A Case of Gastrointestinal Stromal Tumor in a Child (소아에서 발생한 위장관 간질 종양 1예)

  • Gastrointestinal stromal tumors (GISTs) are the most common primary mesenchymal tumors of the digestive tract. They have been commonly observed in adults but have been rarely described in children. They arise typically from the intestinal wall and rarely in the mesentery, omentum, or retroperitoneum. GISTs originate from the interstitial cell of Cajal and are characterized by overexpression of the receptor tyrosine kinase c-kit. Up to 94% of these tumors express the CD117 on immunohistochemical stain. Surgery is the main modality of treatment for primary resectable GIST. Completely resectable GIST with low risk has excellent prognosis after primary surgical intervention, with over 90% of the 5-year survival. We report a case of 10-year-old girl presenting with an upper gastrointestinal bleeding caused by gastrointestinal stromal tumor.

Enhancing the Viability Rate of Probiotic by Co-Encapsulating with Prebiotic in Alginate Microcapsules Supplemented to Cupcake Production

  • The objective of the study was to assess the survival of microencapsulated Lactobacillus plantarum ATCC8014 produced using the emulsion technique in alginate gel combined with pectin and maltodextrin components. The microcapsules were then added to cupcake dough that was further baked at 200℃ for 12 min. The viability of L. plantarum was assessed during baking and the 10 days of storage at 4℃ as well as in simulated gastrointestinal conditions. In addition, yeast-mold and water activity were investigated. After baking, the samples with microencapsulated L. plantarum contained more than 5 log CFU/g, which was higher compared to the bacterial concentration of the control samples. The concentration of L. plantarum was more than 6 logs CFU/g after the end of the storage; therefore, the probiotic functioned as a biopreservative in the cake. The prebiotic component strengthened the microcapsules network and helped protect the viability of L. plantarum in simulated gastric fluid (SGF) and simulated intestinal fluid (SIF) media. The results show that the addition of L. plantarum microencapsules did not affect the sensory scores of the cupcake while ensuring the viability of the probiotic during baking and storing.

Metastatic Bilateral Renal T-Cell Lymphoma in a Persian Cat

  • A two-year-old spayed female Persian cat demonstrated weight loss, anorexia, and vomiting for one week. Hematologic findings suggested chronic renal failure. Radiography and ultrasonography revealed severe bilateral renomegaly with hypoechoic nodules and subcapsular hypoechoic rim. Fine needle aspiration of the kidney revealed malignant lymphoma. The cat received in-hospital treatment for chronic renal failure for seven days, followed by chemotherapy (cyclophosphamide, vincristine, and prednisolone). The cat tolerated chemotherapy well and chronic kidney disease was alleviated. However, complete remission was not achieved. After 93 days of treatment, the cat exhibited anisocoria and mental dullness. Brain magnetic resonance imaging revealed hypertrophy and enhancement of cranial nerves. Chemotherapy was replaced with lomustine (10 mg orally), and two weeks later, cytosine arabinoside (50 mg/㎡ subcutaneously), twice daily for consecutive days. Five days after substitution chemotherapy, the patient showed anemia due to severe intestinal bleeding and died. Post-mortem examination and histopathologic analysis confirmed renal T-cell lymphoma with metastasis to the central nervous system, colon, and nasal cavity. Survival time was 117 days after the diagnosis of renal lymphoma.

Resolution of Protein-Losing Enteropathy after Congenital Heart Disease Repair by Selective Lymphatic Embolization

  • With improving survival of children with complex congenital heart disease (CCHD), postoperative complications, like protein-losing enteropathy (PLE) are increasingly encountered. A 3-year-old girl with surgically corrected CCHD (ventricular inversion/L-transposition of the great arteries, ventricular septal defect, pulmonary atresia, postdouble switch procedure [Rastelli and Glenn]) developed chylothoraces. She was treated with pleurodesis, thoracic duct ligation and subsequently developed chylous ascites and PLE (serum albumin ${\leq}0.9g/dL$) and was malnourished, despite nutritional rehabilitation. Lymphangioscintigraphy/single-photon emission computed tomography showed lymphatic obstruction at the cisterna chyli level. A segmental chyle leak and chylous lymphangiectasia were confirmed by gastrointestinal endoscopy, magnetic resonance (MR) enterography, and MR lymphangiography. Selective glue embolization of leaking intestinal lymphatic trunks led to prompt reversal of PLE. Serum albumin level and weight gain markedly improved and have been maintained for over 3 years. Selective interventional embolization reversed this devastating lymphatic complication of surgically corrected CCHD.

Effect of Dietary Calcium, Casein and Suet on Intestinal Absorption of Cadmium in Mice (생쥐에 있어서 카세인, 칼슘 및 우지가 카드뮴의 장흡수에 미치는 영향)

  • This study was carried out to clarify the effect of dietary calcium, casein and suet on intestinal absorption of cadmium in mice. It was performed for 30 days, from April 11, to May 10, 1988. Ninety mice were devided into 4 experimental groups and control group with 6 mice each dietary group, and measured survival ratio, body weight and weight ratio of organ to body. The contents of cadmium in liver, kidney, spleen, muscle and skin with hair, and feces were analyzed by atomic absorption spectrophotometer after sacrifice by anesthesia. The ratio of survival after 30 day breeding was $100\%$ in control group, but $66.7\%$ in group IV(basal diet + Cd + Ca) with $100{\mu}g$ one dose and with 50ppm cadmium containing fluid free-intake, and group V (basal diet + Cd + suet) with free-intake of 50ppm containing cadmium fluid. The ratio of body weight gains in cases of single dose $100{\mu}g$ of cadmium was highest as $42.3\%$ in group IV and lowest as $26.0\%$ in group V, in cases of free-intake of 50ppm containing cadmium fluid was highest as $24.0\%$ in group IV and lowest as $11.6\%$ in group II (basal diet + Cd). The body weight, in the case of single dose of $100{\mu}g$ of cadmium showed no increase untill 5th day from acute poisoning. While in case of free-intake of 50ppm containing cadmium, it showed very slight increase through all breeding period. The weight ratio of organ to body were lowest in liver of group II in both occasions. The most of cadmium administered was excreated in feces within 2 days after single dose $100{\mu}g$ cadmium. The contents of cadmium in all tissues were significantly high in each group comparing to control group as liver, kidney, spleen and muscle of group II which showed the highest level in both occasions of $100{\mu}g$ one dose and free-intake of 50ppm containing cadmium fluid, and higher in group II of the occasion with $100{\mu}g$ single dose, and in group V of the occasion of 50ppm containing cadmium fluid free-inatke for skin with hair. And the content of cadmium in tissues is generally higher in the occasion of 50ppm containing cadmium free-intake than $100{\mu}g$ single dose. From the above results I conclude that the addition of casein, calcium and suet in cadmium containing diet is effective to inhibtion of intestinal absorption of cadmium by particularly, calcium.

  • In vivo protective and in vitro inhibitory activities of Lactobacillus casei YIT 9018. against typical enteritis causing Salmonella enteritidis KU101 and IgA level after challenge have been determined. In order to identify the strains of lactobacilli the sequences of 16S-23S rRNA intergenic spacer region were determined. All the test strains of Lactobacillus spp. inhibited Salmonella enteritidis, the intensity varied depending upon the species of lactobacilli. Effects on the survival rate of the mouse after challenge with Salmonella enteritidis KU101 on feeding Lactobacillus spp. have shown the highest survival rate in L. helveticus CU 631 followed by L. casei YIT 9018 and L. johnsonii C-4 and the lowest in control mice. The higher level of total Ig A concentration in the intestinal fluid of lactobacilli fed mice than control mice was observed. The sequences of 16S-23S rRNA intergenic spacer region of seven strains of Lactobacillus casei could be utilized as a strain identification, those sequences showed some degree of difference in homology.

Treatment Result of Postoperative Radiotherapy of Carcinoma of the Uterine Cervix (자궁경부암의 수술후 방사선 치료성적)

  • The early carcinoma of the uterine cervix may be treated by either radical surgery or radical radiotherapy according to the patient's characteristics, and the survival is high with either treatment. But, because of the size of the lesion, metastasis to lymph nodes, and vascular space invasion by tumor have all been shown to influence recurrence and survival, postoperative radiotherapy may be considered as their histopathologic finding after radical surgery. However, there are still debates on the increasing survival rates with postoperative radiotherapy. Two hundreds and three patients with carcinoma of the uterine cervix who were treated with postoperative radiotherapy from February 1979 to September 1982 in the Department of Therapeutic Radiology, Seoul National University were analyzed and following results were obtained. 3-year actuarial survival rate and 3-year disease free sruvival rate were $83.4\%\;and\;73.4\%$ respectively and 3-year actuarial survival rates by stages were $90.7\%$ for IB, $69.6\%$ for IIA, and $85.2\%$ for IIB. 3-year disease free survival rates by stage IB, IIA, IIB were $79.8\%,\;07.8\%,\;68.3\%$ respectively. The overall failure rate was $25.1\%(51/203)$; local recurrence rate was $8.4\%$, distant metastasis rate was $14.3\%$ and simultaneous local recurrence and distant metastasis was $2.4\%$. Failure rates by stages were $19.8\%$ (18/19) for IB, $29.1\%$ (16/55) for IIA and $29.8\%$ (17/57) for IIB. The overall acute complication rate was $57.6\%$; tolerable cases was $50.2\%$ and severe cases was $7.4\%$. Late complication rate was $7.9\%$ and the major late complication were intestinal obstruction. aggravated urinary symptom, radiation cystitis in order of frequency.

Effects of Moxi-tar Herbal Acupuncture of LI11 on inflammatory bowel disease induced by TNBS in mice (생쥐의 급 ${\cdot}$ 만성 대장염에 대한 곡지의 구진 약침 효과)

  • Objectives : The purpose of the present study is to evaluate the effects of herbal acupuncture (HA) with Moxi-tar for the treatment to intestinal disease in mice with 2, 4, 6 - trinitrobenzenesulfonic acid (TNBS) induced colitis. Methods : Mice were administered with 5% TNBS at day 1 and day 7. To investigate effects of HA with Moxi-tar at LI11, treatments were carried out at day -1, day 1, day 3, day 5, and day 7. It was checked on the weight and width of colon, diarrhea, edema, survival rate, changes of body weight, and myeloperoxygenase (MPO) activity. Furthermore, we carried out immunohistochemical staining and Western blot and analyzed mRNA expression by RT-PCR. Results : HA of Moxi-tar at LI11 in preventive mode suppressed macroscopic damages and damages of intestinal epithelial cells and infiltration of immune cells in the colon by TNBS. HA in early and preventive mode ameliorated various symptoms by TNBS. TNBS injection increased MPO activity in colon while HA in preventive mode suppressed increase of MPO activity. HA down-regulated NF-kB activity and reduced expression of TNF-a, IL-1b, and ICAM-1 in colon of TNBS treated mice. Similar to experiment at colon, HA down-regulated NF-kB activity and reduced expression of TNF-a, IL-1b, and ICAM-1 by TNBS in mesenteric lymph node. HA in therapeutic mode suppressed errosion and shortening of colon and MPO activity by TNBS and suppressed mRNA expression of TNF-a, IL-1b, and ICAM-1 in the colon. Conclusions : This study demonstrates that HA with Moxi-tar at LI11 represents a potential therapeutic method of inflammatory bowel diseases.

Expression of Heregulin and ErbB Family Proteins in Gastric Adenocarcinomas: Correlation with Clinopathologic Prognostic Factors (위선암에서 Heregulin과 ErbB Family 단백 발현과 임상.병리학적 예후인자와의 상관관계)

  • Purpose: Heregulin is a natural ligand for erbB3 and erbB4. However, very little is known about their roles in the gastric cancer This retrospective study was performed to evaluate the frequencies of heregulin and erbB family protein expression and to compare their expressions with clinicopathologic parameters. Materials and Methods: Immunohistochemical expressions of heregulin and erbB family proteins were examined with tissue micro-array slides. A total of 251 gastric adenocarcinomas were classified as early cancers and advanced cancers and as having and not having lymph node metastases. Results: The positive rates of the heregulin, erbB1, erbB2, erbB3, and erbB4 protein stainings were 64%, 68%, 6%, 88%, and 76%, respectively. Intestinal type gastric adenocarcinomas showed higher expression of heregulin, erbB2, erbB3, and erbB4 proteins. Heregulin and erbB4 proteins showed lower expressions in advanced gastric carcinomas. However, erbB2 protein showed higher expression in advanced gastric carcinomas. The protein expressions of heregulin and erbB family proteins showed no relationship with survival rate. Co-expression groups of heregulin and erbB3 proteins or heregulin and erbB4 proteins showed higher expressions in intestinal type adenocarcinomas and early gastric carcinomas. Conclusion: Heregulin, erbB3, and erbB4 proteins may play a role in the early stage of adenocarcinomas.

  • This study was performed to encapsulate low molecular weight marine collagen and ${\gamma}$-aminobutyric acid (GABA)-producing lactic acid bacteria to inhibit degradation and improve survival rate during exposure to adverse conditions of the gastro-intestinal tract. Calcium-alginate method was used for the manufacture of a double-layered coated capsule. The inner core material was composed of collagen and lactic acid bacteria, and the coating materials were alginate and chitosan. The sizes and shapes of the double-coated capsule were affected mainly by centrifuge speed and pH. Manufactured capsules were observed with a scanning electron microscope and by confocal laser scanning microscopy to confirm the micromorphological changes of capsules and bacterial cells. As a result, double-layered coated capsules were not degraded at pH 1.2, whereas degradation occurred at pH 7.4. In addition, GABA and collagen were maintained in stable state at pH 1.2. Therefore, double-layered coated capsules developed in this study would not be degraded in the stomach and could be stably delivered to the small intestine to benefit intestinal and dermatic health.
