking and diet intake, and the poor sleep increases diet intake. The short sleep duration increases one's body mass index and impairs the function of the endocrine and metabolism, causing increases in the risk of glucose intolerance and diabetes. The poor sleep quality and sleep disorders have similar impact on the metabolic function. In short, the sleep loss and the poor quality of sleep have a detrimental effect on the endocrine and energy metabolism. The improvement of sleep quality by the future research and appropriate clinical treatment would contribute to the decrease of the metabolic diseases such as diabetes.

  • Early nutritional support for preterm infants is critical because such support influences long-term outcome. Minimal enteral feeding should be initiated as soon as possible if an infant is stable and if feeding advancement is recommended as relevant to the clinical course. Maternal milk is the gold standard for enteral feeding, but fortification may be needed to achieve optimal growth in a rapidly growing premature infant. Erythromycin may aid in promoting gastrointestinal motility in cases that exhibit feeding intolerance. Selected preterm infants need vitamins, mineral supplements, and calorie enhancers to meet their nutritional needs. Despite all that is known about this topic, additional research is needed to guide postdischarge nutrition of preterm infants in order to maintain optimal growth and neurodevelopment.

말초 동맥 교감 신경 절제술을 이용한 레이노드 현상의 치료(증례 보고) (Peripheral Periarterial Sympathectomy for the Treatment of Raynaud's Phenomenon(Case Report))

  • The treatment of chronic digital pain and cold intolerance due to Raynaud's phenomenon is quite difficult especially it is combined with scleroderma. Several surgical trial such as cervicothoracic sympathectomy have been attempted for the medically unresponsible Raynaud's phenomenon, but their results were unsatisfactory. We have tried peripheral periarterial sympathectomy for the 44 years old female patient who had medically unresponsible severe Raynaud's phenomenon with scleroderma. Periarterial adventitial stripping was performed at the level of wrist, superficial palmar arch, common digital artery and proper digital artery about 1.5-2 cm in length. Preoperative angiography and radioactive angiography were done and preoperatively and postoperatively the blood flow was measured by the desk top computer-aided histogram. Both hands digital pain were markedly reduced after operation and blood flow increased as compaired with the preoperative measure.

자연치즈의 제조과정 중 숙성의 중요성 (Importance of Ripening during Natural Cheese Making)

  • The ripening of cheese allows for the development of characteristic taste and flavour, nutritional substances, bio-active components and texture, helping to improve quality. Many different microbiological, biochemical and nutritional changes occur during the process depending on the quality of raw milk, added cultures and enzymes, as well as specific processing and ripening conditions. During the ripening lactose is hydrolyzed to lactic, propionic and acetic acid, helping to reduce potential effects of the problem of lactose intolerance. Fat is hydrolyzed to butyric, propionic and conjugated linoleic acid, which function as bio-active substances. Protein is hydrolyzed to different peptides and amino acids which all show various bio-activities. However, errors of cheese ripening can happen and affect the quality of the product. To guarantee good quality cheese the process needs to be managed carefully with the right microbes used and ensuring cleanliness of processing facilities, staff, ventilation and hazard analysis and critical control points (HACCP). Research into and controlling of ripening technology is crucial for producing high quality cheeses.

개의 만성 외이염 수술적 치료예 (The Result of Surgical Treatment of Chronic Otitis Externa in Dog)

  • The otitis externa frequently occurs in dog. The cause is mostlyan increase of bacterias (for example Staphylococcus spp. Streptococcus spp. and Psudomonas spp.) and/or yeast fungi (especially malassizia pachydermatis) and/or infestation with parasites (otodecits cynotis) which growth is promoted by a change of the microclimate in the external auditory meatus. The worse therapeutic results of medicine is explained by an intolerance and resistence of microorganisms which was often observed. In this study of dog with otitis externa developed a generalised pyoderma. This, in fact, could be a causative factor in the development of otitis externa and its recurrence. Concluded, the surgical treatment is the best method for eliminating chronic otitis externa.

Mitral Dysplasia with Papillary Muscle Atrophy in a Dog

  • A 9-month-old, female, mixed-breed dog was presented with dyspnea, exercise intolerance and a left apical heart murmur. Echocardiography revealed mitral systolic regurgitant flow by morphological abnormalities of the mitral apparatus including dilated annulus, excessively asymmetric mitral leaflets, short and stout chordae tendinae, and atrophic papillary muscles. The dog was diagnosed with congenital mitral dysplasia based on the echocardiographic features. Two months later, the dog died and the echocardiographic findings were confirmed by necropsy. This case report describes the echocardiographic features of mitral dysplasia with papillary muscle atrophy, and compares the gross necropsy findings in a dog.

진도견에서 발생한 후천성 갑상선 기능저하증 (Primary hypothyroidism in Jindo dog)

  • A four-year-old female Jindo dog was admitted to the hospital because of bilaterally symmetrical alopecia and inappentence. Systemic abnomalities detected on physical examination were obesity, bradycardia, exercise intolerance, mental dullness and hypothermia. Dermatologically symmetrical alopecia on the back, trunk and dorsal tail, and ventral hyperpigmentation were observed. Also face was very tragic. The level of basal $T_4$ was 0.01ug/dl. In TSH stimulation test $fT_4$ value was 0.08ng/dl. After treatment was initiated with L-thyroxine the dog's general condition was improved at 4 weeks.

  • MELAS is the condition associated with mutant mtDNA that most closely mimics thrombotic cerebrovascular disease. Characteristic abnormalities are two. first, 'ragged-red fibers' in muscle biopsy. second, point mutation in the mitochondrial DNA analyses. The characteristic clinical presentations of MELAS are short stature, recurrent stroke like episodes, migraine-like headache, sensorineural hearng loss, glucose intolerance and neuropathy. We now report a case of MELAS syndrome having mitochondrial DNA mutation with an A to G transition at the 3,243rd position diagnosed in Chung-ang Hospital.

Transvenous occlusion of patent ductus arteriosus using an embolization coil in a Maltese dog

  • A 6-year-old female Maltese dog (body weight 2.0 kg) was referred to the Veterinary Teaching Hospital, Kangwon National University with primary complaints including exercise intolerance and heart murmur. Based on clinical and diagnostic findings including grade V/VI left basal continuous murmur, bounding femoral pulsation, left ventricular enlargement pattern in electrocardiogram, cardiomegaly with aortic bulging on the thoracic radiography, and shunt flow between aorta and pulmonary artery on the echocardiography, the dog was diagnosed as the left-to-right patent ductus arteriosus. The patent ductus arteriosus was successfully treated by lodging a single embolization coil with transjugular approach.

Balloon valvuloplasty for severe subaortic stenosis in a Pomeranian dog

  • A nine-month-old Pomeranian dog with exercise intolerance and syncope was presented. The dog was depressed with grade 4 systolic murmur on cardiac auscultation. Based on cardiac examination, the dog was diagnosed with severe subaortic stenosis with involvement of the anterior mitral valve. ${\beta}-blocker$ administration was initiated and clinical signs were improved, but not fully resolved. Balloon valvuloplasty was performed and the dog survived for nearly one year without clinical sign and the cardiac troponin I level was normalized. This case describes successful management of severe subaortic stenosis in a small breed dog through balloon valvuloplasty.
