  • 폐흡충증은 임상양상이나 방사선학적 소견이 폐결핵과 매우 유사하고 최근 우리나라에서 유병률이 낮아 조기 진단이 어렵다. 저자들은 폐결핵으로 오인된 폐흡충증 1예를 경험 하였으며 폐결핵이 의심되더라도 균이 증명되지 않거나 항결핵제 치료에 반응하지 않는 경우에 폐흡충증에 관한 병력 청취와 충란 검사 및 ELISA 검사를 통한 감별진단이 정확한 진단 및 치료에 도움이 될 것으로 사료된다.

전대뇌동맥 경색으로 인한 뇌량 분리 증후군 환자의 치험 1례 (Case Study of a Patient with Callosal Disconnection Syndrome Secondary to an ACA Infarction)

  • A callosal disconnection syndrome results in the functional independence of each hemisphere caused by ACA infarction, head injury or callosal operation for epilepsy. Clinical features are characterized by left unilateral tactile anomia, left unilateral agraphia, left unilateral apraxia, difficulty in copying drawings, dyscalculia and alien hand syndrome. Taeumchowe-tang(太陰調胃湯) is used to treat Taeumin(太陰人) Wewansuhanpyohanbyung(胃脘受寒表寒病). A 47 year-old man, following callosal infarction, was detected on MRI with left agraphia, dyscalculia and alien hand syndrome, but no left ideomotor apraxia or left apraxia. He was treated with Taeumchowe-tang. Significant improvement of clinical symptoms was seen, so it is here reported.

Clinical Efficacy of Oclacitinib and Lokivetmab in Dogs with Canine Atopic Dermatitis

  • Canine atopic dermatitis (CAD) is a genetically predisposed inflammatory and pruritic skin disease presenting characteristic clinical features in dogs. Despite oclacitinib and lokivetmab being commonly used, no study has compared their efficacies in CAD. This study aimed to compare the efficacy, safety, and control of CAD-associated pruritus and skin lesions between oclacitinib and lokivetmab. It also investigated whether switching to lokivetmab from oclacitinib or prednisolone had any benefits. Twenty-five client-owned dogs, newly diagnosed with CAD, were allocated to the oclacitinib (n = 20) and lokivetmab (n = 5) groups and administered oclacitinib (0.4-0.6 mg/kg orally, twice daily for 14 days, then once daily) and lokivetmab (2 mg/kg subcutaneously, every month) for 8 weeks, respectively. The switching group included five dogs previously administered with oclacitinib (n = 4) or prednisolone (n = 1) who were switched to lokivetmab directly at the start of the study. The pruritus visual analog scale (PVAS) and Canine Atopic Dermatitis Extent and Severity Index (CADESI-04) values were surveyed at weeks 0, 4, and 8. Oclacitinib and lokivetmab significantly reduced the PVAS and CADESI-04 scores. Switching from oclacitinib or prednisolone to lokivetmab maintained the severity of pruritus (4 weeks: p = 0.068; 8 weeks: p = 0.068) and dermatitis (4 weeks: p = 0.144; 8 weeks: p = 0.068) at the levels measured at baseline. Thus, both oclacitinib and lokivetmab reduced CAD-associated pruritus by a similar degree. Switching to lokivetmab maintained the severity of pruritus and dermatitis at the same level as the previous treatment.

시상증후군(Thalamic Pain Syndrome) 환자의 한방 병합치료에 관한 임상적 고찰 (Clinical Study of Combination Treatment of Oriental Medicine on Thalamic Pain Syndrome)

  • Objectives : To evaluate the pain control effect of combination treatment of Oriental Medicine on patients who suffered from thalamic pain syndrome caused by thalamic stroke. Methods : We reviewed the medical records and brain imaging data of all patients with thalamic stroke from September 1998 to August 2000 who visited to Department of Oriental Internal Medicine, National Medical Center. We evaluated clinical features of thalamic pain syndrome, including incidence, onset interval from stroke, nature, pain distribution, and assessed the pain control effect of combination treatment by Visual Analog Scale(VAS). Results : 64 cases were selected under the inclusion criteria, and 17 patients(26.5%) with thalamic pain syndrome were identified from 64 thalamic strokes. VAS proved combination treatment effective to control pain of thalamic pain syndrome. In 12 cases(70.5%), pain onset was within the first week poststroke. The patients with allodynia were 6(35.3%). In 12 cases(70.5%), the lesion was mainly located in the posterolateral areas of thalamus. Conclusion : We conclude that combination treatment of the Oriental Medicine modalities have pain control effectiveness on thalamic pain syndrome.(t=-5.47, p=0.0001)

조등산(Diaoteng-san, Choto-san, 釣藤散)과 GB34(陽陵泉) 전침치료를 활용한 약인성파킨슨증후군 환자의 증례 1례 (A Case Report of Drug-Induced Parkinsonism Treated with Jodeung-san and Electro-acupuncture at GB34)

  • Drug-induced Parkinsonism is the most frequently observed type among the cases of secondary Parkinsonism. Besides typical parkinsonian symptoms, such as tremor, rigidity and bradykinesia, drug-induced Parkinsonism manifests with additional simultaneous symptoms like orobuccolingual dyskinesia, mixed type of tremor (resting, action), and symmetry of expressions. We present a case of drug-induced Parkinsonism, affected by taking the antiulcer drug cimetidine. Jodeung-san extract (Choto-san, Tsumura Co. 10) was administered for 7 days and acupuncture (electronic, auricular, pharmacopuncture) was conducted 3 times. The clinical outcomes were then evaluated through the patient's global impression of change, visual analogue scale, and Hoehn and Yahr stage. After the treatment, the clinical features, such as tremor and orobuccolingual dyskinesia, disappeared. The combination of Jodeung-san and electro-acupuncture at GB34 could therefore be a remedy for the patients with drug-induced Parkinsonism.

소양인(少陽人) 지황백호탕(地黃白虎湯)으로 호전된 경증(輕症) 파킨슨 환자에 대한 임상보고(臨床報告) (A clinical report on the mild case of Parkinson's disease improved with Soyangin-Gihwangbeakho-tang)

  • Parkinson's disease(PD) is characterized by chronic progress of mesencephalic dopaminergic neuronal death. Diagnostic criteria for PD require at least two of three motor sign: tremor, rigidity, or bradykinesia. Levodopa and the dopamine agonists are considerd first-line drug therapy. In the book ‘dongyi soose bowon(東醫壽世保元)’, Soyangin Gihwangbeakho-tang(地黃白虎湯) is used at Soyangin Interior-overheated-disease. This case is patient who is 69 years old lady, suffered by the tremor of jaw and a slight rigidity, bradykinesia etc. This patient was classified as Soyangin by features, somatotype and emotional patterns. She improved in the tremor of jaw and others with Gihwangbeakho-tang for 67days. The result revealed that Soyangin-Gihwangbeakho-tang was effected on the tremor of jaw and others with Parkinson's disease patient.

Investigation of the effect of internal curing as a novel method for improvement of post-fire properties of high-performance concrete

  • Internal curing, a widely used method for mitigating early-age shrinkage in concrete, also offers notable advantages for concrete durability. This paper explores the potential of internal curing by partial replacement of sand with fine lightweight aggregate for enhancing the behavior of high-performance concrete at elevated temperatures. Such a technique may prove economical and safe for the construction of skyscrapers, where explosive spalling of high-performance concrete in fire is a potential hazard. To reach this aim, the physico-mechanical features of internally cured high-strength concrete specimens, including mass loss, compressive strength, strain at peak stress, modulus of elasticity, stress-strain curve, toughness, and flexural strength, were investigated under different temperature exposures; and to predict some of these mechanical properties, a number of equations were proposed. Based on the experimental results, an advanced stress-strain model was proposed for internally cured high-performance concrete at different temperature levels, the results of which agreed well with the test data. It was observed that the replacement of 10% of sand with pre-wetted fine lightweight expanded clay aggregate (LECA) not only did not reduce the compressive strength at ambient temperature, but also prevented explosive spalling and could retain 20% of its ambient compressive strength after heating up to 800℃. It was then concluded that internal curing is an excellent method to enhance the performance of high-strength concrete at elevated temperatures.

RCB법에 의한 원통형 플런지 연삭공정의 싸이클 시간 감소에 관한 연구 (A Study on the Cycle Time Reduction of Cylindrical Plunge Grinding Process with Recursive Constraint Bounding Method)

  • This study presents the recursive constraint bounding(RCB) method to reduce the cycle time in internal cylindrical plunge grinding process. This method can cope with process noise as well as modeling bias. The main features of RCB method are its utilization of measurements at the end of each cycle and its use of monotonicity analysis for determining the extremes of bias and noise. This method is investigated in simulation and evaluated by experiment in internal cylindrical plunge grinding operation. The results from simulation and experiment show that it is effective in reducing cycle time in spite of modeling uncertainty in the forms of process noise and modeling bias.

  • Numerical calculation tool for forging of gear-like components based on kinematically admissible velocity fields for upper bound method applicable to various deformation features of workpiece in forging processes were suggested. Each one of them deals with unidirectional flow of metal on dies, such as external involute spur gear, sequare spline, internal serrations. A complex calcuation tool of gear-like component forging process was built up by combining these kinematically velocity fields. In this paper, the workpiece with both external and internal teeth is divided into two parts. The deformation of each part is analyzed simultaneously using numerical calculation tool form combined kinematically admissible velocity field. The experimental set-up was installed in a 200 ton hydraulic press. As a result, each kinematically admissible velocity field could be combined with other and the calculated solution are useful to predict the capacity of forging equipment.

Structural dynamics: Convergence properties in the presence of damage and applications to masonry structures

  • A numerical model for masonry is proposed by following an internal variable approach originally developed in the field of elastic-plastic analysis. The general features of the theoretical framework are discussed by focussing on finite element models applicable to incremental elastic-plastic problems. An extremum property is derived and its implications in terms of convergence for convenient algorithms are briefly discussed, by including the case of softening materials and damage effects. Next, a numerical model is presented, which is suitable for masonry, can be developed according to the same internal variable formulation and enjoys similar properties. Some numerical results are presented and compared with the response of a masonry shear wall subjected to pseudodynamic tests.
