Hypersensitivity Reactions to Oxaliplatin: Clinical Features and Risk Factors in Koreans

  • Background and Aim: Oxaliplatin hypersensitivity is a well-known adverse reaction but the prevalence varies and data for frequency and clinical features have not been reported for Korea. Here we evaluates the prevalence and risk factors for hypersensitivity reactions to oxaliplatin after chemotherapy. Methods: Clinical information on all patients treated with oxaliplatin was retrospectively reviewed in electronic medical records between August 2009 and July 2010 in Seoul National University Bundang Hospital. Patients who experienced hypersensitivity reactions to oxaliplatin were compared with those who did not. Results: A total of 393 patients received oxaliplatin, with 42 (10.7%) experiencing hypersensitivity reactions including three cases of anaphylaxis. Median cycle of the first hypersensitivity reaction was 8. Reactions correlated with lower dexamethasone doses. Other variables were not significant. Conclusions: The prevalence of hypersensitivity reactions was 10.7%, symptoms being mostly mild and cutaneous. Lower dexamethasone doses could be a predictor for hypersensitivity reactions to oxaliplatin.

  • Coccidioidomycosis is a rare systemic fungal infection in Korea. However, the incidence of coccidioidomycosis has recently begun to increase due to the increasing incidence of people traveling overseas to endemic areas. In previously reported cases of coccidioidomycosis in Korea, the radiographic findings usually showed a solitary pulmonary nodule, pleural effusion, cavitation, and hilar lymphadenopathy, but no miliary nodules. We report a case of disseminated coccidioidomycosis with miliary nodules in an immunocompetent patient. A 32 year old male, who had traveled in Corona, New Mexico, USA, was admitted for an evaluation of persistent cough with fever. Chest radiography revealed initially diffuse multiple small nodules that appeared to be miliary tuberculosis. However, a subsequent evaluation revealed that he had disseminated coccidioidomycosis.

  • Transfusion related acute lung injury (TRALI) is a serious, potentially life-threatening complication of transfusion therapy that is sometimes under diagnosed and under reported. Patients with TRALI present with dyspnea/respiratory distress and fever. The symptoms, signs and chest radiological findings in TRALI are similar to transfusion associated circulatory overload, which makes it is difficult to distinguish it from circulatory overload. Although the mortality rate in cases of TRALI is relatively low, TRALI is the third most common cause of fatal transfusion reactions next to ABO blood type incompatibility and hepatitis. Mild-to-moderate cases of TRALI may be misdiagnosed as volume overload. Recently, we encountered two cases where the patients suffered from dyspnea and fever after a transfusion. and review of the relevant literature.

  • Acute Myocardica Infarction has high mortality and bad prognosis. We treated a 68 year-old female patient of acute myocardiac infacrtion to follow cerebral infarction. The chief complainment is right hemiplegia, dyspnea, chest discomfort, palpitation, anxiety, insomnia. The prognosis cannot be good, Because she has anterior wall infarction, cardiomegaly, conduction block, hypertention and she is 68 years old. We treated her with chiefly Nogyongdaebo-tang and Gongjin-dan, and gradually increased momentum. also we used self west medicine. The chief complainment was improved. She became to do cane walking by herself.

  • With an aging population, the number of patients with difficulty in swallowing due to medical conditions is gradually increasing. In such cases, enteral nutrition is administered through a temporary nasogastric tube. However, the long-term use of a nasogastric tube leads to various complications and a decreased quality of life. Percutaneous endoscopic gastrostomy (PEG) is the percutaneous placement of a tube into the stomach that is aided endoscopically and may be an alternative to a nasogastric tube when enteral nutritional is required for four weeks or more. This paper is the first Korean clinical guideline for PEG developed jointly by the Korean College of Helicobacter and Upper Gastrointestinal Research and led by the Korean Society of Gastrointestinal Endoscopy. These guidelines aimed to provide physicians, including endoscopists, with the indications, use of prophylactic antibiotics, timing of enteric nutrition, tube placement methods, complications, replacement, and tube removal for PEG based on the currently available clinical evidence.

  • Alcoholism is a chronic behavior disorder that disturbs the health, social, and economical functions by intaking alcohol repeatedly. Alcoholism includes some habituation, dependency, and addiction. It may be clinically silent or severe enough to lead to the rapid development of hepatic, renal and gastrointestinal failure. Alcoholism can also cause death. In this case, we administrated saenggangunbitang and sungjoocheonggantang to a patient suffering from alcohoism and its withdrawal symptoms. After administration of saenggangunbitang-sungjoocheonggantang medication, clinical symptoms, including liver function with diabetes mellitus and splenomegaly improved. saenggangunbitang-sungjoocheonggantang showed desirable effect on alcoholism symptoms.

  • This study was designed to evaluate the effects of retention enema therapy on patients with ileus. We retrospectively analyzed the medical records of two patients with ileus whose main symptoms were nausea, vomiting, constipation and abdominal pain. They were treated with retention enema therapy. After treatment, improvement was seen in nausea, vomiting, constipation and abdominal pain. The results of this study suggests that retention enema therapy is effective for the treatment of ileus.

  • Spironolactone reduces potassium excretion in renal collecting ducts and causes hyperkalemia. A 66-year-old female with urinary incontinence had to discontinue diuretics for hyperkalemia. After treatment with Jesaengsinkihwan-gagambang. she reduced urinary incontinence and her total urine volume was increased. BUN decreased from 32.2 to 16 and creatinine decreased from 1.5 th 0.9. So. we report one case of urinary incontinence with discontinuance of diuretics for hyperkalemia treated with Jesaengsinkihwan-gagambang.

  • The purpose of this study is to investigate the efficacy of using Korean medicine treatment with post-concussion syndrome. We primarily treated the patient's post-concussion syndrome during hospitalization (22.04-22.11.11) with Korean medicine treatments (herbal medicine, acupuncture, and Chuna therapy). The KHIT-6 and Numeric Rating Scale were used to assess symptom (headache, dizziness, and nausea) changes. Following treatment, the patient's KHIT-6 and Numeric Rating Scale scores decreased. This study suggests that Korean medicine may be effective in the treatment of post-concussion syndrome.
