Objectives: Patient dose verification is clinically the most important parts in the treatment delivery of radiation therapy. The three dimensional(3D) reconstruction of dose distribution delivered to target volume helps to verify patient dose and determine the physical characteristics of beams used in intensity modulated radiation therapy(IMRT). We present Beam Intensity Scanner(BInS) system for the pre treatment dosimetric verification of two dimensional photon intensity. The BInS is a radiation detector with a custom made software for relative dose conversion of fluorescence signals from scintillator. Methods: This scintillator is fabricated by phosphor Gadolinium Oxysulphide and is used to produce fluorescence from the irradiation of 6MV photons on a Varian Clinac 21EX. The digitized fluoroscopic signals obtained by digital video camera will be processed by our custom made software to reproduce 3D relative dose distribution. For the intensity modulated beam(IMB), the BInS calculates absorbed dose in absolute beam fluence, which are used for the patient dose distribution. Results: Using BInS, we performed various measurements related to IMRT and found the followings: (1) The 3D dose profiles of the IMBs measured by the BInS demonstrate good agreement with radiographic film, pin type ionization chamber and Monte Carlo simulation. (2) The delivered beam intensity is altered by the mechanical and dosimetric properties of the collimating of dynamic and/or static MLC system. This is mostly due to leaf transmission, leaf penumbra, scattered photons from the round edges of leaves, and geometry of leaf. (3) The delivered dose depends on the operational detail of how to make multileaf opening. Conclusions: These phenomena result in a fluence distribution that can be substantially different from the initial and calculative intensity modulation and therefore, should be taken into account by the treatment planing for accurate dose calculations delivered to the target volume in IMRT.
Proton beams have been used for cancer treatment for more than 28 years, and several technological advancements have been made to achieve improved clinical outcomes by delivering more accurate and conformal doses to the target cancer cells while minimizing the dose to normal tissues. The state-of-the-art intensity modulated proton therapy is now prevailing as a major treatment technique in proton facilities worldwide, but still faces many challenges in being applied to the lung. Thus, in this article, the current status of proton therapy technique is reviewed and issues regarding the relevant uncertainty in proton therapy in the lung are summarized.

A novel photo-alignment scheme using normally incident periodically intensity-modulated UV light has been developed and demonstrated to efficiently yield a stable pretilt angle of nematic liquid crystals by single exposure. The scheme, referred to as the "photo-rubbing", consists in unidirectional scan of the intensity-modulated UV light over the photo-alignment film, which causes asymmetrical photo-reactions in the film. We show here its successful application to multi-domain alignment processing. Thanks to the normal incidence, the photo-rubbing removes the instrumental disadvantages of the conventional oblique incidence technique, thereby providing a true practical solution for photo-alignment.
Intensity modulated photovoltage spectroscopy (IMVS) analysis of organic solar cells (OSCs) with a bulk-heterojunction (BHJ) film composed of P3HT and $PC_{61}BM$ was performed. The dynamic response of charge recombination by the post-annealing approach in $P3HT/PC_{61}BM$ BHJ solar cells characterized by IMVS demonstrated that post-annealing reduced the recombination of electron carriers in the device. The recombination times of $P3HT/PC_{61}BM$ BHJ solar cells post-annealed at room temperature, 80, 120, and $140^{\circ}C$ were 0.009, 0.020, 0.024, and 0.030 ms, respectively, at a short-circuit current of 0.18 mA. The results indicated that the IMVS analysis can be effectively used as powerful.
This article reviewed new trends and controversial issues, including the intensification of chemotherapy and recent brachytherapy (BT) advances, and also reviewed recent consensuses from different societies on the management of locally advanced cervical cancer (LACC). Intensive chemotherapy during and after radiation therapy (RT) was not recommended as a standard treatment due to severe toxicities reported by several studies. The use of positron emission tomography-computed tomography (PET-CT) and magnetic resonance imaging (MRI) for pelvic RT planning has increased the clinical utilization of intensity-modulated radiation therapy (IMRT) for the evaluation of pelvic lymph node metastasis and pelvic bone marrow. Recent RT techniques for LACC patients mainly aim to minimize toxicities by sparing the normal bladder and rectum tissues and shortening the overall treatment time by administering a simultaneous integrated boost for metastatic pelvic lymph node in pelvic IMRT followed by MRI-based image guided adaptive BT.
목적 : 비인강암 환자의 국소제어율을 향상시키기 위한 목적으로 보상 여과판을 이용한 전방위 강도변조 방사선치료방법(intensity modulated radiation therapy : IMRT)을 계획하고 기존 3차원 입체조형치료방법과 비교하여 최적의 방사선치료방법을 모색하고자 한다. 대상 및 방법 : 3-차원 입체조형치료계획으로 치료받았던 비강암환자(T4N0M0) 1예를 선택하여 치료면의 굴곡과 뼈, 공동 등 불균질 조직으로 인하여 발생되는 표적체적의 선량분포를 균일하게 만들고 주변 정상장기의 손상을 최소화하기 위한 일차 입사선량의 강도 조절을 보상여과판으로 시행 하였다. 환자는 열변성 plastic mask로 고정시킨 후 치료조준용 CT Scan (PQ5000)을 이용하여 3 mm 간격으로 scan 하고 가상조준장치(virtual simulator)와 3차원 방사선치료계획 컴퓨터$(ADAC-Pinnacle^3)$를 이용하여 보상여과판을 제작하였다. 각 조사면을 세분한 소조사선(beamlet)의 강도 가중치(weighting)를 계산하고 가중치에 따른 선량 감약을 보상여과판의 두께로 환산하여 판별이 쉽도록 도표화하였다. 방사선 치료성과의 기준은 정량적으로 평가할 수 있는 선량체적표(dose volume histogram : DVH)와 종양억제확율(tumor control probability : TCP)및 정상조직 손상확율(normal tissue complication probability : NTCP)의 수학적 관계식을 이용하여 치료효과를 평가하였다. 결과 : 전방위 IMRT에서 계획용표적체적(planning target volume: PTV)내의 최소선량과 최대선량의 차이가 입체조형치료계획보다 약간 증가하였으며 평균선량은 강도조절치료계획에서 약 $10\%$, 더 높았고 전체 방사선량의 $95\%$가 포함되는 체적(V95)은 비교적 양쪽 설계방법에서 비슷한 양상을 보이고 있었다. 주위 건강장기들의 DVH에서 방사선에 민감한 장기인 시신경, 측두엽, 이하선, 뇌간, 척수, 측두하악골관절 등은 강도조절치료계획에서 많이 보호되었다. PTV의 종양제어확율은 입체조형치료계획과 강도변조치료계획에서 모두 비교적 균일하였으며 계획선량이 50 Gy에서 80 Gy로 증가함에 따라 TCP가 0.45에서 0.56으로 완만하게 증가하였다. 척수, 측두하악골 관절, 뇌간, 측두엽, 이하선, 시신경교차, 시신경 등 정상장기의 손상확율은 입체조형치료계획보다 강도조절치료계획에서 월등히 감소되었으며 특히 뇌간(brain stem)의 NTCP는 입체조형치료계획에서 보다 강도조절치료계획에서 훨씬 적은 값(0.3에서 0.15)으로 감소되었다. 계획선량의 증가에 따른 TCP와 NTCP를 입체조형치료계획과 강도조절치료계획에서 TCP는 공히 완만한 증가를 보였으나 NTCP값은 선량증가에 비례적으로 증가하였고 입체조형치료계획이 강도조절치료계획보다 월등히 증가하였다. 결론 : 보상여과판을 이용한 전방위 강도변조 방사선치료에서 PTV내의 선량 균일도의 개선은 없었지만 뇌간, 척수강 등 정상장기의 피폭을 줄일 수 있었다. 특히 인체표면의 굴곡이 심하거나 뼈, 동공 등으로 종양에 도달하는 방사선량분포가 균일하지 않을 경우 매우 유리한 치료방법이였다. 방사선치료성적을 평가함에 있어 DVH와 TCP, NTCP 등 수학적 척도를 이용함으로서 치료성과의 예측, 종양선량의 증가(dose escalation), 방사선수술의 지표 및 방사선치료의 질적 상황을 정량적 수치로 평가할 수 있어 방사선치료성과 향상에 기여할 수 있다고 생각한다.
2원동시 RF magnetron 방법을 이용하여 조건별(sputtering gas압력, buffer충 유무 및 etching여부 등)로 제작된 Pt/co 인공격자 다층막 ([$Pt10.7\AA/Co2.8{\AA}{\times}{12}$])의 구조정보에 대하여 알아보았다. 다층막 구조평가는 X선 회절측정을 이용하여 행하였으며 구조에 대한 정보를 보다 면밀하게 평가하기 위해서 다층막 모델을 제작하여 얻어진 결과를 실험결과와 비교분석 하였다. 제작된 다층막 구조모델의 계산결과를 실측치와 비교하면 peak의 위치나 회절강도가 상당히 일치하였으며 이것은 구조모델이 실제의 Pt/Co 인공격자 다층막구조를 잘 반영하고 있음을 알 수 있었다.
Locoregional failure is the most frequent pattern of failure in locally advanced head and neck cancer patients and it leads to death in most of the patients. Second primary tumors occurring in the other head and neck region reach up to almost 40% of long-term survivors. Recommended and preferred retreatment option in operable patients is salvage surgical resection, reporting a 5-year overall survival of up to 40%. However, because of tumor location, extent, and underlying comorbidities, salvage surgery is often limited and compromised by incomplete resection. Reirradiation with or without combined chemotherapy is an appropriate option for unresectable recurrence. Reirradiation is carefully considered with a case-by-case basis. Reirradiation protocol enrollment is highly encouraged prior to committing patient to an aggressive therapy. Radiation doses greater than 60 Gy are usually recommended for successful salvage. Despite recent technical improvement in intensity-modulated radiotherapy (IMRT), the use of concurrent chemotherapy, and the emergence of molecularly targeted agents, careful patient selection remain as the most paramount factor in reirradiation. Tumors that recur or persist despite aggressive prior chemoradiation therapy imply the presence of chemoradio-resistant clonogens. Treatment protocols that combine novel targeted radiosensitizing agents with conformal high precision radiation are required to overcome the resistance while minimizing toxicity. Recent large number of data showed that IMRT may provide better locoregional control with acceptable acute or chronic morbidities. However, additional prospective studies are required before a definitive conclusion can be drawn on safety and effectiveness of IMRT.

Background: Breast cancers are becoming more frequently diagnosed at early stages with improved long term outcomes. Late normal tissue complications induced by radiotherapy must be avoided with new breast radiotherapy techniques being developed. The aim of the study was to compare dosimetric parameters of planning target volume (PTV) and organs at risk between conformal (CRT) and intensity-modulated radiation therapy (IMRT) after breast-conserving surgery. Materials and Methods: A total of 20 patients with early stage left breast cancer received adjuvant radiotherapy after conservative surgery, 10 by 3D-CRT and 10 by IMRT, with a dose of 50 Gy in 25 sessions. Plans were compared according to dose-volume histogram analyses in terms of PTV homogeneity and conformity indices as well as organs at risk dose and volume parameters. Results: The HI and CI of PTV showed no difference between 3D-CRT and IMRT, V95 gave 9.8% coverage for 3D-CRT versus 99% for IMRT, V107 volumes were recorded 11% and 1.3%, respectively. Tangential beam IMRT increased volume of ipsilateral lung V5 average of 90%, ipsilateral V20 lung volume was 13%, 19% with IMRT and 3D-CRT respectively. Patients treated with IMRT, heart volume encompassed by 60% isodose (30 Gy) reduced by average 42% (4% versus 7% with 3D-CRT), mean heart dose by average 35% (495cGy versus 1400 cGy with 3D-CRT). In IMRT minimal heart dose average is 356 cGy versus 90cGy in 3D-CRT. Conclusions: IMRT reduces irradiated volumes of heart and ipsilateral lung in high-dose areas but increases irradiated volumes in low-dose areas in breast cancer patients treated on the left side.
