중풍 후 딸꾹질의 한약치료에 대한 임상 연구 동향 - 중의학 저널을 중심으로 (Review of Clinical Research about the Treatment of Hiccup After Stroke with Herbal Medicine - Based on Traditional Chinese Medicine Journals)

  • Objective: The purpose of this research is to investigate Chinese clinical studies of the treatment of hiccup after stroke with herbal medicine. Methods: We used the China National Knowledge Infrastructure (CNKI) database to search for clinical studies about using herbal medicine to treat hiccup after stroke. Results: Nineteen clinical articles Until August 31, 2017 were analyzed. The most commonly used herbal prescription was Xuanfudaizhe-tang (旋覆代?湯). According to analysis, the most commonly used medical herbs were Haematitum (代?石), Inulae Flos (旋覆花), and Pinelliae Tuber (半夏). Clinical symptoms, total incidence rate of adverse reaction, recurrence rate, symptom improvement time, patient satisfaction comparison, and dietary status improvement were used to evaluate the treatments. The effective rate of the treatment group was 86.0-97.8%, significantly higher than control group in all papers. Side effects occurred much less frequently in the treatment group than in the western medicine control group. Conclusion: The treatment of hiccup after stroke with herbal medicine was shown to be highly effective in 19 studies. Additional well-designed clinical trials are needed; this study can be used as a basis for further research regarding the treatment of hiccup after stroke.

  • Objectives The purpose of this study was to improve the knowledge of the low-level laser therapy (LLLT) field and to review research reports on LLLT to understand the current situation with respect to the clinical indication and current research trends.Methods A survey was carried out on the subject of low-level laser therapy to September 2012, using the PubMed search engine. Selected literature was checked by two reviewers and was classified according to the International Classification of Diseases 10th (ICD-10) over 10 years.Results We selected 469 studies in total, of which 142 were case reports, 118 were case-controlled trials, and 209 were randomized controlled trials of LLLT. According to the ICD-10 classification of diseases, the K code and M code being the most common, 399 studies have been published in the last 10 years. This shows that the study and clinical indications of low-level laser therapy have rapidly increased over the past 10 years.Conclusions Low-level laser therapy has been used most frequently with respect to dentistry and pain and musculoskeletal disorders. Recently, interest in and research into LLLT has increased for various diseases. With the establishment of standard conditions for low-level laser therapy, supported by aggressive clinical utilization and systematic clinical research, LLLT will be a very useful treatment and a useful alternative method in many medical fields.

폐암 환자에서 방사선치료가 세포성 면역반응에 미치는 영향 (The Effect of Radiation Therapy on Cellular Immune Response in Patients with Squamous Cell Lung Carcinoma)

  • The immune staus is known to be decreased in malignant disease and radiation therapy (RT), used as a therapeutic tool, further decrease this-attenuated immune status. We measured the number of peripheral lymphocytes, its subsets and lymphoblast transformation for PPD, PHA, monoclonal antibodies including anti-CD3 and anti-CD2 before and after RT in 19 patients with squamous cell lung cancer to search the fine mechanism behind the RT-induced attenuation of lymphoblast transformtion for mitogens and antigen. The results were as follows; 1) The number of lymphocytes and its subsets decreased significantly after RT, but the percentages of lymhocyte subsets did not change aftr RT except interleukin-2 receptor positive T lymphocytes. 2) The function of lymphoctes, measured by lymphoblast tranformation for PHA and PPD, decrased after RT and the compositions of PBMC used for lymphoblast transformtion were not different before and after RT. 3) The mitosis of lymphocytes to anti-CD2 or anti-CD3 decreased significantly after RT. And IL-2 plus anti-CD3 increased the mitosis than that of anti-CD3 only after RT, but before RT there was no difference. In conclusion, we suggested the fine mechanism behind the RT-induced attenuation of immune response might be the dysfunction of lymphocytes in terms of impaired synthesis of IL-2 rather than the decrease of circulating lymphocyte numbers.

항암화학요법 유발 말초신경병증에 대한 침치료의 효과 : 체계적 문헌고찰 및 메타 분석 (The Therapeutic Efficacy of Acupuncture for Chemotherapy-Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis)

  • Objective: This study aimed to report the therapeutic effect of acupuncture on chemotherapy-induced peripheral neuropathy (CIPN). Methods: The articles were sourced from databases including PubMed, EMBASE, Cochrane Library, CNKI, CiNii, WHO ICTRP, JSOM, KMBASE, KISS, NDSL, and OASIS as of July 2019. The main search keywords were peripheral neuropathy and acupuncture, and only randomized controlled trials using acupuncture for therapeutic purposes were included. Cochrane's risk of bias was used to assess the risk of bias, and the Review Manager 5.3 program was used for meta-analysis. Results: Six studies with a total 394 participants were included. When combined treatment of acupuncture and usual care was compared with usual care alone, quality of life improved more significantly in the combination treatment group (SMD=-2.71, 95% CI: -5.01 to -0.41, P=0.02, I2=97%). The CIPN pain score was lower among the combination treatment group, but not to a significant degree (SMD=-2.55, 95% CI: -5.14 to 0.04, P<0.05, I2=98%). There were no severe side effects in any studies. Conclusion: Acupuncture combined with usual care may be considered to safely relieve CIPN pain and improve quality of life for cancer patients. However, as there are few randomized controlled trials studying the effect of acupuncture on CIPN, further well-designed research is needed.

뇌졸중 환자의 실어증에 대한 전침 치료 : 체계적 문헌 고찰 (Electro-Acupuncture on Aphasia after Stroke: A Systemic Review of Randomized Controlled Trials)

  • Objectives: This study investigates the effect of electro-acupuncture on aphasia after stroke. Methods: A search of OASIS, NDSL, PubMed, Cochrane, and CNKI was executed between 4 January 2021 and 4 February 2021, with no limitation on publication year. Extraction and selection from the studies were made by 3 authors. The quality of the studies was evaluated using Cochrane's risk of bias (RoB) tool. Results: 10 studies met the selection criteria. As the treatment site for electro-acupuncture, GV20 (Baihui) was used the most. In all studies, the region located on the head was used for treatment without distinguishing between acupoints and areas of scalp acupuncture, and the stimulation was organized into 3 conditions: speed, intensity, and time. The outcome indicators used before and after treatment focused on the evaluation of language function and the degree of aphasia. The results showed that using electro-acupuncture with speech rehabilitation therapy for aphasia after stroke was more effective than using speech rehabilitation therapy alone. Conclusions: In this review, electro-acupuncture for aphasia after stroke was found to have a significant effect compared to the previous treatment alone. However, because of limitations, information was not reliable enough. Additional research is needed to produce more objective evidence.

다낭신의 중의치료에 대한 임상연구 동향 - CNKI검색을 중심으로 (A Review of Clinical Studies of Chinese Medicine Treatment of Polycystic Kidney Disease Using the CNKI Database)

  • Objective: This research reviews and investigates the trends in recent clinical studies of polycystic kidney disease (PKD) in China. Method: We searched for clinical studies discussing Oriental medicine-based treatments for PKD in the China National Knowledge Infrastructure (CNKI) database. Thirteen clinical articles published from 2001 to 2019 were analyzed. The search focused on the authors, publication year, type of study, purposes of study, method and duration of treatment, evaluation criteria, and results of the selected articles. Results: Of the articles from the database, 9 case series and 4 randomized controlled trials (RCTs) were analyzed. Ten articles used herbal medicine; 4 used herbal medicine for external use. Gamigyejibokryeong-hwan was the most common herbal prescription. The most frequently used herb was Polia Sclerotium (茯苓), and Cnidii Rhizoma (川芎) was employed in all the external uses. All 13 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: 1. Scientifically designed and more varied clinical studies are required to develop treatments for PKD. 2. The current study could be used as basic data in future clinical studies on treatment and further studies of PKD.

만성 위염에 대한 이중탕(理中湯)의 효과 : 체계적 문헌 고찰 및 메타 분석 (The Effect of Yijung-tang for Chronic Gastritis: A Systemic Review and Meta-Analysis)

  • Objective: This study assessed the effectiveness of using Yijung-tang to treat chronic gastritis through a systematic review and meta-analysis of randomized controlled trials (RCTs). Methods: The search was conducted using keywords such as "gastritis", "Lizhong", "Yijung", and "Chronic gastritis" on July 31, 2023. A meta-analysis was conducted according to outcome measurements, such as total effective rate (TER), Traditional chinese medicine symptom score (TSS), gastric hormones, and symptom relief time, using the Review Manager website. Results: A total of 11 RCT studies were selected. The treatment group (Yijung-tang-gagam or Yijung-tang-gagam+Western medicine) showed significant improvement effects in terms of TER, TSS, serum gastrointestinal hormones (motilin (MTL), gastrin (GAS), and somatostatin (SS)), and symptom relief time compared to the control group (Western medicine). TER-RR : 1.20, 95% CI : 1.11 to 1.30, P≤0.00001, RR : 1.21, 95% CI : 1.14 to 1.28, P≤0.00001; MTL-MD : 35.99, 95% CI : 30.79 to 41.20, P≤0.00001); GAS-MD : 103.33, 95% CI : 97.62 to 109.04, P≤0.00001); SS : MD : -37.19, 95% CI : -41.75 to -32.64, P≤0.00001; symptom relief time (nausea)-MD : -1.96, 95% CI : -2.47 to -1.45, P≤0.00001; symptom relief time (colic)-MD : -1.75, 95% CI : -2.31 to -1.18, P≤0.00001. Conclusion: This study suggests that Yijung-tang is effective against chronic gastritis. However, the number of studies was insufficient, and the diagnosis and evaluation criteria were not standardized. Furthermore, because of the low quality of the included studies, more clinical studies need to be conducted to increase the possibility of clinical use.

수술 후 발생한 유방암 연관 림프 부종(BCRL)에 대한 황기계지오물탕(黃芪桂枝五物湯)의 효과 : 체계적 문헌 고찰 및 메타 분석 (Effect of Hwanggigyejiomul-tang on Postoperative Breast Cancer-related Lymphedema (BCRL): A Systematic Review and Meta-analysis)

  • Objectives: This study assessed the effectiveness of Hwanggigyejiomul-tang for postoperative breast cancer-related lymphedema (BCRL) by a systematic review and meta-analysis of randomized controlled trials (RCTs). Methods: A search was conducted using keywords such as "breast cancer", "lymphedema", "edema", "Huangqi Guizhi Wuwu decoction", "Huangqi Guizhi Wuwu tang", and "Hwanggigyejiomul tang" in 10 databases (PubMed, Cochrane, ScienceDirect, CNKI, CiNii, RISS, KISS, ScienceON, OASIS, DBpia) on February 11, 2024. There were no limits on the publication period and language, and the quality of the studies was evaluated using Cochrane's risk of bias tool. A meta-analysis was performed based on the outcome measurements, such as total effective rate (TER), increase in shoulder joint mobility (flexion, extension, abduction, adduction), Fugl-Meyer assessment (FMA), and visual analog scale (VAS) using Review Manager Web. Results: Eleven RCTs were selected. The treatment group (Hwanggigyejiomul-tang-gagambang or hapbang with control group intervention) showed a more statistically significant effect compared with the control group (physical therapy or western medicine) in TER (upper limb circumference change), TER (upper limb edema grade change), increase in flexion angle, increase in extension angle, increase in adduction angle, FMA, and VAS. Conclusions: Hwanggigyejiomul-tang is effective in treating postoperative BCRL. However, because of the low quality of the included studies, more clinical studies are required to increase the possibility of clinical use.

뇌졸중 후 우울증에 대한 감맥대조탕(甘麥大棗湯)의 효과 : 체계적 문헌 고찰 및 메타 분석 (The Effect of Gammaekdaejo-tang for Post-stroke Depression: A Systemic Review and Meta-Analysis)

  • Objective: This study assessed the effectiveness of Gammaekdaejo-tang for post-stroke depression through a systematic review and meta-analysis of randomized controlled trials (RCTs). Methods: A search was conducted using keywords such as "Post-stroke Depression", "PSD", "Gammaekdaejo", and "Ganmai-dazao" on April 30, 2024. A meta-analysis was conducted according to outcome measurements, such as total effective rate (TER), HDRS (Hamilton Depression Rating Scale), and NIHSS (National Institute of Health Stroke Scale), using the Review Manager website. Results: A total of 10 RCTs was selected. The treatment group ((Gammaekdaejo-tang) or (Gammaekdaejo-tang combined with other ingredients or decoction)+Western medicine) showed significant improvement effects in terms of TER, HDRS, and NIHSS compared to the control group (Western medicine). [TER] RR: 1.19, 95% CI: 1.11 to 1.27, P<0.00001, RR: 1.24, 95% CI: 1.11 to 1.38, P<0.00001; [HDRS] MD: -2.29, 95% CI: -2.58 to -2.00, P<0.00001), MD: -3.28, 95% CI: -4.21 to -2.35, P<0.00001) [NIHSS] MD: -7.70, 95% CI: -8.52 to -6.89, P<0.00001. Conclusion: This study suggests that Gammaekdaejo-tang is effective in treating PSD. However, there are limitations, such as the small number of included studies, inability to clearly determine the effect of Gammaekdaejo-tang, inability to use various evaluation tools, and risk of bias. This research must be supplemented through systematic research design and implementation.

  • This paper presents the design of a passive biped walking robot based on limit cycle analysis. By using numerical analysis and experiment, we identify better design criterion for biped walking robot. In designing robot parameters we apply global search method to find limit cycles for given robot parameters and ground angle. Internal parameter variation changes limit cycle behavior, total energy, strides, etc and the characteristics of walking is analyzed by simulation and experiments.
