Contents Analysis on the Media about Problems of Long-Term Care Facilities (노인요양시설 사건사고에 관한 미디어 내용 분석)

  • The purpose of this study is to analyze contents on media about the problems of long-term care facilities. For achieving this purpose, we reviewed 'KINDS(http://www.kinds.or.kr/)' from July, 2008 to May, 2012 which was an synthetic newspaper and TV news searching system. Among 320 articles, we finally analyzed 72 articles: 218 articles were duplicated and 30 articles did not directly related or were not objective. The results were divided with seven parts: 'long-term care facilities', 'employee' 'health care and accidents', 'providing and management of service', 'meals related problems', 'maltreatment of client', 'disorders & unfairness long-term care benefit. Among these seven parts, we could confirm specific and major problems, which were lack of safety management, incompleteness facilities & equipments, shortage of long-term care manpower, insufficient service providing low quality of service, hiding money from elderly's accounts, and providing some money to get national subsidies. For resolving these problems, the long-term service guidelines must be prepared and the related facilities must cooperate each other for providing high quality of service. In addition, the efficient systems must be made to find out long-term care problems and unfairness of service providing and strict management rules or sanctions must be needed.

Convergence Effective Factors for Work Performance among Returning to Workers with Industrial Accident (산업재해 직업복귀자의 업무수행능력 융합적 영향 요인)

  • For workers, industrial accidents exert a bad effect on the productivity, quality of life, and depress the morale. This study aimed to examine the overall influence on job performance of employees who returned to work after industrial accidents. This was a cross-sectional study using the data of 2013 and 2014 Panel Study of Worker's Compensation Insurance (PSWCI), and we performed logistic-regression analysis to analyze an affinity between general characteristics and job performance as independent variable and outcome variable, respectively. As a result, the major factor depressed the job performance were the 1 - 7 degree of disabilities and injuries with convalescence period for 6 to 9month or more than 12 months. In other words, this study shown that job performance was decreased as higher degree of disability and longer convalescence period. Job performance is the factor to identify indirectly worker's successful return to work, and it is important in follow-up of workers who returned to work after industrial accidents. Stable job performance of an industrial disaster victim is the key factor to maintain worker's comfortable and qualitative life as well as increase of productive capacity.

  • The converted accident rates that included in Pre-Qualification (PQ) are to evaluate the results of accidents which have occurred. Therefore, it is demanded the development of proactive evaluation contents to evaluate the safety management activities and efforts on construction companies. The purpose of this study is to provide basic data for the establishment of proactive evaluation contents at public construction project in Korea through the collected and analyzed the PQ documentation applied by public agencies in Los Angeles, USA. The results indicated that diverse reactive safety evaluations are implemented to examine accidents which occurred in past projects such as occupational health insurance, regulation violation. And also, when the accidents occurred, it is evaluated the proactive safety management actions such as safety education, accident prevention programs, and safety management plans are reflected on the evaluation. Thus, if proactive safety related evaluation contents are established and reflected in PQ examinations together with diverse existing reactive safety evaluations considering converted accident rates, not only the existing function of converted accident rates can be reinforced but also businesses' active safety management activities can be induced and decreases in the construction industry's accident rate can be expected.

A Study on the Introduction Direction of Private Investigation Law (민간조사업법의 도입방향에 관한 연구)

  • The important items, which should be considered in Private Investigation Law, can include subjects, licenses, the scope of business, qualifying examinations, and supervisory and penal provisions. The subjects of Private Investigation Law should be permitted to be both natural persons and juridical persons in terms of providing various services, but should be permitted to be juridical persons and should be administered on a license system, even in order to ensure public interests. Concretely, the introduction scope of Private Investigation Law can be regulated to include the followings: that is, investigating the whereabouts identification of runaways and missing children, investigating the personal identification, habit, way of action, motivation, whereabouts identification, real child confirmation, association, transaction, reputation, and personality of specific persons or specific groups, investigating the whereabouts identification of missing persons, owners of government-vested properties or renounced properties, investigating the whereabouts of lost properties or stolen properties, investigating the causes of fire, character defamation, slander, damage, accident, physical disability, infringement on real estate or movable property, and investigating all sorts of accidents including traffic accidents, insurance accidents, and medical malpractices. In the qualifying examination, examinees' age should be restricted to be over age 25. The person, who is exempted from its primary examination, should be restricted to be the person, who has the career of over 20 years in related fields, in consideration of its equity with other certificates of qualification. In the supervisory institution, as the policy institution is the supervisory institution in many countries including France (the police) and Japan (public security committee), so the National Policy Agency should be the supervisory institution in consideration of management aspects. In the penal regulations, especially, we should clarify the management of personal information (personal information protection, personal information management), and so should prevent the infringement of people's basic rights, and then should ensure the public interest.

A Study on Risk Factor Identification by Specialty Construction Industry Sector through Construction Accident Cases : Focused on the Insurance Data of Specialty Construction Worker (건설재해사례 분석에 의한 전문건설업종별 위험요인 탐색 : 전문건설업 근로자 공제자료를 중심으로)

  • The number of domestic construction company is expanding every year while the construction workers' exposure to disaster risk is increasing due to technological advancements and popularity of high-rise buildings. In particular, the industry faces greater fatalities and severe large scale accidents because of construction industry characteristics including influx of foreign workers with different language and culture, large number of aged workers, outsourcing, high place work, heavy machine construction. The construction industry is labor-intensive, which is to be completed under given timeline and consists of unique working environment with a lot of night shifts. In addition, when a fixed construction budget is not secured, there is less investment in safety management resulting in poor risk management at the construction site. Taking account that the construction industry has higher accident risk rate and fatality rate, risky and unique working environment, and various labor pool from foreign to aged workers, preemptive safety management through risk factor identification is a mandatory requirement for the construction industry and site. The study analyzes about 8,500 cases of construction accidents that occurred over the past 10 years and identified risk factor by construction industry sector to secure a systematic insight for risk management. Based on interrelation analysis between accident types, work types, original cause materials and assailing materials, there is correlation between each analysis factor and work industry. Especially for work types, there is great correlation between work tasks and industry type. For reinforced concrete and earthwork are among the most frequent types of accidents, and they are not only high in frequency of accidents, but also have a high risk in categories of occurrence.

  • The court sentenced meaningful decisions related to the medical service in 2014. The court assumed the negligence of medical staff in the accident if being broken while using the medical equipment for not an original purpose at the time of surgery and ruled that the compensation for damage can be recognized in recognition of the causal relationship between the explanation duty violation and side effect's happening when unproven surgery on safety is implemented regarding the duty of explanation, that in the case of cosmetic surgery, the subject on the duty of explanation needs to be expanded compared to the general medical practice and that the duty of explanation cannot be accepted for the range that cannot be expectable. Also, the court has provided the requirement and limitation of self-determination exercise in case of the crash between patient's self-determination and doctor's duty of care and has ruled that as automobile insurance contract is a contract with the insurance company to pay regarding liability for car accidents, treating patients and taking the insurance money is not illegal activity even for the unlicensed hospital violating the medical law while established. The judgment stating the opinion that medical practitioners cannot be punished according to the medical law prohibiting the receiving of rebate in case that medical practitioners did not receive benefit while the medical institution itself gained an unfair economic benefit also stands out. And the court has ruled that even if the medical institution who received a business suspension is closed, the suspension is still effective in case that the same operator opens a new medical institution in the same place, ruled on the requirement to conduct a medical service outside of the medical institution that the doctor opened and ruled that the administrative penalty cannot be conducted prior to the conviction on charge of violating the medical law.

Analysis on the actual condition of consciousness for related regulations with revised Water Leisure Safety Regulation (수상레저안전법의 개정에 따른 관련규제에 관한 의식실태분석)

  • Changes such as expanding nationwide road networks, 5-day work system and income increases triggered high interest in water leisure and an increase in water sports population. And safety regulations have been under discussion as a possible way to prevent the water leisure related accidents. Water Leisure Safety Regulation was enacted in 1999 and fully revised in 2005. Therefore, it is necessary to examine the current state cf water leisure activities and how the water leisure safety regulations are perceived. For this purpose, a survey of water leisure people was conducted on the revised safety regulation and relevance and correlation between variables were examined using Frequency analysis and Crosstabs. The results showed that more than $60\%$ of the people surveyed acknowledge the necessity of registration, safety instruction, insurance, and safety inspection. It was also found that the people who are highly dissatisfied with the safety of water leisure apparatus and acknowledge the necessity of insurance showed strong opinion of the necessity of legal controls on water leisure activities.

Nationwide Study on the Usage and Characteristics of Patients Visiting Korean Medical Facilities - Based on the Treatment of Major Disorders, Effectiveness, Satisfaction and Occurrence Rate of Side Effects From the Ministry of Welfares Report on Usage and Consumption of Korean Medicine in 2011 - (전국민을 대상으로 한 한의원과 한방병원 외래이용환자의 이용실태 및 특성비교연구 - 2011년 한방의료이용 및 한약소비실태조사 보고서(보건복지부)중 이용환자의 질병치료방법 및 치료효과를 중심으로 -)

  • A survey conducted on 1,103 patients or caretakers visited Korean medical clinics and hospitals between August 25, 2011 to September 30, 2011 by the Ministry of Health and Welfares and Korea Institute for Health and Social Affairs was analyzed and obtained following results: 1. For sociodemographic distribution, female(813) outnumbered male(290) patients with majority of patients ranging from 30s to 60s. Married patients(793) outnumbered unwed(150) patients and 65.0% with higher than high school education. Statistical significance was seen in gender, age, marital status, and education level but no significant difference for status of employment, income level, and types of insurance between the clinics and hospitals. (P<0.05). 45.1% had less than \2,000,000 in monthly salary and most of coverage was provided by either regional or work insurance. 2. 67.9% of the patients rated health conditions to be better than average and 32.1% listed as poor. People in good health showed tendency to visit Korean medical facilities. Musculo-skeletal conditions such as arthritis, ankle sprain, lumbago, muscular injury, and frozen shoulder were common conditions, followed by gastric disorders, common cold and herbal tonics. No significant difference was observed between the clinics and hospitals for above conditions, but significance was seen in atopic dermatitis, stroke, and sequela from traffic accidents (P<0.05). 3. Ten most common conditions addressed at Korean medical facilities were lumbago, arthritis, muscular injury, back sprain, gastric disorders, ankle sprain, common cold, herbal tonics, frozen shoulder and stroke. Major treatment modalities rendered were herbal medicine, herbal supplements, acupuncture and moxibustion, cupping, tuina, and Korean midical physical therapy. No significant difference existed between the clinics and hospitals. 4. All modalities showed at least 85% effectiveness. No statistical significant difference between the clinics and hospitals except for herbal decoction. (P=0.0452) 5. 88.3% of responses showed treatment satisfaction with significant difference between the clinics and hospitals (P=0.002). The occurrence of side effects was at 2%, mostly corning from treating digestive, skin, kidney disorders and neurological issues. No significant difference was observed between the clinics and hospitals. From the above results, the typical population visiting Korean medical facilities can be summarized as being middle aged female with relatively higher education and moderately low income. The health condition is generally good and the purpose of visit is to receive traditional treatments of acupuncture, herbal medicine, and physical therapy. Treatment efficacy and satisfaction were high with no significant differences between the clinics and hospitals.

Health Behavior Associated with Outpatient Utilization (외래서비스 이용과 건강행태)

  • Objectives: It attempted to analyze influencing factors on the utilization of outpatient services which were adopted to predisposing, enabling, and need factors in Anderson model. Methods: The current study analyzed "2007 Korean National Health Nutrition Survey" data, which selected 3,335 people nationwide by proportional systematic sampling. This study analyzed data of persons who used outpatient services in two weeks. It adopted Anderson Model to control contextual factors including socioeconomic factors. The study compared means and fitted logistic regression models and multilevel model. Results: The logistic regression model showed that persons purchased private medical insurance were less likely to use outpatient services than the persons did not purchase private medical insurance. Persons with hypertension and diabetes mellitus, overweight, and problem drinkers were more likely to use outpatient services. Persons with high school graduates or higher in education level and experience of accidents or intoxications were more likely to use outpatient services according to the multilevel analysis of mixed model which treated region as random effect. Conclusion: Higher level of perceived stress increased the probability to use outpatient service than lower level of perceived stress. As number of days a person had exercised increased, the probability to use outpatient service decreased. Overweight and problem alcohol drinking increased the probability of outpatient service use. Further research should be conducted to find more factors influencing outpatient service use.

Utilization Trends of Health Subcenter for Primary Medical Care in a Korean Rural Area (일개 농촌 면단위지역 주민의 보건지소 의료 이용 추이)

  • This study was analyze through the reports which published on the subject matter of Su Dong-Myun from year of 1972-1993, and analysis of data in 1994 was performed with medical records on the health subcenter by PC-SAS program. The result are as follows: The number of population in Su-Dong Myun(study area) was 5,707 in 1995, 4,641 in 1985 and 5,424 in 1975. In the composition rate of population "0-14" of age group only showed markedly decreasing tendancy from 42.8% in 1975 to 19.1% in 1995. However, "65 and over" showed markedly increasing tendancy from 5.7% in 1975 to 9.8% in 1995. Annual utilization rate showed rapidly increasing tendency from year of 1972 to 1978, such as 314 per showed rapidly decreasing tendency, such as 708 in 1981, 485 in 1984, 272 in 1987, 309 in 190 and in 1993. In the annual age-specific utilization rate, the age group of "0-14" showed the highest rate of 621 per thousand population in 1975, 1159 in 1980, 1021 in 1985 and 538 in 1990. However the age group of "65 and over" showed the highest rate of 481 in 1994. Age specific annual utilization rate showed markedly decreasing tendency in the age group of "0-14" and "15-44", however showed slightly decreasing tendency or same level in the group of "45-64" and "65 and over" from year of 1980 to 1994. In the age specific utilization rate, the lower down the age was, the higher tendency the rate showed, such as 621 per 1,000 population in "0-14" of the age group, 543 in "15-44", 406 in "45-64" and 294 in "65 and over" in 1975. However, the higher up the age was, the higher tendency the rate showed in 1980, 1985 and 1994, except "0-14" of age group. The 5 major diseases were disease of Respiratory system, Gastrointestinal system, Skin and Subcutaneous tissue, Accidents, Poisoning and Violence and Nervous system and Sense organ, in 1975, 1980 and 1985. However, in 1990 and 1994, the 5 major disease were disease of Respiratory system, Gastrointestinal system. Skin and Subcutaneous tissue, Musculoskeletal system and, Connective tissue and Circulatory system. In Composition rate of patient in Su Dong-Myun Health Subcenter by Charged Medical Fee, medical insurance showed almost all the highest rate of 93.9% in year of 1994 and C.H.D.A. of 100% in 1975. Proportion of insurance showed increasing tendency such as 6.6% in 1980, 21.3% in 1985, 69.0% in 1990 and relatively C.H.D.A. showed decreasing tendency.
