Potentials of Chenpi on Metabolic Syndrome: A Review

  • Objectives: Metabolic Syndrome (MetS) is strongly related with central obesity, hypertriglyceridemia, low high-density lipoprotein-cholesterol (HDL-C), hyperglycemia, and hypertension. This study reviewed the potential of Chenpi in treatment of MetS through amelioration of co-related diseases, such as diabetes mellitus, hyperlipidemia, obesity, hepatic steatosis, and inflammation. Methods: Six electronic databases (Oriental Medicine Advanced Searching Integrated System (OASIS), Korean Traditional Knowledge Portal, Korea Institute of Oriental Medicine (KIOM), Research Information Sharing Service (RISS), PubMed, and Embase) were used to search for in vitro, in vivo, and clinical research that discusses the potential effects of Chenpi (Citrus unshiu Markovich, Citrus reticulata Blanco) on diabetes mellitus, hyperlipidemia, obesity, hepatic steatosis, and inflammation. Results: This review suggests the potential of Chenpi as a candidate for the treatment of metabolic syndrome through improvement of co-related diabetes, hyperlipidemia, obesity, hepatic steatosis, and inflammation. However, comparison of the results of each study was limited by a lack of quantification of the experimental materials.

  • Idiopathic interstitial pneumonia (IIP) is a histologically identifiable pulmonary disease without a known cause that usually infiltrates the lung interstitium. IIP is largely classified into idiopathic pulmonary fibrosis, idiopathic non-specific interstitial pneumonia, respiratory bronchiolitis-interstitial lung disease (ILD), cryptogenic organizing pneumonia, desquamative interstitial pneumonia, and acute interstitial pneumonia. Each of these diseases has a different prognosis and requires specific treatment, and a multidisciplinary approach that combines chest high-resolution computed tomography (HRCT), histological findings, and clinical findings is necessary for their diagnosis. Diagnosis of IIP is made based on clinical presentation, chest HRCT findings, results of pulmonary function tests, and histological findings. For histological diagnosis, video-assisted thoracoscopic biopsy and transbronchial lung biopsy are used. In order to identify ILD associated with connective tissue disease, autoimmune antibody tests may also be necessary. Many biomarkers associated with disease prognosis have been recently discovered, and future research on their clinical significance is necessary. The diagnosis of ILD is difficult because patterns of ILD are both complicated and variable. Therefore, as with other diseases, accurate history taking and meticulous physical examination are crucial.

  • The external treatment of internal diseases was first recorded in Hwangjenaegyeong(黃帝內經), and two hundred and fifty odd cases were found in the Bonchogangmok(本草綱目) and Oechisusebang(外治壽世方). Bonchogangmok(本草綱目) is a technical book devoted to explaining materia media as well as the external treatment of internal diseases. Oechisusebang(外治壽世方) is also a technical book, which make 36 references to the disease patterns of internal medicine. The medicinals employed as external treatment of internal diseases are grouped into 300 classes. These medicinals do not set limits to trees and plants but also uses birds and beasts, reptiles, grains, feces and urine, soil, stone etc., applied to various disease patterns. This study researched the medical texts in order to treat the disease patterns of internal medicine.

  • 페인트, 가구 도장제 등에 포함된 Isocyanate는 직업성 천식의 흔한 원인물질로 알려져 있고, 과민성 폐렴을 일으키기도 한다. 저자들은 6개월간 자동차 공장 페인트 도장공으로 일한 중국인 노동자에서 isocyanate에 의한 것으로 추정되는 과민성 폐렴 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Asthma is a chronic inflammatory airway disease that is characterized by variable airflow obstruction. The Korean Asthma Study Group of the Korean Academy of Tuberculosis and Respiratory Diseases has recently updated the Korean Asthma Guideline. This review summarizes the updated Korean Asthma Guideline. Asthma prevalence is increasing worldwide, and in Korea. Variable airflow obstruction can be confirmed by bronchodilator response or other tests, and should be established prior to the controller medication. A low-dose inhaled corticosteroid-formoterol is used to alleviate symptoms in all treatment step, and it can be used as a controller as well as reliever in steps 3-5. This approach is preferred, because it reduces the risk of severe exacerbations, compared to the use of short-acting β2-agonist as reliever. In severe asthma, phenotype/endotype based on the underlying inflammation should be evaluated. For type 2 severe asthma, the biologics should be considered.

악성 종양에 의한 흉막삼출에서 적혈구수 몇 Carcinoembryonic Antigen 그리고 세포진 검사와의 관계 (Correlation of Gross Appearance or RBCs Numbers with Pleural Histocytology and Pleural Fluid Carcinoembryonic Antigen Values in Malignancy Associated Pleural Effusions)

  • 목 적: 악성 종양과 연관된 흉막삼출에서 혈액상을 보이는 빈도를 확인하고 혈액상 여부와 세포진 검사나 흉막조직 검사의 양성률 그리고 흉막액 CEA 양성률과의 연관 관계를 확인해 보고자 하였다. 또한 일반적으로 흉막액 CEA의 증가는 세포진 양성률과 밀접한 연관관계를 보인다고 하는데, 이에 대한 의미를 재조명하고자 하였다. 방 법: 1995년 3월부터 1996년 12월까지 연세대학교 의과 대학 세브란스 병원에서 악성 종양으로 진단된 환자중 흉막삼출이 발생한 98예를 대상으로 흉막액의 통상검사(pH, cell count, glucose, protein, LDH)와 함께 흉막액의 외견, 적혈구 수, CEA, 그리고 세포 조직학적 검사를 시행하였다. 결 과: 악성 종양과 연관된 흉막삼출의 44.9%에서 혈액상이었고 55.1%에서 장액상으로 나타났다. 흉막액의 적혈구 수가 $100,000/mm^3$ 이상 증가된 경우 흉막액 세포진 검사의 양성률은 42.8%였으며 흉막액의 적혈구수와 흉막액 세포진 검사의 양성률 그리고 CEA 양성률 등의 상호간에 연관관계는 없었다. 세포조직검사장 확진된 악성 흉막삼출의 경우 흉막액 CEA는 72.7%에서 10ng/ml 이상으로 증가된 소견을 보였고, 흉막액 CEA가 10ng/ml 이상으로 증가된 경우 새포조직검사의 양성률은 58.2%였다. 종양 진단후 흉막액이 발현되기까지의 기간과 흉막액 세포진 검사의 양성률, 흉막액 CEA의 양성률, 그리고 흉막액 적혈구수 등의 상호간에 연관관계를 발견할 수 없었다. 결 론: 소견을 악성 종양과 연관된 흉막삼출의 44.9%에서 혈액상보였으며 흉막액내 적혈구 수의 증가와 세포조직학적 검사의 양성률이나 CEA의 양성률간에 유의한 상관관계는 없었다. 흉막액 CEA의 측정은 세포조직학적인 양성률과 일치하지는 않았지만 일부 비종양성 흉막삼출에 의한 증가를 임상적으로 배제하면 흉막생검, 세포진 검사와 함께, 악성 흉막삼출의 진단적 접근에서 보조적인 역할을 감당할 수 있을 것으로 생각된다.

The Effects of Retinoic Acid and MAPK Inhibitors on Phosphorylation of Smad2/3 Induced by Transforming Growth Factor β1

  • Background: Transforming growth factor ${\beta}$ (TGF-${\beta}$), retinoic acid (RA), p38 mitogen-activated protein kinase (MAPK), and MEK signaling play critical roles in cell differentiation, proliferation, and apoptosis. We investigated the effect of RA and the role of these signaling molecules on the phosphorylation of Smad2/3 (p-Smad2/3) induced by TGF-${\beta}1$. Methods: A549 epithelial cells and CCD-11Lu fibroblasts were incubated and stimulated with or without all-trans RA (ATRA) and TGF-${\beta}1$ and with MAPK or MEK inhibitors. The levels of p-Smad2/3 were analyzed by western blotting. For animal models, we studied three experimental mouse groups: control, bleomycin, and bleomycin+ATRA group. Changes in histopathology, lung injury score, and levels of TGF-${\beta}1$ and Smad3 were evaluated at 1 and 3 weeks. Results: When A549 cells were pre-stimulated with TGF-${\beta}1$ prior to RA treatment, RA completely inhibited the p-Smad2/3. However, when A549 cells were pre-treated with RA prior to TGF-${\beta}1$ stimulation, RA did not completely suppress the p-Smad2/3. When A549 cells were pre-treated with MAPK inhibitor, TGF-${\beta}1$ failed to phosphorylate Smad2/3. In fibroblasts, p38 MAPK inhibitor suppressed TGF-${\beta}1$-induced p-Smad2. In a bleomycin-induced lung injury mouse model, RA decreased the expression of TGF-${\beta}1$ and Smad3 at 1 and 3 weeks. Conclusion: RA had inhibitory effects on the phosphorylation of Smad induced by TGF-${\beta}1$ in vitro, and RA also decreased the expression of TGF-${\beta}1$ at 1 and 3 weeks in vivo. Furthermore, pre-treatment with a MAPK inhibitor showed a preventative effect on TGF-${\beta}1$/Smad phosphorylation in epithelial cells. As a result, a combination of RA and MAPK inhibitors may suppress the TGF-${\beta}1$-induced lung injury and fibrosis.
