Purpose: Gastric mucosal neoplastic lesions should have characteristic endoscopic features for successful endoscopic submucosal dissection. Materials and Methods: Out of the 1,010 endoscopic submucosal dissection, we enrolled 62 patients that had the procedure cancelled. Retrospectively, whether the reasons for cancelling the endoscopic submucosal dissection were consistent with the indications for an endoscopic submucosal dissection were assessed by analyzing the clinical outcomes of the patients that had the surgery. Results: The cases were divided into two groups; the under-diagnosed group (30 cases; unable to perform an endoscopic submucosal dissection) and the over-diagnosed group (32 cases; unnecessary to perform an endoscopic submucosal dissection), according to the second endoscopic findings, compared with the index conventional white light image. There were six cases in the under-diagnosed group with advanced gastric cancer on the second conventional white light image endoscopy, 17 cases with submucosal invasion on endoscopic ultrasonography findings, 5 cases with a size greater than 3 cm and ulcer, 1 case with diffuse infiltrative endoscopic features, and 1 case with lymph node involvement on computed tomography. A total of 25 patients underwent a gastrectomy to remove a gastric adenocarcinoma. The overall accuracy of the decision to cancel the endoscopic submucosal dissection was 40% (10/25) in the subgroup that had the surgery. Conclusions: The accuracy of the decision to cancel the endoscopic submucosal dissection, after conventional white light image and endoscopic ultrasonography, was low in this study. Other diagnostic options are needed to arrive at an accurate decision on whether to perform a gastric endoscopic submucosal dissection.
Objectives : In order to distinguish morphological characteristics of trunk bark and root bark of Ulmus davidiana var. japonica (Rehder) Nakai and the trunk bark and root bark of Hemiptelea davidii Planchon were sampled and compared in terms of their external and internal features with flour states according to their medical use, through microscopic examination. Methods : The slice of the tested material made by paraffin section technique was colored with Safranine Malachite Green contrast methods, and the flour of it was mounted by the liquid made by the same ratio of each of glycerin, acetic acid, and water, and then observed and photographed by olymphus-BHT. Results : 1. Internal Features 1) A large parenchymatous cell was observed in the phloem of the slice of both trunk bark and root bark of Ulmi Cortex, However, both of the trunk bark and root bark of Hemipteleae Cortex did not have parenchymatous cell in the phloem; instead, stone cells including much square crystal of calcium oxalate were distributed around fiber bundle, and the parenchymatous cell included much druse crystal of calcium oxalate. 2) In both the Ulmi Cortex and Hemipteleae Cortex, rhytidome was observed in trunk bark, but not in root bark, but in the parenchymatous cell of the root bark of the Ulmi Cortex contained starch grain. 2. Flour States 1) In the flour of root bark of the Ulmi Cortex, a large parenchymatous cell was observed. However, in the flour of trunk bark and root bark of Hemipteleae Cortex, no parenchymatous eel was found; instead, stone cell including square crystal of calcium oxalate and druse crystal of calcium oxalate were observed. 2) There was no remarkable difference between the trunk bark and root bark of Hemipteleae Cortex. However, starch grain was contained in the parenchymatous cell of the root bark of Ulmi Cortex but not in the trunk bark of it. Conclusions : There were some morphological differences in external, internal, and flour parts of Ulmi Cortex and Hemipteleae Cortex. In particular, there was a morphological difference in flour states between the trunk bark and root bark of Ulmi Cortex, it is possible to use microscope to distinguish their flour states.
Objective : Gypsum fibrosum has been traditionally used in treatment of febrile diseases and recently been shown to have anti-inflammatory effect. Chronic renal failure has a serious clinical symptoms including proteinuria, azotemia, anemia, and hyperlipidemia and has characteristic histopathological changes, glomerular hypertrophy, infiltration of inflammatory cells, and crescentic sclerosis, We investigated the effects of gypsum fibrosum on renal functional and histopathological disorder in chronic renal failure rat model induced 5/6 nephrectomy. Methods : Using Sprague-Dawley rats, CRF was induced by 5/6 nephrectomy. The rats were divided into 3 groups, normal, conrol, and gypsum administered orally with gypsum fibrosum 500mg/kg/day. Body weight, 24 hr proteinuria, hematologic analysis, and histological morphologic changes were followed up after 8 weeks. The glomerular macrophage/monocyte infiltration, $TGF-{\beta}_1$, type IV collagen, and angiotensin II type1 receptor($AT_1$) were evaluated by immunohistochemistry. Resuls : In the CRF control group, functional parameters and histopathologic changes clearly indicated the development of CRF. 24 hr proteinuria significantly increased in the CRF control group over the normal group, and serum creatinine level was lower in the gypsum group than in the control group, LDL-cholesterol was significantly lower in the gypsum group than in the control group. Morphological investigations showed a variety of characteristic features of CRF, glomerular hypertrophy, increasing cellular density of glomerulus, deposition of extra-cellular matrix, fibrotic change, and glomerular sclerosis in the control group, but in the gypsum group, these features diminished significantly. In observation of renal type IV collagen and $AT_1$ expression, positive area significantly increased in the control group over the normal group, and it significantly decreased in the gypsum group compared to the control group. Conclusions : Our findings suggest that gypsum fibrosum inhibits $AT_1$ and type IV collagen expression in renal tissues and attenuates progression of glomerulosclerosis and interstitial fibrosis in chronic renal failure rats, which lead to amelioration of renal function. From these results, we suggest that gypsum fibrosum may have renoprotective effects and could be a useful remedy agent for treating chronic renal failure.
Fashion is very superficial. However, it is expressed through the culture that is a cause of internal change and therefore its contents are defined by the cultural features. In this study, to study the aesthetic properties of the French fashion, the internal fashion contents are defined according to the cultural characteristics of France and based on the definition, the external features are investigated. The cultural characteristic was analyzed based on the reference literatures on the French culture. Based on the cultural characteristics of France, the aesthetical properties of the contemporary French fashion can be analyzed as follows: ${\cdot}$Dualism is the internal contents of the French culture with multiple sources, and it implies that opposing values such as female and male, traditionalism and modernism, simplicity and decoration are coexisting. Simplicity is pursued but decorative elements are in harmony and new phase of time is reflected while certain style has lasted for a long period of time to become a tradition. Furthermore, a rational trend combining masculinity and femininity, simultaneously exist with a trend that emphasizes feminine image. ${\cdot}$Multiculturism is a mysterious feature that has adapted the oriental or other cultural factors into the forms, patterns, colors and material of costumes. It has also been reconstructed with the French and the modern tastes. ${\cdot}$Avant-gardism is pursuing a novel aesthetic value against the former value system. It recognizes the human body as an object of figurative arts therefore emphasizes the body with geometric shapes to deny the fixed-idea, or engraft new materials and technologies to express the futuristic aspects.
Objectives: The purpose of this study is to determine the clinical features of post-acute COVID-19 syndrome and the effectiveness and safety of Korean medicine treatment at the Public health center. Methods: The study was conducted among 11 patients with post-acute COVID-19 syndrome who attended a single public health center from January to December 2022. We retrospectively analyzed the charts of 11 patients and collected clinical characteristics, previous treatments, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Visual Analog Scale (VAS)), adverse events, etc. Results: Of the 11 patients, six were women, and the average age of all patients was 68.27±12.31 years. The most common symptom were cough(n=9, 81.82%) and sore throat(n=9, 81.82%), followed by sputum, fatigue, rhinorrhea, and loss of appetite, etc. All 11 patients were treated with herbal medicine, with Samso-eum(n=6, 54.55%), Yeonkyopaedok-san(n=5, 45.45%), and Haengso-tang(n=3, 27.27%) being the most commonly used. After herbal medicine treatment, the median cough NRS decreased from 5 to 1, and the median sore throat NRS decreased from 4 to 1, both of which were statistically significant. One patient reported adverse event of dyspepsia and heartburn, but it was mild. Conclusions: The study presented the clinical features of the post-acute COVID-19 syndrome and suggested that Korean medicine treatment at public health centers may be effective and safe in alleviating associated symptoms.

Background and Objectives: Atrial tachycardias (ATs) from noncoronary aortic cusp (NCC) uncovered after radiofrequency ablation for atrial fibrillation (AF) are rarely reported. This study was conducted to investigate the prevalence and clinical characteristics of NCC ATs detected during AF ablation and compare their characteristics with de novo NCC ATs without AF. Methods: Consecutive patients who underwent radiofrequency catheter ablation for AF were reviewed from the multicenter AF ablation registry of 11 tertiary hospitals. The clinical and electrophysiological characteristics of NCC AT newly detected during AF ablation were compared with its comparators (de novo NCC AT ablation cases without AF). Results: Among 10,178 AF cases, including 1,301 redo ablation cases, 8 (0.08%) NCC AT cases were discovered after pulmonary vein isolation (PVI; 0.07% in first ablation and 0.15% in redo ablation cases). All ATs were reproducibly inducible spontaneously or with programmed atrial stimulation without isoproterenol infusion. The P-wave morphological features of tachycardia were variable depending on the case, and most cases exhibited 1:1 atrioventricular conduction. AF recurrence rate after PVI and NCC AT successful ablation was 12.5% (1 of 8). Tachycardia cycle length was shorter than that of 17 de novo ATs from NCC (303 versus 378, p=0.012). No AV block occurred during and after successful AT ablation. Conclusions: Uncommon NCC ATs (0.08% in AF ablation cases) uncovered after PVI, showing different characteristics compared to de-novo NCC ATs, should be suspected irrespective of P-wave morphologies when AT shows broad propagation from the anterior interatrial septum.
Graft-versus-host disease (GVHD) is a common complication of allogeneic stem cell transplantation (allo-SCT). However, a similar syndrome has been reported in autologous stem cell transplantation (ASCT) as well. The target organs of GVHD in ASCT are the skin, liver and gastrointestinal (GI) tract, which are consistent with those in allo-SCT. Histologic findings from the skin and the mucosa of the GI tract also show similar features. Here we describe a case of autologous GVHD involving the skin of a patient who underwent ASCT for multiple myeloma. In this patient, the response to a total prednisone dose of 0.5 mg/kg/day was unsatisfactory, and the patient required more intensive and prolonged immunosuppressive therapy with slow tapering.
This study was performed to investigate the effectiveness of Hangryunhaedocktang (HHT) on epidermal damages induced by allergic contact dermatitis (ACD). The sensitization were caused by one application of $25{\mu}l$ of 5% 2,4-dinitroch1orobenzene (DNCB) onto a back-lumbar skin of BALB/c mice. 2 weeks later, ACD was elicitated with $4{\mu}l$ of 2.5% DNCB and then mice were given HHT extract in doses of 3.3ml/kg/day, for 72 hours. The ACD induced epidermal damages in HHT treated ACD mice was more mitigated than non-treated ACD elicited mice. The features related with epidermal damage such as epidermal hyperplasia, infiltration of inflammatory cells, increase of nuclear shrinkages and vacuolation, and enlargement of intercellular space softened. And the distribution of soybean agglutinin (SBA) positive reaction in stratum spinosum (SS) and stratum basale (SB) were similarly maintained in a normal configuration. The numerical decrease of BrdU, TUNEL, and Fas positive cells observed were prominent in SB. Results suggest a benefit role for HHT in mitigating epidermal damages in mice with allergic contact dermatitis.
그동안 기술적으로 관심을 가져온 외부 정보위협의 방지를 위한 방화벽, 침입탐지시스템, 악성코드백신 등의 보안수단은 피해금액이나 사고건수 면에서 다수를 차지하는 내부 정보보안 위협에 대해서는 거의 효과적이지 않다. 본고에서는 컴퓨터를 통한 내부 정보보안 위협을 방지하기 위해 정보시스템사용에 대한 감시와 통제가 가능한 시스템에 대하여 구조와 특징, 세부기능과 개발방식 등을 정리하였다. 그리고 정리한 내용을 바탕으로 실제 개발사례가 되는 시스템을 통해 구현될 수 있는 기능을 설명하였다. 본고에서 제시한 시스템은 개방된 인터넷 환경 또는 모바일(mobile) 컴퓨팅과 같은 정보환경을 위해서는 보다 개선된 형태의 개발이 필요하다.
