The Value of Periapical Radiograph in the Diagnosis of Interproximal Caries (구내방사선사진의 인접면 치아우식 진단에 대한 유용성 평가)

  • Purpose : To compare the diagnostic performance of clinical and radiologic examination for the interproximal caries on intraoral periapical radiographs and to evaluate the value of periapical radiographs. Methods: One hundred seven dental patients were examined clinically, with a mouth mirror and an explorer, by a dentist at the department of oral medicine, and the presence or absence of interproximal caries lesion was recorded. The patients were prescribed one or more dental periapical radiographs. Radiographs were assessed for the presence of interproximal caries by three oral and maxillofacial radiologists independantly. Two thousand sixty interproximal surfaces were included in this study. The diagnostic accuracies of clinical and radiologic examinations for interproximal caries were calculated. To assess the degree of agreement between clinical and radiologic examinations, Cohen's coefficient of agreement was computed. Results: The specificity of clinical and radiologic examination was 0.991, 0.997 and the sensitivity was 0.279, 0.985 respectively. The diagnostic accuracy of radiologic examination was statistically significantly higher than that of clinical examination (P<0.05). Cohen's kappa value of clinical and radiologic examination was 0.335, 0.942 respectively. These results suggested that clinical examination show only fair agreement, whereas radiologic examination show perfect agreement. Conclusion: The diagnositic performance of the dental periapical radiographs on interproximal caries were higher than that of clinical examination, thus this study showed the validity of periapical radiographs for detecting interproximal caries lesion without bitewing radiograph.

CLINICAL USE OF DENTAL RADIOGRAPHY IN THE DIAGNOSIS OF INTERPROXIMAL CARIES AND PERIODONTAL DISEASE (인접면 치아우식증과 치주질환의 진단에서 방사선 촬영의 이용)

  • The purpose of the present study was to investigate associations between periapical and bitewing techniques by assessing the crestal alveolar bone. This article also reports the ability of these two techniques to correctly detect evidence of interproximal dental caries, and comparison between the interproximal overlapping of teeth. Bitewing and periapical radiographs were used from posterior quardants of 243 dental students in Seoul National University. The distance from cemento-enamel junction to the alveolar crest (CEJ-AC) was measured for each proximal surface from the distal of cuspid to the distal of second molar. Data were arranged according to the proximal surface examined, and bitewing and periapical measurements were compared using paired tests. The obtained results were as follows: 1. In maxilla, a significant t ratio with a P value of 0.05 or less reached for 100% and in mandible, reached for 94%. 2. The anatomic limitations imposed on periapical radiographic technique, most often result in somewhat foreshortened radiographic images. This situation would tend to be accentuated by the anatomical restrictions of the hard palate. 3. Consequently, since the significant differences frequently exist between measurements obtained from bitewing and periapical techniques, it is important to define which technique is used. 4. The number of the interproximal overlapping was the largest medial side of the maxillary second molar, while the smallest at the distal side of the mandibular second premolar. And the overall number of the interproximal overlapping was more (538) in the periapical technique than in the bitewing technique (372). 5. The interproximal dental carious lesions were detected more (74) on the bitewing films than on the periapical ones (23). The fact was resulted from the small number of interproximal overlapping and relative easi- ness of obtaining horizontal angulation in taking the bitewing radiographs.

A STUDY ON THE ARTIFICIAL INTERPROXIMAL CARIES DETECTION WITH THE DIGITAL RADIOGRAPHY (디지털방사선촬영술을 이용한 인접면 치아우식증 진단에 관한 실험적 연구)

  • The purposes of this study were clinical comparison and evaluation between digital radiography and conventional radiography for the detection of artificial interproximal caries. Four freshly extracted, unrestored posterior teeth were obtained and caries was simulated by drilling semicircled defects with variable size at the interproximal surface of each tooth. The experiments were performed with IBM-PC/32 bit-DX compatible, video camera(VM-S8200, Hitachi Co., Japan), and color monitor(Multisync 3D, NEC, Japan). Sylvia Image Capture Board for the ADC(analog to digital converter) was used, and spatial resolution was 512 × 480 with 256 gray levels. The obtained results were as follows: 1. At the condition of under exposure, the radiographs were superior to the digital images in readability. Also, as the size of the artificial lesion was increased, readability of the radiographs was elevated. 2. The digital images were superior to the radiographs in readability especially under over exsposure. 3. As the exposure time and size of lesion was increased, the gray level of region of interest of the digital image was decreased proportionally. 4. As the F-value of average gray level of region of interest at individual exposure time and size of lesion, gray level of the all lesion sizes was decreased in comparison with that of the normal enamel.

Effect of slow forced eruption on the vertical levels of the interproximal bone and papilla and the width of the alveolar ridge

  • Objective: Forced eruption has been proposed for the reconstruction of deficient bone and soft tissue. The aim of this study was to examine the changes in the alveolar ridge width and the vertical levels of the interproximal bone and papilla following forced eruption. Methods: Patients whose hopeless maxillary anterior teeth were expected to undergo severe bone resorption and soft tissue recession upon extraction were recruited. In addition, patients whose maxillary anterior teeth required forced eruption for restoration due to tooth fracture or dental caries were included. Before and after forced eruption, the interproximal bone height was measured by radiographic analysis, and changes in the alveolar ridge width and the interproximal papilla height were measured with an acrylic stent. Results: This prospective study demonstrated that the levels of the interproximal alveolar bone and papilla were significantly increased by 1.36 mm and 1.09 mm, respectively, in the vertical direction. However, the alveolar ridge width was significantly reduced by an average of 0.67 mm in the buccolingual direction. The changes in the level of the interproximal alveolar bone and papilla were positively correlated. Conclusions: Although the levels of the interproximal bone and papilla were significantly increased, the alveolar ridge width was significantly decreased following forced eruption. There was a modest positive and significant correlation between the changes in the height of the interproximal alveolar bone and the papilla. Based on our findings, modification of vertical forced eruption should be considered when augmentation of the alveolar ridge width is required.

PHYSIOLOGIC INTERDENTAL SPACES AND PROXIMAL CARIES IN THE ANTERIOR MAXILLARY PRIMARY DENTITION (상악 유전치부의 치간공간과 인접면 우식에 관한 조사연구)

  • The purpose of this study was to assess the relationship between interdental spaces and proximal caries in maxillary anterior primary teeth. 555 children aged 3-7 inhabit in Iksan were divided into two groups, depending on the presence of interdental space which was detected by a dental explorer. They were determined to have proximal caries if cavity was formed or the enamel surface was softened. The results were as follows : 1. Regarding interdental spaces, 77.4% had primate spaces; 54.4% had developmental spaces between central and lateral incisor, and 39.0% between central incisors. 2. Interproximal caries incidences in right primary canine, lateral incisor, and central incisor were 6.3%, 14.7%, and 33.5%, respectively. Also interproximal caries incidences in left primary central incisor, lateral incisor, and canine were 33.7%, 16.0%, and 4.7%, respectively. 3. Children with more interdental spaces had less caries incidence, but the relationship was weak(r=-0.024). 4. The mean caries incidence was higher in absence of interdental space of maxillary primary incisors than in presence of space. The mean caries incidence with no interdental space was twice as high as that with presence of interdental space.

PREVENTION OF DENTAL CARIES BY XYLITOL GUM IN PRE-SCHOOL CHILDREN DURING 12-MONTHS (자일리톨 껌 저작에 의한 유치 우식증 예방효과 비교분석)

  • The effect of xylitol and sorbitol chewing gums on dental caries levels among 5-year old kindergarteners was investigated. Three kindergartens the operations of which were partly subsidized by municipal government of Daegu city, Korea, were chosen as the study sites. The child base of the kindergartens were considered demographically and ethnically similar, representing middle-income families. The subjects were examined at their own kindergarten by the two dentists. Participation in the program was voluntary. Over 12 months, 123 participants chewed xylitol chewing gums(X group; 42 subjects), sorbitol chewing gum(S group; 42 subjects), or did not receive chewing gum as a control group(C group; 39 subjects). Consumption of xylitol and sorbitol was 4.5 to 5.0 g/day/subject, consumed in five daily chewing episodes of 5 min. Oral examination, Dentocult-SM test and interproximal dental plaque collection were completed at baseline and 12 months later. The dmfs of group C increased 59.2%, but group S increased 33.4% and group X increased only 31.3% during 12 months study period. The caries prevention ratio was 47.1% at group X and 43.6% at group S. There also appeared the reduction of caries activity at group X(1.39), group S(1.50) than control group(1.79). Compared with groups S and C, there was a statistically significant reduction of S. mutans in interproximal plaque in group X. The results suggest that xylitol chewing gum can prevent dental caries of deciduous dentition, and may be a little more effective than a sorbitol-containing product in controlling some caries-associated parameters in kindergarten-age subjects.

Diagnostic accuracy of Insight intraoral film on dental caries (Kodak Insight 구내필름의 치아우식증 진단능에 대한 연구)

  • Purpose: To compare the diagnostic accuracy of Kodak Insight film with other intra-oral films in the detection of dental caries. Materials and Methods : Periapical radiographs of 99 extracted human teeth with sound proximal surfaces and interproximal artificial cavities were made on Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Kodak Insight films and automatically processed. Six dentists examined the presence of dental caries using a five-point confidence rating scale and compared the diagnostic accuracy by ROC (Receiver Operating Characteristic) analysis and ANOVA test. Results: The sensitivity of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.84, 0.77, 0.75 and 0.79 respectively. The specificity of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.97. 0.95. 0.96 and 0.94 respectively. The mean ROC areas (Az) of Kodak Ultra speed, Ektaspeed, Agfa Ektaspeed and Insight film were 0.917, 0.910, 0.894, 0.909 respectively. There was no significant differences between Az of Insight film and other films (p = 0.178). Conclusion: Theses results suggested that Kodak Insight film have the comparative diagnostic accuracy of dental caries with Ultraspeed and Ektaspeed films.

  • The aim of the present study was to describe an safe and convenient method for the early detection of enamel caries using laser fluorescence. Fluorescence from natually carious lesion of human teeth illuminated by an argon laser(488nm) was observed and photographed using barrier filter. Intact enamel was found to fluorescence with a yellowish light. Whereas, incipient caries lesions in the enamel were dearly visible as dark areas in contrast to the fluorescence surroundings. For evaluation of accuracy of this method, lesion depth measured by the laser fluorescence in light microscope was compared with that polarizing microscope. The results from the present study can be summarized as follows : 1. Enamel caries of smooth surface was observed as pale white spot and undefined outline in ordinary light. Whereas, lesion was clearly visible as dark spot in laser fluorescence. 2. There was no difference between ordinary light view and laser fluorescence in occlusal surface and interproximal surface. 3. There was no significant difference between the lesion depth observed by laser fluorescence with light microscope and polarizing microscope. Apparent correlation exists between two groups.

  • The purpose of this study was to evaluate the diagnostic performance of the senior dental students for the proximal dental caries on intraoral radiographs and to compare it with the dental hospital residents, the reference group. It was also investigated the diagnostic performance according to the carious lesion depth. Thirty-five intraoral periapical and bitewing radiographs with 213 proximal surfaces included in this study were selected from the dental patients at Chonnam National University Hospital. The observers were 181 senior dental students from 5 dental schools and 40 dentists who were second year resident from 5 dental hospitals. They were asked to evaluate the presence or the absence of the proximal dental caries. The results were as follows: 1. The mean of the hitting rate for the overall observers was 184.51 surfaces and the diagnostic accuracy was 86.62%. 2. The diagnostic performance of the sound proximal tooth surfaces was very high, i.e., 91.5% true negative rate and 8.5% false positive rate. 3. The diagnostic performance of the dentist group was higher than the student group(P<0.05). 4. The proximal dental caries perceptibility increased as the lesion depth increased significantly(P<0.001) except no difference between the carious lesion depth III and IV (P>0.001).

Determining the reliability of diagnosis and treatment using artificial intelligence software with panoramic radiographs

  • Purpose: The objective of this study was to evaluate the accuracy and effectiveness of an artificial intelligence (AI) program in identifying dental conditions using panoramic radiographs(PRs), as well as to assess the appropriateness of its treatment recommendations. Materials and Methods: PRs from 100 patients(representing 4497 teeth) with known clinical examination findings were randomly selected from a university database. Three dentomaxillofacial radiologists and the Diagnocat AI software evaluated these PRs. The evaluations were focused on various dental conditions and treatments, including canal filling, caries, cast post and core, dental calculus, fillings, furcation lesions, implants, lack of interproximal tooth contact, open margins, overhangs, periapical lesions, periodontal bone loss, short fillings, voids in root fillings, overfillings, pontics, root fragments, impacted teeth, artificial crowns, missing teeth, and healthy teeth. Results: The AI demonstrated almost perfect agreement (exceeding 0.81) in most of the assessments when compared to the ground truth. The sensitivity was very high (above 0.8) for the evaluation of healthy teeth, artificial crowns, dental calculus, missing teeth, fillings, lack of interproximal contact, periodontal bone loss, and implants. However, the sensitivity was low for the assessment of caries, periapical lesions, pontic voids in the root canal, and overhangs. Conclusion: Despite the limitations of this study, the synthesized data suggest that AI-based decision support systems can serve as a valuable tool in detecting dental conditions, when used with PR for clinical dental applications.
